Photodynamic therapy(PDT)is a novel therapeutic approach for combating various cancers.PDT involves the administration of a photosensitizer which generates singlet oxygen after light activation.FosPeg■ is the liposomal formulation of mTHPc.In this in vitro study,the photodynamic efficacy of FosPeg■ on a human colon cancer cell line(HT29)was investigated via studying the cellular uptake of FosPeg■,FosPeg■ PDT mediated photocytotoxicity and the cell death mechanism were triggered.FosPeg■ PDT demonstrated its antitumor effect in a drug and light dose-dependent manner in HT-29 cells.Lethal dose(LD50)was achieved with 0.4g/mL of drug and 3 j/cm^(2) of light dose.FosPeg■ PDT triggered apoptotic cell death via activating caspase cascade and regulating cell cycle progression.In conclusion,FosPegr-PDT is an effective measure to combat human colon cancer cells.

OBjEcTIVE: complete hydatidiform molar pregnancies occur in approximately 1 of 1,000 conceptions. After uterine evacuation of the trophoblastic tissue, women are followed up with serial serum human chorionic gonadotropin (hcG) measurement s. Patients are considered to have attained remission when their hcG level spont aneously declines to an undetectable level and remains there during a 6-month f ollow-up period. This standard effectively detects all disease recurrence; howe ver, it is resource intensive, delays child bearing, and is subject to significa nt noncompliance. Our objective was to determine the risk of disease recurrence after hcG spontaneously declines to undetectable levels. METHODS: We used a data base from the New England Trophoblastic Disease center to analyzehcG levels in p atients with complete molar pregnancies. RESULTS: Among 1,029 women with complet e molar pregnancy and complete data, 15%developed persistent gestational tropho blastic neoplasia. The rate of persistent neoplasm among those whose hcG level f ell spontaneously to undetectable levels was 0.2%(2/876, 95%confidence interva l 0-0.8%). No women developed persistent gestational trophoblastic neoplasia a fter their hcG level fell to undetectable levels using an assay with a sensitivi ty of 5 mIU/mL (n = 82, 95%confidence interval 0-4.5%). cONcLUSION: Based on our experience with women with complete hydatidiform molar pregnancies whose hcG val ues spontaneously fell to undetectable levels after molar evacuation, we conclud e that the risk of recurrent neoplasm after hcG levels fall to less than 5 mIU/m L approaches zero.

Background. central nervous system (cNS) metastases from an ovarian malignancy are uncommon. The long-term prognosis for these patients is poor, with studies reporting a mean survival of less than 12 months. cases. We present three ovarian cancer patients who developed metastatic disease to the brain. All patients were heavily pre-treated prior to the development of cNS disease. Following detection of cNS disease, they all were treated with multi-modality therapy including gammaknife radiosurgery (GKRS). At this time, one patient is alive at 26 months following treatment with GKRS. The second and third patients survived for 88 and 22 months respectively, before succumbing to their disease. conclusion. Local control of ovarian cancer metastatic to the brain can be achieved in some patients with GKRS. Additional investigation into GKRS is warranted.

Twelve cycles of single-agent paclitaxel have been demonstrated to prolong pr ogression-free survival in women with advanced ovarian cancer whom achieved a c linical complete response to a primary platinum/paclitaxel chemotherapy reg-ime n. This trial was conducted to compare the toxicity and disease-free interval o f 3 cycles vs. 12 cycles of paclitaxel consolidation in patients treated with an intensive three-drug front-line regimen of carboplatin, paclitaxel, and gemci tabine. Following cytoreductive surgery, 26 ovarian cancer patients received pri mary chemotherapy with carboplatin (AUc = 5, day 1), paclitaxel (175 mg/m2 over 1 h, day 1), and gemcitabine (800 mg/m2, day 1 day 8), with treatment repeated e very 21 days ×6 cycles. The first 13 patients (group A) received three addition al cycles of paclitaxel (175 mg/m2 over 1 h every 21 days). The second set of 13 patients (group B) also received three cycles of paclitaxel (175 mg/m2 over 1 h every 21 days) and then received nine additional cycles of paclitaxel (135 mg/m2 over 1 h every 21 days) consolidation therapy. The change from 3 cycles to 12 cycles of consolidation therapy for group B was made following the published results of GOG 178. In group A, all 13 patients com pleted three courses of consolidation therapy. One patient experienced grade 3 n eutropenia and two patients exhibited both grade 4 neutropenia and thrombocytope nia. Grade ≥2 neuropathy developed in 3 patients (23%). In group B, 9 of the 1 3 patients whom were intended to receive 12 total cycles of paclitaxel consolida tion were able to complete the program. There was no grade 3-4 neutropenia or a nemia in this population, although 1 patient developed grade 3 thrombocytopenia. Grade ≥2 neuropathy developed in 7 patients (54%). Although not a randomized experience, median progression-free interval was 76 weeks for group B, and 47 w eeks for group A. A case of subretinal hemorrhage associated with cytomegalovirus (cMV) retinitis in a human immunodeficiency virus-positive woman is described. The patient was found to have an exudative retinal detachment involving the fovea, subretinal hemorrhage, cottonwool spots, and active cMVretinitis temporal to the exudative detachment. No evidence of systemic coagulopathy was found. Although unusual, subretinal hemorrhage may be observed in association with cMV retinitis in the absence of a systemic coagulopathy.

Headache is a common complaint in the emergency department (ED). In order to examine headache work-ups and diagnoses across the USA, we queried a representative sample of adult ED visits (the National Hospital Ambulatory Medical care Survey)for the years 1992-2001. Headache accounted for 2.1 million ED visits per year (2.2%of visits). Of the 14%of patients who underwent neuroimaging, 5.5%received a pathological diagnosis. Of the 2%of patients who underwent lumbar puncture, 11%received a pathological diagnosis. On multivariable analysis, a decreased rate of imaging was noted for patients without private insurance [odds ratio (OR) 0.61, confidence interval (cI) 0.44, 0.86] and for those presenting off-hours (OR 0.55, cI 0.39, 0.77). Patients over 50 were more likely to receive a pathological diagnosis (OR 3.3, cI 1.2, 9.3). In conclusion, clinicians should ensure that appropriate work-ups are performed regardless of presentation time or insurance status, and be vigilant in the evaluation of older patients.

china lies at the heart of the global center of freshwater crab diversity in tropical Asia,where the 2 most diverse families occur:Potamidae(505 species,95 genera)and Gecarcinucidae(344 species,59 genera).china stands out as the country with the highest species richness of freshwater crabs globally.Its fauna comprises 243 species in 37 genera and in 2 families,and species discovery is still progressing at a rapid pace.The vast majority of the species are distributed in southwest,south central and eastern china in the Oriental zoogeographical region.china also stands out as having a highly endemic freshwater crab fauna at the species level(96%)and at the genus level(78%).Although the recent International Union for the conservation of Nature(IUcN)red list conservation assessment found only 6 out of 228 species(2%)to be threatened(5 potamids and 1 gecarcinucid),the majority(more than 75%)of chinese species are regarded as data deficient,so the number of threatened species is likely to be a serious underestimate.Threats from increasing habitat destruction and pollution are a major concern due to the rapidly growing economy and massive developments taking place in china.There is therefore an urgent need for increased species exploration and for the development of a conservation strategy for china’s threatened(and potentially threatened)endemic freshwater crab species.
