Sodium and water perturbations in patients who had an acute stroke:clinical relevance and management strategies for the neurologist

Sodium and water perturbations,manifesting as hyponatraemia and hypernatraemia,are common in patients who had an acute stroke,and are associated with worse outcomes and increased *** non-stroke related causes of sodium and water perturbations in these patients include underlying comorbidities and concomitant ***,hospitalised patients who had an acute stroke may receive excessive intravenous hypotonic solutions,have poor fluid intake due to impaired neurocognition and consciousness,may develop sepsis or are administered drugs(eg,mannitol);factors that can further alter serum sodium *** and water perturbations can also be exacerbated by the development of endocrine consequences after an acute stroke,including secondary adrenal insufficiency,syndrome of inappropriate antidiuretic hormone secretion and diabetes ***,COVID-19 infection has been reported to increase the risk of development of sodium and water perturbations that may further worsen the outcomes of patients who had an acute *** there are currently no accepted consensus guidelines on the management of sodium and water perturbations in patients who had an acute stroke,we conducted a systematic review of the literature published in English and in peer-reviewed journals between January 2000 and December 2020,according to PRISMA guidelines,to assess on the current knowledge and clinical practices of this *** this review,we discuss the signs and symptoms of hyponatraemia and hypernatraemia,the pathogenesis of hyponatraemia and hypernatraemia,their clinical relevance,and we provide our recommendations for effective treatment strategies for the neurologist in the management of sodium and water perturbations in commonly encountered aetiologies of patients who had an acute stroke.

This research advocates for the construction of Climate Change Haven Communities across the Appalachian Region. The proposed development plan can be extended to the northern tier states across the US and also to the northern and mountainous regions of Europe and Asia. We present an analogy to the earlier climate change period of the Last Glacial Maximum/“Ice Age” in which these same northern regions of the planet were covered in ice sheets making them uninhabitable for most humans and many plant and animal species. In some significant ways, the Ice Age scenario can be a reverse-model for our current climate crisis. We also advocate strongly for the prevention of upscale real estate development projects in these same regions of the globe, as these will foreclose the possibility of safely sheltering the millions of persons who will be displaced by climate change over the next 5 to 10 years.

Estimation of the density of neural,glial,and endothelial lineage cells in the adult mouse dentate gyrus

The dentate gyrus subregion of the mammalian hippocampus is an adult neural stem cell niche and site of lifelong *** regarding the role of adult-born neuron synaptic integration in hippocampal circuit function are framed by robust estimations of adultborn versus pre/perinatally-born neuron *** contrast,the non-neurogenic functions of adult neural stem cells and their immediate progeny,such as secretion of bioactive growth factors and expression of extracellular matrix-modifying proteins,lack similar framing due to few estimates of their number versus other prominent secretory ***,we apply immunohistochemical methods to estimate cell density of neural stem/progenitor cells versus other major classes of glial and endothelial cell types that are potentially secretory in the dentate gyrus of adult *** the cell types quantified,we found that GFAP^(+)SOX2^(+)stellate astrocytes were the most numerous,followed by CD31^(+)endothelia,GFAP-SOX2^(+)intermediate progenitors,Olig2^(+)oligodendrocytes,Iba1+microglia,and GFAP^(+)SOX2^(+)radial glia-like neural stem *** did not observe any significant sex differences in density of any cell ***,neural stem/progenitor cells were present at a similar density as several cell types known to have potent functional roles via their *** findings may be useful for refining hypotheses regarding the contributions of these cell types to regulating hippocampal function and their potential therapeutic *** experimental protocols were approved by the Ohio State University Institutional Animal Care and Use Committee(protocol#2016A00000068)on July 14,2016.

Potato aphid Macrosiphum euphorbiae performance is determined by aphid genotype and not mycorrhizal fungi or water availability

Intra- and interspecific variation in plant and insect traits can alter the strength and direction of insect-plant interactions, with outcomes modified by soil biotic and abiotic conditions. We used the potato aphid （Macrosiphum euphorbiae Thomas） feeding on cultivated Solanum tuberosum and wild Solanurn berthaulti to study the impact of water availability and plant mutualistic arbuscular mycorrhizal （AM） fungi on aphid performance and susceptibility to a parasitoid wasp （Aphidius ervi Haliday）. Plants were grown under glass with live or sterile AM fungal spores and supplied with sufficient or reduced water supply. Plants were infested with 1 of 3 genotypes ofM. euphorbiae or maintained as aphidfree controls; aphid abundance was scored after 1 week, after which aphid susceptibility to A. ervi was assayed explanta. Solarium tuberosum accumulated c. 20% more dry mass than S. berthaultii, and root mass of S. berthaultii was smallest under reduced water supply in the presence of AM fungi. Aphid abundance was lowest on S. berthaultii and highest for genotype ＂2＂ aphids; genotype ＂1＂ aphid density was particularly reduced on S. berthaultii. Aphid genotype ＂1＂ exhibited low susceptibility to parasitism and was attacked less frequently than the other two more susceptible aphid genotypes. Neither AM fungi nor water availability affected insect performance. Our study suggests a fitness trade-offin M. euphorbiae between parasitism resistance and aphid performance on poor quality Solarium hosts that warrants further exploration, and indicates the importance of accounting for genotype identity in determining the outcome of multitrophic interactions.

