Emergency Neonatal Obstetric Cares at Cocody University Hospital: Overview of Instrumental Extraction

Objectives: The aim of this study was to show an overview of situation of instrumental extraction at the maternity of Cocody University Hospital by determining availability and assessing the maternal and fetal prognosis. Patients and method: it was a retrospective study with a descriptive purpose on 6 months from January 1 to June 31, 2015. It concerned all patients admitted in the expulsive phase of labor delivery room and with an indication of fetal instrumental extraction and the state of their newborns at birth. Results: We saved 2288 vaginal deliveries;including 28 instrumental deliveries on 104 indications of instrumental extraction is an implementation rate of 26.9% (16 by vacuum extractor and 12 forceps). Among patients with indication but without instrumental extraction (n = 76), there is 42.3% vaginal delivery (n = 44) and 30.8% of cesarean section (n = 32). 44 have given birth vaginally (42.3%) and 32 by caesarean section (30.8%). It’s young patient (28 years), nulliparous (42.3%). Average time between instrumental extraction indication and the delivery of the baby was 58 min in the case of instrumental extraction and 1 hour 41 minutes in the case of spontaneous delivery in anticipation of the c-section. Motherhood had 3 instrumental extractors (2 vacuum extractors and 1 forceps) recycled after each use. The Apgar score was good in 85.7% in children born by instrumental extraction and bad in 54.5% in children born vaginally without instrumental extraction. We found 20 stillborn in intra partum occurred only in children born vaginally without instrumental extraction. Twelve (12) cases of bleeding of the issue by uterine atony (27.3%) were recorded in patients pregnant without instrumental extraction. No maternal deaths were observed. Conclusion: The realization of instrumental extraction rate remains low at the maternity of the UH-C. In the event, the fetal prognosis was better.

Hysterectomy for Uterine Inversion: About Two Cases at University Hospital of Cocody (Abidjan—Ivory Coast)

It is a rare clinical condition that may occur to two possible types: puerperal uterine inversion (IUP) and no puerperal uterine inversion (IUNP). Obstetrician will observe often only once in his professional practice. The risk of maternal mortality by hemorrhage is high. We report two cases of uterine inversion requiring a hysterectomy, including a postpartum mode and the other gynecological mode with young women. Hysterectomy remained a last resort treatment in the two types, to reduce morbidity. In the case of uterine inversion, the treatments without hysterectomy could be facilitated if the management is early and adequate. Furthermore, the support should be multidisciplinary and fast in the two types.

The transformation of uterine fibroids is common in relation to their development. Giant forms of cystic degeneration are rare. They raise diagnostic difficulties with other pelvic tumors, such as ovarian tumors and leiomyosarcomas. Magnetic resonance imaging specifies the original organ, the volume and the main relationships of fibromyoma with adjacent structures. The diagnosis of certainty is based on laparotomy coupled with histology. The authors illustrate these difficulties by observing a giant cystic degenerative fibroma in a 26-year-old G1P1 woman in the postpartum period.

Cyclopia is a rare congenital brain malformation frequently associated with facial anomalies. It is characterized by the presence of a single eye with varying degrees of doubling of the intrinsic ocular structures located in the middle of the face. It is the most severe facial expression of holoprosencephaly. Its aetiology is still poorly understood, but several factors could play a role in its occurrence, including certain viruses contracted during pregnancy. Obstetrical ultrasound has made antenatal diagnosis and the search for associated malformations possible. This diagnosis must be made antenatally because the prognosis is poor, hence the decision to terminate the pregnancy. We report a case of cyclopia associated with ambiguity of the external genitalia, discovered intraoperatively in a patient with poor prenatal follow-up, in whom a coronavirus infection (COVID-19) had been diagnosed in early pregnancy.

Malignant melanomas or lymphoma of the skin are malignant tumors of the skin and/or the mucous membranes whose uterine metastases are rare. The secondary uterine localizations, although rare, must be evoked in front of a pelvic tumoral syndrome, or diffuse metastases and a personal past history of melanoma, even after a long time of remission. In our observation, the evolution of the tumor in the pelvis extended to the muscular structures of the uterus and the sigmoid colon of a 72 year old patient, what made it an exceptional case. The diagnosis of these secondary localizations is a diagnosis of elimination, almost always post-operative, made on the histopathological and immunocytochemical study of the surgical specimen, supported by cytogenetics, even molecular biology. The treatment is based on chemotherapy.

