Refractory ulcerative colitis accompanied with cytomegalovirus colitis and multiple liver abscesses: A case report

Various hepato-biliary complications are an increased incidence in patients with inflammatory bowel disease,and portal bacteremia is well documented in patients with ulcerative colitis (UC). However, few reports mention UC in association with liver abscesses. Recently, there are several reports describing cytomegalovirus (CMV) infection in association with disease exacerbation and steroid refractoriness in patients with UC. Here we present a case of refractory UC accompanied with multiple liver abscesses and CMV colitis. The patient, a 72-year-old male, with a five-year history of repeated admissions to our hospital for UC, presented with an exacerbation of his UC.Sigmoidoscopy performed on admission suggested that his UC was exacerbated, then he was given prednisolone and mesalazine orally, and betamethasone enemas.However, he had exacerbated symptoms. Repeat sigmoidoscopy revealed multiple longitudinal ulcers and pseudopolyps in the rectosigmoid colon. Although immunohistochemical staining of biopsy specimens and the serum testing for antigenemia were negative on admission and after the repeat sigmoidoscopy, they became histologically positive for CMV. Nonetheless, the patient developed spiking fevers, soon after ganciclovir was administered. Laboratory studies revealed an increased white cell count with left shift, and Enterococcus fecalis grew in blood cultures. An abdominal computed tomography (CT) scan was obtained and the diagnosis of liver abscesses associated with UC was made, based on CT results. The hepatic abscesses were successfully treated with intravenous meropenem for 6 wk, without further percutaneous drainage. To our knowledge, this is the first reported case of multiple liver abscesses that develop during UC exacerbation complicated by CMV colitis.

Syngeneic implantation of mouse hepatic progenitor cell-derived three-dimensional liver tissue with dense collagen fibrils

BACKGROUND Liver transplantation is a therapy for irreversible liver failure;however,at present,donor organs are in short supply.Cell transplantation therapy for liver failure is still at the developmental stage and is critically limited by a shortage of human primary hepatocytes.AIM To investigate the possibility that hepatic progenitor cells(HPCs)prepared from the portal branch-ligated hepatic lobe may be used in regenerative medicine,we attempted to enable the implantation of extracellular matrices containing organoids consisting of HPC-derived hepatocytes and non-parenchymal cells.METHODS In vitro liver organoid tissue has been generated by accumulating collagen fibrils,fibroblasts,and HPCs on a mesh of polylactic acid fabric using a bioreactor;this was subsequently implanted into syngeneic wild-type mice.RESULTS The in vitro liver organoid tissues generated transplantable tissues in the condensed collagen fibril matrix and were obtained from the mouse through partial hepatectomy.CONCLUSION Liver organoid tissue was produced from expanded HPCs using an originally designed bioreactor system.This tissue was comparable to liver lobules,and with fibroblasts embedded in the network collagen fibrils of this artificial tissue,it is useful for reconstructing the hepatic interstitial structure.

Recurrent Ectopic Pregnancy in the Remnant Fallopian Tube Following Ipsilateral Partial Salpingectomy

A 39-year-old female with a history of partial salpingectomy for tubal pregnancy was diagnosed as having recurrent ectopic pregnancy in the distal portion of the fallopian tube remnant, which was successfully treated by laparoscopic surgery. The patient was multigravida (9 pregnancies) and uniparous. She had undergone right partial salpingectomy by laparotpmy for right isthmic ectopic pregnancy at the age of 31 years. At 6 weeks of the current pregnancy, she was referred to our hospital for suspected ectopic pregnancy. The gestational sac was not observed in the uterus, and a mass was observed in the right adnexal region by transvaginal ultrasonography. Emergency laparoscopic surgery revealed the pregnancy site in the ampulla of the remnant portion of the right fallopian tube;therefore, this portion was resected. Because the proximal portion of the fallopian tube remnant was completely occluded, we concluded that this was a case of ectopic pregnancy resulting from the intraperitoneal migration of a fertilized ovum. With current developments in assisted reproductive technologies, reanastomosis of the fallopian tube is rarely performed. While partial salpingectomy is less likely to contribute to the preservation of fertility, it increases the risk of recurrent ectopic pregnancy. A single-stage total salpingectomy on the affected side should be the first choice of treatment when fallopian tube preservation surgery is not selected.

