Kidney Renal Clear Cell Carcinoma(KIRC)is a malignant tumor that carries a substantial risk of morbidity and *** mmP family assumes a crucial role in tumor invasion and *** study aimed to uncover the mechanistic relevance of the mmP gene family as a therapeutic target and diagnostic biomarker in Kidney Renal Clear Cell Carcinoma(KIRC)through a comprehensive approach encompassing both computational and molecular ***,Cytoscape,UALCAN,GEPIA,OncoDB,HPA,cBioPortal,GSEA,TImER,ENCORI,DrugBank,targeted bisulfite sequencing(bisulfite-seq),conventional PCR,Sanger sequencing,and RT-qPCR based analyses were used in the present study to analyze mmP gene family members to accurately determine a few hub genes that can be utilized as both therapeutic targets and diagnostic biomarkers for *** performing STRING and Cytohubba analyses of the 24 mmP gene family members,mmP2(matrix metallopeptidase 2),mmP9(matrix metallopeptidase 9),mmP12(matrix metallopeptidase 12),and mmP16(matrix metallopeptidase 16)genes were denoted as hub genes having highest degree *** analyzing mmP2,mmP9,mmP12,and mmP16 via various TCGA databases and RT-qPCR technique across clinical samples and KIRC cell lines,interestingly,all these hub genes were found significantly overexpressed at mRNA and protein levels in KIRC samples relative to *** notable effect of the up-regulated mmP2,mmP9,mmP12,and mmP16 was also documented on the overall survival(OS)of the KIRC ***,targeted bisulfite-sequencing(bisulfite-seq)analysis revealed that promoter hypomethylation pattern was associated with up-regulation of hub genes(mmP2,mmP9,mmP12,and mmP16).In addition to this,hub genes were involved in various diverse oncogenic *** mmP gene family members(mmP2,mmP9,mmP12,and mmP16)may serve as therapeutic targets and prognostic biomarkers in KIRC.

AImTo evaluate the reversibility of minimal hepatic encephalopathy (mHE) following liver transplantation (LT) in Egyptian cirrhotic patients. mETHODSThis prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty age- and sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A (TmT A), TmT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score (PHES) was calculated for patients to diagnose mHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTSBefore LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls (P P P P = 0.005). more patients in whom reversal of mHE was observed had a pre-transplant mELD score CONCLUSIONReversal of mHE in cirrhotic patients could be achieved by LT, especially in those with a mELD score

Treatment Outcome and Prognostic Factors for Pediatric medulloblastoma Patients: The Egyptian National Cancer Institute Experience

Purpose: To evaluate treatment outcomes and prognostic factors of pediatric medulloblastoma (mB) patients treated by adjuvant post-operative riskadapted radiotherapy (RT) and chemotherapy (CT). Patients and methods: A retrospective analysis was conducted based on medical records of pediatric patients with pathologically confirmed mB treated between 2006 and 2013 at the National cancer Institute (NCI), Egypt. Various patients’ and disease characteristics, treatment details and outcome data were reviewed. Results: Fifty patients’ records were included in the analysis with a median age of 6 years at diagnosis (range 3 - 18). According to the Chang staging system;38%, 44%, 4%, and 14% were m0, m1, m2, and m3, respectively. All patients underwent primary surgery;gross total resection (with no residual) in 38%, near total resection (with residual ≤1.5 cm2) in 8%, subtotal resection (with residual > 1.5 cm2) in 34%, and 20% had only biopsy. All patients were treated by riskadapted craniospinal irradiation (CSI);high-risk patients were treated by CSI 36 Gy/20 fractions over 4 weeks followed by posterior fossa (PF) boost 18 Gy/10 fractions over 2 weeks (180 cGy per fraction), while standard-risk patients were treated by CSI 23.4 Gy/13 fractions over 2 and half weeks followed by PF boost 30.6 Gy/17 fractions over 3 and half weeks. median overall treatment time (OTT) was 52 days. All patients received adjuvant CT;47 patients (94%) received concomitant chemo radiotherapy (CCRT), while 4 patients (8%) only received neoadjuvant CT (NB: only one patient received all neoadjuvant, concomitant and adjuvant CT). With a median follow up time of 32.5 months, ranging from 6 to 104 months, the whole group estimates of the overall survival (OS) at 1, 3, and 5 years were 83%, 70%, and 64%, respectively, while, the progression-free survival (PFS) rates at 1, 3, and 5 years were 79%, 62%, and 57% respectively. Four patients relapsed. Neural-axis was the commonest site of relapse (3 patients).

