This report describes a patient that developed recurrent metastatic hepatocellular carcinoma(HCC) to a suprapancreatic lymph node four years after being treated for primary HCC via complete left hepatectomy. metastatic HCC was proven by pathologic confirmation. The report addresses the role of surgical resection as a treatment modality for recurrent HCC to solitary lymph nodes. The role of biological chemotherapy as adjuvant treatment is also addressed.

A miniature forward-viewing endoscopic probe that provides high-resolution 3D photoacoustic images is *** probe is of outer diameter 3.2mm and comprised of a transparent Fabry-Pérot(FP)polymer-film ultrasound sensor that is located at the distal end of a rigid optical fiber *** laser pulses are coupled simultaneously into all cores of the bundle and are transmitted through the FP sensor to provide wide-field tissue illumination at the distal *** resulting photoacoustic waves are mapped in 2D by sequentially scanning the input end of the bundle with an interrogation laser beam in order to individually address different points on the FP *** this way,the sensor acts as a high-density ultrasound array that is comprised of 50,000 individual elements,each of which is 12μm in diameter,within the 3.2mm diameter footprint of the *** fine spatial sampling that this affords,along with the wide bandwidth(f_(-3dB)=34 mHz)of the sensor,enables a high-resolution photoacoustic image to be *** measured on-axis lateral resolution of the probe was depth-dependent and ranged from 45-170μm for depths between 1 and 7 mm,and the vertical resolution was 31μm over the same depth *** system was evaluated by acquiring 3D images of absorbing phantoms and the microvascular anatomies of a duck embryo and mouse *** image fidelity was *** is anticipated that this type of probe could find application as a tool for guiding laparoscopic procedures,fetal surgery and other minimally invasive interventions that require a millimeter-scale forward-viewing 3D photoacoustic imaging probe.

Aims The objectives of this study were to(i)determine the importance of soil moisture and soil nitrogen availability as resource requirements for the establishment of Stipa pulchra,(ii)to examine how changes in slope within a local habitat can affect the accumulation of soil moisture and soil nitrogen and(iii)to measure the physiological re-sponses of *** among such slope *** The field experiment occurred in the San JoséHills of Southern California on the California State Polytechnic University,Pomona *** were established within three different slope loca-tions(shallow,moderate and steep)within four separate *** nitrogen treatments(ambient,addition and removal)were replicated within each slope location for all four canyons,and five *** plants were planted in each *** nitrogen accumula-tion and soil moisture,as well as other abiotic variables,were meas-ured across the varied slope locations and nitrogen *** measured the cover,stress,and reproduction of *** plants for 2 years and used a mesocosm experiment to determine how ***-chra responds to varying water and nitrogen *** Findings Stipa pulchra had a strong positive response to the wettest watering treatment and a weak response to nitrogen addition in the meso-cosm *** the field experiment,*** responded positively in moderate slope locations and did not respond to ni-trogen treatments,possibly due to *** patterns of soil moisture were driven by slope,soil compaction and solar *** nitrogen accumulation did not vary among slope *** restoration of ***,priority should be placed on managing soil moisture availability and determining suitable edaphic factors.

Background: Continuous exposure to millimolar (mm) Vitamin C (AA) in vitro kills cancer cells. For superficial bladder carcinoma (SBC), standard chemotherapy is instillation of Bacillus Calmette-Guerin. The recurrence rate with this therapy is 91%. But high dosage vitamins including AA reduced the recurrence to 41%. Aim: To determine the oral dosage of AA that causes the highest concentration of AA [AA] in the bladder. method: We conducted a clinical trial of 14 people who took various dosages of AA, and analyzed the [AA] in their urine. Results: AA above 2 g twice a day was not absorbed. But that intake produced a bladder [AA] above 1 mm in all participants. Conclusion: Taking 2 g of AA BID will increase [AA] in the bladder to a level likely to kill cancer cells that cause SBC. Taking that dosage 2 consecutive days a week is likely to reduce the recurrence rate of SBC substantially.

Urban public transport plays a critical role in stimulating economic development of any nation since most of the revenues come from cities. The majority of the city dwellers of any country use public transport. The evaluation of public transport service quality provides a valuable feedback to commuter operators to ensure continuous improvement of level of service and to the government to take appropriate measures for enhancing the quality of public transport service. This paper analyses and evaluates service quality of Road Public Transport (RPT) (i.e. minibuses and buses) and Urban Rail Transport (URT) in Dar es Salaam City, Tanzania. Since service quality and its attributes are intangible and vague, a fuzzy evaluation model is developed and applied. The formulated model is composed of Fuzzy Entropy method (FEm) and Technique for Order Preference by Similarity to Ideal Solution (TOPSIS). The overall evaluation procedure is as follows: initially, an intensive literature search and experts’ opinions are employed to establish criteria for evaluating the service quality of public transport in Dar es Salaam City. The developed FEm is used to obtain criteria weight. Lastly, the formulated TOPSIS is used to provide an overall ranking of urban public transport service quality. The overall evaluation shows that urban rail transport outperforms road public transport in terms of service quality. Nevertheless, the urban rail transport service in Dar es Salaam City is currently not well developed as it is provided on very limited routes. Thus, the Tanzania government, the rail transport operator i.e. Tanzania Railway Limited (TRL) and the agency responsible for the provision of rail infrastructure i.e. Reli Assets Holding Company (RAHCO) are advised to design and employ Public-Private-Partnership (PPP) schemes i.e. concession contracts to invest more in rolling stocks, locomotives and rail wagons so that the rail transport service is available on many routes of the road public

AIm: To evaluate and validate the national trends and predictors of in-patient mortality of transjugular intrahepatic portosystemic shunt (TIPS) in 15 ***: Using the National Inpatient Sample which is a part of Health Cost and Utilization Project, we identified a discharge-weighted national estimate of 83884 TIPS procedures performed in the United States from 1998 to 2012 using international classification of diseases-9 procedural code 39.1. The demographic, hospital and co-morbility data were analyzed using a multivariant analysis. Using multi-nominal logistic regression analysis, we determined predictive factors related to increases in-hospital mortality. Comorbidity measures are in accordance to the Comorbidity Software designed by the Agency for Healthcare Research and ***: Overall, 12.3% of patients died during hospitalization with downward trend in-hospital mortality with the mean length of stay of 10.8 ± 13.1 d. Notable, African American patients (OR = 1.809 vs Caucasian patients, P mergency admissions (OR = 3.032 vs elective cases, P mortality. Number of diagnoses and number of procedures showed positive correlations with in-hospital death (OR = 1.249 per one increase in number of procedures). Patients diagnosed with acute respiratory failure (OR = 8.246), acute kidney failure (OR = 4.359), hepatic encephalopathy (OR = 2.217) and esophageal variceal bleeding (OR = 2.187) were at considerably higher odds of in-hospital death compared with ascites (OR = 0.136, P morbidity measures with the highest odds of in-hospital death were fluid and electrolyte disorders (OR = 2.823), coagulopathy (OR = 2.016), and lymphoma (OR = 1.842).CONCLUSION: The overall mortality of the TIPS procedure is stea
