Magnetic resonance imaging for prostate cancer before radical and salvage radiotherapy: What radiation oncologists need to know

External beam radiotherapy(EBRT) is one of the principal curative treatments for patients with prostate cancer(PCa). Risk group classification is based on prostate-specific antigen(PSA) level, Gleason score, and T-stage. After risk group determination, the treatment volume and dose are defined and androgen deprivation therapy is prescribed, if appropriate. Traditionally, imaging has played only a minor role in T-staging due to the low diagnostic accuracy of conventional imaging strategies such as transrectal ultrasound, computed tomography, and morphologic magnetic resonance imaging(MRI). As a result, a notable percentage of tumours are understaged, leading to inappropriate and imprecise EBRT. The development of multiparametric MRI(mp MRI), an imaging technique that combines morphologic studies with functional diffusion-weighted sequences and dynamic contrastenhanced imaging, has revolutionized the diagnosis and management of PCa. As a result, mpM RI is now used in staging PCa prior to EBRT, with possible implications for both risk group classification and treatment decisionmaking for EBRT. mpM RI is also being used in salvageradiotherapy(SRT), the treatment of choice for patients who develop biochemical recurrence after radical prostatectomy. In the clinical context of biochemical relapse, it is essential to accurately determine the site of recurrence-pelvic(local, nodal, or bone) or distant-in order to select the optimal therapeutic management approach. Studies have demonstrated the value of mpM RI in detecting local recurrences-even in patients with low PSA levels(0.3-0.5 ng/m L)-and in diagnosing bone and nodal metastasis. The main objective of this review is to update the role of mpM RI prior to radical EBRT or SRT. We also consider future directions for the use and development of MRI in the field of radiation oncology.

Coronavirus disease 2019(COVID-19)was discovered after unusual cases of severe pneumonia emerged in December 2019 in Wuhan Province(China).Coronavirus is a family of single-stranded RNA viruses.Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is transmitted from person to person.Although asymptomatic individuals can transmit the virus,symptomatic patients are more contagious.The incubation period ranges from 3-7 d and symptoms are mainly respiratory,including pneumonia or pulmonary embolism in severe cases.Elevated serum levels of interleukins(IL)-2,IL-6,IL-7 indicate the presence of cytokine release syndrome,which is associated with disease severity.The disease has three main phases:Viral infection,pulmonary involvement,and hyperinflammation.To date,no treatment has proved to be safe or effective.Chest X-ray and computed tomography(CT)are the primary imaging tests for diagnosis of SARS-CoV-2 pneumonia,follow-up,and detection of complications.The main radiological findings are ground-glass opacification and areas of consolidation.The long-term clinical course is unknown,although some patients may develop pulmonary fibrosis.Positron emission tomography-computed tomography(PETCT)is useful to assess pulmonary involvement,to define the affected areas,and to assess treatment response.The pathophysiology and clinical course of COVID-19 infection remain poorly understood.However,patterns detected on CT and PETCT may help to diagnose and guide treatment.In this mini review,we analyze the clinical manifestations and radiological findings of COVID-19 infection.

Oligometastatic non-small cell lung cancer(NSCLC)describes an intermediate stage of NSCLC between localized and widely-disseminated disease.This stage of NSCLC is characterized by a limited number of metastases and a more indolent tumor biology.Currently,the management of oligometastatic NSCLC involves radical treatment(radiotherapy or surgery)that targets the metastatic lesions and the primary tumor to achieve disease control.This approach offers the potential to achieve prolonged survival in patients who,in the past,would have only received palliative measures.The optimal therapeutic strategies for the different scenarios of oligometastatic disease(intracranial vs extracranial disease,synchronous vs metachronous)remain undefined.Given the lack of head-to-head studies comparing radiotherapy to surgery in these patients,the decision to apply surgery or radiotherapy(with or without systemic treatment)must be based on prognostic factors that allow us to classify patients.This classification will allow us to select the most appropriate therapeutic strategy on an individualized basis.In the future,the molecular or microRNA profiles will likely improve the treatment selection process.The objective of the present article is to review the most relevant scientific evidence on the management of patients with oligometastatic NSCLC,focusing on the role of radiotherapy and surgery.We also discuss areas of controversy and future directions.

The importance of the bite angle of metal(Ⅲ) salen catalysts in the sequestration of CO_(2) with epoxides in mild conditions

Located in the middle of the fever to solve the problem of CO_(2)emissions in the environment,CO_(2)sequestration by reaction with epoxides is one of the key tools,as it not only fixes CO_(2),but also makes it functional by leading to cyclic carbonates.Herein,the results are focused specifically on the formation of cyclic organic carbonates catalyzed by metal-salen complexes,previously achieved with yttrium and scandium,that are compared with those of analogous complexes containing metals from the first transition series,such as cobalt or chromium.Density functional theory(DFT)calculations allow to determine whether this switch of metals will be feasible and provide the basis for instigating future experimental efforts in this regard.The calculations analyzing the structure and electronics of the catalysts allow us to give not only a clear picture of whether these catalysts will be efficient,but also allow us to assess which metal center is the most convenient and/or whether the catalytic reaction will occur under mild conditions.Advanced buried volume calculations with the SambVca packages shed light on the different catalytic pockets of monometallic first row transition metals vs.group III salen complexes.Our predictive catalysis results show that the bite O-M-O angle plays an essential role in the catalysis.

