Epithelial cell identity in hyperplastic precursors of breast cancer

Introduction:In the adult human breast,hyperplastic enlarged lobular unit(HELU) and atypical ductal hyperplasia(ADH) are two common abnormalities that frequently coexist with ductal carcinoma in situ(DCIS).For this reason,they have been proposed as the early steps in a biological continuum toward breast ***:We investigated in silico the expression of 369 genes experimentally recognized as involved in establishing and maintaining epithelial cell identity and mammary gland remodeling,in HELUs or ADHs with respect to the corresponding patient-matched normal ***:Despite the common luminal origin,HELUs and ADHs proved to be characterized by distinct gene profiles that overlap for 5 genes *** HELUs were associated with the overexpression of progesterone receptor(PGR),ADHs were characterized by the overexpression of estrogen receptor 1(ESR1) coupled with the overexpression of some proliferation-associated ***:This unexpected finding contradicts the notion that in differentiated luminal cells the expression of estrogen receptor(ER) is dissociated from cell proliferation and suggests that the establishing of an ER-dependent signaling is able to sustain cell proliferation in an autocrine manner as an early event in tumor *** clinical evidence indicates that only a fraction of HELUs and ADHs evolve to invasive cancer,present findings warn that exposure to synthetic progestins,frequently administered as hormone-replacement therapy,and estrogens,when abnormally produced by adipose cells and persistently present in the stroma surrounding the mammary gland,may cause these hyperplastic lesions.

Differential expression of genes involved in the epigenetic regulation of cell identity in normal human mammary cell commitment and differentiation

The establishment and maintenance of mammary epithelial cell identity depends on the activity of a group of proteins, collectively called maintenance proteins, that act as epigenetic regulators of gene transcription through DNA methylation, histone modification, and chromatin remodeling. Increasing evidence indicates that dysregulation of these crucial proteins may disrupt epithelial cell integrity and trigger breast tumor initiation. Therefore, we explored in silico the expression pattern of a panel of 369 genes known to be involved in the establishment and maintenance of epithelial cell identity and mammary gland remodeling in cell subpopulations isolated from normal human mammary tissue and selectively enriched in their content of bipotent progenitors, committed luminal progenitors, and differentiated myoepithelial or differentiated luminal cells. The results indicated that, compared to bipotent cells, differentiated myoepithelial and luminal subpopulations were both characterized by the differential expression of 4 genes involved in cell identity maintenance: CBX6 and PCGF2, encoding proteins belonging to the Polycomb group, and SmARCD3 and SmARCE1, encoding proteins belonging to the Trithorax group. In addition to these common genes, the myoepithelial phenotype was associated with the differential expression of HDAC1, which encodes histone deacetylase 1, whereas the luminal phenotype was associated with the differential expression of SmARCA4 and HAT1, which encode a Trithorax protein and histone acetylase 1, respectively. The luminal compartment was further characterized by the overexpression of ALDH1A3 and GATA3, and the down-regulation of NOTCH4 and CCNB1, with the latter suggesting a block in cell cycle progression at the G2 phase. In contrast, myoepithelial differentiation was associated with the overexpression of mYC and the down-regulation of CCNE1, with the latter suggesting a block in cell cycle progression at the G1 phase.

In 2012, the Italian ministry of Education introduced in the Guidelines for preschool the need to evaluate one aspect of coordination related to the eye-hand relationship. meinel＇s theoretical model provides an overview of the eye hand coordination, based on capacity to integrate anatomic-functional skills with specific and synchronic hand＇s actions. meinel＇s model is rich of possible teaching ideas which are consistent with the Italian school system. They might also be a topical theoretical support to choose and use the motor evaluation tests in the Italian preschool, in order to evaluate the eye hand coordination and its constituent elements.

由于人口的不断增长和全球化的不断发展,海上交通量也得以不断增加。近几十年来,由于持续不断的海上交通事故,海上安全和监视的课题受到了广泛关注。现有研究表明,海上交通的风险是由于操作和管理失误造成的。因此,该研究集中研究海上安全和监管方面存在的不足,旨在分析海运行业的重大安全问题,特别是在坦桑尼亚。本研究利用文献分析开展定性研究,总结出海运行业的安全问题,如工作人员对安全文化缺乏了解,不遵守规章制度,安全管理落实不到位;采用CRISP-Dm模型进行海运风险评估,以辅助分析船舶交通管理。研究发现坦桑尼亚事故是由于管理上的缺陷、人为失误和缺乏先进技术造成的。研究建议,遵守法规,在海运系统中建立弹性,开展风险评估,以及在操作、通信和船舶设计方面运用高技术可以减少事故,进而有助于提升海上交通安全的整体水平。

