Teaching chronic obstructive airway disease patients using a metered-dose inhaler

Asthma and chronic obstructive airway disease ,（COAD） are chronic inflammatory disorders of the airways which are usually associated with widespread airway obstruction that is often relieved by treatment. β2-adrenoreceptor agonists and corticosteriods are the mainstay of the management of this disease. The preferred route of administration of these agents is by inhalation. This allows the drug to be delivered directly to the airway with more rapid relief and fewer side effects than systemic route. The main disadvantage of metered-dose inhaler （MDI） use is that the correct use requires good technique;

本文主要讨论香港“修例风波”中的青年问题,试图说明香港的社会结构是让香港成为中国“化外之民”的“元凶”,进一步加剧了青年对国家的错误认知。回归后,香港青年对国家的认同感一直下降,而大量青年参与反修例活动,更是进一步反映了他们对特区和中央政府的强烈不满和厌恶感。本文综合分析香港本土和外国势力的互动关系,指出香港的意识形态和外国的势力干预是引发大规模青年社运的主要成因。笔者认为,社会的持续发展和青年参与相辅相成,透过了解青年问题认清社会的现实需求,并为解决青年问题提供新思路。本文分为四个部分。第一部分对“修例风波”进行背景概述,阐述青年人参与的方式和动机等。第二部分对“修例风波”的本地成因进行分析,指出香港的历史、社会、政治和经济体系都是构建青年反抗精神的因素。第三部分对“修例风波”的国际背景进行分析,从动员、渗透和干预三个方面说明“修例风波”是以青年人为对象的政治阴谋。第四部分就香港在“修例风波”之后的出路进行思考,从教育发展和社会结构上提出建议,为陆港交流和青年教育方面提供更多的视角。本文可供读者透过青年问题了解香港的深层次矛盾,更是提倡香港社会以积极态度与青年相处和了解他们的想法,培育更健康的年轻群体,帮助青年重回正轨。

Embryonic stem cells(ESCs)are derived from the inner cell mass of *** can self-renew and are pluripotent,meaning that they can proliferate indefinitely and have the ability to differentiate into all cells in our *** may therefore act as a source of different cell types for future regenerative *** the present study,the role of voltage-gated K(Kv)channels in maintaining the self-renewal and pluripotent characteristics of undifferentiated mouse(m)ESCs was *** channel blocker tetraethylammonium(TEA)was shown to decrease mESC proliferation without affecting cell *** of Kv channels resulted in cell cycle arrest at the G/G *** addition,decreased expression of pluripotent markers such as Oct-4 was detected in Kv channel -blocked *** Kv blockade led to depolarization,and Kv channel-blocked ESCs failed to progress through the cell cycle and had a loss of pluripotency,the present results suggested a possible relationship between the Kv channel functions,membrane potential,cell cycle progression and differentiation potential in *** study shall provide new information about the basic biology of ESCs and provide insights into the possible strategies of controlling ESC self-renewal and differentiation.

Objectives The predisposition of intracranial atherosclerotic disease(ICAD)to East Asians over Caucasians infers a genetic basis which,however,remains largely *** prevalence of vascular risk factors(VRFs)in Chinese over Caucasian patients who had a stroke,and shared risk factors of ICAD with other stroke subtypes indicate genes related to VRFs and/or other stroke subtypes may also contribute to *** Unrelated symptomatic patients with ICAD were recruited for genome sequencing(GS,60-fold).Rare and potentially deleterious single-nucleotide variants(SNVs)and small insertions/deletions(InDels)were detected in genome-wide and correlated to genes related to VRFs and/or other stroke *** aneuploidies,copy number variants(CNVs)and chromosomal structural rearrangements were also ***,candidate genes were used for pathway and gene ontology enrichment *** Among 92 patients(mean age at stroke onset 61.0±9.3 years),GS identified likely ICAD-associated rare genomic variants in 54.3%(50/92)of ***-eight patients(52.2%,48/92)had 59 rare SNVs/InDels reported or predicted to be deleterious in genes related to VRFs and/or other stroke *** of the 59 rare variants were identified in local subjects without ICAD(n=126).31 SNVs/InDels were related to conventional VRFs,and 28 were discovered in genes related to other stroke *** study also showed that rare CNVs(n=7)and structural rearrangement(a balanced translocation)were potentially related to ICAD in 8.7%(8/92)of ***,candidate genes were significantly enriched in pathways related to lipoprotein metabolism and cellular lipid catabolic *** Our GS study suggests a role of rare genomic variants with various variant types contributing to the development of ICAD in Chinese patients.

