Gastroesophageal reflux after per-oral endoscopic myotomy:Management literature

In this editorial,we respond to a review article by Nabi et al,in which the authors discussed gastroesophageal reflux(GER)following peroral endoscopic myotomy(POEM).POEM is presently the primary therapeutic option for achalasia,which is both safe and effective.A few adverse effects were documented after POEM,including GER.The diagnostic criteria were not clear enough because approximately 60%of patients have a long acid exposure time,while only 10%experience reflux symptoms.Multiple predictors of high disease incidence have been identified,including old age,female sex,obesity,and a baseline lower esophageal sphincter pressure of less than 45 mmHg.Some technical steps during the procedure,such as a lengthy or full-thickness myotomy,may further enhance the risk.Proton pump inhibitors are currently the first line of treatment.Emerging voices are increasingly advocating for the routine combining of POEM with an endoscopic fundoplication method,such as peroral endoscopic fundoplication or transoral incisionless fundoplication.However,more research is necessary to determine the safety and effectiveness of these procedures in the long term for patients who have undergone them.

A 56-day feeding period was performed to investigate the possible impacts of dietary nano curcumin(0,50,100,150,and 200 ppm)on the growth,nutrient utilization,non-specific immune parameters,antioxidants in Nile tilapia under chronic low temperature(21.02±0.11◦C).Fishes(n=225;Initial weight=4.39±0.08 g/fish)were randomly stocked at 15 fish/tank for five experimental groups in triplicates.Under low-temperature circumstances,dietary curcumin in nano form showed no notable alteration in growth variable,nutrient efficiency,digestive enzymes efficiency,biometric indices,survival rates,and hematological components.Meanwhile,the serum of fishes with nano curcumin diets under low-temperature stress displayed higher total protein as well as lower glucose,cortisol,and total cholesterol compared with the control group.Moreover,fish fed nano curcumin diets displayed higher lysozyme and bactericidal activities compared to the control group and the best performance was found at dietary nano curcumin level of≥100 ppm.Also,groups fed the basal diet demonstrated the poorest antioxidant capacity,and the best superoxide dismutase(SOD)and glutathione peroxidase(GPx)existed in fish with nano curcumin diets while the best catalase(CAT)efficiency occurred at higher nano curcumin levels≥100 ppm.In addition,higher counts of intestinal microbiota in terms of total bacterial count(TBC),total yeast and molds count(TYMC),and coliform were noticed in fish consumed the basal diet compared to groups fed on nano curcumin diets.In conclusion,incorporating nano curcumin at a level of≥100 mg/kg diet(particularly at 150 mg/kg)improved a non-specific immune response,antioxidant,and healthier gastrointestinal microbiota in Nile tilapia under chronic low-temperature stress.

In order to maintain the optimal operating temperature of the battery surface and meet the demand for thermal storage technology,battery thermal management system based on phase change materials has attracted increasing interest.In this work,a kind of core-shell structured microcapsule was synthesized by an in-situ polymerization,where paraffin was used as the core,while methanol was applied to mod-ify the melamine-formaldehyde shell to reduce toxicity and improve thermal stability.Moreover,three different types of heat conductive fillers with the same content of 10 wt.%,i.e.,nano-Al_(2)O_(3),nano-ZnO and carbon nanotubes were added,generating composites.The microcapsules were uniform,and were not affected by the thermal fillers,which were evenly dispersed around.The composite sample with carbon nanotubes(10 wt.%)showed the highest thermal conductivity of 0.50 W/(m K)and latent heat of 139.64 J/g.Furthermore,according to the leakage testing and battery charge/discharge experiments,compared with Al_(2)O_(3)and ZnO,the addition of carbon nanotubes remarkably enhances the heat storage ability as latent heat from 126.98 J/g for the prepared sample with Al_(2)O_(3)and 125.86 J/g for the one with ZnO,then to 139.64 J/g,as well as dissipation performance as a cooling effect by decreasing the sur-face temperature of battery from 2%to 12%of microcapsule,composite sample with carbon nanotubes presents a broad application prospect in battery thermal management system and energy storage field.

