The impact of intensity modulated radiotherapy on the skin dose for deep seated tumors

Objective: The purpose of this study was to investigate the impact of intensity modulated radiotherapy (IMRT) on surface doses for brain, abdomen and pelvis deep located tumors treated with 6 MV photon and to evaluate the skin dose calculation accuracy of the XIO 4.04 treatment planning system. Methods: More investigations for the influences of IMRT on skin doses would increase its applications for many treatment sites. Measuring skin doses in real treatment situations would reduce the uncertainty of skin dose prediction. In this work a pediatric human phantom was covered by a layer of 1 mm bolus at three treatment sites and thermoluminescent dosimeter (TLD) chips were inserted into the bolus at each treatment site before CT scan. Two different treatment plans [three-dimensional conformal radiation therapy (3DCRT) and IMRT] for each treatment sites were performed on XIO 4.04 treatment planning system using superposition algorism. Results: The results showed that the surface doses for 3DCRT were higher than the surface doses in IMRT by 1.6%, 2.5% and 3.2% for brain, abdomen and pelvis sites respectively. There was good agreement between measured and calculated surface doses, where the calculated surface dose was 15.5% for brain tumor calculated with 3DCRT whereas the measured surface dose was 12.1%. For abdomen site the calculated surface dose for IMRT treatment plan was 16.5% whereas the measured surface dose was 12.6%. Conclusion: The skin dose in IMRT for deep seated tumors is lower than that in 3DCRT which is another advantage for the IMRT. The TLD readings showed that the difference between the calculated and measured point dose is negligible. The superposition calculation algorism of the XIO 4.04 treatment planning system modeled the superficial dose well.

Background: For decades, traditional open surgical techniques were used to treat lumbar disc herniation and lumbar canal stenosis (LCS). However, seeking for better outcomes for patients and avoiding extensive bony loss with its sequences had raised minimally invasive technique for treating these disorders as an alternative surgery. Methods: This is a retrospective study in which 54 patients of LCS were operated upon via unilateral minimally invasive technique to decompress the canal in a 360 degrees fashion through laminotomy, deroofing of opposite laminar side, sublaminar ligamintectomy, bilateral foraminotomies and discectomy. We used VAS scores and ODI to assess clinical outcomes with a period of one year follow-up. Results: Our results demonstrated that minimally invasive techniques for treating these disorders are effective procedures. Minimally invasive 360 degrees decompression for treating LCS had better outcomes regarding postoperative back pain, smaller incisions, less bony loss and early ambulation. Conclusion: Minimally invasive techniques for treating lumbar canal stenosis of different causes could be considered a better option instead of traditional full laminectomy with better outcomes as regards respecting the anatomical layers such as posterior spinal integrity and musculature, postoperative pain, accompanied with less blood loss, shorter hospital stays, and shorter recovery periods.

An annular aortic abscess is a fatal complication of infective endocarditis. Echocardiography is the initial imaging modality to confirm the diagnosis in suspected infective endocarditis. Here, we present a case of a bicuspid aortic valve infective endocarditis caused by Staphylococcus aureus and complicated with annular aortic abscess resulting in acquired Gerbode defect (type II) with tricuspid valve vegetation which was undiagnosed preoperatively. The intraoperative transoesophageal echocardiography yields a new finding prior to the surgical incision, which impacted the clinical decision-making and increased the burden of the procedure.

AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

The identification of control chart patterns is very important in statistical process control. Control chart patterns are categorized as natural and unnatural. The presence of unnatural patterns means that a process is out of statistical control and there are assignable causes for process variation that should be investigated. This paper proposes an artificial neural network algorithm to identify the three basic control chart patterns;natural, shift, and trend. This identification is in addition to the traditional statistical detection of runs in data, since runs are one of the out of control situations. It is assumed that a process starts as a natural pattern and then may undergo only one out of control pattern at a time. The performance of the proposed algorithm was evaluated by measuring the probability of success in identifying the three basic patterns accurately, and comparing these results with previous research work. The comparison showed that the proposed algorithm realized better identification than others.

Experimental Investigation of Progressive Collapse of Steel Frames

This paper reports two new tests conducted to augment available data highlighting the structural performance of multistory steel frames under progressive collapse. The investigated steel frames had different geometries, different boundary conditions, different collapse mechanisms, different damping ratios and different connections. Overall, the paper addresses how multistory frames would behave when subjected to local damage or loss of a main structural carrying element. The obtained results can form a data base for nonlinear finite element models. The deformations of the investigated steel frames and failure modes under progressive collapse were predicted from the finite element analysis, with detailed discussions presented.

This paper investigates the behavior of steel frames under progressive collapse using the finite element method. Non-linear finite element models have been developed and verified against existing data reported in the literature as well as against tests conducted by the authors. The nonlinear material properties of steel and nonlinear geometry were considered in the finite element models. The validated models were used to perform extensive parametric studies investigating different parameters affecting the behavior of steel frames under progressive collapse. The investigated parameters are comprised of different geometries, different number of stories and different dynamic conditions. The force redistribution and failure modes were evaluated from the finite element analyses, with detailed discussions presented.

The Dosimetric Effects of Different Beam Energy on Physical Dose Distributions in IMRT Based on Analysis of Physical Indices

This work aimed at evaluating the effect of 6- and 10-MV photon energies on intensity-modulated radiation therapy (IMRT) treatment plan outcome in different selected diagnostic cases. For such purpose, 19 patients, with different types of non CNS solid tumers, were selected. Clinical step-and-shoot IMRT treatment plans were designed for delivery on a Siemens Oncor accelerator with 82 leafs;multi-leaf collimators (MLCs). To ensure that the similarity or difference among the plans is due to energy alone, the same optimization constraints were applied for both energy plans. All the parameters like beam angles, number of beams, were kept constant to achieve the same clinical objectives. The Comparative evaluation was based on dose-volumetric analysis of both energy IMRT plans. Both qualitative and quantitative methods were used. Several physical indices for Planning Target Volume (PTV), the relevant Organs at Risk (OARs) as mean dose (Dmean), maximum dose (Dmax), 95% dose (D95), integral dose, total number of segments, and the number of MU were applied. Homogeneity index and conformation number were two other evaluation parameters that were considered in this study. Collectively, the use of 6 MV photons was dosimetrically comparable with 10 MV photons in terms of target coverage, homogeneity, conformity, and OAR savings. While 10-MV plans showed a significant reduction in the number of MUs that varied between 4.2% and 16.6% (P-value = 0.0001) for the different cases compared to 6-MV. The percentage volumes of each patient receiving 2 Gy and 5 Gy were compared for the two energies. The general trend was that 6-MV plans had the highest percentage volume, (P-value = 0.0001, P-value = 0.006) respectively. 10-MV beams actually decreased the integral dose (from average 183.27 ± 152.38 Gy-Kg to 178.08 ± 147.71 Gy-Kg, P-value = 0.004) compared with 6-MV.
