Climate change impacts on eutrophication in the Po River(Italy):Temperature-mediated reduction in nitrogen export but no effect on phosphorus

Rivers worldwide are under stress from eutrophication and nitrate pollution,but the ecological consequences overlap with climate change,and the resulting interactions may be unexpected and still unexplored.The Po River basin(northern Italy)is one of the most agriculturally productive and densely populated areas in Europe.It remains unclear whether the climate change impacts on the thermal and hydrological regimes are already affecting nutrient dynamics and transport to coastal areas.The present work addresses the long-term trends(1992-2020)of nitrogen and phosphorus export by investigating both the annual magnitude and the seasonal patterns and their relationship with water temperature and discharge trajectories.Despite the constant diffuse and point sources in the basin,a marked decrease(-20%)in nitrogen export,mostly as nitrate,was recorded in the last decade compared to the 1990 s,while no significant downward trend was observed for phosphorus.The water temperature of the Po River has warmed,with the most pronounced signals in summer(+0.13℃/year)and autumn(+0.16℃/year),together with the strongest increase in the number of warm days(+70%-80%).An extended seasonal window of warm temperatures and the persistence of low flow periods are likely to create favorable conditions for permanent nitrate removal via denitrification,resulting in a lower delivery of reactive nitrogen to the sea.The present results show that climate change-driven warming may enhance nitrogen processing by increasing respiratory river metabolism,thereby reducing export from spring to early autumn,when the risk of eutrophication in coastal zones is higher.

Ensuring a sufficient oxygen supply is pivotal for the success of bioprinting applications since it fosters tissue integration and natural regeneration.Variation in oxygen concentration among diverse tissues necessitates the precise recreation of tissue-specific oxygen levels in imprinted constructs to support the survival of targeted cells.Although oxygen-releasing biomaterials,such as oxygen-generating microparticles(OMPs),have shown promise for enhancing the oxygen supply of microenvironments in injured tissues,whether this approach is scalable for large tissues and whether tissue-specific bioinks with varying OMP concentrations remain printable remain unknown.This study addresses this critical gap by introducing an innovative class of engineered oxygenated bioinks that combine colloidal-based microgels with OMPs.We report that incorporating nanosized calcium peroxide(nCaO_(2))and manganese oxide nanosheets(nMnO_(2))into hydrophobic polymeric microparticles enables precise modulation of oxygen release while controlling hydrogen peroxide release.Moreover,the fabrication of oxygenating and cytocompatible colloidal gels is achieved using an aqueous two-phase system.This study thoroughly evaluates the fundamental characteristics of the resulting bioink,including its rheological behaviors,printability,shape fidelity,mechanical properties,and oxygen release properties.Moreover,this study demonstrates the macroscopic scalability and cytocompatibility of printed constructs produced via cell-laden oxygenating colloidal bioinks.By showcasing the effectiveness of extrusion-based bioprinting,this study underscores how it can be used to fabricate biomimetic tissues,indicating its potential for new applications.The findings presented here advance the bioprinting field by achieving scalability with both high cell viability and the possibility of mimicking specifically oxygenated tissues.This work thereby offers a promising avenue for the development of functional tissues with enhanced physiological relevance.

Balanced propofol sedation administered by nonanesthesiologists:The first Italian experience

AIM:To assess the efficacy and safety of a balanced approach using midazolam in combination with propofol,administered by nonanesthesiologists,in a large series of diagnostic colonoscopies.METHODS:Consecutive patients undergoing diagnostic colonoscopy were sedated with a single dose of midazolam(0.05 mg/kg)and lowdose propofol(starter bolus of 0.5 mg/kg and repeated boluses of 10 to 20 mg).Induction time and deepest level of sedation,adverse and serious adverse events,as well as recovery times,were prospectively assessed.Cecal intubation and adenoma detection rates were also collected.RESULTS:Overall,1593 eligible patients were included.The median dose of propofol administered was 70 mg(range:40120 mg),and the median dose of midazolam was 2.3 mg(range:24 mg).Median induction time of sedation was 3 min(range:14 min),and median recovery time was 23 min(range:1040 min).A moderate level of sedation was achieved in 1561(98%) patients,whilst a deep sedation occurred in 32(2%) cases.Transient oxygen desaturation requiring further oxygen supplementation occurred in 8(0.46%;95% CI:0.2%0.8%)patients.No serious adverse event was observed.Cecal intubation and adenoma detection rates were 93.5%and 23.4%(27.8%for male and 18.5%for female,subjects),respectively.CONCLUSION:A balanced sedation protocol provided a minimalization of the dose of propofol needed to target a moderate sedation for colonoscopy,resulting in a high safety profile for nonanesthesiologist propofol sedation.