Influence of dosimetric considerations in evaluating second cancer risks in prostate cancer. material and methods: Fifteen patients in this study suffering from early stage of prostate cancer, each patient underwent three plans: 1) Three-dimensional conformal radiation therapy (3DCRT), 2) Rotation therapy (Arc therapy), and 3) intensity-modulated radiation therapy (ImRT) plan. Estimate secondary metastasis risk models: Excess Relative Risk (ERR) and Excess absolute risk (EAR) based on age of exposure by taking dosimetry data from Dose Volume Histograms (DVHs) to calculate risk models. Result: The second cancer risk models (ERR and EAR) for organs at risk OARs decrease with increasing age of exposure for 3D-CRT, ARC and ImRT and there is no significant difference for ERR and EAR model for developing second cancer risk in 3D-CRT, ARC and ImRT.

Amphetamine Versus Non Amphetamine-Related First Episode Psychosis in Saudi Arabian Patients

Background: Amphetamines are illicit psychostimulant drugs that can induce psychotic symptoms. Very few studies have been conducted in Kingdom of Saudi Arabia (SA) on amphetamine abuse and related psychosis. Recently, the pattern of amphetamine abuse in SA showed a significant trend of increased frequency. Objectives: To investigate the extent of amphetamine abuse in a sample of Saudi patients hospitalized for first episode of acute psychosis. Also, to compare in that sample between amphetamine psychosis and other psychoses regarding demographic data, premorbid personality and symptoms profile. method: 106 patients with acute psychosis were hospitalized and screening of urine for amphetamine was conducted for all. Patients’ psychiatric evaluation included interviewing, and ICD-10 criteria for personality disorders. 30 healthy subjects were also included for comparison with patients. Results: 34/106 of psychotic patients (32%) were positive for amphetamine in urine (≥ 1000 ng/ml). The frequency of personality disorders was significantly higher (P mpared with healthy subjects (6/30;20%). Also, the incidence of personality disorders was significantly higher (P mphetamine positive psychosis (25/34;73.5%) compared with amphetamine negative psychosis (29/72;40%). Cluster B personality disorders particularly the antisocial and borderline were significantly higher in amphetamine positive psychosis (13/34;38%) compared with amphetamine negative psychoses (6/72;8%). The symptom profile showed significant difference between amphetamine positive and amphetamine negative psychosis as regards ideas of reference (50% vs. 14%), suspiciousness (44% vs. 11%), suicidal ideation (38% vs. 23%), paranoid delusions (29% vs. 17%) and increased pulse or blood pressure (29% vs. 7%) respectively. Conclusion: Screening of amphetamine in urine among patients with first episode of acute psychosis can help and support the clinical distinction of

Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIm To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

为了提高桑叶发酵品质,制作良好的青贮饲料以满足动物营养需要,本试验研究了发酵底物(葡萄糖,糖蜜)和乳酸菌制剂对桑叶青贮发酵品质的影响。1.不同浓度葡萄糖(morus Alba)对桑叶青贮发酵品质,化学成分和体外消化率的影响。本试验研究目的是研究添加不同水平葡糖糖对桑叶青贮发酵品质,化学成分和体外消化率的影响。将采收自生长阶段的新鲜桑叶切碎处理,采用实验室小型青贮窖进行青贮。试验设不同浓度的葡萄糖添加组,分别为(1)对照无添加;(2)1%葡萄糖组,G1;(3)2%葡萄糖组,G2;(4)3%葡萄糖组,G3,在青贮第1,7,14,30,60和90天开窖分析发酵品质。结果表明,桑叶干物质(dry matter;Dm)为 424.92 g/kg 鲜重(fresh matter;Fm),可溶性糖(WSC)为 80.31 g/kg干重,缓冲能和粗蛋白含量分别为223.02 mE/kg Dm和21.86%Dm。表面附着乳酸菌数量为1.0×103CFU/gFm。青贮14天后,G2和G3组pH显著(Pm,AN/TN,ADF和NDF均无显著影响。G3组中有较高含量的PA表明梭菌发酵或异型发酵的发生。不同处理组在整个青贮过程中产气量均无显著差异。添加葡萄糖对桑叶体外干物质消化率(IVDmD)和体外中性洗涤纤维消化率(IVNDF)均无显著影响。2.葡萄糖和乳酸菌对桑叶青贮发酵品质,化学成分和体外消化率的影响本试验采用小型实验室青贮窖。试验设:对照无添加(C);商业乳酸菌(Lactobacillus Plantarum)(LAB1);乳酸片球菌(Pediococuss acidilactici)(LAB2);鼠李糖乳杆菌(Lactobacillus rhamnosus)(LAB3)。所有乳酸菌组添加水平为106(CFU)g-1。青贮窖置于室温下贮藏(25-30℃),分别于1,7,14,30,60和90天开窖分析发酵品质。原材料Dm WSC和CP含量分别为424.92 g/kg FW,80.31g/kg Dm和21.86%Dm,缓冲能为61.17 mE/kg Dm。表面附着乳酸菌数量为1.0×105cfu/gFm,酵母菌数量少于1.0×103CFU/gFm。青贮7天后,所有处理组pH均显著(Pm。不同乳酸菌添加组在整个培养过程中产气量存在显著差异(PmD)和体外中性洗涤纤维消化率(IVNDF)显著差异。3.添加糖蜜,葡萄糖和乳酸菌对桑叶青贮特性、发酵品质和化学成分的影响。为开发桑叶作为非常规饲料资源,本试验探讨了添加乳酸菌、葡萄糖或糖蜜对桑叶青贮发酵品质的影响,试验设对照组(C)、乳酸菌组(P)、葡萄糖组(G)、糖蜜组(m)、乳酸菌+葡萄糖组(P+G)、乳酸菌+糖蜜组(P+m),青贮后第7,14,30和60天开窖取样分析桑叶青贮饲料发酵品质。结果表明,添加乳酸菌加速了桑叶青贮过程中乳酸发酵,青贮第7天P,P+G和P+m组乳酸含量已达到C组的6倍以上,pH值降至4.30以下,其中P+G和P+m在青贮结束时降至4.0左右。补充发酵底物并未有效改善桑叶青贮发酵品质,C、G和m组pH值在青贮前30 d始终保持在5.85以上,青贮60d时C组为5.96,G和m组分别下降至5.35和5.24,显著(Pm组,青贮7 d后开始显著(Pm组。青贮第7天p组氨态氮/总氮显著(Pm组氨态氮/总氮显著(Pm组直至青贮结束。认为单独添加乳酸菌明显提高了桑叶青贮发酵品质,而组合添加并未进一步得到大的改善.

