Alcohol consumption is a predominant etiological factor in the pathogenesis of chronic liver diseases, resulting in fatty liver, alcoholic hepatitis, fibrosis/cirrhosis, and hepatocellular carcinoma (HCC). Although the pathogenesis of alcoholic liver disease (ALD) involves complex and still unclear biological processes, the oxidative metabolites of ethanol such as acetaldehyde and reactive oxygen species (ROS) play a preeminent role in the clinical and pathological spectrum of ALD. Ethanol oxidative metabolism influences intracellular signaling pathways and deranges the transcriptional control of several genes, leading to fat accumulation, fibrogenesis and activation of innate and adaptive immunity. Acetaldehyde is known to be toxic to the liver and alters lipid homeostasis, decreasing peroxisome proliferator-activated receptors and increasing sterol regulatory element binding protein activity via an AMP-activated protein kinase (AMPK)-dependent mechanism. AMPK activation by ROS modulates autophagy, which has an important role in removing lipid droplets. Acetaldehyde and aldehydes generated from lipid peroxidation induce collagen synthesis by their ability to form protein adducts that activate transforming-growth-factor-β-dependent and independent profibrogenic pathways in activated hepatic stellate cells (HSCs). Furthermore, activation of innate and adaptive immunity in response to ethanol metabolism plays a key role in the development and progression of ALD. Acetaldehyde alters the intestinal barrier and promote lipopolysaccharide (LPS) translocation by disrupting tight and adherent junctions in human colonic mucosa. Acetaldehyde and LPS induce Kupffer cells to release ROS and proinflammatory cytokines and chemokines that contribute to neutrophils infiltration. Ulcerative colitis(UC), a chronic, relapsing, remitting disease of the colon and rectum, is characterized by inflammatory ulceration of the mucosa. Current UC therapy relies on controlling acute episodes and preventing relapse. To predict modifications in the natural course of UC, mucosal healing(MH) has emerged as a major treatment goal. Endoscopic evaluation is considered the gold standard for assessing MH, which can be achieved by conventional drugs and biologics in many, but not all, patients. Consequently, interest is focusing on the development of new substances for UC therapy, and new oral agents are in the pipeline. This review will focus on the ability of newly developed oral drugs to induce and maintain MH in UC patients.

Cat scratch disease(CSD),an infection caused by Bartonella henselae,is uncommon in Europe,whereas 24,000 cases per year have been reported in the United States[1,2].The typical manifestation of CSD consists of fever and subacute,solitary or regional,painful and self-limited lymphadenopathy in a patient with a history of previous scratching,biting,or contact with a kitten or cat[3].In 85%of patients,only one regional lymph node is involved,which is located in the axillary and epitrochlear regions in approximately half of the cases[4,5].One or more round,red-brown nontender papules can develop at the inoculation site or near the scratch after a few days and may persist until two to three weeks[6].

Frailty is an issue of paramount importance for cardiologists,because of the aging of patients admitted to hospital for acute coronary syndrome(ACS)and the straight relationship between aging and *** tools have been provided in this setting,in order to objectively assess frailty status,but important questions are still *** are conflicting data about a unique definition of frailty in subjects with cardiovascular diseases,the timing to perform a frailty evaluation in the context of an acute myocardial infarction,the mean to assess frailty in these patients and the usefulness of the information derived from the frailty *** results from the analysis of several items and a multidomain evaluation including laboratory values,clinical data and physical performance assessment is required for a comprehensive frailty ***,regardless of the frailty tool,the prevalence of frailty in older ACS patients is high and it could add important information to the decision-making process about invasive strategy,the multivessel disease management,dual antiplatelet therapy and secondary prevention *** present overview tries to summarize the current knowledge about the definition and prevalence of frailty in older adults admitted to hospital for ACS,suggesting how frailty assessment may improve the management of older ACS patients.