The Use of Exergames for Adaptive Physical Activity among the Elderly

The research program proposed in this study promotes the development of a pilot study for the promotion of physical and motor activities for the elderly, with particular reference to the over 80 years old. The study aims to promote the practice of adapted physical activity for the elderly and proposes the design and development of specific exergames for the promotion of a healthy and active lifestyle among the elderly even when they can not do motor activities in dedicated places such as gyms or specialized centers or adequate natural environments. The project involved elderly people living in the Salerno area who can not do motor activity in a regular way due to cultural, socio-economic, environmental or personal reasons. The identification of the experimental group for the pilot study required a preliminary survey on the levels of autonomy, intellectual efficiency and motor functioning： Instrumental activities of daily living, activities of daily living, short portable mental status questionnaire, clock drawing test, geriatric depression scale （GDS-15） and the short physical performance battery were administered. A specific program of adapted physical activities was planned and realized for 8 months. The study could open interesting perspectives of research that involve interesting results also from a social point of view.

Background: COVID-19 brought challenges that did not end after a two-year pandemic. From more straightforward changes in habits to studying to understand the enigmatic parasite-host relationship, we can better manage the patient infected with SARS-CoV-2 even with a vaccine full of doubts and antivirals that do not correctly cover the viral period. SARS-CoV-2 brought the chronic inflammation now called “The Long COVID-19 Syndrome” (LCS), something still little talked about, but we already see deaths due to non-identification of this inflammatory syndrome that can lead to shock. Theory: LCS Shock is due to a long period of metabolic stress, reflecting the shift from inflammation to oxidative stress and innate immunity, and does not respond to antimicrobials, as its main component is inflammatory, although there may be conjoined bacterial translocation. Thus, we are losing patients to a new syndrome confused with sepsis and septic shock. While septic shock (SS) responds to antimicrobials, Inflammatory Shock (ISc) does not respond to antimicrobials alone, requiring high doses of corticosteroids. Review: This study shows that we need to differentiate SS and ISC, as the treatment is different. The review shows that Lactate, LDH and the presence of new/recent cardiac changes and bradycardia in the face of a status where there should be tachycardia as the usual response can differ ISC from SS. maybe the main responsible for high LDH is Warburg Effect. Conclusion: We have a dilemma that requires clinical studies that routinely match high doses of corticosteroids (until there is something better to be done) and bring laboratory and imaging differences to diagnose SS vs ISc better.

Surgical management of genital lymphoedema:experience and critical considerations from a tricenter study

Aim:Genital lymphoedema(GL)is a chronic and debilitating disease,which can severely affect the patient’s quality of life with significant socio-economic ***,no gold standard algorithm exists for GL from diagnosis to *** study proposes our therapeutic flowchart based on the three senior consultants’experience in lymphatic ***:A retrospective investigation was conducted on a prospectively maintained database(2018-2022).Inclusion criteria involved all patients who underwent surgical procedures for treating GL in three plastic surgery departments(Lausanne,Bari,and Genova).Outcomes were assessed in terms of oedema reduction,stage regression,and functional reported ***:16 patients with GL were included:50%underwent debulking surgery,18.8%microsurgery,and 31.2%debulking+*** recorded a significant regression of the GL stage:62.5%shifted from stage II/III to postoperative stage ***,we found an infection recurrency resolution in 50%,a scrotal oedema reduction in 62.5%,and a scrotal oedema resolution in 37.5%of the patients *** almost half of the patients(53.3%)with associated penile oedema described persistent postoperative penile oedema,only two patients complained of persistent ***:According to our clinical experience,preoperative and postoperative physical functional therapy is always *** stages I and IIA,after the failure of the conservative treatment,lymph-venous shunts and lymph node transplantation surgery are proposed at the early *** GL is already diagnosed at stages IIB and III,the debulking surgery,together with functional procedures,represents our first approach.

Hepatitis B virus (HBV) infection affects a large part of the world population. Within the different virological HBV categories that have been identified, patients with occult HBV infection represent a peculiar group. These individuals harbor a replication competent virus, inhibited in its replicative function. Accordingly, cases of reactivations have been observed in immunosuppressed individuals who lose immunological control over the infection. Patients with hematological malignancies (Hm) are treated with intense myeloand immunosuppres-sive chemotherapy regimens which favor HBV reactivation. This event can have severe consequences, such as hepatitis flare, hepatic failure and even death. In addition, it can lead to delays or interruptions of curative treatments, resulting in a decreased disease free and overall survival. In this review, we will examine the event of HBV reactivation in patients with signs of resolved HBV infection undergoing treatment for Hm and propose possible management strategies.
