The significance of clinical practice guidelines on adult varicocele detection and management

Varicoceles are the most common correctable etiology of male factor infertility. However, the detection and management of varicoceles have not been standardized. This has led to decades of debate regarding the effect of varicocele on male infertility and subsequently whether repair leads to an improved fertility status. The current body of evidence investigating the role of varicocele and varicocelectomy is weak and conflicting. The stance taken by the AUA and ASRM suggests that there is insufficient outcomes data to support evidenced-based guidelines, citing evidence used to provide current recommendations are generally of a low quality level. On the other hand, the EAU Guidelines give a level la of evidence for management of varicoceles that are clinically palpable, associated with subnormal semen analyses and having otherwise unexplained fertility. Besides aiding with clinical varicocele detection and management, clinical practice opinion statements and guidelines aim to direct and strengthen the infrastructure of future studies. We review the current status of opinion statements and guidelines in varicocele and management detection with focus on their application in practice.

过去一个世纪里，勃起功能障碍（Ed）的治疗一直是一件引人关注的事情，涉及到泌尿、心血管以及手术领域的多个学科。随着Ed与心血管疾病的相关性被充分证实，血管造影技术已被用来鉴别血管性阳痿。由于腔内血管药物洗脱支架植入术在治疗冠状动脉疾病获得成功，采用该技术来治疗血管性Ed已激起了医务人员的浓厚兴趣。最近报道了针对阴茎流入血管狭窄的男性，植入药物洗脱支架绕过病变处动脉已获得了较高的成功率。同样，血管内栓塞治疗静脉闭塞功能障碍也取得了惊人的手术成功率。然而，查阅文献发现动脉介入只推荐应用于治疗年轻的孤立性血管损伤的患者，通常为那些先前经历过创伤的患者。该手术的短期功能恢复没有达到最佳，长期疗效也有待进一步观察。总之，用微创方法治疗勃起功能障碍的希望仍然存在，但是在获得普遍认可之前仍需要进一步的研究。

虽然手术是肢体和躯干原发软组织肉瘤的根治性治疗手段,但对于高危患者单纯手术常难以获得疾病的长期控制。治疗失败的主要原因为远处转移,其次为局部复发。一些局部进展期的患者可能面临无法手术切除,或仅边缘可切除,因此预后很差。围手术期的系统治疗和放疗可提高手术疗效。如何确立围手术期系统治疗的价值颇具挑战。系统治疗可以缩小肿瘤。但要证明其对局部控制的价值从而促进实施更为保守的外科手术的证据仍不充分。对于那些原发肿瘤被认定为难以或不可能切除的患者,新辅助化疗可能为疾病的局部控制提供一种选择。围手术期的系统治疗也可以避免远处转移的发生。近期的研究表明,以蒽环类药物/异环磷酰胺为基础的辅助治疗能够使患者获益,虽然证据来自于对一项大型研究的再次分析。一项新辅助化疗的随机研究提示,对某些经过选择的肉瘤组织学亚型,以蒽环类药物/异环磷酰胺为基础的系统化疗可能改善患者无病生存和总生存期。其疗效的获得主要是通过改善远处无转移生存而不是局控率的提高。这两项研究所采用的方法均存在一定的局限性,因此需要进一步探索。尽管如此,我们相信这些结果支持在局部进展期、肢体和躯干原发的软组织肉瘤患者中进行新辅助化疗。放疗与手术结合在软组织肉瘤的治疗中有确切的疗效。虽然预后并不因放疗和手术的先后顺序而异,但不良反应却存在差异。辅助放疗较新辅助放疗有更低的围手术期伤口并发症,但相对于新辅助放疗而言,辅助放疗需要更高的放射剂量,对长期功能的预后差于新辅助放疗。因此,我们相信围手术期放疗与化疗一样,在可能的情况下,应尽可能在术前进行。

The aim of this study is to review four case-based scenarios regarding the treatment of symptomatic hypogonadism in men. The article is designed as a review of published literature. We conducted a Pub Med literature search for the time period of 1989-2014, concentrating on 26 studies investigating the efficacy of various therapeutic options on semen analysis, pregnancy outcomes, time to recovery of spermatogenesis, as well as serum and intratesticular testosterone levels. Our results demonstrated that exogenous testosterone suppresses intratesticular testosterone production, which is an absolute prerequisite for normal spermatogenesis. Cessation of exogenous testosterone should be recommended for men desiring to maintain their fertility. Therapies that protect the testis involve human chorionic gonadotropin(h CG) therapy or selective estrogen receptor modulators(SERMs), but may also include low dose h CG with exogenous testosterone. Off-label use of SERMs, such as clomiphene citrate, are effective for maintaining testosterone production long-term and offer the convenience of representing a safe, oral therapy. At present, routine use of aromatase inhibitors is not recommended based on a lack of longterm data. We concluded that exogenous testosterone supplementation decreases sperm production. It was determined that clomiphene citrate is a safe and effective therapy for men who desire to maintain fertility. Although less frequently used in the general population, h CG therapy with or without testosterone supplementation represents an alternative treatment.

