Impact of cigarette smoking on response to interferon therapy in chronic hepatitis C Egyptian patients

AIm:Smoking may affect adversely the response rate to interferon-α.Our objective was to verify this issue among chronic hepatitis C patients. mETHODS:Over the year 1998,138 chronic hepatitis C male Egyptian patients presenting to Cairo Liver Center, were divided on the basis of smoking habit into:group I which comprised 38 smoker patients(>30 cigarettes/d) and group Ⅱ which included 84 non-smoker patients. Irregular and mild smokers(16 patients)were excluded. Non eligible patients for interferon-α therapy were excluded from the study and comprised 3/38(normal ALT)in group I and 22/84 in group Ⅱ(normal ALT,advanced cirrhosis and thrombocytopenia).Group I was randomly allocated into 2 sub-groups:group Ia comprised 18 patients who were subjected to therapeutic phlebotomy while sub-group Ib consisted of 17 patients who had no *** sub-group la,3 patients with normal ALT after repeated phlebotomies were excluded from the ***-α 2b 3 mU/TIW was given for 6 mo to 15 patients in group Ia,17 patients in group Ib and 62 patients in group Ⅱ. Biochemical,virological end-of-treatment and sustained responses were evaluated. RESULTS:At the end of interferon-α treatment,ALT was normalized in 3/15 patients(20%)in group Ia and 2/17 patients(11.8%)in group Ib compared to17/62 patients (27.4%)in group Ⅱ(P=0.1).Whereas 2/15 patients(13.3%) in group *** 2/17 patients(11.8%)in group Ib lost viraemia compared to 13/62 patients(26%)in group Ⅱ (P=0.3).Six months later,ALT was persistently normal in 2/15 patients(13.3%)in group 1a and 1/17 patients (5.9%)in group Ib compared to 9/62 patients(14.5%)in group Ⅱ(P=0.47).Viraemia was eliminated in 1/15 patients (6.7%)in group Ia and 1/17 patients(5.9%)in group Ib compared to 7/62 patients(11.3%) in group Ⅱ,but the results did not mount to statistical significance(P=0.4). Smart and interconnected devices can generate meaningful patient data and exchange it automatically without any human intervention in order to realize the Internet of Things(IoT)in healthcare(HIoT).Due to more and more online security and data hijacking attacks,the confidentiality,integrity and availability of data are considered serious issues in HIoT *** this regard,lightweight block ciphers(LBCs)are promising in resourceconstrained environment where security is the primary *** prevalent challenge while designing an LBC for the HIoT environment is how to ascertain platform performance,cost,and *** of the existing LBCs primarily focus on text data or grayscale *** main focus of this paper is about securing color images in a cost-effective *** emphasis high confidentiality of color images captured by cameras in resource-constrained smartphones,and high confidentiality of sensitive images transmitted by low-power sensors in IoT *** order to reduce computational complexity and simulation time,the proposed Lightweight Symmetric Block Cipher(LSBC)exploits chaos-based confusion-diffusion operations at the inter-block level using a single *** strength of LSBC is assessed by cryptanalysis,while it is ranked by comparing it to other privacy-preserving *** results show that the proposed cipher produces promising results in terms of key sensitivity and differential attacks,which proves that our LSBC is a good candidate for image security in HIoT.

Ammoniacal Nitrogen and Organics Removal modelling in Vertical Flow Wetlands Treating Strong Wastewaters

This paper reports a comparative evaluation between 2 kinetic models for predicting nitrification and biodegradable organics(BOD5)removal rates in 5 vertical flow(VF)wetland systems,that received strong wastewaters(***,textile and municipal effluents).The models were formulated by combining first order and monod kinetics,with continuous-stirred tank reactor(CSTR)flow *** performance of the 2 models had been evaluated with3 statistical parameters:coefficient of determination(R2),relative root mean square error(RRmSE),and model efficiency(mE).The statistical parameters indicated better performance of the monod CSTR model(over first order CSTR approach),for correlating ammoniacal nitrogen(NH4+—N)and BOD5removal profiles across VF *** monod coefficient values(from monod CSTR model)coincided with greater input NH4+—N and BOD5loading,and experimentally measured removal rate(g/(m2·d))*** trends indicate that NH4+—N and BOD5removals in the VF systems were mainly achieved via biological *** the other hand,the rate constants(from the first order CSTR model)did not exhibit such correlations(of monod coefficients),elucidating their inefficiencies in capturing overall removal *** interference of organics removal on nitrification process(in VF wetlands)was identified through monod *** deviation between NH4+—N and BOD5monod coefficients imply incorporation of both coefficients,for calculating the area of a single VF ***,closer performance of the monod CSTR model for predicting NH4+—N and BOD5removals indicate its potential application,as a design tool for VF systems.