Are we leveling the playing field？ Trends and disparities in sports participation among sexual minority youth in Canada

Purpose: Sports participation and physical fitness are widely beneficial for young people, yet activity levels among young people are *** growing popular media attention on the participation of sexual minority(e.g., lesbian, gay, and bisexual) youth in sports and various campaigns to improve the often homophobic climate of sports, there is limited evidence that sexual minority youth participate in sports. Our aim was to provide a current portrait of sports participation among 3 groups of sexual minority youth(e.g., lesbian, gay, and bisexual) in British Columbia, Canada, as well as to document population ***: Pooled population-level data from British Columbia, Canada(n = 99,373) were used to examine trends and disparities in sports participation among sexual minority and heterosexual youth. Age-adjusted logistic regression models were used to examine changes in participation over time and disparities in participation over time(1998—2013).Results: We found an overall decline in sports participation and physical activity(PA) for all youth. Sexual minority students were less likely to participate in formal sports(with a coach) and informal sports(without a coach) compared with their heterosexual peers. The disparity in participating in informal sports between heterosexual and sexual minority youth has narrowed over time for some sexual orientation groups, whereas the disparity in participating in formal sports has widened over time in some ***: This study provides a comprehensive examination of sports participation among sexual minority youth over the past 15 *** changing societal attitudes and laudable efforts to reduce homophobia in sports, results suggest that there are continued barriers to participation for sexual minority youth. Effect of sedative agent selection on morbidity, mortality and length of stay in patients with increase in intracranial pressure

BACKGROUND: To identify the effects of sedative agent selection on morbidity, mortality, and length of stay in patients with suspected increase in intracranial pressure. Recent trends and developments have resulted in changes to medications that were previously utilized as pharmacological adjuncts in the sedation and intubation of patients with suspected increases in intracranial pressure. Medications that were previously considered contraindicated are now being used with increasing regularity without demonstrated safety and effectiveness. The primary objective of this study is to evaluate and compare the use of Ketamine as an induction agent for patients with increased intracranial pressure. The secondary objective was to evaluate and compare the use of Etomidate, Midazolam, and Ketamine in patients with increased intracranial pressure. METHODS: We conducted a retrospective chart review of patients transported to our facility with evidence of intracranial hypertension that were intubated before trauma center arrival. Patients were identifi ed during a 22-month period from January 2014 to October 2015. Goals were to evaluate the impact of sedative agent selection on morbidity, mortality, and length of ***: During the review 148 patients were identifi ed as meeting inclusion criteria, 52 were excluded due to incomplete data. Of those the patients primarily received; Etomidate, Ketamine, and Midazolam. Patients in the Ketamine group were found to have a lower mortality rate after injury stratifi cation. CONCLUSION: Patients with intracranial hypertension should not be excluded from receiving Ketamine during intubation out of concern for worsening outcomes.

BACKGROUND Presence of liver metastatic disease in pancreatic ductal adenocarcinoma(PDAC),either synchronous or metachronous after pancreatic resection,is a terminal diagnosis that warrants management with palliative intent as per all international practice ***,there is an increasing interest on any potential value of surgical treatment of isolated oligometastatic disease in selected *** To present the published evidence on surgical management of PDAC liver metastases,synchronous and metachronous,and compare the outcomes of these treatments to the current standard of *** A systematic review was performed in line with the Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines to compare the outcomes of both synchronous and metachronous liver metastases resection to standard *** 356 studies were identified,31 studies underwent full-text review and of these 10 were suitable for *** synchronous resection of liver metastases was compared to standard care,most studies did not demonstrate a survival benefit with the exception of one study that utilised neoadjuvant ***,resection of metachronous disease appeared to confer a survival advantage when compared to treatment with chemotherapy *** A survival benefit may exist in resection of selected cases of metachronous liver oligometastatic PDAC disease,after disease biology has been tested with time and systemic *** survival benefit is less clear in synchronous cases;however an approach with neoadjuvant treatment and consideration of resection in some selected cases may confer some *** studies should focus on pathways for selection of cases that may benefit from an aggressive approach.

Medication beliefs predict medication adherence in ambulatory patients with decompensated cirrhosis

AIM To investigate the impact of medication beliefs, illness perceptions and quality of life on medication adherence in people with decompensated *** One hundred adults with decompensated cirrhosis completed a structured questionnaire when they attended for routine outpatient hepatology review. Measures of self-reported medication adherence(Morisky Medication Adherence Scale), beliefs surrounding medications(Beliefs about Medicines Questionnaire), perceptions of illness and medicines(Brief Illness Perception Questionnaire), and quality of life(Chronic Liver Disease Questionnaire) were examined. Clinical data were obtained via patient history and review of medical records. Least absolute shrinkage and selection operator and stepwise backwards regression techniques were used to construct the multivariable logistic regression model. Statistical significance was set at alpha = *** Medication adherence was " High " in 42 % o f participants, "Medium" in 37%, and "Low" in 21%. Compared to patients with "High" adherence, those with "Medium" or "Low" adherence were more likely to report difficulty affording their medications(P < 0.001), lower perception of treatment helpfulness(P = 0.003) and stronger medication concerns relative to medication necessity beliefs(P = 0.003). People with "Low" adherence also experienced greater symptom burden and poorer quality of life, including more frequent abdominal pain(P = 0.023), shortness of breath(P = 0.030), and emotional disturbances(P = 0.050). Multivariable analysis identified having stronger medication concerns relative to necessity beliefs(Necessity-Concerns Differential ≤ 5, OR = 3.66, 95%CI: 1.18-11.40) and more frequent shortness of breath(shortness of breath score ≤ 3, OR = 3.87,95%CI: 1.22-12.25) as independent predictors of "Low"*** The association between "Low" adherence and patients having strong concerns or doubting the necessity or helpfulness of their medications should be explor