Distribution of Initial Vegetation Recruitment on Bare Bar in Sand Bed River

Riparian vegetation is known to affect the flood flow and the riparian environment, and it is important for river engineers to know the vegetation dynamics in river. Despite a number of researches have been performed for vegetation dynamics, the mechanism of initial vegetation recruitment is still not clear. In this study, two field surveys by using Unmanned Aerial Vehicle (UAV) remote sensing, UAV aerial photographs analysis and a numerical simulation were conducted to detect the location of initial vegetation recruitment and explore its relationship with hydrology and river morphology. The initial vegetation recruitment was identified by comparing vegetation distributions before and after vegetation germination. Most of the initial vegetation recruitment locations were found either along dunes on sandbar or along the wet/dry rotational area of shore bank. Results of the aerial photographs analysis showed that the initial vegetation recruitment occurred at the downstream side of crest of dunes. As for the vegetation recruitment zone located along shore bank, the relative elevation of different recruitment zones is almost same, and the mean value of the relative elevation is around 0.33 m. With the comparison of the contour map of river morphology and the initial vegetation recruitment zone, the shape of initial vegetation recruitment zone is positively consistent with the shape of river morphology distribution. The results of numerical analysis show that the initial vegetation recruitment zone located along shore bank experienced intermediate flood pulse. The inundation frequencies during seed dispersal and germination period for the internal and external boundaries of vegetation recruitment zone are 23.87% and 6.2%, respectively.

BACKGROUND Hypoglycemia due to non-insulin-producing tumors is referred to as non-islet cell tumor hypoglycemia(NICTH).As NICTH is a rare lesion,the natural course of NICTH is not well understood.We report a case of NICTH that was observed 30 years before the onset of hypoglycemia.CASE SUMMARY A 50-year-old man was diagnosed with an abnormal right chest shadow during a routine X-ray examination,but no further examination was undertaken because the lesion appeared benign.Thirty years after the tumor discovery,the patient was admitted to the hospital with symptoms of severe hypoglycemia,which was diagnosed as NICTH based on a complete examination.The tumor was resected and found to be a solitary fibrous mass(15.6 cm×13.7 cm×10.4 cm);thereafter,the patient’s blood glucose levels normalized and he completely recovered.CONCLUSION NICTH can have an acute onset,even if the tumor has been present and asymptomatic over a long time period.

Unusually Broadened Spectral Profiles Observed in Solar Prominences

After surveying the spectra of 39 prominences observed by the Multi-channel Infrared Solar Spectrograph at Purple Mountain Observatory and the 25 cm coronagraph at the Norikura Solar Observatory, we found that about 28% of them show small spatial scale (6''–8'') and short time scale (tens of seconds to a few minutes), unusual large broadening and large shift velocities in spectral lines including Hα, Hβ, H, Ca II H, Ca II K, Ca II 8542?, He I D3 and He I 10830?. We present in detail two typical events observed respectively on 2002 May 27 and 1981 August 2. The full-width at half maximum of the widest profile of the 2001 prominence is 1.8? for Hα and 2.9 ? for He I 10830?, while that of the 1981 prominence is 5.3 ? for Hβ, 3.6 ? for Ca II K, 4.0 ? for Ca II H and 2.8 ? for He I D3. Such broadenings generally occur at a level of several-thousand kilometres above the chromosphere. Further, most of these prominences manifest a rotation of (0.4–1.35)×10-2 rads s?1 pointing to the Sun and large line-of-sight velocities of 20–200 km s?1. Some of these events correspond in time to an enhancement or a small peak in the GOES X-ray flux, indicating the existence of high energy process at work. These prominences generally display discernible changes in the Hα morphology around the time of large broadening, but do not show Hα brightening or overall eruption except for a few small surge-like events, hence, they are hardly observed in daily Hα patrols. According to the characteristics of their Hα structures and spectral properties, we infer these events are small-scale eruptions similar to nano-flares, which may contribute to the mass and energy transported into the corona. Large turbulent velocities of 25–120 km s?1 are responsible for the observed broadenings.

Spindle Cell Hemangioma and Decorin Expression

Spindle cell hemangioma (SCH) is a rare benign vascular tumor and we report 2 cases of SCH who are 50-year-old man and 16-year-old girl. A growing amount of evidence indicates that multifunctional ECM molecule decorin regulate endothelial cell-matrix interactions during angiogenesis. Furthermore, a current publication shows that no detectable decorin expression was found in the stromas of tumor nests in Kaposi’s sarcoma or angiosarcoma groups while the expression was clearly observed in the connective tissue stromas within benign vascular tumors. SCH exhibits spindle cell proliferation and resembles Kaposi’s sarcoma. Therefore, this study examined decorin expression in the tissues of our SCH cases by immunohistochemical analysis. The results demonstrated that immunoreactivity of decorin was clearly detectable in the connective tissue stroma within tumor nests. SCH was first perceived as sarcoma because of spindle cell proliferation, but this proliferation was subsequently found to be reactive. The presence of decorin expression in our cases also indicates that SCH have benign properties. Moreover, this study further increases the reliability of examination of decorin expression for differentiation between benign and malignant vascular tumors.