Conservative management of an early abdominal complication during a Tullio percutaneous endoscopic excision of renal pelvis urothelial carcinoma:A case report

BACKGROUND Upper tract urothelial carcinoma(UTUC)representing only a small fraction of all urothelial tumors.It predominantly affects the renal pelvis in men,often coexisting with bladder carcinoma.UTUC displays a more aggressive genetic profile compared to bladder neoplasms,with the majority of patients presenting with advanced disease.Histologically,about a quarter of UTUC cases exhibit high-grade subtypes,associated with a worse prognosis.Tobacco use and past exposure to aromatic amines are significant risk factors for UTUC.Imaging modalities such as Uro-computed tomography and magnetic resonance imaging play crucial roles in diagnosis.Ureterorenoscopy is vital for direct visualization and biopsy sampling,but its limited sampling capacity presents challenges in determining tumor depth and staging.Traditionally,nephroureterectomy with bladder cuff excision has been the gold-standard treatment for UTUC.Endoscopic conservative treatment has recently emerged as a viable option for selected patients,offering comparable oncological outcomes to radical surgery.Percutaneous access is also feasible for larger intrarenal tumors.CASE SUMMARY We present a case of an 84-year-old woman who underwent percutaneous endoscopic excision of a renal pelvis neoplasm using the Tullio laser.Despite successful tumor removal,the patient experienced a postoperative complication with abdominal fluid leakage.Conservative management effectively resolved the complication.Given the patient's age and refusal for radical surgery,the conservative approach proved to be a valid therapeutic choice.CONCLUSION Overall,UTUC remains a diagnostic and therapeutic challenge due to its rarity.However,advances in endoscopic and percutaneous techniques provide valuable alternatives for selected patients,warranting further exploration in this evolving field.

Optimized design and integration of energy storage in Solar-Assisted Ground-Source Heat Pump systems

The integrated use of multiple renewable energy sources to increase the efficiency of heat pump systems,such as in Solar Assisted Geothermal Heat Pumps(SAGHP),may lead to significant benefits in terms of increased efficiency and overall system performance especially in extreme climate contexts,but requires careful integrated optimization of the different system components.In particular,thermal storages take a fundamental role in optimizing the integration of renewable energy sources and the system operation.This work investigates the potential design optimization of a SAGHP system in a mountain site by exploring many different alternatives to optimize the mutual relationship between the solar field,the geothermal field and the water thermal storages.This is done through an original simulation-based multi-objective optimization framework considering energy efficiency and economic feasibility,which allows appraising the impact of the different design alternatives on the overall system performance and on the dynamics of the different system components.Results identify a set of optimized system configurations that optimize the integrated exploitation of the different thermal sources showing a potential increase of the overall system performance leading to 34%lower global cost compared to the initial design.High robustness of the optimal design solutions is reported with respect to the current context of high economic uncertainty.

Chronic pain is sustained by a phenomenon of hyper-activation of nociceptive neurons both at peripheral(peripheral sensitization)and central(central sensitization)levels.The onset and maintenance of pain,however,is to be found in the interaction among the various cell populations in the nervous tissue including neurons and glia(Nam et al.,2016).The pathogenesis of neuropathic pain is extremely complex depending on the primary cause of nerve damage,e.***.traumatic nerve injury is associated with a robust inflammatory response while chemotherapy-induced pain is characterized by a modest phlogistic component(Di Cesare Mannelli et al.,2013).Despite these differences,a powerful common concept to explain the cellular mechanisms underlying pain is the activation of glia.

Social skills and psychopathology are associated with autonomic function in children:a cross-sectional observational study

In recent years,the increase of psychopathological disorders in the population has become a health emergency,leading to a great effort to understand psychological vulnerability mechanisms.In this scenario,the role of the autonomic nervous system(ANS)has become increasingly important.This study investigated the association between ANS,social skills,and psychopathological functioning in children.As an ANS status proxy,we measured heart rate variability(HRV).Infants admitted to the neonatal intensive care unit of the University Hospital of Padova because of preterm birth or neonatal hypoxic-ischemic encephalopathy were sequentially recruited from January 2011 to June 2013 and followed long-term up to school age in this cross-sectional observational study.We recorded 5 minutes of HRV immediately before measuring performance in social abilities tasks(affect recognition and theory of mind,NEPSY-II)in 50 children(mean age 7.4±1.4 years)with and without risk factors for developing neuropsychiatric disorders due to pre-/perinatal insults without major sequelae.Children also completed extensive cognitive,neuropsychological,and psychosocial assessment.Parents were assessed with psychopathological interviews and a questionnaire(CBCL 6-18).Analysis in a robust Bayesian framework was used to unearth dependencies between HRV,social skills,and psychopathological functioning.Social task scores were associated with HRV components,with high frequency the most consistent.HRV bands were also associated with the psychopathological questionnaire.Only normalized HRV high frequency was able to distinguish impaired children in the affect recognition task.Our data suggest that ANS may be implicated in social cognition both in typical and atypical developmental conditions and that HRV has cross-disease sensitivity.We suggest that HRV parameters may reflect a neurobiological vulnerability to psychopathology.The study was approved by the Ethics Committee of the University Hospital of Padova(Comitato Etico per la Sperimentazione,Azienda Opedal

