High-resolution computed tomography findings in humoral primary immunodeficiencies and correlation with pulmonary function tests

AIM To compare high-resolution computed tomography(HRCT) findings between humoral primary immunodeficiencies(hPIDs) subtypes; to correlate these findings to pulmonary function tests(PFTs).METHODS We retrospectively identified 52 consecutive adult patients with hPIDs who underwent 64-row HRCT and PFTs at the time of diagnosis. On a per-patient basis, an experienced radiologist recorded airway abnormalities(bronchiectasis,airway wall thickening, mucus plugging, tree-in-bud, and air-trapping) and parenchymal-interstitial abnormalities(consolidations, ground-glass opacities,linear and/or irregular opacities, nodules, and bullae/cysts) found on *** chi-square test was performed to compare the prevalence of each abnormality among patients with different subtypes of hPIDs. Overall logistic regression analysis was performed to assess whether HRCT findings predicted obstructive and/or restrictive PFTs results(absent-to-mild vs moderate-tosevere).RESULTS Thirty-eight of the 52 patients with hPIDs showed common variable immunodeficiency disorders(CVID), while the remaining 14 had CVID-like conditions(i.e., 11 had isolated IgG subclass deficiencies and 3 had selective IgA deficiencies). The prevalence of most HRCT abnormalities was not significantly different between CVID and CVID-like patients(P > 0.05), except for linear and/or irregular opacities(prevalence of 31.6% in the CVID group and 0 in the CVID-like group; P = 0.0427). Airway wall thickening was the most frequent HRCT abnormality found in both CVID and CVID-like patients(71% of cases in both groups). The presence of tree-in-bud abnormalities was an independent predictor of moderate-to-severe obstructive defects at PFTs(Odds Ratio, OR, of 18.75, P < 0.05), while the presence of linear and/or irregular opacities was an independent predictor of restrictive defects at PFTs(OR = 13.00; P < 0.05).CONCLUSION CVID and CVID-like patients showed similar HRCT findings. Role of Disinfection in Food Industry

At present microbiological risk are still the most frequent in the food industry and the disinfection is one of the procedures that can be carried out in the general prevention of foodborne infectious diseases. In the industrial process of food transformation the cleaning and disinfection procedures of surfaces and machinery have to be considered an integrate system with food production,a technological aspect that surely affects the finished product quality and safety. First of all to carry out properly these operations it is necessary that the factory and the equipment have been designed and constructed with high standard of hygiene to reduce the risk of contamination and to facilitate the sanitation. The procedures involve several stages which generally include pre-washing with water,cleaning,real disinfection,and a final rinsing with water. In order to assure a right disinfections of surfaces areas and of processing equipment the choice of disinfectant takes on particular importance. Some of the most widely used disinfectants include the hypochlorous acid,the chlorine dioxide and the peroxides like peracetic acid; the latter is widely used in soft drink and mineral water industries. The efficiency of these chemical agents depends on various factors as pH,temperature and the presence of organic substances with which the disinfectants can react to give by-products which can reduce the activity and can present toxicity. The use of hypochlorite may led to the formation of carcinogenic substance such as trialomethanes (THM) while the use of chlorine dioxide may lead to the formation of chlorites and chlorates. The peracetic acid is a good alternative compounds and it doesn’t lead to the formation of carcinogenic by-products. We really appreciated Geramizadeh, " Bile spillage and incidental gall bladder adenocarcinoma ", and Sandstrom et al. ,"Bile spillage should be avoided in elective cholecystectomy " (1,2). Both papers have been recently published in HepatoBiliary Surgery and Nutrition, 2019 (1,2). Starting from an analysis of the recent paper by Horkoff et al. "Adverse outcomes after bile spillage in incidental gall bladder cancer " in Annals of Surgery April 2019, they introduced a comprehensive discussion about epidemiology, diagnosis, therapy, prognostic factors and outcomes of incidental gallbladder cancer (1-3). They analyzed the possible risks related to bile spillage after cholecystectomy, in particular (1,2).

We read with pleasure papers by Cai et al. and Jia et al. published in HepatoBiliary Surgery and Nutrition, 2018, Volume 7, Issue 2 and 4, respectively (1,2). Both papers introduced comprehensive up-to-date literature reviews on indications and patient selection, surgical instruments, techniques, and outcomes of laparoscopic liver resections (LLRs) (1,2). Papers mentioned final statements by 2008 Louisville Statement and 2014 Morioka Consensus Conference (1,2). They stressed LLR non-inferiority to open procedures, in both minor and major liver resections as far as oncological and intraoperative/perioperative outcomes are concerned: reduced blood loss, lower postoperative morbidity and shorter length of hospitalization (1,2). Such results were corroborated by recent meta-analyses (3,4). Furthermore, some previous limitations for LLR look like they have been recently overcome (1-5). For example, tumor size, patient's old age and high BMI are no longer limitations for LLR, as Southampton Consensus guidelines suggested (5). Likewise, cirrhosis has been suggested as a further indication of LLR in highly selected patients with hepatocellular carcinoma and where it was performed by experienced surgeons (1-5).

We really appreciated Lin et al.’s“The influence of ^(18)F-fluorodeoxyglucose positron emission tomography/computed tomography on the N-and M-staging and subsequent clinical management of intrahepatic cholangiocarcinoma”in HepatoBiliary Surgery and Nutrition(1).A 291-patient population affected by intrahepatic cholangiocarcinoma(ICC)was retrospectively analyzed with the aim of assessing the accuracy of preoperative staging and treatment reallocation by ^(18)F-fluorodeoxyglucose positron emission tomography/computed tomography(^(18)F-FDG PET/CT)(1).