Dogs engage in play behavior at every age and the play bow is their most iconic playful ***,the function of this posture is still under ***,we selected the Czechoslovakian Wolfdog(CWD)as a model breed to clarify the function of the play *** analyzed frame-by-frame 118 sessions of 24 subjects and recorded 76 play bow *** found that all the play bows were performed in the visual field of the playmate suggesting that the sender takes into account the attentional state of the receiver when releasing the *** drawing survival curves and using log-rank test we found that play bow was mainly performed during a short pause in an ongoing session and that its performance triggered the playmate’s reaction *** findings show that play bow functions in restoring the partner motivation to ***,by using a sequential analysis and a generalized mixed model,we found no evidence supporting the metacommunicative function of the play *** signal did not necessarily precede a contact offensive behavior(e.g.,play biting and play pushing)and it was not affected by the level of asymmetry of the play *** conclusion,in CWDs play bow can be considered a visual signal useful to maintain the motivation to play in the ***,we suggest that the mismatched number of play bows emitted by the 2 players in a given session can be predictive of their different motivations to play.

In this note, we consider a Fremond model of shape memory alloys. Let us imagine a piece of a shape memory alloy which is fixed on one part of its boundary, and assume that forcing terms, e.g., heat sources and external stress on the remaining part of its boundary, converge to some time-independent functions, in appropriate senses, as time goes to infinity. Under the above assumption, we shall discuss the asymptotic stability for the dynamical system from the viewpoint of the global attractor. More precisely, we generalize the paper dealing with the one-dimensional case. First, we show the existence of the global attractor for the limiting autonomous dynamical system; then we characterize the asymptotic stability for the non-autonomous case by the limiting global attractor.

Treatment of active steroid-refractory inflammatory bowel diseases with granulocytapheresis: Our experience with a prospective study

AIM： To report our experience with the use of granulocytapheresis （GCAP） in 14 patients with active steroid-refractory inflammatory bowel disease （IBD） in order to evaluate its efficacy in achieving remission and maintaining a long lasting symptom-free period. METHODS： The activity of the disease was evaluated by clinical activity index （CAI） and endoscopic index （EI） in ulcerative colitis （UC）, while by Crohn＇s disease activity index （CDAI） in Crohn＇s disease （CD）. The patients were treated using the AdacolumnTM system, an adsorption column which selectively binds to granulocytes and monocytes. One session/week of GCAP was performed for 5 wk. Steroids were stopped during apheresis. RESULTS： All the patients completed the five-week course showing no complications. At the end of the last session, 93% of patients showed a clinical remission of the disease that persisted for 6 too. Nine months after the end of the treatment, 60% of the cases maintained remission, while 23% of the patients were still in clinical remission after 12 too. CONCLUSION： Even if the number of our patients with steroid-refractory IBDs was not big, we can assert that GCAP is well tolerated and effective, especially in the first six months after the treatment, in a significant percentage of cases. The rate of sustained response drops slightly after 6 mo and significantly after 12 too, however the absence of severe side effects can be a stimulus for further evaluating new schedules of treatment.

AIM: To compare the endotics system (ES), a set of new medical equipment for diagnostic colonoscopy, with video-colonoscopy in the detection of polyps. METHODS: Patients with clinical or familial risk of colonic polyps/carcinomas were eligible for this study. After a standard colonic cleaning, detection of polyps by the ES and by video-colonoscopy was performed in each patient on the same day. In each single patient, the assessment of the presence of polyps was performed by two independent endoscopists, who were randomly assigned to evaluate, in a blind fashion, the presence of polyps either by ES or by standard colonoscopy. The frequency of successful procedures (i.e. reaching to the cecum), the time for endoscopy, and the need for sedation were recorded. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of the ES were also calculated. RESULTS: A total of 71 patients (40 men, mean age51.9 ± 12.0 years) were enrolled. The cecum was reached in 81.6% of ES examinations and in 94.3% of colonoscopies (P = 0.03). The average time of endoscopy was 45.1 ± 18.5 and 23.7 ± 7.2 min for the ES and traditional colonoscopy, respectively (P < 0.0001). No patient required sedation during ES examination, compared with 19.7% of patients undergoing colonoscopy (P < 0.0001). The sensitivity and specificity of ES for detecting polyps were 93.3% (95% CI: 68-98) and 100% (95% CI: 76.8-100), respectively. PPV was 100% (95% CI: 76.8-100) and NPV was 97.7% (95% CI: 88-99.9). CONCLUSION: The ES allows the visualization of the entire colonic mucosa in most patients, with good sensitivity/specificity for the detection of lesions and without requiring sedation.
