National preparedness survey of pediatric intensive care units with simulation centers during the coronavirus pandemic

The coronavirus disease pandemic caught many pediatric hospitals unpreparedand has forced pediatric healthcare systems to scramble as they examine and planfor the optimal allocation of medical resources for the highest priority *** is limited data describing pediatric intensive care unit (PICU) preparednessand their health worker *** describe the current coronavirus disease 2019 (COVID-19) preparedness effortsamong a set of PICUs within a simulation-based network *** A cross-sectional multi-center national survey of PICU medical director(s) fromchildren’s hospitals across the United States. The questionnaire was developedand reviewed by physicians with expertise in pediatric critical care, disasterreadiness, human factors, and survey development. Thirty-five children’shospitals were identified for recruitment through a long-established nationalresearch network. The questions focused on six themes: (1) PICU and medicaldirector demographics;(2) Pediatric patient flow during the pandemic;(3)Changes to the staffing models related to the pandemic;(4) Use of personalprotective equipment (PPE);(5) Changes in clinical practice and innovations;and(6) Current modalities of training including *** report on survey responses from 22 of 35 PICUs (63%). The majority of PICUswere located within children’s hospitals (87%). All PICUs cared for pediatricpatients with COVID-19 at the time of the survey. The majority of PICUs (83.4%)witnessed decreases in non-COVID-19 patients, 43% had COVID-19 dedicatedunits, and 74.6% pivoted to accept adult COVID-19 patients. All PICUsimplemented changes to their staffing models with the most common changesbeing changes in COVID-19 patient room assignment in 50% of surveyed PICUsand introducing remote patient monitoring in 36% of the PICU units. Multidrug resistance(MDR) remains a major clinical obstacle to successful cancer *** diverse mechanisms of MDR have been well elucidated, such as dysregulation of drugs transporters, defects of apoptosis and autophagy machinery, alterations of drug metabolism and drug targets, disrupti on of redox homeostasis, the exact mechanisms of MDR in a specific cancer patient and the cross-talk among these different mechanisms and how they are regulated are poorly *** RNAs(mi RNAs) are a new class of small noncoding RNAs that could control the global activity of the cell by post-transcriptionally regulating a large variety of target genes and proteins *** evidence shows that mi RNAs play a key regulatory role in MDR through modulating various drug resistant mechanisms mentioned above, thereby holding much promise for developing novel and more effective individualized therapies for cancer treatment. This review summarizes the various MDR mechanisms and mainly focuses on the role of mi RNAs in regulating MDR in cancer treatment.

Preliminary assessment of nasopharyngeal carcinoma incidence in the Philippines: a second look at published data from four centers

In endemic regions such as southern China and Southeast Asia, the annual incidence of nasopharyngeal carcinoma(NPC) ranges from 3 to 30 per 100,000. In the Philippines, the estimated incidence in 2010 was 1.2 per 100,000. However, this rate is based on data collected from registries covering only two regions in the country. Here, we report the findings from our study to better approximate the incidence of NPC in the Philippines. Between September 1, 2011 and August 31, 2012, data were collected from 49 patients from 4 different institutions—University of Santo Tomas, Makati Medical Center, Philippine Oncology Center Corporation, and Cardinal Santos Memorial Medical Center—using a NPC screening questionnaire. Crude incidence was 0.09 per 100,000. Age-standardized incidences using Segi and WHO standards were 2.08 and 1.79 per 100,000, respectively. Of the 49 patients, 31 were males and 18 were females, and 71% of patients were between 30 and 59 years old. WHO types II and III represented 22% and 78% of the subjects, respectively, and 75.5% of cases were locally advanced(stages III–IVB). Although the age-standardized incidence from the 4 institutions was numerically higher than the published age-standardized incidence(2.07 per 100,000 vs. 1.2 per 100,000), two-proportion z-test showed no significant difference between them(P = 0.68). A more concerted effort is needed for a better approximation of the country's NPC disease burden.

A new numerical technique for testing and evaluation of quenching media and quenching systems is outlined. The measured time-temperature samples as a result of cooling curve test are analyzed by the new software developed, in order to characterize quantitatively the quenchants. The method applied is based on Fourier analysis. Examples for evaluation and comparison of cooling performance of quenchants are presented the applicability of the computational technique.

Are gender inclusiveness and rural transformation interlinked?The case of Bangladesh

Gender inclusiveness is important in the rural transformation process of Bangladesh as rural women play key roles in both agricultural and non-agricultural *** inclusiveness also leads to gender *** empirically evaluate relationships between rural transformation and gender inclusiveness in *** consider three rural transformation indicators:high-value commodities’share in agricultural output values,non-farm employment’s share in rural labor employment,and non-agricultural GDP’s share in total *** capturing gender inclusiveness include the per capita rural income of males and females,ratio of investments into gender programs,female access to education,access to healthcare,employment participation,land ownership,and asset *** test the effect of the difference in per capita rural income of males and females and the ratio of their incomes and check for the robustness of the gender variables across different model *** 128 district-level observations from 32 districts of Bangladesh across four time periods(2000,2005,2010 and 2016),we use both ordinary least squares and fixed effects panel regression *** find that female land and asset ownership and access to education and healthcare are robust determinants of various stages of rural ***,our results suggest that improving women’s ownership of land and assets and investing in women’s education and healthcare will likely contribute to a more inclusive rural transformation.

