Prolonged existence of SARS-CoV-2 RNAs in the extracellular vesicles of respiratory specimens from patients with negative reverse transcription-polymerase chain reaction

Background and aim:Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is primarily in the respiratory tract,particularly in patients with underlying *** study aimed to investigate the presence of the virus inside the extracellular vesicles(EVs)in patients with and without chronic liver disease(CLD).Methods:Eighty patients with positive SARS-CoV-2,including twenty-four patients with CLD and fiftysix patients without CLD,and five healthy controls with negative SARS-CoV-2 were *** swab specimens were tested for the detection of SARS-CoV-2 using reverse transcription-polymerase chain reaction(RT-PCR).Patients with coronavirus disease 2019(COVID-19)were followed up on days 7 and *** swab,collected in viral transport media(VTM),and plasma samples were investigated at each time *** were isolated from the nasal swabs(collected in VTM)and plasma using differential ultracentrifugation and estimated at each time *** transmission or replication by the EVs was assessed in Vero E6 ***:In patients with baseline RT-PCR positive,SARS-CoV-2 RNAs inside the EVs were found in 68/80(85%)patients with higher viral load in the nasal swabs than in the EVs(cycle threshold(Ct)value,23.4±5.7 vs.30.3±5.0,P<0.001).On follow-up at day 7,of the 32 patients negative for COVID-19,15(46.9%)had virus persistence in the EVs(Ct value,30.7±2.7),and on day 14,of the 56 patients with negative SARS-CoV-2,16 patients(28.6%)had positive SARS-CoV-2 RNAs in the EVs(Ct value,31.4±3.0).The mean viral load decreased on days 7 and 14 compared to baseline in the nasal swabs(P<0.001)but not in the ***,SARS-CoV-2 RNAs were undetectable in the plasma,but 12.5% of patients were positive in the plasma *** prolonged and high viral load was found in the EVs on day 14 in COVID-19 patients combined with CLD compared with COVID-19 patients(P?0.0004).We found significant higher levels of EV-associated with endothelial cells and hepatocytes

在印度次大陆的亚热带地区,秋天冷而干燥,春天湿润。变温性果蝇所具有的抗干燥性有助于其度过较为干旱的气候条件。Drosophila jambulina具有体色二型性。已有研究表明,随湿度变化,***热带种群始终保持体色多型性,这与热条件下体色黑化相反,且该热带物种中体色分化频率随季节性变化,这符合黑化-干燥假说。但是两种色型的***产生这类气候适应的机理尚不明了。为了检验干燥相关性状生理基础的分化与对气候条件的色型特异性适应相关这一假说,我们利用分别在17℃和25℃、低湿(40%RH)和高湿(80%RH)条件下饲养获得的两种色型的***,检测了其水分平衡对相对湿度、温度、及温湿度相互作用的反应。我们发现,在低相对湿度下,两种温度下饲养的深色型果蝇的生理和脱水性状数值显著高于浅色型。对两种色型果蝇的水分收支情况进行的比较分析表明,在低相对湿度下,深色型果蝇的含水量较高、水分损失率较低、抗脱水能力较强,使其具有更强的抗干燥性。在干燥胁迫过程中,两种色型的果蝇均以碳水化合物作为代谢燃料,但是在低湿条件下,深色果蝇中贮存碳水化合物的含量明显要高。而且,在两种湿度条件下,这两种色型果蝇之间的总能量收支显著不同。据此认为,***的水分平衡相关性状表现出的色型特异性分化与其对湿热生境的适应相关。

Non-Operating Room Anesthesia and the COVID-19 Patient: Evidence Based Strategies

As non-operating room anesthesia (NORA) vastly increases, so does the complexity of both the patients and the procedures. Unfamiliar environment, remote locations, and limited access to resources are some of the many challenges faced by the anesthesiologist in this environment. Understanding the environmental factors such as the room layout, machine placement and patient positioning needs advanced planning in the COVID-19 patient to determine airway management, intravenous access and overall patient safety. Communication with the proceduralist and healthcare workers (HCW) is of essence as both the procedure and anesthetic requirements may need to be altered as the case progresses. Standard monitoring guidelines should apply to all patients regardless of depth of anesthesia. The COVID-19 patient adds to these challenges. Aerosol generating medical procedures (AGMP’s) need to be identified ahead of time in order to ensure the safety of both the staff and patients. This allows for planning and preparation required for transportation of the patient to the room. HCWs may need to be identified and be available to assist with the procedure. Time should be allocated for transportation, briefing, room preparation, personal protective equipment needs of the staff and donning and doffing procedures as per institutional protocol. Communication means with staff outside of the room should be discussed in advance should an emergent situation arise during the procedure. Debriefing should be performed at the end of every procedure to improve patient safety and outcomes.