Development and Validation of a Liquid Chromatographic method for the Determination of Leflunomide： Application to in vitro Drug metal Interactions

Leflunomide is a leading drug for the treatment of rheumatoid arthritis. The principle aim of this study was to develop and validate an RP-HPLC method for the determination of leflunomide in bulk and pharmaceutical dosage form using diclofenac sodium as an internal standard. For this purpose, chromatography was accomplished on a Purospher Start, C18 （5 μm, 12.5 cm×0.46 cm） column at ambient temperature. methanol ： water （80 ： 20, V/V） solvent system was selected as mobile phase, the pH of which was adjusted to 3.4 by ortho-phosphoric acid and de- livered at a flow rate of 1.2 mLomin-1. Seperation of leflunomide and diclofenac sodium was carried out on a Purospher Start, C18 equipped with a UV-visible detector at 248 nm. The suitability of the method for the quantita- tive determination of the drugs is proven by validation in accordance with the requirements laid down by the Inter- national Conference on Harmonization （ICH） guidelines. The method was accurate （99.55%--100.03%）, specific, linear （R2〉0.999） and precise （intra-day precision 0.023%--0.93% and inter-day precision 0.26%--0.944%） in the range of 0.5--20 μg·mL^-1. The minimum limit of detection and quantification in pharmaceutical formulation were 0.05 and 0.15 μ***^-1, respectively. Thus the proposed method is simple, accurate, reproducible and suitable for the routine analysis of leflunomide in pharmaceutical formulations and was applied to study in vitro drug-metal interactions.

mHD maxwell Fluid with Heat Transfer Analysis under Ramp Velocity and Ramp Temperature Subject to Non-Integer Differentiable Operators

The main focus of this study is to investigate the impact of heat generation/absorption with ramp velocity and ramp temperature on magnetohydrodynamic(mHD)time-dependent maxwell fluid over an unbounded plate embedded in a permeable ***-dimensional parameters along with Laplace transformation and inversion algorithms are used to find the solution of shear stress,energy,and velocity ***,new fractional differential operators are used to define ramped temperature and ramped *** obtained analytical solutions are plotted for different values of emerging *** time derivatives are used to analyze the impact of fractional parameters(memory effect)on the dynamics of the *** making a comparison,it is observed that the fractional-order model is best to explain the memory effect as compared to classical *** results suggest that the velocity profile decrease by increasing the effective Prandtl *** existence of an effective Prandtl number may reflect the control of the thickness of momentum and enlargement of thermal *** incremental value of the m is observed for a decrease in the velocity field,which reflects to control resistive ***,it is noted that the Atangana-Baleanu derivative in Caputo sense(ABC)is the best to highlight the dynamics of the *** influence of pertinent parameters is analyzed graphically for velocity and energy *** for skin friction and Nusselt number are also derived for fractional differential operators.

Design of a test structure based on chevron-shaped thermal actuator for in-situ measurement of the fracture strength of mEmS thin films

A novel test structure to characterize the fracture strength of mEmS(micro-electro-mechanical Systems)thin films is *** test structure is comprised of a micro fabricated chevron-shaped thermal actuator and test *** test structure is capable of producing large displacement and stresswhile keeping a relatively low temperature gradient across the test specimen.A voltage is applied across the beams of the chevron-shaped actuator,producing thermal expansion force to fracture the test *** deflection is computed based on elastic analysis of *** verify the test structure,simulations have been implemented using COmSOL multiphysics.A 620μmlong,410μm wide,10μm thick test structure produced stress of 7.1 GPawhile the applied voltage is 5 *** results indicate that the test structure is suitable for in-situ measurement of the fracture strength of mEmS thin films.

One of the leading causes of mortality worldwide is liver *** earlier the detection of hepatic tumors,the lower the mortality *** paper introduces a computer-aided diagnosis system to extract hepatic tumors from computed tomography scans and classify them into malignant or benign *** hepatic tumors from computed tomography scans is considered a challenging task due to the fuzziness in the liver pixel range,intensity values overlap between the liver and neighboring organs,high noise from computed tomography scanner,and large variance in tumors *** proposed method consists of three main stages;liver segmentation using Fast Generalized Fuzzy C-means,tumor segmentation using dynamic thresholding,and the tumor’s classification into malignant/benign using support vector machines *** performance of the proposed system was evaluated using three liver benchmark datasets,which are mICCAI-Sliver07,LiTS17,and *** proposed computer adided diagnosis system achieved an average accuracy of 96.75%,sensetivity of 96.38%,specificity of 95.20%and Dice similarity coefficient of 95.13%.

Evaluation of Neurological Disorder Using Computed Tomography and magnetic Resonance Imaging

m>Background: m>The integration of different imaging technologies such as computed tomography (CT) and magnetic resonance imaging (mRI) assists clinicians in diagnosing several neurological diseases at an initial stage, thereby inhibiting their development into serious medical conditions. With respect to the severity of neurological diseases and the increasing popularity of imaging tools, the current study has evaluated the findings of CT and mRI in examining neurological disorders. m>methods: m>A retrospective study was conducted at the faculty of applied medical sciences, King Abdul-Aziz University from September 2017 to 2018 including details of 43 patients with complaints of potential neurological disturbances such as headache, oligodendroglioma, CNS infiltration, hypoxia, weakness, epilepsy and multiple sclerosis. The history of patients including their age, gender, reason for examination and findings of CT and mRI was evaluated in the study. m>Results: m>The results showed that patients in the age group of 21 to 40 years were more prone to developing neurological disorders. The major reasons for neurological examination in individuals were found to be headaches (n = 7), multiple sclerosis (n = 5) and epilepsy (n = 3). The evaluation of patients’ records revealed that only 4 patients underwent CT scans;however, all of them went through the mRI. Statistically significant results were obtained for the patients scanned and examined through mRI rather than CT. m>Conclusions: m>The results revealed mRI as more successful in diagnosing different neurological conditions in comparison with CT scan. The study further suggests that mRI can be used as an integral diagnostic approach for neurological understanding of major diseases of the central nervous system.