Hydrogen production from methane and carbon dioxide mixture using all-solid-state electrochemical cell based on a proton-conducting membrane and redox-robust composite electrodes

In recent years, interest in hydrogen as a fuel has sharply increased in the field of alternative and green energy due to its high energy capability and zero-emission behaviour. As a result, research in the development of new highly efficient methods for producing high-purity hydrogen is relevant. This paper presents, for the first time, the test results of an electrochemical cell with a proton-conducting La_(0.9)Sr_(0.1)ScO_(3-δ) electrolyte and symmetrical Sr_(1.95)Fe_(1.4)Ni_(0.1)Mo_(0.5)O_(6-δ)+ La_(0.9)Sr_(0.1)Sc_(0.9)Co_(0.1)O_(3-δ) electrodes as a hybrid setup for electricity generation in proton ceramic fuel cell mode, for hydrogen separation from H_(2)+ Ar mixture and the production of high-purity hydrogen from methane with simultaneous CO_(2) *** was found that this electrochemical cell generates high flow rates of hydrogen during its separation through a proton-conducting membrane from H_(2)+ Ar mixture, about 500 cm^(3)h^(-1)cm^(-2)at a current density of 0.6 A cm^(-2)as well as about 370 cm^(3) h^(-1)cm^(-2)at a current density of 0.5 A cm^(-2) from CH_(4)+ CO_(2) mixture at 800 ℃ which shows that these cells are promising for hydrogen production.

The interest for land navigation has increased for the recent years. With the advent of the Global Position System (GPS) we have now the ability to determine the absolute position anywhere on the globe. The problem is that the GPS systems work well only in open environments with no overhead obstructions and they are subject to large unavoidable errors when the reception from some of the satellites are blocked. This occurs frequently in urban environments, forests and tunnels. GPS systems require at least four “visible” satellites to maintain a good position fix. In many situations in which higher level of accuracy is required, the navigation cannot be achieved by GPS alone. This paper discusses the design of a reliable multisensor fusion algorithm using GPS and Inertial Navigation System in order to decrease the implementation cost of such systems on land vehicles. The major contribution of this paper is in the definition of the possible developments and research axes in land navigation.

Preliminary study of mollusk fossils in the Qinan Miocene loess-soil sequence in western Chinese Loess Plateau

The first near continuous mollusk fossil record from the Qinan Miocene loess-soil se-quence is presented. Results show that mollusk species are abundant in the QA-I section between 22 and 6.2 Ma, and both loess layers and soil layers yield mollusk fossil individuals. All mollusk species are terrestrial and well preserved in the section. Most species are identical to those identified in the Quaternary loess-soil sequence of the Loess Plateau. The thermo-humidiphilous group generally is distributed in soil layers, whereas cold-aridiphilous group can be observed in loess layers. From the Miocene to Quaternary, the occurrence and divergence of new species of Metodontia and Cathaica correspond to the crucial periods of regional and global environmental changes, indicating that mol-lusk species are sensitive to environmental variations. Preliminary study on the QA-I mollusk record indicates that the Qinan Miocene loess-soil sequence may have a similar sedimentary environment to the Quaternary loess-soil sequence of the Loess Plateau.

Edge and lithium concentration effects on intercalation kinetics for graphite anodes

Graphite interfaces are an important part of the anode in lithium-ion batteries(LIBs),significantly influencing Li intercalation *** anodes adopt different stacking sequences depending on the concentration of the intercalated Li *** this work,we performed first-principles calculations to comprehensively address the energetics and dynamics of Li intercalation and Li vacancy diffusion near the no n-basal edges of graphite,namely the armchair and zigzag-edges,at high Li *** find that surface effects persist in stage-Ⅱ that bind Li strongly at the edge ***,the pronounced effect previously identified at the zigzag edge of pristine graphite is reduced in LiC_(12),penetrating only to the subsurface site,and eventually disappearing in LiC_(6).Consequently,the distinctive surface state at the zigzag edge significantly impacts and restrains the charging rate at the initial lithiation of graphite anodes,whilst diminishes with an increasing degree of *** diffusion time for Li hopping to the bulk site from either the zigzag edge or the armchair edge in LiC_(6) was observed during high state of charge due to charge *** controlling Li occupation and diffusion kinetics at this stage is also crucial for enhancing the charge rate.
