Risk of fracture in celiac disease:Gender,dietary compliance,or both?

AIM:To determine the incidence of peripheral fractures in patients with celiac disease (CD) and the effect of treatment on fracture ***:We compared the incidence and risk of peripheral fractures before and after diagnosis between a cohort of 265 patients who had been diagnosed with CD at least 5 years before study entry and a cohort of 530 age-and sex-matched controls who had been diagnosed with functional gastrointestinal *** were collected through in-person interviews with an *** overall assessment window for patients was 9843 patient-years (2815 patient-years after diagnosis).RESULTS:Compared with the control group,the CD cohort showed significantly higher incidence rate and risk of first peripheral fracture before diagnosis [adjusted hazard ratio (HR):1.78,95% CI:1.23-2.56,P ociated with the classic CD presentation with gastrointestinal symptoms (P od after diagnosis,the risk of fractures was comparable between the CD cohort and controls in both sexes (HR:1.08,95% CI:0.55-2.10 for women;HR:1.57,95% CI:0.57-4.26 for men).CoNCLUSIoN:CD patients have higher prevalence of fractures in the peripheral skeleton before *** is associated with male sex and classic clinical *** fracture risk was reduced after the treatment.

AIM: To establish the diagnostic performance of sev-eral serological tests, individually and in combination, for diagnosing celiac disease (CD) in patients with different pretest probabilities, and to explore potential se- rological algorithms to reduce the necessity for biopsy. METHoDS: We prospectively performed duodenal biopsy and serology in 679 adults who had either high risk (n = 161) or low risk (n = 518) for CD. Blood samples were tested using six assays (enzyme-linked immunosorbent assay) that detected antibodies to tissue transglutaminase (tTG) and deamidated gliadin peptide (DGP). RESULTS: CD prevalence was 39.1% in the high-risk population and 3.3% in the low-risk group. In high-risk patients, all individual assays had a high diagnostic efficacy [area under receiving operator characteristic curves (AU RoC): 0.968 to 0.999]. In contrast, assays had a lower diagnostic efficacy (AU RoC: 0.835 to 0.972) in the low-risk group. Using assay combinations, it would be possible to reach or rule out diagnosis of CD without biopsy in 92% of cases in both pretest populations. We observed that the new DGP/tTG Screen assay resulted in a surplus compared to more conventional assays in any clinical situation. CoNCLUSIoN: The DGP/tTG Screen assay could be considered as the best initial test for CD. Combinations of two tests, including a DGP/tTG Screen, might be able to diagnose CD accurately in different clinical scenarios making biopsy avoidable in a high proportion of subjects.

BACKGRoUND Life-long removal of gluten from the diet is currently the only way to manage celiac disease(CeD). Until now, no objective test has proven useful to objectively detect ingested gluten in clinical practice. Recently, tests that determine consumption of gluten by assessing excretion of gluten immunogenic peptides(GIP) in stool and urine have been developed. Their utility, in comparison with conventional dietary and analytical follow-up strategies, has not been fully *** To assess the performance of enzyme-linked immunosorbent assay(ELISA) and point-of-care tests(PoCTs) for GIP excretion in CeD patients on gluten-free diet(GFD).METHoDS We conducted an observational, prospective, cross-sectional study in patients following a GFD for at least two years. Using the Gastrointestinal Symptom Rating Scale questionnaire, patients were classified at enrollment as asymptomatic or symptomatic. Gluten consumption was assessed twice by 3-d dietary recall and GIP excretion(by ELISA in stool and PoCTs(commercial kits for stool and urine) in two consecutive samples. These samples and dietary reports were obtained 10 day apart one from the other. Patients were encouraged to follow their usual GFD during the study *** Forty-four patients were enrolled, of which 19(43.2%) were symptomatic despite being on a GFD. overall, 83 sets of stool and/or urine samples were *** out of 44 patients(25.0%) had at least one positive GIP test. The occurrence of at least one positive test was 32% in asymptomatic patients compared with 15.8% in symptomatic patients. GIP was concordant with dietary reports in 65.9% of cases(Cohen′s kappa: 0.317). PoCT detected dietary indiscretions. Environmental Impact Assessment Follow-Up of Seismic Survey offshore Activities in Brazil

Follow-up of environmental impacts is an integral part of the Environmental Impact Assessment (EIA) process, closely related to the effectiveness of the instrument. EIA follow-up has been receiving a lot of interest from scientists and practitioners, though it is recognized as one of the weakest points of EIA systems globally. Also, EIA follow-up is influenced by the context, mainly in terms of the types of projects or activities and their related impacts on the environment. Therefore, the present paper is focused on the investigation of the follow-up stage applied to the activity of seismic survey coupled with offshore oil & gas exploitation in Brazil. Research was based on a qualitative approach that included document analysis and semi-structured interviews with analysts involved in EIA processes, and sought to generate evidence of effectiveness of the EIA follow-up as conducted by the Federal Environment Agency (Ibama) in order to situate the practice of follow-up in the broader context of international best practice principles. Based on the findings, it was concluded that, due to the peculiarities of offshore seismic survey, it is necessary to promote adaptations in the procedures for monitoring impacts in order to ensure proper alignment with the principles and conceptual foundations that guide EIA practice. Specifically, the timing of the execution of the activity imposes challenges for its integration into the “conventional” cycle that has guided the monitoring of the impacts in the EIA of projects.

