Biliary stenosis may represent a diagnostic and therapeutic challenge resulting in a delay in diagnosis and initiation of therapy due to the frequent difficulty in distinguishing a benign from a malignant *** such cases,the diagnostic flowchart includes the sequential execution of imaging techniques,such as magnetic resonance,magnetic resonance cholangiopancreatography,and endoscopic ultrasound,while endoscopic retrograde cholangiopancreatography is performed to collect tissue for histopathological/cytological diagnosis or to treat the stenosis by insertion of *** execution of percutaneous transhepatic drainage with subsequent biopsy has been shown to increase the possibility of tissue diagnosis after failure of the above *** the diagnostic yield of histopathology and imaging has increased with improvements in endoscopic ultrasound and peroral cholangioscopy,differential diagnosis between malignant and benign stenosis may not be easy in some patients,and strictures are classified as *** these cases,a multidisciplinary workup including biochemical marker assays and advanced technologies available may speed up a diagnosis of malignancy or avoid unnecessary surgery in the event of a benign ***,we review recent advancements in the diagnosis and management of biliary strictures and describe tips and tricks to increase diagnostic yields in clinical routine.

AIM: To investigate the prevalence of human leukocyte antigen (HLA) DQ2/8 alleles in Southern Italians with liver and gastrointestinal (GI) diseases outside of celiac disease. METHODS: HLA DQ2/8 status was assessed in 443 patients from three ambulatory gastroenterology clinics in Southern Italy (University of Federico Ⅱ, Naples, Loreto Crispi Hospital, Ruggi D'Aragona Hospital, Salerno). Patients were grouped based on disease status [pre-post transplant liver disease, esophageal/gastric organic and functional diseases, irritable bowel syndrome (IBS) and inflammatory bowel disease (IBD)] and DQ2/8 alleles, which correspond to a celiac disease genetic risk gradient. Subject allele frequencies were compared to healthy Italian controls. RESULTS: One hundred and ninety-six out of four hundred and forty-three (44.2%) subjects, median age 56 years and 42.6% female, were DQ2/8 positive. When stratifying by disease we found that 86/188 (45.7%) patients with liver disease were HLA DQ2/8 positive, 39/73 (53.4%) with functional upper GI diseases and 19/41 (46.3%) with organic upper GI diseases were positive. Furthermore, 38/105 (36.2%) patients with IBS and 14/36 (38.9%) with IBD were HLA DQ2/8 positive (P = 0.21). Compared to healthy controls those with functional upper GI diseases disorders had a 1.8 times higher odds of DQ2/8 positivity. Those with liver disease had 1.3 times the odds, albeit not statistically significant, ofDQ2/8 positivity. Both those with IBS and IBD had a lower odds of DQ2/8 positivity compared to healthy controls. CONCLUSION: The proportion of individuals HLA DQ2/8 positive is higher in those with liver/upper functional GI disease and lower in IBS/IBD as compared to general population estimates.

Molecular pathogenesis of bone metastases in breast cancer: Proven and emerging therapeutic targets

Metastatic occurrence is the principal cause of death in breast cancer patients. The high osteotropism makes breast cancer the most common primary tumor type associated with metastatic bone disease. The peculiar clinical aspects associated with metastases limited to the skeletal system suggest considering these cases as a distinctive subset of metastatic patients with a better prognosis. Because bone is frequently the first metastatic site in disease relapse, it is feasible that the next improvement in therapeutic options for bone metastatic disease could be associated with an improvement of survival expectation and quality of life in breast cancer patients. Study of the molecular basis of bone remodeling and breast cancer osteotropism has allowed identification of several therapeutic candidates involved in formation and progression of bone metastases. These targets are frequently the determinants of positive feedback between the tumor and bone cells whose clinical outcome is osteolytic lesions. In this review, we discuss the physiopathologic features underlying targeted therapeutic strategies aimed at interfering with the aberrant bone remodeling associated with breast cancer metastases.

本文探讨了在进行有效调整和限制的条件下,多层级治理的概念框架对中国政治行政体制的可适用性。多层级治理在中国的应用能够解释全球影响和非政府力量在公共政策制定中的参与。本文认为多层级治理的发展可能是新权威主义条件下提高政治体制的社会合法性的一种方式。我们提出一种吸引人但也可能存在过度延伸的假设,即多层级治理的发展可能是在中国建立政治合法性过程中的重要部分,正如它可能成为欧洲探索重建陷入困境的传统自由民主制度的一个路径。中国和欧洲可能会采用多层级治理以加强各自政治制度的合法性并强化其基础。这预示者对多层级治理极强的规范性和目的性的应用。

