AIM: To study the efficacy of low-dose imipramine in relieving symptoms associated with the irritable bowel syndrome (IBS). METHODS: A randomized, double-blind trial of 25 mg imipramine vs matched placebo for 12 wk was performed. Doubling the dose was allowed once at week 2 in case of an unsatisfactory early response. Primary efficacy variables were subjective global symptom relief and quality of life (QoL) using SF-36 at week 12. RESULTS: One hundred and seven patients were enrolled by advertisement or referral by general practitioners and 56 (31 imipramine: 25 placebo) completed the 16-wk study. Baseline characteristics were comparable. A high overall dropout rate was noted in the imipramine and placebo arms (47.5% vs 47.9%, P > 0.05), a mean of 25.0 and 37.4 d from enrollment, respectively (P elief with imipramine over placebo (per-protocol: 80.6% vs48.0%, P = 0.01) and a trend on intent-to-treat (ITT) analysis (42.4% vs 25.0%, P = 0.06). This improvement was evident early and persisted to week 16 (P = 0.024 and 0.053 by per-protocol and ITT analyses, respectively). Mean cumulative and componentspecific SF-36 scores improved in the imipramine group only (per-protocol, P 0.05). CONCLUSION: Imipramine may be effective in the treatment of IBS patients and is associated with improved QoL. Careful patient selection, initiation of a low dose with gradual escalation and monitoring for side effects may result in an improved therapeutic response.

Particle size fractionation by sieving is used to optimize antioxydant potential of natural substances. The aim of the present study was to evaluate particle size effects on antioxidant and hepatoprotective potential of Eucalyptus camaldulensis essential oils (EO) on CCl4-induced hepatic damage in Wistar rats. Animals were daily orally treated with the EOs extracted by hydrodistillation from powder sieved at four particle sizes (≥355, 200 - 355, 100 - 200, ≤100 μm) and those of the unsieved powder at dose of 50 mg/Kg for 7 days. Compounds that are evaluated for these activities are hydrocarbons and oxygenated terpenes that were identified and quantified by GC/MS. Activities of enzymes markers of hepatocellular damage in serum and antioxidant enzymes in the liver homogenates were measured. In this research, EOs significantly prevented the increase in serum ALT and AST (p 4 and significant increase level of plasma HDL-cholesterol. Also, significantly (p < 0.05) decreased the extent of malondialdehyde (MDA) formation and elevated the activities of superoxide dismutase (SOD) and catalase (CAT) liver in comparison to negative control group. The best antioxidant and hepatoprotective activities were those of EOs from two fine powder fractions (≤100 μm and 100 - 200 μm) was correlated to their high concentration in oxygenated terpenes (70.9% and 46.4%, respectively), when compared to the large particles (200 - 355 μm and ≥355 μm, with 33.3% and 41.8%, respectively) and unsieved powder (37.4%).

Phelipanche aegyptiaca Management with Glyphosate in Potato

Two years field and greenhouse studies were carried out to evaluate the efficacy of sub-lethal doses of glyphosate (Round upR), ammonia gas, phosphoric acid and sulfuric acid against Phelipanche aegyptiaca in potato. Results showed that sequential application of sub-lethal doses of glyphosate at all tested rates significantly reduced P. aegyptiaca shoot number and shoot dry weight. While, the use of ammonia gas, phosphoric acid and sulfuric acid had no significant effect on the total level of P. aegyptiaca infection as compared to the control. The best results considering both P. aegyptiaca control and selectivity in potato were obtained by sequential application of sub-lethal doses of glyphosate at 60 and 80 g·ai·ha-1. Sequential application of glyphosate at 60 g·ai·ha-1 reduced P. aegyptiaca infection by 100% after 100 days after potato emergence (DAPE). Except for sequential application of glyphosate at 60 and 80 g·ai·ha-1, all tested rates enhanced the maturity rate of potato plants and decreased the number of marketable potato tubers.

Introduction: The addiction to games of chance and money is frequently encountered among some players. The general objective of this study was to describe the level of the gambling addiction among the players of the Togolese National Lottery (LONATO) in the city of Lomé. Method: This was a descriptive cross-sectional study through non-random sampling by convenience surveys, conducted on October 09, 2019 in 20 LONATO plying sites which affected 611 players. The screening was done using the South Oaks Gambling Screen (SOGS) questionnaire. Results: Of the 611 players surveyed, 55% were aged 18 to 35 years with an average age of 36 years and the extremes ranging from 14 to 78 years. The male gender was in the majority (84.26%). The prevalence of pathological gambling was 62.52%. Among the pathological gamblers, 16.89% had a family history of pathological gambling, 19.00% had a mood disorder, 28.50% had excessive alcoholism, and 17.15% consumed another psychoactive substance. Cannabis was the most consumed substance (6.87%) followed by tobacco (6.07%) and soporific (6.07%). More than half of players (62.03%) thought that pathological gambling is not a disease. Conclusion: The pathological gambling is a global phenomenon and a reality in Togo. Raising everyone’s awareness will reduce its negative impact on the players.

Ampullary carcinoma(AC) is a rare gastrointestinal tumor without clear treatment *** management of this tumor is usually extrapolated from the treatment of pancreatic,biliary duct and intestinal *** papers have studied the AC as an independent entity and yet succombs to several *** studies were retrospective single institutional experiences with limited sample sizes recruited over a long period of *** metastatic ACs where chemotherapy is the only recommended option,localized AC once excised may be approached by either chemotherapy alone or concomitant chemoradiation *** this review,we report the overall survival and recurrence factors of more than 1000 patients from all the studies treating exclusively *** also review the medical treatment of this tumor and conclude to the necessity of multi-institutional randomized controlled studies for AC exclusively.

Outpatient Management of Sickle Cell Disease: Assessment of 13 Years Follow-Up of Pediatric Patients at the Sylvanus Olympio University Hospital

Objective: To evaluate thirteen years of ambulatory follow-up of patients with major forms of sickle cell disease. Methods: This was a study of 1055 records of sickle cell patients aged 6 months to 45 years followed between 2006 and 2018. Results: Six hundred and eighty-one (64.5%) homozygous SS, 283 (26.8%) SC, 86 (8.2%) SF, 4 (0.4%) S? thalassemia and 1 (0.1%) SD sickle cell patients were followed. The majority of the patients (97.9%) were living in the capital city of Lomé and its surrounding suburbs. Most children (67.3%) were less than 5-year-old when they were diagnosed with sickle cell disease, and only 21.3% of cases were discovered before the first year birthday of the patients. Osteo-articular vaso occlusive crisis (VOC) was the principal symptom at the onset of the diagnosis, approximately 28.9% of the cases, followed by severe anemia (19%), then hand-foot syndrome in 15.6%. The systematic screening was performed in 5.4%. Twenty-six percent were G6PD deficient, 4 were HIV immunocompromised, and 29.7% were transfused. Among the degenerative complications, 2.4% had retinopathy, 1.9% osteonecrosis of the femoral head, 1.4% leg ulcers, and 0.8% cardiomyopathy. Salmonella typhi was the main cause of infection, 51.4% of the cases. The mean baseline hemoglobin level (n = 27) was 7.67 ± 1.64 g/dl. Four (0.4%) patients had a splenectomy. The mortality rate was 2.4%. The primary cause of death was anemia (28%), followed by sepsis in 20% of the cases. One case of suicide was documented as the cause of death. Conclusion: Outpatient medical follow-up of sickle cell patients is essential in sub-Saharan African countries where neonatal screening does not exist, and diagnosis is often late. The improvement in quality of life observed should be studied.
