Addition of Insulin to Treatment Regimen in Type 2 Diabetes: The Significance of Age and Duration among Sudanese Diabetic Patients

Background: Type 2 diabetes is a progressive disease and as the result of patient’s age and diabetes duration, β-cell ability for insulin secretion deteriorates continually and finally fails to meet body requirement. Intensification of treatment drug regimen including addition of insulin is critical to maintain glycemic target and reduce developing of long-term complications. Objective: The main purpose of this study was to investigate the effect of diabetes duration and patients’ age on addition of insulin to treatment regimen. Methods: A single centre, cross-sectional hospital-based study was done among 351 diabetes Mellitus (DM) patients visiting Ribat University Hospital Diabetic Clinic, Khartoum State, Sudan. Data was collected using constructed, validated and multiple-choice questionnaire. Results: From 351 patients enrolled in the study, female were 65.8% and male 34.2%. About 64.4% were on oral anti-diabetic;35.6% on insulin;about 31.6% shifted to insulin when their ages above 40 years;and 22.8% when diabetes duration was above 5 years. Co-morbidity was 54.4% and 60.2% of patients had hypertension. Conclusion: Interactions between age and longer duration of diabetes were mostly significant reasons for transferring patients to insulin. Percent of insulin users and co-morbidity was within international range.

Assessing Knowledge of Hypoglycemia Symptoms among Type 2 Diabetic Patients Using Insulin, Khartoum, Sudan

Background: Hypoglycemia is a traumatic dilemma that has a critical impact on not only the patient’s quality of life but also their families and communities. Improving Patient’s awareness and knowledge regarding hypoglycemia symptoms is crucial to avoid serious implications like loss of consciousness and even death. Objective: This study aimed to assess patients’ knowledge of hypoglycemia symptoms and source of diabetes information among insulin users type 2 diabetes. Methods: A single centre, hospital-based, analytical, and cross-sectional study was carried out among diabetic patients using insulin attended to Ribat University Hospital Diabetes Clinic (RUHDC), Khartoum State, Sudan during a period of study. A validated questionnaire was used to obtain socio-demographic characteristics of respondents and the patients’ knowledge regarding hypoglycemia symptoms was assessed whether good, medium or poor. Data were analyzed, using (SPSS). Descriptive (frequencies) and inferential statistics (association used chi-square) were used at a level of confidence p ≤ 0.05. Results: A majority of patients (52%) had poor knowledge of hypoglycemia symptoms and the common symptoms known to patients were hands tremor (67.2%), blurred vision (52.8%) and sweating (65.6%). No associations were found among hypoglycemia symptoms knowledge, socio-demographic and diabetes duration. Only (25.6%) had regular visiting to educating center and physicians were the major source of diabetes knowledge (74.4%). Conclusion: This study proved low patients hypoglycemia symptoms knowledge and physician were the main sources of information about diabetes at RUHDC. Further ideas are required in collaboration and integrated approach to filling the gap of patients’ awareness and multidisciplinary team in diabetes management.

Renewable and Storage Energy Integration for Smart Grid Housing

This paper presents the design, implementation and testing of an embedded system that integrates solar and storage energy resources to smart homes within the smart mierogrid. The proposed system provides the required home energy by installing renewable energy and storage devices. It also manages and schedules the power flow during peak and off-peak periods. In addition, a two-way communication protocol is developed to enable the home owners and the utility service provider to improve the energy flow and the consumption efficiency. The system can be an integral part for homes in a smart grid or smart microgrid power networks. A prototype for the proposed system was designed, implemented and tested by using a controlled load bank to simulate a scaled random real house consumption behavior. Three different scenarios were tested and the results and findings are reported. Moreover, data flow security among the home, home owners and utility server is developed to minimize cyber-attaeks.

Antiulcer Activities of Commiphora molmol (Myrrh) Extract in Male Rats

Medicinal plants used in folk medicine contain a wide range of substances that can prevent and treat many diseases. The Antiulcer Activities effects of myrrh or Commiphora molmol extract (CME) were assessed in rats. In this experiment, thirty rats were divided into 6 groups as follows: G1) normal control (vehicle);G2) received aspirin without any treatment;G3) pretreated with Omeprazole (antiulcer drug) and groups (4), (5) and (6) were pretreated with CME at 125, 250 and 500 mg/kg ***, respective-ly for 2 weeks. At the end of experiment the volume, pH and total acidity of gastric juices;mucus content and ulcer lengths were measured and protection percentages were calculated. Gross and histopathological examinations of stomachs were also performed. CME induced an antiulcer effect manifested by decreased volume and total acidity of gastric juice and increased mucus content and percentages of protec-tion from ulcer as well as partial amelioration of gross and histopathological lesions seen in stomach of ulcerated rats. In conclusion, the results denote that Commiphora molmol extract possess antiulcer effects in rats. These results affirm the traditional use of Myrrh extract for the treatment of gastric ulcer.

