Objective: This multicenter clinical study was to assess the clinical usability of an oral moisture-checking device in detecting the dry mouth patients and evaluating the optimal measurement site. Materials and Methods: The study group comprised 250 patients with dry mouth and 241 healthy volunteer subjects at 13 medical centers. This device was used to measure the moisture degrees of the lingual mucosa and the buccal mucosa. Subjective oral dryness, objective oral dryness, and saliva flow rates were also compared between the two groups. For statistical analysis, receiver-operating characteristic analysis was performed to calculate the area under the curve (AUC). Results: The moisture degree of the lingual mucosa was significantly lower in the dry mouth group (27.2 ± 4.9) than that in the healthy group (29.5 ± 3.1, AUC = 0.653). When a lingual mucosa moisture degree of 31.0 or higher was defined as normal, less than 27.0 as dry mouth, and 27.0 to less than 31.0 as borderline zone of dry mouth, both the sensitivity and the specificity for the diagnosis of dry mouth were close to 80%. Conclusion: These results suggest that the oral moisture-checking device is a usable screening device for dry mouth.

Background: Although exercise echocardiography(EE) is not clearly indicated in patients with normal electrocardiogram(ECG) as the first evaluation, there is a lack of data regarding its superiority over the Duke score for prognosis. We investigate whether EE has incremental value over the Duke score for predicting outcome in patients with normal ECG. Methods: One thousand six hundred forty-seven patients with interpretable ECG referred for EE were followed up for 2.5±1.4 years. There were 58 hard events(myocardial infarction or cardiovascular death). Results: There were 38 events in 735 patients with abnormal EE versus 20 events in 912 with normal EE(P< .0001). The Duke score, resting wall motion score index, and ischemia were independently associated to events(incremental P value of EE=.03). The Duke score allowed stratification of patients with abnormal EE(P=.001) or ischemia(P=.01) into different risk categories but did not stratify patients without these characteristics. Exercise echocardiography variables stratified patients with the low Duke score(left anterior descending artery territory P=.04, left anterior descending artery ischemia P=.03) and with the intermediate Duke score(abnormal EE P=.005, necrosis P=.0009, ischemia P=.004, resting ejection fraction P< .00001, resting wall motion score index P< .00001, peak ejection fraction P< .00001, peak wall motion score index P< .0001, number of territories P=.002, left anterior descending artery territory P=.001, and left anterior descending artery ischemia P=.002) but did not with the high Duke score. Conclusions: Exercise echocardiography has incremental value over clinical variables, the Duke score, and resting echocardiography for the prediction of hard cardiovascular events in patients with normal resting ECG.

Neurocognitive Disorders in Patients with HIV Infection with Virologic Suppression for More than 10 Years

Background: HIV-associated neurocognitive disorder (HAND) may appear in patients with viral and immunological response to treatment and remain unnoticed during the initial stage of the infection. The goal of this study is to evaluate the development and/or progression of HAND in patients with undetectable viral load for more than ten years. Methods: We included adult HIV-infected patients who were under antiretroviral treatment and had undetectable plasma viral load for more than ten years (blips were included). These patients had already been subjected to neurocognitive evaluation five years previously. Demographic, clinical and analytical data were analysed. For the neurocognitive evaluation, the WAIS-III subtests (digit symbol coding and symbol search), trail making test (TMT) A and B, Stroop test and categorical verbal fluency (animals) tests were used. SPSS??version 22.0 for Windows was used for statistical analysis. Results: In this re-evaluation, performed 4.76 (±1.82) years after the first one, 9 (36%) patients showed deficits in processing speed (WAIS-III and TMT A), 8 (32%) executive function (TMT B and Stroop) and 12 (48%) verbal fluency. There were significant statistical differences between the past and current executive function tests (p = 0.029 and p = 0.01), highlighting worsening of deficits. No differences were found for the other tests. No association was found between deficit progression and the studied variables. Conclusions: Although not generally noticed on regular appointments, in this small population, worsening of executive function deficits (mental flexibility and divided attention) was found. Classical risk factors for HAND did not appear to interfere in its progression. Speed of information processing and categorical verbal fluency remained stable.