Childhood asthma biomarkers including zinc: An exploratory crosssectional study

BACKGROUND Childhood bronchial asthma(BA)is a chronic inflammatory respiratory disease.Nutritional conditions,including zinc deficiency,can affect such allergic disorders.AIM To outline the difference in serum zinc levels between asthmatic children and healthy controls.METHODS A cross-sectional study was carried out at Children’s Hospital,Cairo University,investigating serum zinc levels in children with BA(n=40)and healthy children(n=21).Other markers included serum ferritin,iron,hemoglobin(Hb),and immunoglobulin E(IgE)levels.Independent t-tests and Mann-Whinny tests were used for comparisons.The Kruskal-Wallis test was applied to compare serum ferritin and IgE levels with regard to asthma severity.Spearman's rank correlation was performed to explore the relationship between serum ferritin levels and both iron and Hb levels in asthmatic children.RESULTS Children with BA had higher levels of zinc,yet the difference was not significant(P=0.115).Serum ferritin and IgE levels were significantly higher in asthmatic children(P=0.006 and 0.001,respectively),yet their levels did not differ significantly by severity(P=0.623 and 0.126,respectively).There was a nonsignificant weak correlation between serum ferritin levels and both serum iron and Hb levels.CONCLUSION Serum zinc levels do not seem to differ between asthmatic children and healthy children.Serum ferritin levels may be a marker of asthma control.Serum IgE levels are not markers of asthma severity.

BACKGROUND Helicobacter pylori(H.pylori)infection is a worldwide problem with increasing burden on the health sector due to its increasing rate of resistance.The conventional triple therapy(TT)is becoming obsolete with a high failure rate of eradication,necessitating the need for better alternatives or regimens.AIM To investigate H.pylori eradication rate of TT vs modified bismuth quadruple therapy.METHODS Ninety-two patients with dyspepsia symptoms and positive ^(13)C-urea breath test were randomly assigned to two groups.The first group(control group)was treated for 14 d using standard TT protocol:Esomeprazole(40 mg twice daily),amoxicillin(1 g twice daily)and clarithromycin(500 mg twice daily).On the other hand,the second group was prescribed a 10-d course of modified bismuth quadruple therapy fortified with zinc carnosine:TT in addition to bismuth subcitrate(240 mg twice daily)and zinc carnosine(75 mg twice daily).A repeated 13C-urea breath test was done 4 wk after the completion of the eradication therapy.RESULTS Among the 92 subjects,67.4%were males and 32.6%were females.There were no differences in demographic characteristics(age,body mass index,smoking history,previous antibiotics use and ethnicity)between the modified bismuth quadruple therapy group and TT group.The eradication rate was higher[93.5%(43/46)]in the modified bismuth quadruple therapy group compared to 69.6%(32/46)in the standard TT group(P=0.003).Of the tested predictor variables,only nationality,smoking and therapy type were statistically significant.Besides dizziness,which was recorded in modified bismuth quadruple therapy group,there were no significant differences in side effects between the two groups.CONCLUSION Ten days of modified bismuth quadruple therapy fortified with zinc carnosine is superior to 14 d of conventional TT in eradicating H.pylori infection,with no additional significant adverse events.

Objective: The great saphenous vein (GSV) is commonly used as a conduit for grafting during CABG surgery, and open GSV harvesting (OVH), commonly used with long incision to expose the vein. However, endoscopic vein harvesting (EVH) is an alternative approach, utilizing specialized instruments and small incisions to harvest the vein. Methods: A retrospective analysis was conducted on a cohort of patients who underwent Coronary artery bypass graft (CABG) requiring great saphenous vein (GSV) harvesting which was done by EVH or OVH procedures. Demographic variables, including age and gender, were assessed for both groups. Intraoperative variables such as the number of grafts, cardiopulmonary bypass time, X clamp time, and type of procedure were analyzed. Postoperative variables, including infection and bleeding rates, were also evaluated. Results: The study included 30 patients each undergoing Coronary artery bypass graft (CABG) with need of great saphenous vein harvesting which was done by EVH and OVH. Demographic variables were well-matched between the two groups in terms of age, while a significant difference in gender distribution was observed. Obesity and smoking were more prevalent in the OVH group, and EVH was associated with a higher mean number of grafts compared to OVH. Conversion to an open technique occurred in a portion of the EVH cases, and infection rates did not significantly differ between the EVH and OVH groups. However, the incidence of postoperative bleeding was significantly higher in the EVH group. Conclusion: This study provides valuable insights into the demographic, intraoperative, and postoperative variables associated with EVH and OVH techniques. EVH demonstrated advantages in terms of reduced infection rates compared to OVH. However, the higher incidence of postoperative bleeding associated with EVH raises concerns about potential risks.