This study examined if the anti-tumorigenesis effect of Exendin-4 in HT29 and HCT116 colorectal cancer(CRC)involves modulation of SIRT1 and Akt/GSR3K/β-catenin/NF-κB ***29 and HCT116 cells were treated either with increasing levels of Exendin-4(0.0-200μm)or with Exendin-4(at its IC50)in the presence or absence of EX-527(10μm/a selective SIRT1 inhibitor)or Exendin-4(9-39)amide(E(9-39)A)(1μm/an Exendin-4 antagonist).In a dose-dependent manner,Exendin-4 inhibited cell survival,but enhanced levels of lactate dehydrogenase(LDH)and single-stranded DNA(ssDNA)in both HT29 and *** both cell lines and at it has an IC50(45μm for HT29 and 35μm for HCT1165),Exendin-4 also significantly reduced cell survival,migration,and invasion of both cell types,with no effect on the expression GLP-1 receptors(GLPRs)nor of the activity of *** these doses,Exendin-4 also increased the expression of SIRT1 but reduced the acetylation of NF-κB and the expression of Bax and cleaved caspase-3 and in both cell ***,protein levels of p-GS3Kβ(Ser9),total and acetylatedβ-catenin,and Anix2 were significantly decreased,but levels of p-GS3Kβ(Ser9)and p-β-catenin(Ser33/37/Thr41)were significantly increased in both HT29 and HCT116-exendin-4 treated *** the effects exerted by Exendin-4 were completely prevented by Ex527 or E(9-39)*** conclusion,Exendin-4 suppresses the tumorigenesis of HT29 and HCT116 CRC cell activation of GS3Kβ-induced inhibition ofβ-catenin and NF-κβin a SIRT1-dependent mechanism.

Background: Hepatocellular carcinoma (HCC) is characterized by the deficiency of specific symptoms in the early stages of the disease and a poor prognosis. Biomarkers that have the diagnostic ability to distinguish between inflammation, cirrhosis, and HCC are needed for early case detection. midKine (mK) is overexpressed in many malignancies, including hepatocellular carcinoma, gastric carcinoma, colon carcinoma, lung carcinoma, urinary bladder carcinoma and prostate carcinoma. Objective: The objective is to evaluate serum midKine as a marker of HCC in HCV-related liver cirrhosis. Patients and methods: This study included 50 patients with HCC (on top of hepatitis C virus-related cirrhosis), 20 cirrhotic patients due to chronic HCV infection, and 10 normal subjects as a control group. History, examination, and laboratory investigations were performed on all subjects in the three groups including mK and Alpha fetoprotein (AFP) serum levels. Results: Serum levels of AFP and mK were higher in the HCC group than in the other study groups (P Conclusion: midKine is a novel sensitive biomarker that could be used for early detection of HCC and has a high significance in differentiation between cirrhosis and HCC cases. midKine levels are correlated with HCC stages and sizes.
