We investigate the effect of the westerly rotation of the lithosphere on the active margins that surround the Americas and find good correlations between the inferred easterly-directed mantle counterflow and the main structural grain and kinematics of the Andes and Sandwich arc *** the Andes,the subduction zone is shallow and with low dip,because the mantle flow sustains the slab;the subduction hinge converges relative to the upper plate and generates an uplifting doubly verging *** Sandwich Arc is generated by a westerly-directed SAM(South American) plate subduction where the eastward mantle flow is steepening and retreating the subduction *** this context,the slab hinge is retreating relative to the upper plate,generating the backarc basin and a low bathymetry single-verging accretionary *** Central America,the Caribbean plate presents a more complex scenario:(a) To the East,the Antilles Arc is generated by westerly directed subduction of the SAM plate,where the eastward mantle flow is steepening and retreating the subduction zone.(b) To the West,the Middle America Trench and Arc are generated by the easterly-directed subduction of the Cocos plate,where the shallow subduction caused by eastward mantle flow in its northern segment gradually steepens to the southern segment as it is infered by the preexisting westerly-directed subduction of the Caribbean *** the frame of the westerly lithospheric flow,the subduction of a divergent active ridge plays the role of introducing a change in the oceanic/continental plate’s convergence angle,such as in NAM(North American)plate with the collision with the Pacific/Farallon active ridge in the Neogene(Cordilleran orogenic type scenario).The easterly mantle drift sustains strong plate coupling along NAM,showing at Juan de Fuca easterly subducting microplate that the subduction hinge advances relative to the upper *** lower/upper plate convergence coupling also applies along strike to the neigh

Fecal calprotectin(FC) has emerged as one of the most useful tools for clinical management of inflammatory bowel diseases(IBD). Many different methods of assessment have been developed and different cutoffs have been suggested for different clinical settings. We carried out a comprehensive literature review of the most relevant FC-related topics: the role of FC in discriminating between IBD and irritable bowel syndrome(IBS) and its use in managing IBD patients In patients with intestinal symptoms, due to the high negative predictive value a normal FC level reliably rules out active IBD. In IBD patients a correlation with both mucosal healing and histology was found, and there is increasing evidence that FC assessment can be helpful in monitoring disease activity and response to therapy as well as in predicting relapse, post-operative recurrence or pouchitis. Recently, its use in the context of a treat-to-target approach led to a better outcome than clinically-based therapy adjustment in patients with early Crohn's disease. In conclusion, FC measurement represents a cheap, safe and reliable test, easy to perform and with a good reproducibility. The main concerns are still related to the choice of the optimal cut-off, both for differentiating IBD from IBS, and for the management of IBD patients.

An update on microbiological causes of canine otitis externa in Campania Region,Italy

Objective: To update the recent knowledge of the microbiological causes of canine otitis externa in Campania Region(Italy) and the antibiotic susceptibility patterns of the isolated ***: A total of 122 dogs were examined by otoscopy, and auricular swab samples were collected from both ears in 74 dogs presenting clinical bilateral otitis and from single ears in 48 dogs displaying clinical unilateral otitis. Cytological examination,bacteriological analysis and antimicrobial susceptibility tests were ***: Thirty-one out of 122 dogs were positive for yeast species(25.4%, 95% confidence interval(CI): 18.2%–34.2%) with a higher prevalence of Malassezia pachydermatis(21/31 isolates, 67.7%, CI: 48.5%–82.7%), and a total of 91 out of 122 dogs were positive for bacterial species(74.6%; CI: 65.8%–81.8%) with a higher prevalence of Staphylococcus pseudintermedius(45/143 isolates, 31.5%, CI: 24.1%–39.8%). These results are the first description of Streptococcus agalactiae-associated otitis. The yeasts isolated showed high levels of susceptibility to all antifungal agents tested; on the contrary all the isolated bacterial strains were highly resistant to at least four out of ten antimicrobial classes. Both Gram-positive and Gram-negative bacteria showed high resistance to amoxicillin/clavulanate and kanamycin hence they are not recommended as initial empirical therapy for the otitis ***: This update illustrates an increase in antibiotic resistances providing an insight into the current knowledge of the therapeutic procedures followed on canine otitis externa in Italy. It also emphasizes the importance of considering the results of the microbiological and sensitivity tests to decide on an appropriate antibiotic therapy.

麦卡因大型VMS型Au-Ag-Cu-Zn-Pb矿床位于中哈萨克斯坦西北部的Baidaulet-Akbastau地区。地质剖面测量和岩石薄片研究表明,麦卡因矿区发育中基性火山岩(玄武岩和安山岩)和次火山岩(辉绿-辉长岩和辉绿岩),矿化主要与安山岩有关。对矿区火山岩地球化学分析表明,矿区岩石分为两种类型:类型Ⅰ为拉斑-钙碱性系列中基性火山岩(玄武岩和安山岩)和次火山岩(辉绿-辉长岩和辉绿岩),岩石具平坦的稀土配分模式,亏损Nb和富集Th/Yb,显示俯冲特点;类型Ⅱ为钙碱性系列安山岩,具典型的右倾型稀土配分模式,亏损Nb和富集Th/Yb,指示其形成于岛弧环境。含矿岩石为类型Ⅱ安山岩。对类型Ⅱ安山岩进行了锆石Lu-Hf同位素和SIMS U-Pb年龄分析,结果表明,锆石εHf(t)为较高的正值(+4.28^+14.84),指示其是地幔物质部分熔融的产物,锆石SIMS U-Pb年龄为459.1±4.8Ma,指示其成岩年龄属于晚奥陶世。因此,哈萨克斯坦麦卡因大型VMS型矿床含矿火山岩为钙碱性系列安山岩,形成于晚奥陶世岛弧环境。对比研究认为,哈萨克斯坦早古生代波谢库尔-成吉思斑岩型-VMS型成矿带向东延伸到我国西准噶尔沙尔布提山,沙尔布提山具有形成VMS型矿床的岩浆条件和构造环境。

BACKGROUND Pathological complete response(pCR) is rare in hormone receptor-positive(HR+)HER2-negative breast cancer(BC) treated with either endocrine therapy(ET) or chemotherapy. Radical resection of locoregional relapse, although potentially curative in some cases, is challenging when the tumor invades critical *** oral cyclin-dependent kinase 4/6 inhibitor palbociclib in combination with ET has obtained a significant increase in objective response rates and progression-free survival in patients with advanced BC and is now being evaluated in the neoadjuvant setting. We present a clinical case of a patient with an inoperable locoregional relapse of HR+ HER2-negative BC who experienced p CR after treatment with *** SUMMARY We report the clinical case of a 60-year-old patient who presented with an inoperable locoregional relapse of HR+, HER2-negative BC 10 years after the diagnosis of the primary tumor. During a routine follow-up visit, breast magnetic resonance imaging and positron emission tomography/computed tomography revealed a 4-cm lesion in the right subclavicular region, infiltrating the chest wall and extending to the subclavian vessels, but without bone or visceral involvement. Treatment was begun with palbociclib plus letrozole, converting the disease to operability over a period of 6 mo. Surgery was performed and a p CR achieved. Of note, during treatment the patient experienced a very uncommon toxicity characterized by burning tongue and glossodynia associated with dysgeusia, paresthesia, dysesthesia, and xerostomia. A reduction in the dose of palbociclib did not provide relief and treatment with the inhibitor was thus discontinued, resolving the tongue symptoms. Laboratory exams were unremarkable.