Background:Rapid climate changes lead to an increase in forest disturbance,which in turn lead to growing concerns for biodiversity.While saproxylic beetles are relevant indicators for studying different aspects of biodiversity,most are smaller than 2 mm and difficult to sample.This,together with a high number of species and trophic roles,make their study remarkably challenging,time-consuming,and expensive.The Landsat mission provides data since 1984 and represents a powerful tool in this scenario.While we believe that remote sensing data cannot replace on-site sampling and analysis,in this study we aim to prove that the Landsat Time Series(TS)may support the identification of insects’hotspots and consequently guide the selection of areas where to concentrate field analysis.Methods:With this aim,we constructed a Landsat-derived NDVI TS(1984–2020)and we summarised the NDVI trend over time by calculating eight Temporal Metrics(TMs)among which four resulted particularly successful in predicting the amount of saproxylic insects:(i)the slope of the regression line obtained by linear interpolating the NDVI values over time;(ii)the Root Mean Square Error(RMSE)between the regression line and the NDVI TS;(iii)the median,and the(iv)minimum values of the NDVI TS.The study area consists of four monitoring sectors in a Mediterranean-managed beech forest located in the Apennines(Molise,Italy),where 60 window flight traps for flying beetles were installed.First,the saproxylic beetle's biodiversities of monitoring sectors were quantified in terms of species richness and alpha-diversity.Second,the capability of TMs in predicting the richness of saproxylic beetles family and trophic categories was assessed in terms of Pearson's product-moment correlation.Results:The alpha diversity and species richness analysis indicate dissimilarities across the four monitored sectors(Shannon and Simpson's index ranging between 0.67 to 2.31 and 0.69 to 0.88,respectively),with Landsat TS resulting in effective predictors for estimating saproxylic b

Modelling the daily reference evapotranspiration in semi-arid region of South India: A case study comparing ANFIS and empirical models

The estimation of evapotranspiration(ETo)is one of the main tools for the control of crop growth and to make a rational use of water resources.To estimate this parameter accurately,it is necessary to have a daily measurement of four meteorological variables,these are:temperature,solar radiation,relative humidity and wind speed.It is not always possible to count on all the variables,that is why there are empirical methods that use a limited number of variables that make an approximate estimate of the ETo value.Each of these models are applicable to different regions with completely different climates.In this paper,a study has been carried out to define the model of ETo estimation that best adapts to the semi-arid region in South India.Two different datasets for the same period from different meteorological stations were used.In addition to the empirical methods for estimating ETo,computer models ANFIS(Adaptive Neuro-Fuzzy Inference System)were implemented.These models consist in the future estimation of a certain parameter by using current variables and a history of variables and past results.The results of this work show that ANFIS 11 model makes the best estimate with RMSD=0.002 and r=0.999.The RITCHIE method is the most suitable empirical model for this region,which reaches RMSD=0.507 and r=0.851.In addition,ranking of equations is elaborated for both datasets for daily estimates of ETo.Finally,comparison is made with the results for each case and thus confirm or reject the convenience of one model over the rest.To achieve this,a series of statistical indicators were used:Index of agreement(d),MAE(Mean absolute error),SEE(Standard error of estimate)and RMSD(Root mean square difference).Moreover,a sensitivity analysis was performed in order to compare and show the stability of the best models when an error is introduced within the input parameters.In this case,the empirical models demonstrate a better performance than the ANFIS models.This work demonstrates that the Ritchie method is a good estimator of the ETo

Background:Pyrogeography is a major field of investigation in wildfire science because of its capacity to describe the spatial and temporal variations of fire disturbance.We propose a systematic pyrogeographic analytical approach to cluster regions on the basis of their pyrosimilarities.We employed the Affinity Propagation algorithm to cluster pyroregions using Italian landscape as a test bed and its current wildfire metrics in terms of density,seasonality and stand replacing fire ratio.A discussion follows on how pyrogeography varies according to differences in the human,biophysical,socioeconomic,and climatic spheres.Results:The algorithm identified seven different pyroregion clusters.Two main gradients were identified that partly explain the variability of wildfire metrics observed in the current pyroregions.First,a gradient characterized by increasing temperatures and exposure to droughts,which coincides with a decreasing latitude,and second,a human pressure gradient displaying increasing population density in areas at lower elevation.These drivers exerted a major influence on wildfire density,burnt area over available fuels and stand replacing,which were associated to warmdry climate and high human pressure.The study statistically highlighted the importance of a North–South gradient,which represents one of the most important drivers of wildfire regimes resulting from the variations in climatic conditions but showing collinearity with socioeconomic aspects as well.Conclusion:Our fully replicable analytical approach can be applied at multiple scales and used for the entire European continent to uncover new and larger pyroregions.This could create a basis for the European Commission to promote innovative and collaborative funding programs between regions that demonstrate pyrosimilarities.

