Skin disorders in peritoneal dialysis patients:An underdiagnosed subject

AIM： To examine all skin changes in peritoneal dialysis （PD） patients followed up in our ***： Patients on PD program for at least three months without any known chronic skin disease were included in the study. Patients with already diagnosed skin disease, those who have systemic diseases that may cause skin lesions, patients with malignancies and those who did not give informed consent were excluded from the study. All patients were examined by the same predetermined dermatologist with all fndings recorded. The demographic, clinical and laboratory data including measures of dialysis adequacy of patients were recorded also. Statistical Package for Social Sciences （SPSS） for Windows 16.0 standard version was used for statistical ***： Among the patients followed up in our PD unit, those without exclusion criteria who gave informed consent, 38 patients were included in the study with male/female ratio and mean age of 26/12 and 50.3 ± 13.7 years, respectively. The duration of CKD was 7.86 ± 4.16 years and the mean PD duration was 47.1 ± 29.6 mo. Primary kidney disease was diabetic nephropathy in 11, nephrosclerosis in six, uropathologies in four, chronic glomerulonephritis in three, chronic pyelonephritis in three, autosomal dominant polycystic kidney disease in three patients while cause was unknown in eight patients. All patients except for one patient had at least one skin lesion. Loss of lunula, onychomycosis and tinea pedis are the most frequent skin disorders recorded in the study group. Diabetic patients had tinea pedis more frequently （ P = 0.045）. No relationship of skin findings was detected with primary renal diseases, comorbidities and medications that the patients were ***： Skin abnormalities are common in in PD patients. The most frequent skin pathologies are onychomycosis and tinea pedis which must not be overlooked.

Background Hand ge stur e interfaces are dedicated programs that principally perform hand tracking and hand gesture prediction to provide alternative controls and interaction *** take advantage of one of the most natural ways of interaction and communication,proposing novel input and showing great potential in the field of the human-computer *** a flexible and rich hand gesture interface is known to be a time-consuming and arduous *** published studies have demonstrated the significance of the finite-state-machine(FSM)approach when mapping detected gestures to GUI *** In our hand gesture interface,we broadened the FSM approach by utilizing gesture-specific attributes,such as distance between hands,distance from the camera,and time of occurrences,to enable users to perform unique GUI *** attributes are obtained from hand gestures detected by the RealSense SDK employed in our hand gesture *** means of these gesture-specific attributes,users can activate static gestures and perform them as dynamic *** also provided supplementary features to enhance the efficiency,convenience,and user-friendliness of our hand gesture ***,we developed a complementary application for recording hand gestures by capturing hand keypoints in depth and color images to facilitate the generation of hand gesture *** We conducted a small-scale user survey with fifteen subjects to test and evaluate our hand gesture *** feedback obtained from the users indicates that our hand gesture interface is adequately facile and self-explanatory to *** addition,we received constructive feedback about minor flaws regarding the responsiveness of the *** We proposed a hand gesture interface along with key concepts to attain use r-friendlines s and effectiveness in the control of existing GUIs.

Osteoarthritis(OA)is a complex"whole joint"disease pursued by inflammatory mediators,rather than purely a process of"wear and tear".Besides cartilage degradation,synovitis,subchondral bone remodeling,degeneration of ligaments and menisci,and hypertrophy of the joint capsule take parts in the *** is the hallmark symptom of OA,but the extent to which structural pathology in OA contributes to the pain experience is still not well *** the knee OA,intraarticular(IA)injection(corticosteroids,viscosupplements,blood-derived products)is preferred as the last nonoperative modality,if the other conservative treatment modalities are *** corticosteroid injections provide short term reduction in OA pain and can be considered as an adjunct to core treatment for the relief of moderate to severe pain in people with *** hyaluronic acid(HA)injections might have efficacy and might provide pain reduction in mild OA of knee up to 24 *** for HA injections,the costeffectiveness is an important concern that patients must be informed about the efficacy of these *** more high-quality evidence is needed,recent studies indicate that IA platelet rich plasma injections are promising for relieving pain,improving knee function and quality of life,especially in younger patients,and in mild OA *** current literature and our experience indicate that IA injections are safe and have positive effects for patient ***,there is no data that any of the IA injections will cause osteophytes to regress or cartilage and meniscus to regenerate in patients with substantial and irreversible bone and cartilage damage.

