Neurological complications after cardiac surgery, rang- ing from permanent stroke to transient dysfunction, repre- sent a key issue in the management of geriatric patients. Many patients aged 70 or more have history of neurological dysfunctions, which increases the risk of complications and sequelae, Severe neurologic diseases, such as strokes, occur in up to 6% of patients undergoing cardiac surgery. Heart rate variability （HRV） refers to the variations between consecutive heartbeats, which depend on the continuous modulation of the sympathetic and parasympathetic branches of the autonomic nervous system. HRV has been shown to be effective as a predictor of risk after myocardial infarction and an early warning sign of diabetic neuropathy, and in the cardiology setting is now recognized to be a useful tool for risk-stratification after hospital admission and after discharge. Recent evidences suggest that HRV analysis might predict complications even in patients undergoing cardiac surgery, and the present review summarizes the importance of HRV analysis in adult cardiac surgery and the perspectives for HRV use in current clinical practice. Although future larger studies are warranted before HRV can be included into daily clinical practice in adult cardiac surgery, HRV is a novel tool which might detect autonomic instability in the early postoperative phase and during hospital stay, thus predicting or prompt-diagnosing many of the post-operative complications.

AIM： To analyze the long-term prognosis in a cohort of western cirrhotic patients with single hepatocellular carcinoma treated with ethanol injection.METHODS： One-hundred forty-eight patients with solitan/hepatocellular carcinoma were enrolled. The tumor diameter was lower than 2 cm in 47 patients but larger in the remaining 101 patients. The impact of some pre- treatment clinical and laboratory parameters and of tu- mor recurrence on patients＇ survival was assessed.RESULTS： Among the pre-treatment parameters, only a tumor diameter of less than 2 cm was an independent prognostic factor of survival. The occurrence of new nodules in other liver segments and the neoplastic portal invasion were linked to a poorer prognosis at univariate analysis. Patients with a single hepatocellular carcinoma smaller than 2 cm showed a better 5-year cumulative survival （73.0% vs 47.9%）（P = 0.009）, 3-year local re- currence rate （29.1% vs 51.5%）（P = 0.011）, and 5-year distant intrahepatic recurrence rate （52.g% vs 62.8%）（P = 0.054） compared to patients with a larger tumor. CONCLUSION： The 5-year survival rate of patients with single hepatocellular carcinoma 〈 2 cm undergoing ethanol injection is excellent and comparable to that achieved using radiofrequency ablation.

Coronavirus disease 2019(COVID-19)has several extrapulmonary ***(GI)symptoms are among the most frequent clinical manifestations of COVID-19,with severe consequences reported in elderly ***,the impact of COVID-19 on patients with pre-existing digestive diseases still needs to be fully elucidated,particularly in the older *** review aimed to investigate the impact of COVID-19 on the GI tract,liver,and pancreas in individuals with and without previous digestive diseases,with a particular focus on the elderly,highlighting the distinctive characteristics observed in this ***,the effectiveness and adverse events of the anti-COVID-19 vaccination in patients with digestive disorders and the peculiarities found in the elderly are discussed.

Snow depth is the easiest snowpack property to measure in the field and is used to estimate the distribution of snow for quantifying snow storage. Often the mean of three snow depth measurements is used to represent snow depth at a location. This location is used as a proxy for an area, typically a digital elevation model (DEM) or remotely sensed pixel. Here, 11, 17, or 21 snow depth measurements were used to represent the mean snow depth of a 30-m DEM pixel. Using the center snow depth measurement for each sampling set was not adequate to represent the pixel mean, and while the use of three snow depth measurements improved the estimate of mean, there is still large error for some pixels. These measurements were then used to determine the variability of snow depth across a pixel. Estimating variability from few points rather than all in a measurement was not sufficient. The sampling size was increased from one to the total per pixel (11, 17, or 21) to determine how many point samples were necessary to approximate the mean snow depth per pixel within five percent. Binary regression trees were constructed to determine which terrain and canopy variables dictated the spatial distribution of the snow depth, the standard deviation of snow depth, and the sample size to within 5% of the mean per pixel. One location was measured in two years just prior to peak accumulation, and it is shown that there was little to no inter-annual consistency in the mean or standard deviation.

Background:COVID-19 caused by SARS-CoV-2 virus is characterized by respiratory compromise and immune system involvement,even leading to serious disorders,such as cytokine ***:We then conducted a literature review on the topic of sepsis and covid-19,and in parallel conducted an experimental study on the histological finding of patients who died from SARS-Covid 19 infection and a control ***:Sepsis associated with covid-19 infection has some similarities and differences from that from other ***:In this paper the complex interplay between the 2 disorders was discussed,focusing on the similarities and on the effect that one could have on the other.A preliminary experimental section that demonstrates the multisystemic involvement in subjects who die from SARS-CoV-2 is also proposed.