AIM:To evaluate whether the effectiveness of Granulomonocyto apheresis(GMA),a technique that consists of the extracorporeal removal of granulocytes and monocytes from the peripheral blood,might vary according to the severity of ulcerative colitis(UC)in patients with mild to moderate-severe disease UC ***:We retrospectively reviewed prospectively collected data of patients undergoing GMA at our inflammatory bowel disease centre who had at least a 6 mo of *** demographics,clinical and laboratory data were extracted from the patients’charts and electronic *** severity of UC was scored according to the Modified Truelove Witts Severity Index(MTWSI).A clinical response was defined as a decrease from baseline of≥2 points or a value of MTWSI≤2 ***:A total of 41(24 males/17 females;meanage 47 years)patients were included in the *** GMA cycle completion,21/28(75%)of mild UC patients showed a clinical response compared with 7/13(54%)of patients with moderate to severe disease(P=0.27).At 6-mo,14/28(50%)of the mild UC patients maintained a clinical response compared with 2/13(15%)of the patients with moderate to severe disease(P=0.04).After the GMA cycle completion and during the 6-mo follow up period,13/16(81%)and 9/16(56%)of mild UC patients with intolerance,resistance and contraindications to immunosuppressants and/or biologics showed a clinical response compared with 2/6(33%)and 0/6(0%)of patients with moderate to severe disease activity with these characteristics(P=0.05and P=0.04,respectively).CONCLUSION:Patients with mild UC benefit from GMA more than patients with moderate to severe disease in the short-term *** should be considered a valid therapeutic option in cases of contraindications to immunosuppressants,corticosteroids and/or biologics.

Use of chondral fragments for one stage cartilage repair: A systematic review

AIM: To investigate the state of the art regarding Cartilage Autograft Implantation System(CAIS) or Particulated Juvenile Allograft Cartilage(PJAC).METHODS: The authors searched the English literature regarding CAIS and PJAC. The search strategy was:(particulated cartilage) OR autologous cartilage fragments. All basic science articles were included. Clinical articles with less than 10 patients treated and less than 6 mo of follow-up were excluded. With these criteria, a total of 17 articles were available for the present review. RESULTS: PJAC and CAIS are relatively novel techniques for cartilage repair. Good basic science evidence was described to support the concept. Although the preliminary clinical reports show encouraging results, clinical data are still limited, especially for CAIS. The indications for both techniques need to be precisely defined(age of the patients, size of the lesion, and involvement of the subchondral bone), together with other debated issues. CONCLUSION: In conclusion, the authors can state that encouraging preliminary results are available for both techniques. However, further studies are necessary to precisely determine the indications, surgical techniques, and long term outcomes for PJAC and CAIS.

Achieving energy efficiency in government buildings through mandatory policy and program enforcement

Government buildings are responsible for a significant proportion of energy consumption worldwide,for example, in Australia, up to 41.5 PJ energy was consumed by government buildings in 2011–2012. While the newly constructed buildings may be energy efficient,the existing buildings, which account for more than 85% of the total building stock, were built prior to the time when energy rating systems was put in practice and are consequently energy inefficient to a large degree. Reducing the energy consumption in existing government buildings is essential, as it will not only reduce the costs and environmental impacts, but also show governments' strong commitment towards the reduction of greenhouse gas emission. Furthermore, successful building energy retrofit projects are the showcases to the general public, encouraging other sectors(e.g. commercial) to conduct building retrofits for energy savings. Recognising these benefits,several state governments in Australia have introduced building energy efficiency policies and programs. This paper reviewed the energy efficiency policies/programs in five States in Australia: Victoria, New South Wales, South Australia, Western Australia, and Queensland in terms of respective policies and targets, implementation methods and current progress. The lessons learned from these programs were also discussed. This research revealed that the key factors for a successful government buildingenergy retrofitting program are 1) having a properly enforced energy efficiency mandate with clear energy saving targets, 2) establishing an expert facilitation team and 3) implementing suitable financing and procurement methods.

BRCA1/2 pathogenic variants are widely known as major risk factors mainly for breast and ovarian cancer,while their role in gastrointestinal(GI)malignancies such as colorectal cancer(CRC),gastric cancer and oesophageal cancer(OeC)is still not well *** main objective of this review is to summarise the available evidence on this *** studies included in the review were selected from PubMed/GoogleScholar/ScienceDirect databases to identify published articles where BRCA1/2 pathogenic variants were assessed either as a risk factor or a prognostic/predictive factor in these *** review suggests that BRCA1/2 might have a role as a risk factor for colorectal,gastric and OeC,albeit with differences among these diseases:In particular BRCA1 seems to be much more frequently mutated in CRC whereas BRCA2 appears to be much more closely associated with gastric and ***-onset cancer seems to be also associated with BRCA1/2 mutations and a few studies suggest a positive prognostic role of these *** assessment of a potentially predictive role of these mutations is hampered by the fact that most patients with these diseases have been treated with platinum compounds,where it is expected that a higher probability of response should be seen.A few clinical trials focused on poly(ADPribose)polymerase inhibitors use in GI cancers are currently ongoing.