Tuberculosis in kidney transplant recipients:A case series

Solid organ transplant recipients have an elevated risk of tuberculosis(TB) with high mortality. Data about TB in this population in the United States is sparse. We present four cases of active tuberculosis in kidney transplant recipients at our center. All patients had possible TB exposure prior to transplant and all were diagnosed with active TB within the first year of transplant. Disseminated TB was seen in half of the patients with extra-pulmonary TB being more common affecting lymph nodes, pericardium, and the kidney allograft. Delay in diagnosis from onset of symptoms ranged from fifteen days to two months. Two patients died from TB. TB is a largely preventable and curable disease. However, challenges remain in the diagnosis due to most recipients presenting with atypical symptoms. Physicians should maintain a high degree of suspicion for TB to promptly diagnose and treat posttransplant thereby minimizing complications. A review of the literature including the epidemiology, pathogenesis, clinical presentation, diagnosis and treatment options are discussed.

Delonix regia （Bojer ex Hook） Raffin （Fabaceae）, also known as flame of forest, is a semi-deciduous tree, distributed throughout Madagascar, India, Africa, and Northern Australia. Various parts of the plant are traditionally used for the treatment of different ailments such as inflammation, rheumatism, bronchitis, diabetes, anemia, fever, gynecological disorders, and pneumonia. The plant possess antioxidant, hepatoprotective, gastroprotective, wound healing, antiarthritic, larvicidal, antimalarial, antiemefic, antibacterial, antifungal, antiinflammatory, analgesic, antidiarrhoeal, antiheamolytic, diuretic, and anthelmintic activities. This review is an up-to-date compilation on its traditional uses in context to phytochemical and pharmacological perspectives.

目的:核黄素角膜胶原交联早期治疗轻度至中度圆锥角膜的预后。方法:三级眼科诊疗中心的前瞻性研究。共38例47眼轻至中度圆锥角膜接受核黄素角膜胶原交联治疗的患者纳入本研究。术前数据包括参与眼数,视力,眼压,角膜厚度与角膜地形图。术后数据包括最佳矫正视力,眼压,角膜厚度,角膜地形图和术后并发症。结果:研究包含年龄16~30岁的患者38例47眼。平均术前视力为0.58±0.40 logM AR,术后随访2a显著提高(0.40±0.27logM AR)(P=0.005)。平均术前角膜曲率为50.5±4.6 D,术后显著降低。随访2a平均角膜曲率为48.2±4.1 D(P=0.011)。术后随访2a眼压(15.1±3.0mmH g)较术前(12.9±2.5 mmH g)显著增加(P=0.035)。术前角膜厚度467.9±38.8μm,术后随访2a(465.0±39.3μm)明显降低。所有患者均无并发症出现。结论:早期使用核黄素角膜胶原交联治疗轻至中度圆锥角膜可获得长期的良好视力,而不必等其发展为进展期。

Performance evaluation of NeuMoDx 96 system for hepatitis B and C viral load

BACKGROUND Hepatitis B virus(HBV)and hepatitis C virus(HCV)viral load(VL)estimation is essential for the management of both HBV and HCV *** to a longer turnaround time for VL estimation,many patients drop out from the cascade of *** achieve the global goals of reducing morbidity and mortality due to HBV/HCV and moving towards their elimination by 2030,molecular diagnostic platforms with faster and random(*** sample)access are *** To evaluate the performance of the recently launched NeuMoDx 96 random access system with the conventional COBAS^(■)AmpliPrep/COBAS TaqMan system for HBV and HCV VL *** Archived once-thawed plasma samples were retrieved and tested on both *** between the assays was determined by linear regression and Bland-Altman *** study included samples from 186 patients,99 for HBV of which 49 were true infected HBV cases(hepatitis B surface antigen,antihepatitis B core antibody,and HBV DNA-positive)and 87 for HCV assay in which 39 were true positives for HCV infection(anti-HCV and HCV RNA-positive).RESULTS The median VL detected by NeuMoDx for HBV was 2.9(interquartile range[IQR]:2.0-4.3)log_(10)IU/mL and by COBAS it was 3.70(IQR:2.28-4.56)log_(10)IU/mL,with excellent correlation(R2=0.98).In HCV,the median VL detected by NeuMoDx was 4.9(IQR:4.2-5.4)log_(10)IU/mL and by COBAS it was 5.10(IQR:4.07-5.80)log_(10)IU/mL with good correlation(R2=0.96).CONCLUSION The overall concordance between both the systems was 100%for both HBV and HCV VL ***,no genotype-specific bias for HBV/HCV VL quantification was seen in both the *** findings reveal that NeuMoDx HBV and HCV quantitative assays have shown overall good clinical performance and provide faster results with 100%sensitivity and specificity compared to the COBAS AmpliPrep/COBAS TaqMan system.