Alcohol-related liver disease(ALD)and metabolic dysfunction-associated steatotic liver disease(MASLD)are the most common causes of chronic liver disease worldwide(1).Daily alcohol consumption thresholds(or women andor men)are used to arbitrarily differentiate MASLD from ALD(2).However,“safe”levels of alcohol intake are difficult to define because of wide variations in the factors that contribute to its susceptibility and multiple effect *** MASLD,conflicting results on whether light alcohol intake is detrimental or beneficial.

Assembly structures of coastal woody species of eastern South America:Patterns and drivers

Tropical regions have provided new insights into how ecological communities are *** dry coastal communities,water stress has been hypothesized to determine plant assembly structure by favoring preadapted lineages from neighboring ecosystems,consistent with functional ***,it is unclear whether this hypothesis is sufficient to explain how coastal communities in tropical ecosystems are ***,we test whether water stress or other factors drive community assembly in woody plant communities across the coastal zone of Brazil,a tropical *** characterized functional and phylogenetic structures of these communities and determined the underlying environmental factors(e.g.,water stress,historical climate stability,edaphic constraints,and habitat heterogeneity)that drive their community *** of coastal woody species show geographically varied patterns,including stochastic arrangements,clustering,and overdispersion of species relative to their traits and phylogenetic *** complexity,water vapor pressure,and soil nutrient availability best explained the gradient in the functional *** deficit,water vapor pressure,and soil organic carbon were the best predictors of variation in phylogenetic *** results support the water-stress conservatism hypothesis on functional and phylogenetic structure,as well as the effect of habitat heterogeneity on functional structure and edaphic constraints on functional and phylogenetic *** effects are associated with increased phenotypic and phylogenetic divergence of woody plant assemblages,which is likely mediated by abiotic filtering and niche opportunities,suggesting a complex pattern of ecological assembly.

During the rainy season in Brazil,landslides can have catastrophic consequences,including loss of life,damage to urban infrastructure and significant economic ***,more than eight million people in Brazil live in disaster-prone *** objective of this article is to present a chronological review of the principal policies and programmes at the national,state,and municipal levels,with an emphasis on their key actions aimed at mitigating the impact of disasters related to natural hazards,particularly *** analysis was based on official records of public policies from the national,state,and municipal *** results show that several initiatives were initially developed at the local and regional ***,it was not until the late 1980s that a comprehensive organisation of civil protection at the national level emerged as a response *** is possible to highlight three phases(understanding,coexistence,and prevention)of institutional action focused on risk management in the face of *** the growing knowledge of risk scenarios,disasters continue to occur in Brazil on an annual basis,revealing several obstacles to reducing their impact,particularly given the social and economic disparities between Brazilian regions and the applicability of public policies that must overcome the limitations of each municipality.

We present a method to fabricate handcrafted thermoelectric devices on standard office paper *** devices are based on thin films of WS_(2),Te,and BP(P-type semiconductors)and TiS_(3)and TiS_(2)(N-type semiconductors),deposited by simply rubbing powder of these materials against *** thermoelectric properties of these semiconducting films revealed maximum Seebeck coefficients of(+1.32±0.27)mV K^(-1)and(-0.82±0.15)mV K^(-1)for WS_(2)and TiS_(3),***,Peltier elements were fabricated by interconnecting the P-and N-type films with graphite electrodes.A thermopower value up to 6.11 mV K^(-1)was obtained when the Peltier element were constructed with three *** findings of this work show proof-of-concept devices to illustrate the potential application of semiconducting van der Waals materials in future thermoelectric power generation as well as temperature sensing for low-cost disposable electronic devices.

We conducted the first light curve study of the binary star EL Tuc within the Binary Systems of South and North project's *** photometric observations were made using standard multiband BVR_cI_c filters at an observatory in *** presented a new ephemeris for EL Tuc and a linear fit to the o–C diagram,utilizing our extracted times of minima and additional *** employed the PHysics of Eclipsing BinariEs Python code and the Markov chain Monte Carlo approach for the system's light curve *** target system's light curve solution required a cold starspot on the hotter *** conclude that EL Tuc is a total contact binary system with a low mass ratio of q=0.172±0.002,an orbital inclination of i=83°.74±0°.40,and a fillout factor of f=53.7%±1.6%.We used the P-a relationship and the Gaia Data Release 3 parallax method to determine the absolute parameters of EL Tuc to compare the precision of our *** system was classified as W-type based on the mass and effective temperature of the companion *** positions of the systems were depicted on the M-L,M-R,T-M,and *** relationship between the spectroscopic and photometric mass ratios of binaries was discussed.