Machine-learned metrics for predicting the likelihood of success in materials discovery

Materials discovery is often compared to the challenge of finding a needle in a haystack.While much work has focused on accurately predicting the properties of candidate materials with machine learning(ML),which amounts to evaluating whether a given candidate is a piece of straw or a needle,less attention has been paid to a critical question:are we searching in the right haystack?We refer to the haystack as the design space for a particular materials discovery problem(i.e.,the set of possible candidate materials to synthesize),and thus frame this question as one of design space selection.In this paper,we introduce two metrics,the predicted fraction of improved candidates(PFIC),and the cumulative maximum likelihood of improvement(CMLI),which we demonstrate can identify discovery-rich and discovery-poor design spaces,respectively.A combined classification system,composed of the CMLI and PFIC metrics,is then used to identify optimal design spaces with high precision,and thus show the potential to significantly accelerate ML-driven materials discovery.

Background:This study aimed at investigating the effects of mycotoxin challenge on the growth and physiology of nursery pigs with different weaning weights.Results:At weaning,10 pigs were euthanized to collect jejunal mucosa and 90 pigs were assigned following a randomized complete block design in a 2×2 factorial arrangement of treatments with 3 pigs per pen.Factors were:weaning weight(light:body weight,BW9.0 kg);and dietary mycotoxins(supplementation of 0.2 mg/kg aflatoxins,2.0 mg/kg deoxynivalenol).All diets had titanium dioxide as an external marker at 0.5%.Growth performance and fecal score were recorded until pigs achieved 20 kg BW(light pigs average BW=21.1 kg and heavy pigs average BW=20.5 kg).Pigs were sampled for blood,ileal digesta,jejunal tissue and mucosa at 20 kg BW.data were analyzed using the mixed procedure of SAS.At weaning,light pigs had decreased(Pd(Pd increased(Pdiversity indexes of jejunal mucosa-associated microbiota.At 20 kg of BW,light pigs had decreased(Pdaily gain(AdG),average daily feed intake(AdFI),and gain to feed ratio(G/F).Mycotoxins decreased(PdG,AdFI,and G/F.Light pigs tended to have increased fecal score on d 0(P=0.080),d 10(P=0.069),and increased(Pdecreased the apparent ileal digestibility of nitrogen(Pd increased(Pdialdehydes and interleukin 8.Mycotoxins tended to increase(P=0.060)intestinal tumor necrosis factor-α.Conclusions:Nursery pigs with light weaning weight were more susceptible to jejunal inflammation and had impaired intestinal health due to weaning stress,whereas mycotoxins diminished the health and growth of nursery pigs regardless of weaning weight.

A direct aqueous supercritical fluid extraction （SFE） system using carbon dioxide provides a sustainable means by which a vast range of industries may continue to depend on well established infrared （IR） techniques to determine oil-in-water. The SFE-IR method provides an environmentally friendly substitute for current national standard IR reference methods for measuring oil-in-water that rely on using increasingly restricted ozone depleting solvents whose manufacture is being phased out in accordance with international law. The SFE-IR analysis of a 500 mL water sample can be accomplished in 15 min. A rapid on-line SFE-IR calibration method has been implemented. With this calibration method, SFE-IR accuracy for determining diesel oil in 500 mL spiked water samples using single wave number measurement was 86.0%-98.8% with precision （RSd） ranging from 2.5%-7.0%. Using a general purpose calculation which involves measuring infrared absorbance values at three different wave numbers, SFE-IR method accuracy for determining diesel oil in 500 mL spiked water samples was 83.7%-92.2% with RSd 1.0%-9.3%. data is presented that indicates current long established national standard IR reference methods involving three wave number calculations should be reviewed since, without careful consideration, the inclusion of calculated aromatic hydrocarbon species contributions to final oil-in-water concentration values may provide less accurate results.