Is it possible to stop nucleos(t)ide analogue treatment in chronic hepatitis B patients?

Chronic hepatitis B(CHB) remains a challenging global health problem, with nearly one million related deaths per year. Nucleos(t)ide analogue(NA) treatment suppresses viral replication but does not provide complete cure of the hepatitis B virus(HBV) infection. The accepted endpoint for therapy is the loss of hepatitis B surface antigen(HBs Ag), but this is hardly ever achieved. Therefore, indefinite treatment is usually required. Many different studies have evaluated NA therapy discontinuation after several years of NA treatment and before HBs Ag loss. The results have indicated that the majority of patients can remain off therapy, with some even reaching HBs Ag seroconversion. Fortunately, this strategy has proved to be safe, but it is essential to consider the risk of liver damage and other comorbidities and to ensure aclose follow-up of the candidates before considering this strategy. Unanswered questions remain, namely in which patients could this strategy be effective and what is the optimal time point at which to perform it. To solve this enigma, we should keep in mind that the outcome will ultimately depend on the equilibrium between HBV and the host's immune system. Viral parameters that have been described as good predictors of response in HBe Ag(+) cases, have proven useless in HBe Ag(-) ones. Since antiviral immunity plays an essential role in the control of HBV infection, we sought to review and explain potential immunological biomarkers to predict safe NA discontinuation in both groups.

The Role of Work Motivation Based on Values in Employee’s Retention in the 21st Century

Organizations in the 21th century have to address the problem of employee’s retention as it has direct and indirect costs for the organisation.In regards to this issue several theories have emerged pointing out the strong relationship between retention and employee’s motivation.This last topic is not trivial,as it includes factors from different disciplines such as the psychological or managerial ones.The aim of this paper is to analyse the current literature related to different work motivation theories in order to identify patterns,which might help to understand better the dynamics between motivation at work and its impact on employee retention.In order to do so a detailed revision of the literature has been done classifying the main motivational theories in needs,traits,values,and cognition.Results of the above analysis highlight the“satisfaction of a need”as a common denominator in the motivational theories.Need theories explain why someone must act but they do not explain why particular actions are followed in specific situations.One of the insights is that values are the ones that unveil what drives individuals to achieve a goal.Therefore it is vital for organizations to learn and understand which ones are the employees work values in order to retain them and keep them motivated.

Segmental arterial mediolysis, reparative phase: An analysis and case report showing conversion to fibromuscular dysplasia with renal infarction

Segmental arterial mediolysis (SAM), an uncommon arteriopathy putatively caused by norepinephrine released by alpha-1 adrenergic agonists or some Beta-2 agonists capable of releasing norepinephrine from the peripheral sympathetic nervous system potentially can present ischemic and organ injury symptoms caused by sequelae created in its reparative phase in lieu of catastrophic hemorrhages announced in its injurious phase. The case documents this presentation—the patient presenting renal infarcts and ischemic lesions causing abdominal angina, hypertension and a nephrectomy event developing 10 years after prolonged ritodrine treatment for premature labor. This agent may have directly caused SAM or sensitized the patient to agonists causing SAM encountered at a latter date. A variety of lesions derived from injurious phase arterial injuries characterize reparative phase SAM. All were encountered in a hilar branch of the resected renal artery. These included side-by-side sequela aneurysms grossly forming a large fusiform aneurysm, granulation tissue filling adventitial medial tear spaces in which a dissecting hematomas developed, medial muscle loss centered to the outer media repaired with fibrous tissue, arterial stenosis created by reparative intimal plaques, and arterial thrombo-embolism. These lesions were mirrored in accompanying radiologic studies. The accompanying renal vein exhibited changes consistent with repair of the spastic venous angiopathy that often accompanies abdominal SAM. This angiopathy, putatively induced by Endothelin-1, suggested that this agent played a role in the genesis of the arterial lesions. Angiographic resolution of non-treated sequelae occurred in 5 months either spontaneously or due to treatment with bosentem. Conclusions: The histologic and angiographic changes demonstrate that the clinical onset of reparative SAM may be significantly delayed to produce ischemic lesions, renal infarction and in this case report, medial fibromuscular dysplasia in the hilar branch of the renal artery.
