AIM To compare gallstones removal rate and incidence of bleeding, pancreatitis, use of mechanical lithotripsy, cholangitis and perforation between isolated sphincterotomy vs sphincterotomy associated with balloon dilation of papilla in choledocholithiasis through the meta-analysis of randomized clinical trials. METHODS We conducted a systematic review according to the PRISMA guidelines. Literature search was restricted to randomized controlled trials(RCTs) on Med Line, Cochrane Library, LILACS, and EMBASE database platforms in July 2017. The manual search included references of retrieved articles. We extracted data focusing on outcomes: The primary endpoint was the stones removal rate; Secondary endpoints were rates of pancreatitis, bleeding, use of mechanical lithotripsy(ML), perforation and cholangitis. RESULTS Eleven RCTs with 1824 patients were included. EST was associated with more post-endoscopic retrograde cholangiopancreatography(ERCP) bleeding [FE RD-0.02, CI(-0.03,-0.00), I2 = 33%, P = 0.05] and more need of mechanical lithotripsy in general [RE RD-0.16, CI(-0.25,-0.06), I2 = 90%, P = 0.002] and in subgroup analysis of stones greater than 15 mm [RE RD-0.20, CI(-0.38,-0.02), I2 = 82%, P = 0.003]. Incidence of pancreatitis [FE RD-0.01, CI(-0.03, 0.01), I2 = 0, P = 0.36], cholangitis [FE RD-0.00, CI(-0.01, 0.01), I2 =0, P = 0.97] and perforation [FE RD-0.01, CI(-0.01, 0.00), I2 = 0, P = 0.23] was similar between the groups as well as similar stone removal rates in general [FE RD-0.01, CI(-0.01, 0.04), I2 = 0, P = 0.23] and pooled analysis of stones greater than 15 mm [FE RD-0.02, CI(-0.02, 0.07), I2 = 11%, P = 0.31]. CONCLUSION Through meta-analysis of randomized clinical trials we found that isolated sphincterotomy was associated with more post-ERCP bleeding and more need for mechanical lithotripsy. Acute appendicitis is rare in the postoperative period of liver transplantation;only 23 cases were described in the literature to date, including late and immediate postoperative. Our case reports a patient who was presented with acute appendicitis in the immediate post-transplant and died in the subsequent postoperative period. The article reviews the available literature and all cases known until now, commenting on incidence, casual factors, symptoms, diagnostic and management.

GTD (Gestational Trophoblastic Disease) is a pathology that encompasses benign and malignant clinical forms, affects women of childbearing age, has a variable incidence and is more frequent in developing or underdeveloped countries, colliding with the economic barrier. The frequent absence of clear protocols and guidelines for the correct diagnosis of the pathology results in inadequate classification, imprecise treatment and failed post-therapeutic observation, increasing the risk of relapses, morbidity and mortality. The present study aims to point out updated national and international practice protocols of diagnosis of GTD, through an integrative review. Seven articles were selected and it was observed that the main international reference centers are agreed with the management suggested by the IFGO (International Federation of Gynecology and Obstetrics), being the conduct in the Hydatidiform Mole (HM): evacuation by suction and curettage under ultrasound guidance, followed by hCG monitoring every 1 - 2 weeks until normalized (usually one month for Partial Hydatidiform Mole six months for Complete Hydatidiform Mole and one year for Gestational Trophoblastic Neoplasia). Unfortunately, regarding the diagnosis of MH, the guidelines of some countries show the absence or difficulty of access to the karyotype test and ploid p57 or pelvic ultrasound accompanying the uterine curettage, contrary to what is proposed by the IFGO guideline. Establishing and complying with consistent guidelines can improve patient care, with early diagnosis of the pathology and its complications, reducing the rate of recurrence, morbidity and mortality, especially in less developed countries.

The EFVM （Vitoria Minas Railroad） is one of the main railways in Brazil. It transports freight trains of ore, 220 wagons each. These wagons have 2 boogies of 2 axles each and 32 metric tons on metre gauge. Elastic strains were measured on a special part of this railway due to these trains. The main load to evaluate stresses and strains was a G 16 Locomotive, a C-C kind from Vale, a Brazilian Company. The measurements were obtained by dynamic deflectometer installed on a main line of this railway, near Ipatinga, a city from Minas Gerais, one of Brazil states. This track was equipped to obtain stresses under an equal repeated static load A simulation of the stresses was made under critical strain by Ferrovia 1.0 software. It was also made an evaluation of unequal results from neighbor sleepers taking in comparison two equipped parts of this railway, one with compacted ballast and no compaction to the other. The results were strain limited, avoiding breakage or damage to the studied rails. This work analyses these measurements focusing on the improvement of track quality.

This paper presents an automatic system for failure detection in hydro-power generators. The main idea of this system is to detect failure using current and voltage signals acquired without any type of internal interference in the generator operation. The detected failures could be mechanical or electrical origins, such as: problems in bearings, unwanted vibrations, partial discharges, misalignment, unbalancing, among others. It is possible because the generator acts as a transducer for mechanical problems, and they appear in current and voltage signals. This automatic system based on electric signature analysis has been installed in Itapebi Power Plant generators since 2012. Some results are presented in this paper.

The species Piper aduncum is a plant with great economic potential, because the essential oil has antimicrobial activity and insecticides. Thus, as the light directly affects photosynthesis process, the present study evaluated the gas exchanges’ variables and photosynthetic pigments’ production of P. aduncum grown under different irradiances. Treatments consisted in environments with 100%, 70% and 50% of irradiance and environments with colored nets (red and blue). After 150 days of culture, photosynthetic activity (A), stomatal conductance (gs) transpiration rate (E), internal CO2 concentration (Ci), deficit vapor pressure (DVP), leaf temperature, concentration of internal/external CO2 (Ci/Ca), carboxylation efficiency (A/Ci) and efficiency water use (A/E) were evaluated. Pigments chlorophyll a, b, carotenoids, total and chlorophyll a/b ratio were also measured. From the observed data, it can be concluded that the species Piper aduncun developed various mechanisms for adaptation to different irradiance conditions. Moreover, it can be inferred that their photosynthetic process is more efficient and has larger spectral intensities of light, with higher efficiency photosynthetic when grown under 70% and 100% of radiation.

This paper aims to conduct a study of the problems associated with the wear of the needles and fuel injection nozzles utilized in diesel engines. The wear found on the needles is mainly associated to impurities in the fuel oil and microcavitation occurred due to high pressure in the phase of the air compression for combustion of the combustible fluid. These pressures associated with the temperature and the fluid velocity results in the occurrence of vaporization, which releases shock waves that cause damage to the affected surface. The impurities solid particles from the fuel oil cause problems inside the nozzles as obstruction of the holes and wear on the needle tip and nozzle seat surface. These failures affect in the atomization of the fuel, since the deterioration of the internal passages of the nozzles interferes in the spray formation and in the end passage of the fluid. For the execution of this study it will be used digital microscopic analysis in specimens that suffered damage, in order to investigate the effects of fuel property, and the temperature conditions and pressure in the formation of the wear on the needles and injector nozzle.