Rural transformation can improve poverty reduction,living standards,and health outcomes in developing ***,impacts associated with rural transformation vary by region,household,and individual trait(including gender).While research on rural transformation has been increasing over the last decade,there has been no comprehensive review conducted on the relationships between gender and rural ***,we conduct a systematic literature review to investigate the impacts of rural transformation on gender and the influence of gender inclusiveness on rural *** reviewed 82 studies from 1960-2021 that explore the relationships between rural transformation and *** then developed a framework that captures incidences and flow directions between *** show that most studies examined the impacts of rural transformation on women and between gender *** investigated the role of women and the influence of gender inclusiveness on rural ***,studies showed that rural transformation typically leads to positive outcomes for women regarding employment,income,and ***,negative impacts on women’s control over income,stability of new income sources,and access to healthy food are also *** future development policies and programs to explicitly account for gender inclusiveness can lead to more successful rural transformation.

Histologic features and genomic alterations of primary colorectal adenocarcinoma predict growth patterns of liver metastasis

BACKGROUND Different histological growth patterns(HGPs)of colorectal carcinoma(CRC)liver metastasis are associated with patients’prognosis and response to antiangiogenic ***,the relationship between HGPs of liver metastasis and clinicopathological and genomic characteristics of primary cancer has not been well *** To assess whether certain clinicopathological and genomic features of primary CRC could predict the HGPs of liver *** A total of 29 patients with paired resections of both primary CRC and liver metastasis were divided into two groups:A(15 cases with desmoplastic liver metastasis)and B(14 cases with replacement liver metastasis).Clinical information was obtained from patients’*** repair proteins,BRAFV600E,and PD-L1 were evaluated by *** cases were selected randomly from each group for whole exome sequencing(WES)*** In the primary tumor,expanding growth pattern,low tumor budding score(TBS),and Crohn’s disease-like response(CDR)were associated with desmoplastic liver metastasis and better overall survival,whereas infiltrating growth pattern alone of primary carcinoma could predict the replacement liver metastasis and worse overall survival(P<0.05).On WES analysis,primary carcinoma with desmoplastic liver metastasis showed mutations in APC(4/5);TP53(3/5);KRAS,PIK3CA,and FAT4(2/5);BRCA-1,BRCA2,BRAF,and DNAH5(1/5),whereas primary carcinoma with replacement liver metastasis showed mutations in APC and TP53(3/5);KRAS,FAT4,DNH5,SMAD,ERBB2,ERBB3,LRP1,and SDK1(1/5).CONCLUSION The HGPs,TBS,and CDR of primary CRC as well as the presence of specific genetic mutations such as those in PIK3CA could be used to predict the HGPs of liver metastasis,response to therapy,and patients’prognosis.

Te content of the rectifed motor evoked potential(MEP)induced by transcranial magnetic stimulation(TMS)has ambiguously been assessed without the precision that energy calculation *** fact has misled data interpretation and misguided biomedical *** defnitively fll the gap that exits in the neurophysics processing of these signals,we computed,in Walls(̂�),the bioenergy within the rectifed MEP recorded from the human frst digitorum index(FDI)muscle at rest and under isometric *** also gauged the biowork exerted by this *** we show that bioenergy and biowork can accurately and successfully be assessed,validated,and determined in̂�from MEP signals induced by TMS,regardless of knowing the mathematical expression of the function of the *** novel neurophysics approach represents a dramatic paradigm shif in analysis and interpretation of the content of the MEP and will give a true meaning to the content of rectifed ***,this innovative approach allowed unveiling that women exerted more bioenergy than men at the magnetic stimulations used in this *** of conclusions drawn from studies published elsewhere assessing rectifed EMG signals that have used ambiguous units is strongly recommended.

对AISI 5 16 0H弹簧钢圆柱试样在不同温度的 2 5 %水溶性聚 (亚烷基 )二醇UCONHT淬火液和Arco5 2 1淬火油中淬火产生的残余应力及畸变进行了对比性研究。试棒的直径分2 0 5 6mm (0 81inches)和13 4 5mm (0 5 3inches)两种。先用INC PHATRAN代码计算出试样随时间变化的传热系数 ,并用ABAQUS/标准有限元软件估算热处理过程中的残余应力和畸变。在试样中心放置热电偶 ,测出冷却曲线 ,根据所测的冷却曲线 ,用INC PHATRAN解决了伴随相变时的逆向热传导问题。试验了这两种淬火介质 ,力图将热处理畸变和开裂减少到最少 ,并得出了颇有价值的结论。
