AIM： To examine the risk of renal events in patients with biopsy-proven diabetic nephropathy （DN） and its possible associated ***： Clinical and histological data of 60 pa-tients diagnosed with diabetic nephropathy were retro-spectively collected. patients with evidence or suspicion of other nephropathies were excluded from the study. The fnal event was defned as renal replacement ther-apy （RRT） initiation or progression of chronic kidney disease （CKD）, according to the KDIGO 2012 defnition of a decrease in CKD category and a decrease in GFR of 25% or more. RESULTS： A total of 45 patients with a follow-up of at least 3 mo were included. Most of the patients presented type 2 DM, with a mean age of 58.3 years old. The time of evolution of DM was 9.6 ± 7.8 years, al-though in 13 patients, it was less than 5 years. A total of 62% of patients reached the fnal event in a mean period of 3.4 years （95%CI： 2.1-4.7）, with 21 of them requiring dialysis. The factors that were indepen-dently associated with renal survival were estimated glomerular fltration rate （eGFR） at the time of biopsy, cardiovascular disease （CVD） history and HbA1c less than 7%. Therefore, for each 10 mL/min per 1.73 m2 reduction in eGFR, we obtained a DN progression risk of HR = 2 （1.3-3.0）（p = 0.001）; patients with CVD were at greater risk for DN progression （HR = 2.8, 1.1-7.1, p = 0.032）, and CKD patients with HbA1c 〈 7% demonstrated greater renal risk than patients with HbA1c ≥ 7%, with an HR of 2.9 （1.0-8.4）（p = 0.054）.CONCLUSION： A past history of CVD is a risk fac-tor for DN progression. Levels of HbA1c less than 7% could favor an eGFR decrease in these patients.

Located in the middle of the fever to solve the problem of CO_(2)emissions in the environment,CO_(2)sequestration by reaction with epoxides is one of the key tools,as it not only fixes CO_(2),but also makes it functional by leading to cyclic ***,the results are focused specifically on the formation of cyclic organic carbonates catalyzed by metal-salen complexes,previously achieved with yttrium and scandium,that are compared with those of analogous complexes containing metals from the first transition series,such as cobalt or *** functional theory(DFT)calculations allow to determine whether this switch of metals will be feasible and provide the basis for instigating future experimental efforts in this *** calculations analyzing the structure and electronics of the catalysts allow us to give not only a clear picture of whether these catalysts will be efficient,but also allow us to assess which metal center is the most convenient and/or whether the catalytic reaction will occur under mild *** buried volume calculations with the SambVca packages shed light on the different catalytic pockets of monometallic first row transition metals *** III salen *** predictive catalysis results show that the bite O-M-O angle plays an essential role in the catalysis.

We analyze the well-posedness of an anisotropic,nonlocal diffusion *** an equivalence between weighted and unweighted anisotropic nonlocal diffusion operators in the vein of unified nonlocal vector calculus,we apply our analysis to a class of fractional-order operators and present rigorous estimates for the solution of the corresponding anisotropic anomalous diffusion ***,we extend our analysis to the anisotropic diffusion-advection equation and prove well-posedness for fractional orders s∈[0.5,1).We also present an application of the advection-diffusion equation to anomalous transport of solutes.

We theoretically investigate the electromagnetic response of a novel class of multilayered metamaterials obtained by alternating graphene sheets and dielectric layers,the whole structure not exhibiting a plane of reflection symmetry along the stacking *** show that the electromagnetic response of the structure is characterized by a magneto-electric coupling described by an effective chiral *** the intrinsic tunability of graphene–light coupling,we prove that one can tune both the dielectric and the chiral electromagnetic response by varying the graphene chemical potential through external voltage gating.

New skin closure system facilitates wound healing after cardiovascular implantable electronic device surgery

The manuscript describes the efficacy of a new skin closure system(Zip Line?) for wound closure after pacemaker/implantable cardioverter defibrillator surgery. The system is particularly useful when wound healingis difficult with traditional methods and in patients at high risk for surgical site infections(SSIs). This skin closure option is easy and quick to apply and remove, and produces excellent cosmetic results. Although it is associated with a minimal expense upcharge, the benefits, including the potential for decrease in SSI, make it attractive and worth considering for skin closure in device patients, particularly those at increased risk of complications.

