This study aims to evaluate the torsional dynamics and 2D speckle tracking in heart failure with reduced ejection fraction on patients taking angiotensin receptor neprilysin inhibitors (ARNI) and none.?ARNI has been shown to be superior to enalapril in?the?long term reducing the mortality and hospitalization of heart failure (HF). However short-term effects on diastolic function remain unclear. We sought to evaluate 6 months' effects of ARNI on left ventricular (LV) diastolic parameters determined by speckle tracking and tissue Doppler imaging. This study was carried out in Menoufia University Hospitals including 60 patients of HFrEF during the period from August 2019 to January 2020. (50%) of patients were?treated with traditional treatment of heart failure and the remaining were?treated with ARNI. Data were collected including history and clinical examination. ECG and speckle tracking and tissue Doppler imaging were?done to evaluate the diastolic function. Our results showed no significant difference between the two groups at baseline. There was?a?significant improvement?in?the LV diastolic function parameters. Untwisting?time and untwisting?onset showed the highly significant parameters of improvement. This study concluded that ARNI showed short term improvement of diastolic function of the left ventricle that could be detected by STE.

This paper investigates and reports on the extent and nature of corporate internet disclosure and the determinants of internet financial disclosure （IFD） by companies listed in three Gulf Corporation Council （GCC） countries. This paper uses data from 207 listed companies in Muscat Securities Market （MSM）, Dubai Financial Market （DFM）, and Qatar Exchange （QE）. Binary logistic regression analysis is used to examine the determinants of IFD. Kruskal-Wallis test is used to examine the differences in disclosure characteristics among the three countries. The results of this study reveal that firm size is the major factor influencing intemet financial reporting in the GCC. The results reveal that the three countries differ significantly in all the disclosure attributes with the exception of the existence of email link. This paper extends the stream of research that confirms the widespread use of internet in disclosing financial information. The results are consistent with previous literature that corporate size is a major determinant of internet financial reporting. This paper provides insights into corporate internet disclosure in the GCC that will benefit all stakeholders with an interest in corporate reporting in this important region of the world.

Objective: The dosimetric characteristics for linear accelerators with the same model, and nominal energy are known to be very similar, as long as the machines are unaltered from the manufacturer's original specifications. In this preliminary study, a quantitative investigation of the similarity in the basic photon and electron dosimetry data from the Siemens Oncor linear accelerators at our hospital(Children's Cancer Hospital, Cairo, Egypt) was reported. Methods: The output factor(OF), wedge factors(WF), percentage depth dose(PDD), and beam profile for the 6 and 10 MV photon beams were measured. Results: The measured output factors varied by less than about 1% for each field size. The difference between the maximum and minimum PDD values at each depth was less than about 1%. The difference between the beam flattnes and symetry was no more than 1% at all off-axis distances. For electron the results showed that the PDD, OF, and the beam profiles were matched within 1% differences. Conclusion: These results strongly suggest that it is feasible to establish one reference photon and electron dosimetry data set for the two machines and nominal energies.

Amphetamine Versus Non Amphetamine-Related First Episode Psychosis in Saudi Arabian Patients

Background: Amphetamines are illicit psychostimulant drugs that can induce psychotic symptoms. Very few studies have been conducted in Kingdom of Saudi Arabia (SA) on amphetamine abuse and related psychosis. Recently, the pattern of amphetamine abuse in SA showed a significant trend of increased frequency. Objectives: To investigate the extent of amphetamine abuse in a sample of Saudi patients hospitalized for first episode of acute psychosis. Also, to compare in that sample between amphetamine psychosis and other psychoses regarding demographic data, premorbid personality and symptoms profile. Method: 106 patients with acute psychosis were hospitalized and screening of urine for amphetamine was conducted for all. Patients’ psychiatric evaluation included interviewing, and ICD-10 criteria for personality disorders. 30 healthy subjects were also included for comparison with patients. Results: 34/106 of psychotic patients (32%) were positive for amphetamine in urine (≥ 1000 ng/ml). The frequency of personality disorders was significantly higher (P < 0.01) in the patients (54/106;51%) compared with healthy subjects (6/30;20%). Also, the incidence of personality disorders was significantly higher (P < 0.01) in amphetamine positive psychosis (25/34;73.5%) compared with amphetamine negative psychosis (29/72;40%). Cluster B personality disorders particularly the antisocial and borderline were significantly higher in amphetamine positive psychosis (13/34;38%) compared with amphetamine negative psychoses (6/72;8%). The symptom profile showed significant difference between amphetamine positive and amphetamine negative psychosis as regards ideas of reference (50% vs. 14%), suspiciousness (44% vs. 11%), suicidal ideation (38% vs. 23%), paranoid delusions (29% vs. 17%) and increased pulse or blood pressure (29% vs. 7%) respectively. Dynamic Increase Factor (DIF) due to strain rate effect was examined with documented experimental work done by Razaqpur, et al. In the experiment work, two 1000 × 1000 × 70 mm reinforced concrete slabs were constructed. The slabs were subjected to blast loads generated by the detonation of either 22.4 kg or 33.4 kg of ANFO located at a 3.0 m standoff. Blast wave characteristics, including incident and reflected pressures and reflected impulses were measured. The slabs were modeled by explicit analysis with or without strain rate effect to study their behavior under blast load to compare their predicted and observed behavior. The predicted post-blast damage and mode of failure for each model is compared with the observed damage of experimental work. It was concluded that when the dynamic increase factor added to concrete and reinforcement materials due to strain rate effect, the behavior of model under blast load become closer to experimental work.

