Since the beginning of the pandemic caused by the new Coronavirus (SARS- CoV-2), critically ill patients care has been challenging. It is necessary to perform the best available practice and protect the health team, preserving human resources and rationalizing costs. Changes in tracheostomies institutional protocols are necessary according to each health care services reality. This is particular important in developing countries. The aim of this study is to establish a practical and sucint guideline to minimize controversies regarding the proper timing to perform elective tracheostomies in critically ill coronavirus infected patients.

The aim of this study is to apply the Nutrient Analysis Critical Control Point (NACCP) process to ensure that the highest nutrient levels in food can determine a beneficial effect on the health of the consumer. The NACCP process involves a sequence of analysis and controls that depart from raw material production to the evaluation of the effect of nutrition on health. It is articulated through the following points: 1) identification of nutrient level in the food;2) identification of critical control points (environmental, genetic data, chemical and physical data, production technology, distribution and administration);3) establishing critical limits that can impoverish and damage the nutrient;4) establishing measures to monitor;5) establishing corrective actions. We selected as biomarkers the total phenolic content (TPC) and total antioxidant capacity (TAC) of a genotyped Italian hazelnut cultivars (Corylus e avellana L.). We performed a clinical study evaluating: a) nutritional status;b) clinical-biochemical parameters;c) low density lipoprotein oxidation (LDL-ox);d) the expression level changes of oxidative stress pathway genes in the blood cell at baseline and after 40 g/die of hazelnut consumption. In this study, we found a significant lowering (p ≤ 0.005) of LDL oxidized proteins, in association with the consumption of 40 g/d of hazelnuts. Also, we found a significant variation (p ≤ 0.005) of gene expression of antioxidant and pro-oxidant genes, between the intake of dietary with and without hazelnuts. This results support the hypothesis that the NACCP process could be applied to obtain significant benefits in terms of primary prevention and for contributing to the amelioration of food management at the consumer level.

Background Asthma is a heterogeneous disease and its different phenotypes need to be better characterized from a biochemical-inflammatory standpoint. This study aimed to apply the metabolomic approach to exhaled breath condensate (breathomics) to discriminate different asthma phenotypes, with a particular focus on severe asthma in *** In this cross-sectional study, we recruited 42 asthmatic children (age, 8–17 years): 31 with nonsevere asthma (treated with inhaled steroids or not) and 11 with severe asthma. Fifteen healthy children were enrolled as controls. Children performed exhaled nitric oxide measurement, spirometry, exhaled breath condensate (EBC) collection. Condensate samples were analyzed using a metabolomic approach based on mass *** A robust Bidirectional-Orthogonal Projections to Latent Structures-Discriminant Analysis (O2PLS-DA) model was found for discriminating both between severe asthma cases and healthy controls (R2 = 0.93; Q2 = 0.75) and between severe asthma and nonsevere asthma (R2 = 0.84; Q2 = 0.47). The metabolomic data analysis leads to a robust model also when the 3 groups of children were considered altogether (K = 0.80), indicating that each group is characterized by a specific metabolomic profile. Compounds related to retinoic acid, adenosine and vitamin D (Human Metabolome Database) were relevant for the discrimination between *** The metabolomic profiling of EBC could clearly distinguish different biochemical-metabolic profiles in asthmatic children and enabled the severe asthma phenotype to be fully discriminated. 晚期外阴鳞状细胞癌的复发模式和无瘤存活率

To compare patterns of recurrence and disease-free survival (DFS) of node-po sitive and node-negative patients with advanced vulval squamous cell carcinoma (SCC). Fifty-five patients with FIGO stage III/IVA vulval SCC who had surgery a t the Queensland Centre for Gynaecological Cancer from 1989 to 1999 were include d. Patients were grouped as follows: Group A, pT3 N0; Group B, pT3 N1; Group C, pT4 N2. Treatment included surgery ±.postoperative radiotherapy. Multivariate C ox models were calculated to identify independent prognostic factors. After a me dian follow-up of 96 months, 25 patients (45.5%) experienced recurrence at the vulva (n = 2), pelvis (n = 8), or distant sites (n = 15)-. Recurrence in the p elvis and at distant sites was more likely for patients in groups B and C (P 0.0 03). At 5 years the probability of DFS was 66.6%, 35.3%, and 39.8%for patient s in groups A, B, and C, respectively (P 0.085)-. Patients with negative nodes (n = 15), one microscopic positive node (n = 11), and two or more positive nodes (n = 29) had a probability of DFS of 66.6%, 67.3%, and 26.1%at 5 years, resp ectively (P 0.005). Patients with ≥2 positive groin nodes are at risk for dista nt failure. The DFS of patients with negative groin nodes and those with only on e microscopic positive node is very similar. The prognosis of patients with ≥2 positive unilateral or bilateral groin nodes is similar. The current FIGO stagin g system inaccurately reflects prognosis for patients with advanced vulval cance r. Clinical trials are warranted to investigate the benefit of systemic treatmen t.
