Maximum standardized uptake value on PET/CT in preoperative assessment of lymph node metastasis from thoracic esophageal squamous cell carcinoma

The presence of lymph node metastasis is an important prognostic factor for patients with esophageal *** assessment of lymph nodes in thoracic esophageal carcinoma is essential for selecting appropriate treatment and forecasting disease *** emission tomography combined with computed tomography(PET/CT)is becoming an important tool in the workup of esophageal ***,we evaluated the effectiveness of the maximum standardized uptake value(SUVmax)in assessing lymph node metastasis in esophageal squamous cell carcinoma(ESCC)prior to ***-nine surgical patients with pathologically confirmed thoracic ESCC were retrospectively *** patients underwent radical esophagectomy with pathologic evaluation of lymph *** all had18F-FDG PET/CT scans in their preoperative staging *** had a prior history of *** pathologic status and PET/CT SUVmax of lymph nodes were collected to calculate the receiver operating characteristic(ROC)curve and to determine the best cutoff value of the PET/CT SUVmax to distinguish benign from malignant lymph *** node data from 27 others were used for the validation.A total of 323 lymph nodes including 39 metastatic lymph nodes were evaluated in the training cohort,and 117lymph nodes including 32 metastatic lymph nodes were evaluated in the validation *** cutoff point of the SUVmax for lymph nodes was 4.1,as calculated by ROC curve(sensitivity,80%;specificity,92%;accuracy,90%).When this cutoff value was applied to the validation cohort,a sensitivity,a specificity,and an accuracy of 81%,88%,and 86%,respectively,were *** results suggest that the SUVmax of lymph nodes predicts ***,when an SUVmax of 4.1 was used instead of 2.5,FDG-PET/CT was more accurate in assessing nodal metastasis.

Ultrasound Value in the Management of Parapneumonic Pleural Effusions in a Limited-Resource Setting: A Case Report

Pleural effusion is a common complication of acute lung infection, with rising morbidity and mortality. If poorly treated, parapneumonic effusion evolves to the fibrino-purulent stage wherein antibiotic therapy alone becomes inadequate. Chest CT is the gold standard diagnostic imaging tool, however, in a resource-limited context, it may not be performed. Chest ultrasound can therefore be an alternative for drainage and intermittent follow-up of complicated parapneumonic pleural effusions. We report the case of a 4-year-old child who presented with cough, breathing difficulties and fever for over two weeks and in whom an initial chest X-ray revealed a left hemithorax white-out with an air-fluid level. Chest ultrasound revealed a left pleuropulmonary massive fluid collection with an encysted empyema. It also allowed ultrasound-guided pleural effusion drainage of a fibrino-purulent liquid which tested positive for Kocuria kristinae, a bacterium sensitive to gentamycin, vancomycin, norfloxacin and clindamycin. The next follow-up ultrasound checks showed improvement and the control chest X-ray performed one month later demonstrated pulmonary functional recovery. This case highlights the importance of ultrasound in the management and follow-up of this chest pathology in resource-limited settings.

The amount of data that is traveling across the internet today, including very large and complex set of raw facts that are not only large, but also, complex, noisy, heterogeneous, and longitudinal data as well. Companies, institutions, healthcare system, mobile application capturing devices and sensors, traffic management, banking, retail, education etc., use piles of data which are further used for creating reports in order to ensure continuity regarding the services that they have to offer. Recently, Big data is one of the most important topics in IT industry. Managing Big data needs new techniques because traditional security and privacy mechanisms are inadequate and unable to manage complex distributed computing for different types of data. New types of data have different and new challenges also. A lot of researches treat with big data challenges starting from Doug Laney’s landmark paper, during the previous two decades;the big challenge is how to operate a huge volume of data that has to be securely delivered through the internet and reach its destination intact. The present paper highlights important concepts of Fifty-six Big Data V’s characteristics. This paper also highlights the security and privacy Challenges that Big Data faces and solving this problem by proposed technological solutions that help us avoiding these challenging problems.