Diverse models have been proposed for the role of the Tarim Craton within the Paleoproterozoic Columbia supercontinent assembly. Here we report a suite of-1.71 Ga Nb-enriched meta-gabbro lenses in the eastern Quanji Massif, within the Tarim Craton in NW China. The meta-gabbroic rocks have Nb contents of 11.5-16.4 ppm with Nb/La ratios varying from 0.84 to 1.02((Nb/La)_N = 0.81-0.98) and Nb/U ratios from 38.0 to 47.2. They show low SiO_2(45.1-48.5 wt.%) and MgO(5.96-6.81 wt.%) and Mg#(Mg# = Mg/(Mg + Fe) = 43.5-47.7), high FeO^t(13.0-15.7 wt.%) and moderate Ti02(1.70-2.51 wt.%).with tholeiitic affinities. These rocks possess low fractionated REE patterns without obvious Eu anomalies(Eu/Eu~* = 0.87-1.02). Their primitive mantle-normalized elements patterns display significant Zr-Hf troughs, positive Nb anomalies, weak negative Ti and P anomalies, and high contents of Rb and Ba,resembling Nb-enriched basalts generated in arc-related tectonic settings. Their arc-like geochemical signatures together with whole rock εNd(t) values of 0.4-2.1 and corresponding old T_(DM)(2.22-2.37 Ga)as well as(^(143)Nd/^(144)Nd)_t and(^(87)Sr/^(86)Sr)t(t = 1712 Ma) values of 0.5104-0.5105 and 0.7030-0.7058,respectively, suggest that their precursor magma originated from mantle wedge peridotite metasomatised by subduction-derived melts. The results from our study reveal subduction along the eastern periphery of the Tarim Craton and marginal outgrowth continuing to ~1.7 Ga within the Columbia supercontinent.

Bacterial Deterioration in the Limestone Minaret of Prince Muhammad and Suggested Treatment Methods, Akhmim, Egypt

el-Amir Muhammad’s minaret in Akhmim, Sohag, Egypt, is constructed of limestone and has been exposed to many factors of damage as a result of the high levels of ground water. Limestone is strongly affected by ground water, especially when being impure. The current work discusses the results of analytical techniques including chemical testing to determine the types of soluble salts through optical microscopy, electronic scanning electron microscope with an X-ray energy dispersion system (ESEM) to study and determine the causes of rapid degradation. Microbial weathering phenomena toward limestone were also studied. Different bacteria and fungi were isolated from outdoors and indoors of air and limestone of the building of which Bacillus cereus OK447647, B. subtilis OK447648, Serratia marcescens OK447650, Pseudomonasoryzihabitans OK447649, Aspergillus flavus, A. niger, Penicillium chrysogenum and Cladosporium cladosporoids were the most representative. B. cereus OK447647 and B. subtilis OK447648 have shown ability for calcium carbonate dissolving. The minimal inhibitory concentrations (MICs) of sodium azide were investigated against the growth of microbial isolates. Sodium azide at 100 ppm was found to be the best treatment for bacterial isolates although it had no significant effect against fungi.

Susceptibility of Newly Introduced Potato Cultivars to Libya to Infection with Bacterial Soft Rot and the Associated Physiological Changes

Tuber slices of newly introduced potato cultivars （cvs） in Libya, namely Spunta, Pamela, Daisy, Daifla, Atlas and Apollina were tested for their susceptibility to soft rot bacterium （Erwinia carotovora subsp, carotovora）. Atlas cv was proved to be the most susceptible cv, meanwhile, Daisy cv was the most resistant one. The activities of pectolytic and oxidative enzymes in diseased tubers were profoundly higher than these in healthy ones. The highest enzymatic activities of pectin methyl esterase （PME） and polygalacturonase （PG） were observed in diseased tubers of Daifla cv. However, diseased tubers of Spunta gave the highest activities of polymethyl galacturonase （PMG）. On the other hand, the highest significant activities of the enzyme PME in healthy tubers was detected in Pamela cv compared to other cvs. Moreover, there were insignificant differences in PG and PMG enzymes activities between healthy tubers of the different cvs. The cv Daisy gave the highest activity of the enzyme peroxidase （PO） in diseased tubers, whereas the highest activity of enzyme polyphenol oxidase （PPO） was detected in infected tubers of Atlas cv. Concerning healthy tubers, the highest activities of enzymes PO and PPO were noticed in Atlas and Pamella cvs respectively as compared to other cvs. electrolyte leakage from plant cells was estimated by measuring electrical conductivity as indicator for permeability changes in potato tissues. Values of electrolyte leakage in infected tubers of all tested cvs showed significant increase compared with those of healthy ones, especially, with the most susceptible one.