Neutrophil-to-lymphocyte ratio in occlusive vascular diseases： the literature review of the past 10 years

BACKGROUND: This study aims to evaluate the results of studies investigating neutrophilto-lymphocyte ratio(NLR) and to identify the prognostic and diagnostic value of NLR in occlusive vascular ***: With the aim of identifying the studies related to NLR, a search was performed on http://***/pubmed by using the key words "neutrophil lymphocyte ratio" between January 2005 and December 2014. All of the original articles were evaluated according to date of publications, countries, clinics and topics. Studies about occlusive vascular diseases were evaluated according to their qualifications, review methods and results. SPSS for Windows 16.0 was used in data analysis and data were expressed as mean, standard deviation and ***: A total of 735 original research articles were investigated. The number of publications have shown a regular logarithmic increase over the years. Thirty-two percent of all publications were performed by clinics in Turkey and 56.4% were performed by general-oncological surgery and cardiology clinics. A total of 107 publications were identified to be about occlusive vascular diseases, 80.3% of these publications were found to be prognostic and 19.6% to be diagnostic, 82.2% of them were found to be planned as retrospective and 17.7% as prospective. In 95.3% of prognostic publications, there was a positive correlation between high NLR values at admission and poor prognosis. In 95.3% of diagnostic publications high NLR values at admission were identifi ed to be signifi cant ***: Elevated neutrophil-to-lymphocyte ratio at admission, could be used as a diagnostic and/or prognostic parameter in occlusive vascular diseases.

Uveitis associated with multiple sclerosis: complications and visual prognosis

AIM: To determine the frequency, subtype,complications, treatment and visual prognosis of uveitis in patients with multiple sclerosis(MS). METHODS: A total of 1702 MS patients’ medical records were reviewed for a history of uveitis both with a neurologist and an ***: Nine patients(0.52%) with uveitis were detected. Eight of them were female, one was male. The mean age was 42.0±14.1y(range 22-66). Seven patients were relapsing remitting MS, two were secondary progressive MS. The mean duration of MS was 10.8 ±10.3y, and the mean duration of uveitis 10.3 ±9.9y. The onset of uveitis preceded that of MS(four patients) by a mean of 5.0±4.3y(range 1-11). MS diagnosed prior to the onset of uveitis(five patients) by an interval of 0.75-16y(mean 4.95 ±6.24y). There were 16 affected eyes of nine patients. The most common types of uveitis were panuveitis and intermediate uveitis. Uveitis was bilateral in most patients. The most common complications were cataract and glaucoma, and patients with such complications were surgically treated. The range of visual acuity of affected eyes was 20/800 to 20/22, with only six of 16 affected eyes better than 20/40. After treatment, the visual acuity of the affected eyes was better than 20/40 in11 of 16 eyes. CONCLUSION: Uveitis should be considered when assessing an MS patient with visual loss, as surgical interventions other than medical treatments may be needed to improve visual function. Complications could be seen more often when posterior segment is involved.

Associating liver partition and portal vein ligation for staged hepatectomy for extensive alveolar echinococcosis:First case report in the literature

Alveolar echinococcosis(AE) is a zoonotic disease that is caused by Echinococcus multilocularis that affects liver and a variety of organs and tissues. It differs from other echinococcal disease because it shows tumor like behavior in the affected organ and tissues. The treatment of choice is concomitant medical therapy and resection with negative margins. Nevertheless, resection with the intent of negative margins(R0) may lead to serious complications such as liver failure. In the present case report, we used Associating Liver Partition and Portal Vein Ligation for Staged Hepatectomy(ALPPS) procedure, which was defined in 2012 by Schnitzbauer et al, in a 28-year-old male patient to avoid complications of major liver resection in order to treat alveolar echinococcosis. Until now, we have not encountered any study using ALPPS procedure for the treatment of alveolar echinococcosis. In the present case report we aimed to show that ALPPS procedure can be safely performed for marginnegative resection of primary or recurrent AE that shows a tumor like behavior. It is our opinion that this procedure should be performed in centers that have expertise and sufficient technical capacity to perform liver transplantation and advanced liver surgery.

Neuroendocrine tumor incidentally detected during living donor hepatectomy: A case report and review of literature

To our best knowledge, no case of a tumor that was incidentally detected during living donor hepatectomy(LDH) has been reported in the English language medical literature. We present two cases in which grade Ⅰ neuroendocrine tumors(NET) were incidentally detected during our twelve-year LDH experience. First Case: A 26-yearold male underwent LDH for his brother suffering from HBV-related chronic liver disease(CLD). After right lobe LDH, intestinal length was measured as part of a study concerning the relationship between small intestinal lengths and surgical procedure. At this stage, a mass lesion with a size of 10 mm × 10 mm was detected on the antimesenteric surface, approximately 90 cm proximal to the ileocecal valve. A wedge resection with primary intestinal anastomosis was performed. Second Case: A 29-year-old male underwent right lobe LDH for his father with hepatitis B virus(HBV)-related CLD. An abdominal exploration immediately prior to the closure of the incision revealed that the appendix vermiformis was edematous and had firmness with a size of 8-10 mm at its tip. An appendectomy was performed. The pathological examinations of the specimens of both patients revealedgrade 1 NET. In conclusion, even if patients undergoing LDH are healthy individuals, whole abdominal cavity should be gently palpated and all findings recorded after completing laparotomy. Suspected masses or lesions should be confirmed by frozen section examination. Such an approach would avert potential medicolegal issues.
