Pelvic exenterations for primary rectal cancer:Analysis from a 10-year national prospective database

AIM To identify short-term and oncologic outcomes of pelvic exenterations(PE) for locally advanced primary rectal cancer(LAPRC) in patients included in a national prospective *** Few studies report on PE in patients with LAPRC. For this study, we included PE for LAPRC performed between 2006 and 2017, as available, from the Rectal Cancer Registry of the Spanish Association of Surgeons [Asociación Espa?ola de Cirujanos(AEC)]. Primary endpoints included procedure-associated complications, 5-year local recurrence(LR), disease-free survival(DFS) and overall survival(OS). A propensitymatched comparison with patients who underwent non-exenterative surgery for low rectal cancers was performed as a secondary *** Eight-two patients were included. The mean age was 61.8 ± 11.5 years. More than half of the patients experienced at least one complication. Surgical site infections were the most common complication(abdominal wound 18.3%, perineal closure 19.4%). Thirty-three multivisceral resections were performed, including two hepatectomies and four metastasectomies. The long-term outcomes of the 64 patients operated on before 2013 were assessed. The five-year LR was 15.6%, the distant recurrence rate was 21.9%, and OS was 67.2%, with a mean survival of 43.8 mo. R+ve resection increased LR [hazard ratio(HR) = 5.58, 95%CI: 1.04-30.07, P = 0.04]. The quality of the mesorectum was associated with DFS. Perioperative complications were independent predictors of shorter survival(HR = 3.53, 95%CI: 1.12-10.94, P = 0.03). In the propensity-matched analysis, PE was associated with better quality of the specimen and tended to achieve lower LR with similar *** PE is an extensive procedure, justified if disease-free margins can be obtained. Further studies should define indications, accreditation policy, and quality of life in LAPRC.

Technological improvements are crucial in the evolution of ***-time fluorescence-guided surgery(FGS)has spread worldwide,mainly because of its usefulness during the intraoperative decision-making *** success of any gastrointestinal oncologic resection is based on the anatomical identification of the primary tumor and its regional lymph *** allows also to evaluate the blood perfusion at the gastrointestinal stumps after colorectal or esophageal ***,a reduction on the anastomotic leak rates has been postulated as one of the foreseeable benefits provided by the use of FGS in these *** the use of fluorescence in lymph node detection was initially described in breast cancer surgery,the technique is currently applied in gastric or splenic flexure cancers,as they both present complex and variable lymphatic *** allows also to perform intraoperative lymphograms or sentinel lymph node *** applications of FGS are being developed to assist in the detection of peritoneal metastases or in the evaluation of the tumor resection *** present review aims to provide a general overview of the current status of real-time FGS in gastrointestinal oncologic *** put a special focus on the different applications of FGS,discussing the main findings and limitations found in the contemporary literature and also the promising near future applications.

Total mesorectal excision(TME)is the standard surgical treatment for the curative radical resection of rectal *** invasive TME has been gaining ground favored by the continuous technological *** procedures,such as transanal TME(TaTME),have been introduced to overcome some technical limitations,especially in low rectal tumors,obese patients,and/or narrow *** earliest TaTME reports showed promising results when compared with the conventional laparoscopic ***,recent publications raised concerns regarding the high rates of anastomotic leaks or local recurrences observed in national *** TaTME(R-TaTME)has been proposed as a novel technique incorporating the potential benefits of a perineal dissection together with precise control of the distal margins,and also offers all those advantages provided by the robotic technology in terms of improved precision and *** short-term results have been reported for R-TaTME,but further studies are needed to assess the real role of the new technique in the long-term oncological or functional *** present review aims to provide a general overview of R-TaTME by analyzing the body of the available literature,with a special focus on the potential benefits,harms,and future perspectives for this novel approach.

Green ionic metal-organic frameworks as nanocatalysts for CO_(2)fixation

Co,Ni,and Zn iso-reticular coordination polymers of 2,5-dihydroxyterphthalic acid(H_(2)dot)can be rapidly obtained under room temperature and aqueous conditions using either sodium or ammonium hydroxide as the bases to deprotonate the *** use of metal bromides and tetrabutylammonium hydroxide(TBAOH)as the base allows the green synthesis of ionic metal-organic frameworks(MOFs),characterized by X-ray diffraction(XRD),Fourier transform infrared(FTIR),scanning electron microscopy(SEM),thermo gravimetric analysis(TGA),H-nuclear magnetic resonance(NMR),elemental analysis,and X-ray photoelectron spectroscopy(XPS),and employed in the pressure,solvent,and additive-free CO_(2)fixation to epoxides by its cycloaddition to epichlorohydrin,exhibiting a cooperative behavior between the MOF and the ionic liquid formed within the framework,achieving TONs of the confined tetrabutylammonium bromide(TBABr)up to 700.

Hydrothermal alteration of the surface volcanic rocks at the Acoculco geothermal field,Mexico:a multi-parametric approach