Two-Fold and Symmetric Repeatability Rates for Comparing Keypoint Detectors

The repeatability rate is an important measure for evaluating and comparing the performance of keypoint detectors.Several repeatability rate measurementswere used in the literature to assess the effectiveness of keypoint detectors.While these repeatability rates are calculated for pairs of images,the general assumption is that the reference image is often known and unchanging compared to other images in the same dataset.So,these rates are asymmetrical as they require calculations in only one direction.In addition,the image domain in which these computations take place substantially affects their values.The presented scatter diagram plots illustrate how these directional repeatability rates vary in relation to the size of the neighboring region in each pair of images.Therefore,both directional repeatability rates for the same image pair must be included when comparing different keypoint detectors.This paper,firstly,examines several commonly utilized repeatability rate measures for keypoint detector evaluations.The researcher then suggests computing a two-fold repeatability rate to assess keypoint detector performance on similar scene images.Next,the symmetric mean repeatability rate metric is computed using the given two-fold repeatability rates.Finally,these measurements are validated using well-known keypoint detectors on different image groups with various geometric and photometric attributes.

Thermal performance of domed roof in air-conditioned spaces

Due to their thermal performance,domed roofs are one of the passive solutions that affect energy consumption in buildings.The thermal performance of domed roofs has been investigated in many naturally ventilated spaces.However,few studies have discussed their performance in conditioned spaces.Therefore,this study introduces a computational comparison between domed and flat roofs to investigate their impact on thermal comfort inside a conditioned mosque.At an earlier stage,field measurements were carried out inside a Bahraini mosque to acquire its indoor air conditions during the summer period of 2021,in addition to validating the computational model.The findings of this study confirm that,under mechanical cooling conditions,the flat roof offers a lower indoor temperature than the domed roof by 0.4℃and 0.1℃for open and closed doors,respectively.Similarly,the air velocity is lower by approximately 0.01 m/s for both door modes.The overall PMV values of the flat roof are also lower by 0.07 and 0.01,while the PPD values are lower by 0.20,and 0.34 for open and closed doors,respectively.Based on these small differences,it can be concluded that the thermal performance of both roofing systems behaves equally in conditioned spaces.However,the air patterns are substantially different,the overall thermal performance is similar.This similarity drives building designers to rethink the thermal performance of the domed roofs in air-conditioned spaces with such a hot climate,regardless of their aesthetic and acoustical behaviour.

Keypoint Description Using Statistical Descriptor with Similarity-Invariant Regions

This article presents a method for the description of key points using simple statistics for regions controlled by neighboring key points to remedy the gap in existing descriptors.Usually,the existent descriptors such as speeded up robust features(SURF),Kaze,binary robust invariant scalable keypoints(BRISK),features from accelerated segment test(FAST),and oriented FAST and rotated BRIEF(ORB)can competently detect,describe,and match images in the presence of some artifacts such as blur,compression,and illumination.However,the performance and reliability of these descriptors decrease for some imaging variations such as point of view,zoom(scale),and rotation.The intro-duced description method improves image matching in the event of such distor-tions.It utilizes a contourlet-based detector to detect the strongest key points within a speciﬁed window size.The selected key points and their neighbors con-trol the size and orientation of the surrounding regions,which are mapped on rec-tangular shapes using polar transformation.The resulting rectangular matrices are subjected to two-directional statistical operations that involve calculating the mean and standard deviation.Consequently,the descriptor obtained is invariant(translation,rotation,and scale)because of the two methods;the extraction of the region and the polar transformation techniques used in this paper.The descrip-tion method introduced in this article is tested against well-established and well-known descriptors,such as SURF,Kaze,BRISK,FAST,and ORB,techniques using the standard OXFORD dataset.The presented methodology demonstrated its ability to improve the match between distorted images compared to other descriptors in the literature.