Which are the most favourable conditions for reducing soil CO_(2)emissions with no-tillage?Results from a meta-analysis

No-tillage practices have a recognised beneficial impact on soil and water conservation while reducing erosion processes and enhancing soil organic matter content.However,scientists continue to debate the effectiveness of no-tillage in reducing soil carbon dioxide(CO_(2))emissions from farming.Following the same line of inquiry pursued by the authors who reviewed the impact of conservative practices on direct CO_(2)emissions,we applied meta-analytic and machine learning techniques to unravel the effect of no-tillage under contrasting pedo-environmental conditions and agricultural management.We analysed fifty-six experimental studies investigating direct soil CO_(2)emissions from no-tillage and conventional tillage practices(102 paired observations),considering pedological(soil texture,soil organic carbon content),environmental(climate type),and management(crop rotation,experiment duration)factors.We estimated the effect of different practices on the daily amount of soil CO_(2)emissions,and the impact of tillage in the period immediately following the event.The main insights of this study are:(i)the conditions leading to the highest reduction of CO_(2)emissions due to no-tillage were long-term experiments(standardised mean differenceβ=0.64)conducted in arid environments(β=0.76)and clay soils(β=0.81),with low organic carbon content(β=0.79)where crop rotations(β=0.65)were performed;(ii)the same conditions were associated with the lowest absolute CO_(2)emissions,irrespective of soil management;(iii)the highest contribution to the variability of absolute soil CO_(2)emissions was associated with soil texture(mean decrease in accuracy of Random Forest models,MDA=4.57),rotation(MDA=3.07),experiment duration(MDA=2.93)and soil organic carbon content(MDA=2.24),rather than to tillage practices;(iv)soil CO_(2)emissions almost doubled in the first day after a tillage event,consistently across studies(p=0.001).This meta-analysis offers quantitative figures on the impact of tillage practices on soil CO_(2)emissions and releases

Background Aicardi-Goutières syndrome(AGS)is a genetically determined disorder with a variable phenotype.Since the original description of AGS,advances in gene sequencing techniques have resulted in a significant broadening of the phenotypic spectrum associated with AGS genes,and new clinical pictures have emerged beyond the classic presentation.The aim of this review is to provide a comprehensive analysis of the clinical spectrum of AGS and report currently available treatments and new immunosuppressive strategies.Data sources Literature reviews and original research articles were collected from databases,including PubMed and Clini-calTrials.gov.Relevant articles about AGS were included.Results The involvement of the nervous system certainly represents the major cause of mortality and morbidity in AGS patients.However,other clinical manifestations,such as chilblains,hepatosplenomegaly,and hematological disturbances,may lead to the diagnosis and considerably impact the prognosis and overall quality of life of these patients.Therapeutic approaches of AGS are limited to interventions aimed at specific symptoms and the management of multiple comorbidities.However,advances in understanding the pathogenesis of AGS could open new and more effective therapies.Conclusions The over-activation of innate immunity due to upregulated interferon production plays a critical role in AGS,leading to multi-organ damage with the main involvement of the central nervous system.To date,there is no specific and effective treatment for AGS.New drugs specifically targeting the interferon pathway may bring new hope to AGS patients.

Slow degrading Mg-based materials induce tumor cell dormancy on an osteosarcoma-fibroblast coculture model

Osteosarcoma is one of the most common cancers in young adults and is commonly treated using surgery and chemotherapy.During the past years,these therapy approaches improved but failed to ameliorate the outcomes.Therefore,novel,targeted therapeutic approaches should be established to enhance treatment success while preserving patient’s quality of life.Recent studies suggest the application of degradable magnesium(Mg)alloys as orthopedic implants bearing a potential antitumor activity.Here,we examined the influence of Mg-based materials on an osteosarcoma-fibroblast coculture.Both,Mg and Mg-6Ag did not lead to tumor cell apoptosis at low degradation rates.Instead,the Mg-based materials induced cellular dormancy in the cancer cells indicated by a lower number of Ki-67 positive cancer cells and a higher p38 expression.This dormancy-like state could be reversed by reseeding on non-degrading glass slides but could not be provoked by inhibition of the protein kinase R-like endoplasmic reticulum kinase.By investigating the influence of the disjunct surface-near effects of the Mg degradation on cell proliferation,an increased pH was found to be a main initiator of Mg degradation-dependent tumor cell proliferation inhibition.