Data Migration is a multi-step process that begins with analyzing old data and culminates in data uploading and reconciliation in new applications. With the rapid growth of data, organizations constantly need to migrate data. Data migration can be a complex process as testing must be done to ensure data quality. Migration also can be very costly if best practices are not followed and hidden costs are not identified in the early stage. On the other hand, many organizations today instead of buying IT equipment (hardware and/or software) and managing it themselves, they prefer to buy services from IT service providers. The number of service providers is increasing dramatically and the cloud is becoming the preferred tool for more cloud storage services. However, as more information and personal data are transferred to the cloud, to social media sites, DropBox, Baidu WangPan, etc., data security and privacy issues are questioned. So, academia and industry circles strive to find an effective way to secure data migration in the cloud. Various resolving methods and encryption techniques have been implemented. The relevance of serum albumin among elderly patients with acute decompensated heart failure

Objective To assess the prognostic utility of serum albumin among elderly patients admitted for acute decompensated heart failure(ADHF)in terms of all-cause mortality and also to identify the predictors of *** Retrospective cohort study of 119 elderly patients admitted for *** patients were defined as patients over the age of 65 *** patients were followed up for approximately 11 *** with advanced renal failure,liver disease not due to HF,cancer and other causes of low life expectancy were *** was defined as serum albumin≤2.9 g/*** The study was made up of 65 females and 54 males with age ranging from 65 to 96 *** the 119 elderly patients with ADHF,there were 26 deaths.A significantly higher proportion of patients in the mortality group had an admission serum albumin level of≤2.9 g/dL than those surviving(P=0.011).After Cox’s logistic regression,low albumin(P=0.016),elevated direct bilirubin(P=0.03),age greater than 85(P=0.008),lack of use of beta blockers(P=0.0001)and left ventricular ejection fraction less than 35%(P=0.005)increased the risk of *** serum creatinine(P=0.0357)was the only predictor of hypoalbuminemia following multiple linear *** Hypoalbuminemia may be an unrecognized marker of death in elderly patients with ADHF.

Phytochemical and anti-neuropathic investigations of Crocus sativus via alleviating inflammation, oxidative stress and pancreatic beta-cells regeneration

Objective:The aim of this study is to investigate the phytochemical and the long-term anti-neuropathic potentials of Crocus sativus cultivated in the University botanical garden,and explore its most bioactive compounds and their underlying mechanisms of ***:Phytochemical analysis and bio-guided isolation-procedures including RP-HPLC and1 H and13 C NMR utilizing biological models of diabetes,inflammation,and diabetic-neuropathy were *** saffron(S-RCED)and Spanish-saffron stigma(S-SP)alone or in combination with Camellia sinus(CS)were ***:The RP-HPLC analyses showed the presence of picrocrocin,crocin I,crocin II,crocin I’,crocin II’,and safranal(SAF)in both S-SP and S-RCED extracts with *** had been shown that SAF was the most bioactive-compound in Crocus *** S-SP and S-RCED possessed significant(P<0.05)anti-diabetic activities in acute(6 h),subchronic(8 d)and chronic(8 weeks)models.S-RCED had been proven with more hypoglycemic potentials when compared to S-SP and SAF.S-SP,S-RCED,and SAF produced significant anti-inflammatory and anti-nociceptive activities against carrageenan-induced inflammatory,hyperalgesic and tactile diabetic-neuropathy models,respectively.S-SP,S-RCED,and SAF elevated serum catalase,reduced glutathione,and insulin serum levels,ameliorated lipid peroxidation and Hb A1 c levels,and histopathologically regenerated the pancreatic *** with CS showed more significant efficacy than the single ***:The oxidative stress reduction,insulin secretagogue,and pancreatic beta-cells regeneration potentials might be responsible for the mechanism underlying the anti-diabetic,anti-inflammatory and anti-diabetic neuropathy ***,the cultivated Crocus sativus might be clinically useful for protecting against many serious-disorders.

A comparative analysis of spontaneous fertility after myomectomy by laparotomy associated or not with a“second look”laparoscopy

Objective:The importance of a“second look”laparoscopy for adhesion formation after myomectomy by laparotomy in improving fertility is not clearly defined in our *** aim of this study was to compare spontaneous fertility after myomectomy by laparotomy between women who underwent a“second look”laparoscopy and those who did ***:We conducted an analytical cross-sectional study with retrospective data collection from January 1,2008 to December 31,2015,comparing spontaneous fertility between patients who underwent“second-look”laparoscopy after myomectomy by laparotomy to those who did ***,data was collected from the records of patients included in the ***,through phone calls,informed consent for each participant was obtained after which inquiries were made regarding their fertility ***:After a period of 24 months,23 patients(47.9%)with“second look”laparoscopy conceived,compared to 37 patients(35.2%)without“second look”laparoscopy(p=0.136);and 24 patients(50.0%)with“second look”had term pregnancies compared to 39 patients(37.1%)without“second look”laparoscopy(p=0.134).Conclusion:Spontaneous fertility rates seemed to be improved after a“second look”laparoscopy,however,there was no statistically significant difference between spontaneous fertility rates obtained after myomectomy by laparotomy,irrespective of the fact that the patient had undergone a“second look”laparoscopy or not at Yaounde Gyneco-Obstetric and Pediatric Hospital.
