BACKGROUND Acute esophageal necrosis(AEN)is a rare entity with multifactorial etiology,usually presenting with signs of upper gastrointestinal *** To systematically review all available data on demographics,clinical features,outcomes and management of this medical *** A systematic literature search was performed with respect to the PRISMA statement(end-of-search date:October 24,2018).Data on the study design,interventions,participants and outcomes were extracted by two independent *** Seventy-nine studies were included in this ***,114 patients with AEN were identified,of whom 83 were males and 31 *** patient age was 62.1±*** most common presenting symptoms were melena,hematemesis or other manifestations of gastric bleeding(85%).The lower esophagus was most commonly involved(92.9%).The most widely implemented treatment modality was conservative treatment(75.4%),while surgical or endoscopic intervention was required in 24.6%of the *** overall followup was 66.2±101.8 *** 29.9%of patients died either during the initial hospital stay or during the follow-up *** symptoms on presentation[Odds ratio 3.50(1.09-11.30),P=0.03]and need for surgical or endoscopic treatment[surgical:Odds ratio 1.25(1.03-1.51),P=0.02;endoscopic:Odds ratio 1.4(1.17-1.66),P<0.01]were associated with increased odds of complications.A sub-analysis separating early versus late cases(after 2006)revealed a significantly increased frequency of surgical or endoscopic intervention(9.7%vs 30.1%respectively,P=0.04)CONCLUSION AEN is a rare condition with controversial pathogenesis and unclear optimal *** the frequency of surgical and endoscopic intervention has increased in recent years,outcomes have remained the ***,further research work is needed to better understand how to best treat this potentially lethal disease.

Feasibility of Performing Sentinel Lymph Node Biopsy (SLNB) after Mastectomy: A Case Report

Introduction: Previous mastectomy remains a contraindication to SLNB as normal drainage patterns of the breast can be disturbed. Patients diagnosed with DCIS on core biopsy and later found to have microinvasive or invasive carci-noma at the time of mastectomy are deprived of the opportunity for SLNB and need to undergo axillary dissection. We explored the option and feasibility of performing SLNB in a 39-year-old female who underwent a simple mastectomy without axillary sampling for extensive DCIS and later found to have microinvasive ductal carcinoma on permanent pathology. Results: Lymphatic mapping using subdermal injection of 99mTc-labeled sulfur colloid and blue dye led to the identification of five SLNs. Histopathologic examination showed no metastasis. Conclusion: SNLB is feasible in this setting. However, before its use is routinely adopted, its feasibility and accuracy has to be demonstrated in larger num-bers of patients in whom a negative SLNB is followed by a completion axillary dissection.

Use of Sunflower Cake in the Diet of ISA Brown Pullets

Recently, feed ingredient prices and availability concerns have resulted in the examination of diet formulations using alternative, locally available, high protein feed ingredients. An experiment was carried out to evaluate sunflower cake （SC） dietary effect on the performance of ISA brown pullets. The duration of the experiment was 20 weeks. A total of 48 one-day-old pullets were randomly allocated in three groups （C, SC6.25 and SC12.5） with four replications. The pullets of the control group （C） were given proper full rations without sunflower cake for the 1 st, 2nd and 3rd rearing ages, whereas in the treatment groups 5C6.25 sunflower cake was added at 6.25% of rations and in group SC12.5 at 12.5% of rations. The pullets were housed in floor pens with litter and were offered feed and water ad libitum. According to the results of statistical analysis, the addition of 6.25% sunflower cake in the diet significantly （P 〈 0.05） affected body weight gain and feed conversion ratio. The results of this study showed that sunflower cake can be added to the ration of ISA brown pullets of 0-20 weeks of age up to 12.5% without any adverse effect on their performance.

This work deals with the experimental study of a surface dielectric-barrier discharge,as a part of the ongoing interest in the control of plasma induced electro-fluid dynamic effects(*** actuators).The discharge is generated using a plasma reactor consisting of a fused silica plate which is sandwiched between two printed circuit boards where the electrodes are *** reactor is driven by narrow high voltage square pulses of asymmetric rising(25 ns)and falling(2.5μs)parts,while the discharge evolution is considered in a temporarily and spatially resolved manner over these *** is,conventional electrical and optical emission analyzes are combined with high resolution optical emission spectroscopy and ns-resolved imaging,unveiling main characteristics of the discharge with a special focus on its propagation along the dielectric-barrier *** voltage rising part leads to cathode-directed ionization waves,which propagate with a speed up to 105m s~(-1).The voltage falling part leads to cathode sheath formation on the driven electrode.Τhe polarization of the dielectric barrier appears critical for the discharge dynamics.

Dementia and osteoporosis in a geriatric population: Is there a common link?

AIM To determine the existence of a common pathological link between dementia and osteoporosis through reviewing the current evidence base. METHODS This paper reviews the current literature on osteoporosis and dementia in order to ascertain evidence of a common predisposing aetiology. A literature search of Ovid MEDLINE(1950 to June 2016) was conducted. The keywords "osteoporosis", "osteoporotic fracture", "dementia" and "Alzheimer's disease"(AD) were used to determine the theoretical links with the most significant evidence base behind them. The key links were found to be vitamins D and K, calcium, thyroid disease, statins, alcohol and sex steroids. These subjects were then searched in combination with the previous terms and the resulting papers manually examined. Theoretical, in vitro and in vivo research were all used to inform this review which focuses on the most well developed theoretical common causes for dementia(predominantly Alzheimer's type) and *** Dementia and osteoporosis are multifaceted disease processes with similar epidemiology and a marked increase in prevalence in elderly populations. The existence of a common link between the two has been suggested despite a lack of clear pathological overlap in our current understanding. Research to date has tended to be fragmented and relatively weak in nature with multiple confounding factors reflecting the difficulties of in vivo experimentation in the population of interest. Despite exploration of various possible mechanisms in search for a link between the two pathologies, this paper found that it is possible that these associations are coincidental due to the nature of the evidence available. Comparative Evaluation of Mannan Oligosaccharides and Acidifier Calcium Formate on the Quail Digestive Tract

Three hundred, one-day-old Japanese quail （Coturnixjaponica） divided into four groups, with three subgroups each, were fed a basal diet that served as control, or a basal diet containing 6 g calcium formate （CF）/kg or 2 g mannan oligosaccharides （MOS）/kg or both 6 g CF/kg plus 2 g MOS/kg. On 21st and 42nd days the pH of the quail crop, proventriculus, gizzard, duodenum, jejunum, ileum, large intestine and ceca were measured in situ and the total aerobic bacteria, lactic acid bacteria, Enterobacteriaceae and coliforms counts of the cecal content were recorded. Results showed that MOS decreased the cecal pH on the 42nd day and the total aerobic bacteria counts on 21st day. Moreover, CF decreased the cecal pH on the 42nd day and the lactic acid bacteria counts on 21st day. Interaction between MOS and CF was noticed on the cecal pH on the 42nd day, as the combined dietary addition of MOS and CF resulted in lower pH, compared to each separate addition. Also, interaction was noticed on the cecal bacterial counts on the 21st day, as the combined dietary addition of MOS and CF resulted in lower total aerobic bacteria, Enterobacteriacea and coliform counts, compared to each separate addition.