目的研究乙型肝炎病毒(HBV)整合肝癌基因组位点附近基因表达调控的改变。方法先后应用Southern blot、基因组文库构建与杂交筛选技术,在1例肝癌组织中克隆、筛选与鉴定了2个HBV DNA在人基因组中的阳性整合克隆,并对全长序列进行了测序分析。采用Western blot方法检测病毒插入点附近基因Cyclin E1的表达。免疫组化方法检测Cyclin E1在由143例肝癌组织构成的组织芯片中的表达水平。结果肝癌组织中有2条>3.2kb的阳性杂交带,分别为4.5kb和12kb,并有2个不同的整合位点。测序分析表明,其中一个病毒整合点的两端均位于人染色体19q12,插入的片段长度为1108bp,整合过程造成了2107bp HBV DNA缺失。另一个病毒序列整合到染色体的5p13,插入的病毒序列长度为2587bp,发生了基因重排。病毒插入位点附近Cyclin E1基因呈高表达。51.7%(74/143例)肝癌组织Cyclin E1表达阳性。结论 HBV DNA插入人基因组可能导致细胞Cyclin E1高表达,提示病毒整合人染色体的潜在致癌机制。

Randomised controlled trial comparing modified Sano's and narrow band imaging international colorectal endoscopic classifications for colorectal lesions

AIM To assess the utility of modified Sano′s(MS) vs thenarrow band imaging international colorectal endoscopic(NICE) classification in differentiating colorectal *** Patients undergoing colonoscopy between 2013 and 2015 were enrolled in this *** on the MS or the NICE classifications,patients were randomised for real-time endoscopic *** was followed by biopsies,endoscopic or surgical *** endoscopic diagnosis was then compared to the final(blinded) *** primary endpoint was the sensitivity(Sn),specificity(Sp),positive predictive value(PPV) and negative predictive value(NPV) of differentiating neoplastic and non-neoplastic polyps(MSⅡ/Ⅱo/Ⅲa/Ⅲb vs I or NICE 1 vs 2/3).The secondary endpoints were "endoscopic resectability"(MSⅡ/Ⅱo/Ⅲa vs Ⅰ/Ⅲb or NICE 2 vs 1/3),NPV for diminutive distal adenomas and prediction of post-polypectomy surveillance *** A total of 348 patients were *** Sn,Sp,PPV and NPV in differentiating neoplastic polyps from non-neoplastic polyps were,98.9%,85.7%,98.2% and 90.9% for MS;and 99.1%,57.7%,95.4% and 88.2% for NICE,*** area under the receiver operating characteristic curve(AUC) for MS was 0.92(95%CI:0.86-0.98);and AUC for NICE was 0.78(95%CI:0.69,0.88).The Sn,Sp,PPV and NPV in predicting "endoscopic resectability" were 98.9%,86.1%,97.8% and 92.5% for MS;and 98.6%,66.7%,94.7% and 88.9% for NICE,*** AUC for MS was 0.92(95%CI:0.87-0.98);and the AUC for NICE was 0.83(95%CI:0.75-0.90).The AUC values were statistically different for both comparisons(P = 0.0165 and P = 0.0420,respectively).The accuracy for diagnosis of sessile serrated adenoma/polyp(SSA/P) with high confidence utilizing MS classification was 93.2%.The differentiation of SSA/P from other lesions achieved Sp,Sn,PPV and NPV of 87.2%,91.5%,89.6% and 98.6%,*** NPV for predicting adenomas in diminutive rectosigmoid polyps(n = 150) was 96.6% and 95% with MS and NICE ***

The liver is an immunologically tolerant organ and a common metastatic site of multiple cancer *** a role for cancer cell invasion programs has been well characterized,whether and how liver-intrinsic factors drive metastatic spread is incompletely ***,we show that aberrantly activated hepatocyte-intrinsic cell cycle-related kinase(CCRK)signaling in chronic liver diseases is critical for cancer metastasis by reprogramming an immunosuppressive *** an inducible liverspecific transgenic model,we found that CCRK overexpression dramatically increased both B16F10 melanoma and MC38 colorectal cancer(CRC)metastasis to the liver,which was highly infiltrated by polymorphonuclear-myeloid-derived suppressor cells(PMNMDSCs)and lacking natural killer T(NKT)*** of PMN-MDSCs in CCRK transgenic mice restored NKT cell levels and their interferon gamma production and reduced liver metastasis to 2.7% and 0.7%(metastatic tumor weights)in the melanoma and CRC models,***,CCRK activated nuclear factor-kappa B(NF-κB)signaling to increase the PMN-MDSC trafficking chemokine C-X-C motif ligand 1(CXCL1),which was positively correlated with liver-infiltrating PMN-MDSC levels in CCRK transgenic ***,CRC liver metastasis patients exhibited hyperaaivation of hepatic CCRK/NF-κB/CXCL1 signaling,which was associated with accumulation of PMN-MDSCs and paucity of NKT cells compared to healthy liver transplantation *** summary,this study demonstrates that immunosuppressive reprogramming by hepatic CCRK signaling undermines antimetastatic *** findings offer new mechanistic insights and therapeutic targets for liver metastasis intervention.
