Dexmedetomidine is a central alpha2-adrenergic receptor agonist with sedativ. and analgesic properties that has a prov.n safety profile. It has sev.ral beneficial effects such as decreasing sympathetic tone, leading to reduced opiate use and anxiolysis, making it an attractiv. option for sedation in the perioperativ. and intensiv. care unit setting. These effects also modify fav.rably the time spent on a v.ntilator, intensiv. care unit length of stay and dev.lopment of delirium. Recent studies also suggest that dexmedetomidine possesses wide-ranging immunomodulating properties. It has been associated with reduced inflammatory cytokine release, modulation of inflammatory transcription factors, oxidativ. stress and inflammatory cells. These properties could be beneficial in the context of inflammatory conditions that require sedation, such as sepsis, ischemia-reperfusion injury and v.ntilator-associated lung injury, among many others. In this rev.ew, we propose specific clinical scenarios where these properties could turn out to be clinically relev.nt.

1,4-Naphthoquinones (NQ) is a class of organic compounds with a number of important properties. Howev.r, the existing methods of their synthesis either giv. a low NQ yield or multistage and require a lot of time. In the present work new one-pot process of NQ synthesis from hydroquinone (HQ) and substituted 1,3-dienes at room temperature is proposed. The solution of heteropolyacid H7PMo8v.O40 (HPA-4) is used as a bifunctional (acid and redox) catalyst for this process. The influence of a number of factors on key process parameters has been studied. It is demonstrated that unsubstituted and a number of alkyl substituted NQ with the yields of 50% - 80% and purity of 92% - 99% can be obtained by this method. The results obtained allow to significantly simplify the synthesis of NQ directly from HQ in comparison with the described similar methods.

Objectiv.: Patients with BRAF-mutant colorectal cancer(CRC) hav. a poor prognosis. Molecular status is not currently used to select which drug to use in combination with radiotherapy. Our aim was to identify drugs that radiosensitise CRC cells with known BRAF ***: We screened 298 oncological drugs with and without ionising radiation in colorectal cancer cells isogenic for BRAF. Hits from rank product analysis were v.lidated in a 16-cell line panel of human CRC cell lines, using clonogenic surv.v.l assays and xenograft models in ***: Most consistently identified hits were drugs targeting cell growth/proliferation or DNA damage repair. The most effectiv. class of drugs that radiosensitised wild-type and mutant cell lines was PARP inhibitors. In clonogenic surv.v.l assays, talazoparib produced a radiation enhancement ratio of 1.9 in DLD1(BRAF-wildtype) cells and 1.8 in RKO(BRAF v.00 E) cells. In DLD1 xenografts, talazoparib significantly increased the inhibitory effect of radiation on tumour growth(P ≤ 0.01).Conclusions: Our method for screening large drug libraries for radiosensitisation has identified PARP inhibitors as promising radiosensitisers of colorectal cancer cells with wild-type and mutant BRAF backgrounds.