External beam radiotherapy(EBRT) is one of the principal curative treatments for patients with prostate cancer(pCa). Risk group classification is based on prostate-specific antigen(pSA) level, Gleason score, and T-stage. After risk group determination, the treatment volume and dose are defined and androgen deprivation therapy is prescribed, if appropriate. Traditionally, imaging has played only a minor role in T-staging due to the low diagnostic accuracy of conventional imaging strategies such as transrectal ultrasound, computed tomography, and morphologic magnetic resonance imaging(MRI). As a result, a notable percentage of tumours are understaged, leading to inappropriate and imprecise EBRT. The development of multiparametric MRI(mp MRI), an imaging technique that combines morphologic studies with functional diffusion-weighted sequences and dynamic contrastenhanced imaging, has revolutionized the diagnosis and management of pCa. As a result, mpM RI is now used in staging pCa prior to EBRT, with possible implications for both risk group classification and treatment decisionmaking for EBRT. mpM RI is also being used in salvageradiotherapy(SRT), the treatment of choice for patients who develop biochemical recurrence after radical prostatectomy. In the clinical context of biochemical relapse, it is essential to accurately determine the site of recurrence-pelvic(local, nodal, or bone) or distant-in order to select the optimal therapeutic management approach. Studies have demonstrated the value of mpM RI in detecting local recurrences-even in patients with low pSA levels(0.3-0.5 ng/m L)-and in diagnosing bone and nodal metastasis. The main objective of this review is to update the role of mpM RI prior to radical EBRT or SRT. We also consider future directions for the use and development of MRI in the field of radiation oncology.

On the calibration of rotational augmentation models for wind turbine load estimation by means of CFD simulations

In this work the improved version of an engineering model which accounts for rotational augmentation effects by means of computational fluid dynamics(CFD)calibration is explored and *** on an analysis of the NREL phase VI wind turbine,the novel modeling is presented,which uses as base line the formulation proposed by Chaviaropoulos and *** model is calibrated based on CFD simulations using *** corresponding correction of the two dimensional polars is straightforward implemented within MoWiT,an in-house software for load *** novel formulation results in improved lift and drag coefficients prediction in all considered cases,reducing the deviation with respect to the rotating CFD cases down to few *** optimal configuration including the correction for tip effects of Shen shows better agreements at the very tip of the *** the range of applicability for large wind turbine rotor blades based on a virtual 10MW rotor model is discussed.

Surface science approach to the heterogeneous cycloaddition of CO_(2) to epoxides catalyzed by site-isolated metal complexes and single atoms: a review

The cycloaddition of CO_(2) to epoxides to afford cyclic organic carbonates is an increasingly relevant non-reductive strategy to convert CO_(2) to useful products able to serve as high-boiling solvents,chemical intermediates,and monomers for the preparation of more sustainable *** development of efficient and robust hetero-geneous catalysts for such transformation is,therefore,crucial and can be carried out by several strategies that often require the preparation of sophisticated and/or expensive organic networks,linkers,or compounds.A different approach to the preparation of heterogeneous catalysts for CO_(2)-epoxide coupling is by applying surface science methodologies to graft molecular fragments or single atoms on various supports leading to well-defined active *** this context,surface organometallic chemistry(SOMC),along with comparable methodologies,is a valuable approach for the preparation of efficient,single-site Lewis acids and catalysts for the target cycloaddition reaction on metal oxides,whereas,other grafting methodologies,can be applied to prepare analogous catalysts on different kinds of ***,we discuss very recent advances in the application of surface methodologies for the preparation of single atom catalysts as an increasingly relevant approach towards highly active Lewis acids for the cycloaddition of CO_(2) to ***,we show that Lewis acids and catalysts prepared by facile surface methodologies hold significant potential for future application is the synthesis of cyclic carbonates from CO_(2).