Novel Applications for Fog Water Harvesting

In a scenario of climate changes and increasing stress upon available fresh water resources like rivers, lakes and aquifers, collecting fog water is a promising yet relatively unexplored potentiality. Providing sufficient water and reducing water extraction’s environmental impact at the same time can be a challenge with conventional ways, but fog harvesting technology presents itself as a powerful and efficient alternative. Water availability profoundly determined regional economic benefit, social relations and it also influenced environmental security and ecosystem services. Underdeveloped countries have been dealing with water scarcity issue for decades, but also wealthy countries will have to face the water crisis soon, due to unsustainable development processes. A review of the state of the art highlights the most relevant parameters to deal with when discussing about fog water harvesting. In regions with frequent fog events, this technology already proved to be a sustainable drinking water resource for rural communities and their low per capita water usage was provided by basic devices utilization. Nevertheless, in this paper, different fog water harvesting applications are investigated, besides the already existing fresh-water collection, reforestation and agricultural use. Further options, such as building components, outdoor activities and domestic devices are considered, according to different parameters, such as economic benefits, possibility of standardized production, life cycle and market attractiveness. A desirable novel concept would become relevant in specific contexts, thanks to multiple functions, offering locals designed and customized solutions. Also noteworthy are the landscape impact of such devices and the effects of the project in terms of places regeneration, raising awareness and “green” conscience creation. A Case of Sinonasal Extranodal NK/T-Cell Lymphoma with Indolent Behaviour and Low-Grade Morphology

Extranodal NK/T-cell lymphoma, nasal type (ENKL), is an uncommon subtype of EBV associated lymphoma usually characterized by aggressive clinical course. We report an atypical sinonasal ENKL case with long-lasting indolent behaviour, developed in the setting of a polymorphic EBV-associated lymphoproliferative disorder (LPD). A 52-year-old woman had suffered from chronic sinusitis and nasal obstruction since 2000, moderately worsened during the last years (marked enlargement of the sino-nasal mucosa at MRI in 2011) with elevated anti-VCA IgG and IgM titers. Three subsequent biopsies revealed slightly increasing morphophenotypic atypia, ranging from a polymorphic B- and T-cell EBV positive proliferation (diagnosed in 2011, but not fulfilling CAEBV diagnostic criteria) to an overt monomorphic mildly atypical T LPD without necrosis and angiocentricty diagnosed as ENKL in 2013 upon immunophenotype and TCR-γ gene clonal rearrangement. Clinically indolent ENKL with low-grade morphology is extremely rare and diagnostically challenging;while the few reports in the literature describe long-survival in ENKL treated patients comparing histologically neoplastic lesions at onset and recurrences, no reports are published on the slow progression from a polymorphic EBV-related T/NK proliferation to a histologically overt clinically indolent ENKL in an untreated patient who only received occasional steroid administration.

按需喷墨印刷技术正在成为装饰陶瓷的主要技术。大多数情况下这种技术使用在色釉中,通过传统陶瓷釉微粉化(0.2-0.6nm)来制备。虽然在理论上已经提出通过减小颜料的尺寸可以很大程度上改善他的光学性能,但是并不是期待中的所有优点都能出现,关于微粉化的染料的增强颜色并不能得到。这是第二部分的研究,目的在于公开在颜料的微细化处理过程中发生了什么;及总相组成和颜色上。基于这个目的,代表性的工业颜料选择了:Cr-Sb负载的金红石相(橘黄色)、Co-Cr-Fe-Mn-Ni尖晶石、V负载锆英石.这种颜料在小规模试验工厂中被微粒化,通过SEN、HR-TEM来表征微粒尺寸和形貌,通过XRD,DRS表征相组成、晶粒尺寸、晶胞参数,以及瓷砖快速烧到12001C时油料的颜色。结果突出了微粉化的行为:晶体结构和光学特征就会发生改变一旦颜料颗粒变化到微观尺寸。颗粒尺寸逐渐减小是伴随着晶粒尺寸的减小和晶格缺陷增加的频率产生的。无定形相的形成发生在关键的晶粒尺寸(30-40nm),它区分两种快慢速粉碎速率。这种结构的改变通过亚微米区域降低颜色强度而增加明度。

喷墨打印越来越多的用于装饰陶瓷瓷砖,大部分情况通过使用有颜色的油墨。这些油墨通过将传统的陶瓷颜料微粉化处理至平均直径为0.2-0.6微米的颗粒来获得,尽管这种尺寸的减小理论上对于颜料颗粒的光学和流体力学性能有很大的优势,但是对于颜色的强度和磨粉过程的效率却还存在着未解决的问题。当前的研究主要是论述微粉化如何影响颜料颗粒的尺寸、形状和比表面积。为此,工业颜料被选来表征其不同密度、硬度、断口和断裂韧性的晶体结构,即金红石相、尖晶石相和锆石相结构。颜料在试验车间进行微粉化处理,控制载体、固体含量、分散剂类型和浓度、旋转速度、研磨介质的量和尺寸、磨粉时间和温度。颜料的颗粒尺寸分布通过激光衍射和动态光散射进行表征,形貌通过扫描电子显微镜表征。结果表明:颜料在微粉化过程中具有不一样的表现,颗粒尺寸的变化和局部形状的变化都与文献一致。颜料的易磨性不同:锆石相>金红石相>尖晶石相。