The studies on hydrothermal alteration-induced eff ects in surface and subsurface rocks provide useful information in the characterization and exploitation of a geothermal ***,these studies are based on traditional,and reliable methods like petrography(primary and secondary minerals,and grade of alteration),and geochemistry(mobility of elements,changes in mass and concentration of elements,and fluid inclusions).Recently,apart from these established methods,some methods based on the geochemical(Chemical Index of Alteration,CIA;Weathering Index of Parkar,WIP;Loss on Ignition,LOI;and Sulfur,S)and rock magnetic properties(magnetic susceptibility,χlf;and percentage frequency-dependent susceptibility,χfd%)are also being applied in the identification of whether a rock is an altered or a fresh *** Acoculco Geothermal Field(AGF),Mexico,is characterized by high temperature and very low permeability,and it is considered a promissory Enhanced Geothermal *** following changes are observed in the rocks as a result of an increase in hydrothermal alteration:(1)an increase in CIA,LOI,and S values,and a decrease in WIP;(2)an increase in quartz and quartz polymorph minerals(silicification),and clay minerals(argillization);and(3)decrease inχlf *** AGF,the most altered surface acid rocks are characterized by entirely quartz and its polymorphs,and clay *** present study also indicates the applicability of the binary plots of major elements(felsic vs mafic component)and rock magnetic parameters(χlf vs.χfd%).The rock withχfd%value of 2-10 andχlf value<0.5×10^(-6)m^(3) kg^(-1)indicate the presence of single domain and stable single domain grains,which in turn suggests that it is an altered *** methods are simple to apply,rapid,reliable,and have the potential to become eff ective tools for the identifi cation of hydrothermally altered rocks during the initial stage of geothermal exploration.

Chemotherapy for gastric cancer

Metastatic gastric cancer remains a non-curative disease. Palliative chemotherapy has been demonstrated to prolong survival without quality of life compromise. Many single-agents and combinations have been confirmed to be active in the treatment of metastatic disease. Objective response rates ranged from 10-30% for single-agent therapy and 30-60% for polychemotherapy. Results of phase Ⅱ and Ⅲ studies are reviewed in this paper as well as the potential efficacy of new drugs. For patients with localized disease, the role of adjuvant and neoadjuvant chemotherapy and radiation therapy is discussed. Most studies on adjuvant chemotherapy failed to demonstrate a survival advantage, and therefore, it is not considerecl as standard treatment in most centres. Adjuvant immunochemotherapy has been developed fundamentally in Korea and Japan. A meta-analysis of phase Ⅲ trials with OK-432 suggested that immunochemotherapy may improve survival of patients with curatively resected gastric cancer. Based on the results of US Intergroup 0116 study, postoperative chemoradiation has been accepted as standard care in patients with resected gastric cancer in North America. However, the results are somewhat confounded by the fact that patients underwent less than a recommended D1 lymph node dissection and the pattern of recurrence suggested a positive effect derived from local radiotherapy without any effect on micrometastatic disease. Neoadjuvant chemotherapy or chemoradiation therapy remains experimental, but several phase Ⅱ studies are showing promising results. Phase Ⅲ trials are needed.

Recent studies highlight the strong correlation between infectious diseases and the development of neuropsychiatric *** this editorial,we comment on the article“Anti-infective therapy durations predict psychological stress and laparoscopic surgery quality in pelvic abscess patients”by Zhang et al,published in the recent issue of the World Journal of Psychiatry 2023;13(11):*** discussion highlighted the potential consequences of anxiety,depression,and psychosis,which are all linked to bacterial,fungal,and viral infections,which are relevant to the impact of inflammation on the sequelae in mental health as those we are observing after the coronavirus disease 2019 *** focus specifically on the immune mechanisms triggered by inflammation,the primary contributor to psychiatric ***,pathophysiological mechanisms such as organ damage,post-injury inflammation,and infectioninduced endocrine alterations,including hypocortisolism or autoantibody formation,significantly contribute to the development of chronic low-grade inflammation,promoting the emergence or development of psychiatric alterations in susceptible *** inflammation can have long-term effects on patients,a multidisciplinary treatment plan can avoid complications and debilitating health issues,and it is crucial to recognize and address the mental health implications.

Should capecitabine replace 5-fluorouracil in the first-line treatment of metastatic colorectal cancer?

Fluoropyrimidines play a central role in the first-line treatment of metastatic colorectal cancer. Our aim was to review whether capecitabine was a safer, non-inferior, economically superior and more convenient alternative to 5-fluorouracil. Capecitabine has previously been compared to 5-fluorouracil-either as a monotherapy or in combination with oxaliplatin, irinotecan, or biological drugs-and has been found to have comparable efficacy and safety profiles. Furthermore, pharmacoeconomic data and patients’ preferences for oral chemotherapy further favor capecitabine. Therefore, capecitabine appears to be an effective and safe alternative to fluorouracil in the first-line treatment of metastatic colorectal cancer.

Enhancement of the benthic communities around an isolated island in the Antarctic Ocean

Insular marine biotas are often richer in faunal diversity than those from the open sea in the same geo- graphical region. The existence of particular island effects were tested under polar conditions by compar- ing infaunal benthic assemblages on Peter I Island with those of similar latitudes in the open ocean at the Bellingshausen Sea and also from the coast of the Antarctic Peninsula. Sampling was carried out aboard the R/V Hesperides during the expedition named BENTART-2003 from 24 January to 3 March 2003. Benthic samples were collected at 18 stations ranging from 90 to 2 044 m depth, using an USNEL-type box corer （BC） dredge. Representatives of 32 higher taxa of invertebrates were found. Univariate and multivariate analyses revealed various patterns in the data. First, significant differences in polychaete abundance were detected between the stations located in the open sea and the remaining sites （island plus mainland sites）. Bivalve abundances were also distinct between island and mainland sampling sites. Sediment columns taken from the island seafloor exhibited the highest rate of bioturbation by the infauna. These findings confirm the model that islands develop distinct assemblages characterized by the enhancement of the benthic commu- nities even in cold waters. Several abiotic factors were measured simultaneously at the seafloor and along the water column to investigate faunal distribution patterns. Significant correlations were found between the benthic assemblages and a combination of two environmental variables：＂island effect＂（measured as a categorical variable） and the redox state of sediments. Richer and more complex benthic assemblages were found in Peter I Island＇s sea bottom, whereas the more depauperate bottoms remained in the open sea.