Role of Endotoxin and TNF in Developing NAFLD in Non-Obese Egyptian Patients

Background and aim of the work: Non-alcoholic fatty liver disease (NAFLD) was considered the hepatic presentation of insulin resistance and obesity for a long time. Studies on lean weighted Asian subjects with NAFLD revealed that NAFLD pathogenesis may be multifactorial. NAFLD is associated with disturbances in gut flora and excess expression of inflammatory mediators. This study aims to find out the relation of endotoxins absorbed from gut and the tumor necrosis factor alpha with NAFLD in non-obese Egyptian patient in comparison to obese patients and healthy control subjects. Patients and methods: This study was performed on three groups group I: Patients with NAFLD and body mass index 2. Group II: Patients with NAFLD and body mass index >25 kg/m2. Group III: healthy control subjects. Results: Group I had significantly higher endotoxin, tumor necrosis factor(TNF) alpha and ALT than group II (endotoxin 11.7 ± 1.7 ug/L vs 9.5 ± 1.4) (TNF 14.8 ± 5.3 vs 11.3 ± 3.3) (ALT 67.8 ± 5.3 IU/L vs 51.8 ± 4.2). There was a highly significant correlation between TNF, endotoxin levels and level of liver enzymes in group I and II. Conclusion: Endotoxemia and TNF alpha may contribute in the pathogenesis of NAFLD especially in non-obese patients.

Low Ankle Brachial Index in Acute Ischemic Stroke: Does ApoE Gene Polymorphism Have a Role?

Background: The existence of asymptomatic peripheral arterial disease among patients with acute ischemic stroke has been studied and proved. Low ankle brachial index (ABI) is considered as a marker of atherosclerosis, and its relation to stroke severity was documented in some studies. The effect of different alleles of ApoE gene on acute ischemic stroke presentation in patients with low ABI is not known. Objective: To study the effect of ApoE gene polymorphism on stroke severity, outcome and recurrence in patients with asymptomatic peripheral arterial disease identified by low ABI. Methods: Patients with acute ischemic stroke were screened for the presence of asymp-tomatic peripheral arterial disease by estimating the ABI using a pocket Doppler ultrasound device. Assay of ApoE gene was done using the real-time PCR technique. Results: Low ABI was present in 31% of patients with acute ischemic stroke. There was no significant difference among patients with different ApoE alleles regarding the severity of their symptoms. Also, there was no significant difference among patients with normal ABI and those with abnormal ABI regarding the ApoE gene polymorphism. Conclusion: The current study showed that there was no significant relation between ApoE gene polymorphism and low ABI in ischemic stroke patients who had asymptomatic peripheral arterial disease.

The Effects of Energy Drink Consumption on Kidney and Liver Function: A Comparative Study

The current study aimed to investigate the potential health impacts associated with Energy Drink (ED) consumption and its effects on general health, liver function, and kidney function among students at Jazan University. EDs contain a variety of ingredients such as taurine, arginine, caffeine, acidity regulators, B vitamins, vitamin C, herbal extracts, and sugar. Previous studies have reported that EDs contain more caffeine content more a cup of coffee. Fifty-seven volunteer female students from the same socioeconomic background were eligible for the study based on their ED consumption habits. The students were divided into four groups: the control group (1) which did not consume EDs (19 students), group (2) which consumed 2 - 3 cans per week (19 students), group (3) which consumed 4 - 7 cans per week (14 students), and group (4) which consumed more than 7 cans per week (5 students). Five milliliters of venous blood were drawn from all subjects to determine ALT, AST, and GGT levels for liver function tests and creatinine, urea, uric acid, and BUN levels for kidney function tests. The results of the kidney function analysis showed that uric acid levels significantly increased (P 0.05) in all groups compared to the control group. The liver function analysis illustrated that, compared to the control group, only AST was significantly increased (P < 0.05) in groups 1 and 2. There was no significance in ALT and GGT in all groups. Overall, the study found that long-term ED consumption had a significant relationship to increased uric acid and AST concentrations, while urea and BUN decreased significantly. Further research is needed to understand the long-term health implications of ED consumption.
