Serologic diagnosis of celiac disease: May it be suitable for adults?

The diagnosis of coeliac disease(CD)in adult patients requires the simultaneous assessment of clinical presentation,serology,and typical histological picture of villous ***,several years ago,the European Society of Pediatric Gastroenterology,Hepatology,and Nutrition guidelines approved new criteria for the diagnosis in children:Biopsy could be avoided when antitransglutaminase antibody(TGA)values exceed the cut-off of×10 upper limit of normal(ULN)and anti-endomysium antibodies are positive,independently from ***“no biopsy”approach is a decisive need for pediatric population,allowing to avoid stressful endoscopic procedures in children,if *** approach relies on the correlation existing in children between TGA levels and assessment of mucosal atrophy according to Marsh’s *** lines of evidence have shown that patients with villous atrophy have markedly elevated TGA ***,we aim to perform a narrative review on the topic in *** that some studies confirmed that the×10 ULN threshold value has a very good diagnostic performance,several lines of evidence in adults suggest that TGA cut off should be different from that of pediatric population for reaching a good correlation with histological *** conclusion,the heterogeneity of study reports as well as some conditions,which may hamper the serological diagnosis of CD(such as seronegative CD and non-celiac villous atrophy)and are much more common in adults than in children,could represent a limitation for the“no biopsy”approach to CD diagnosis in patients outside the pediatric age.

Psychological effects of colorectal cancer screening: Participants vs individuals not invited

AIM To investigate the possible long-term psychological harm of participating in colorectal cancer(CRC)screening in *** In a prospective, randomized trial, 14294 participants(aged 50-74 years) were invited to either flexible sigmoidoscopy(FS) screening, or a faecal immunochemical test(FIT)(1:1). In total, 4422 screening participants(32%) completed the questionnaire, which consisted of the Hospital Anxiety and Depression Scale and the SF-12, a generic health-related quality of life(HRQOL) measurement, when invited to screening and one year after the invitation. A control group of 7650 individuals was invited to complete the questionnaire only, at baseline and one year after, and 1911(25%) completed the *** Receiving a positive or negative screening result and participating in the two different screening modalities did not cause clinically relevant mean changes in anxiety, depression or HRQOL after one year. FS screening, but not FIT, was associated with an increased probability of being an anxiety case(score ≥ 8) at the one-year follow-up(5.6% of FS participants transitioned from being not anxious to anxious, while 3.0% experienced the reverse). This increase was moderately significantly different from the changes in the control group(in which the corresponding numbers were 4.8% and 4.5%, respectively), P = 0.06. CONCLUSION Most individuals do not experience psychological effects of CRC screening participation after one year, while FS participation is associated with increased anxiety for a smaller group.

Substances of abuse(SoA),as well as smoking and alcohol consumption,are well known for their impact on male fertility status,erectile function,and *** assessed SoA consumption habits in a cohort of men seeking medical attention for uro-andrological *** from 7447 men seeking medical attention for the first time for uro-andrological purposes were analyzed.A complete medical and sexual history was collected for each ***,alcohol,and SoA consumption were *** statistics was used to describe the whole *** primary motivations for their evaluation were lower urinary tract symptoms(LUTS),erectile dysfunction(ED),and infertility in 1912(25.7%),2944(39.5%),and 2591(34.8%)men,*** use of SoA was reported by 378(5.1%)men,and 190(2.6%)individuals were current *** seeking medical attention for infertility were more frequently current SoA users(107;4.1%)than men with ED(66;2.2%)and LUTS(17;0.9%)(both P<0.001).Current users of SoA were younger than those with past or no SoA history(P<0.001).Current SoA users were more frequently smokers(P<0.001)and alcohol consumers(P<0.001)than those with a previous history or those who had never tried *** conclusion,approximately 3%of men seeking medical attention for uro-andrological purposes were current SoA *** men reported a higher use of SoA than those with ED or *** SoA users were younger and more frequently concomitant smokers and alcohol consumers compared to those who did or had never used SoA.

Thrombus aspiration in acute myocardial infarction:Rationale and indication

Reperfusion of myocardial tissue is the main goal of primary percutaneous coronary intervention(PPCI) with stent implantation in the treatment of acute ST-segment elevation myocardial infarction(STEMI). Although PPCI has contributed to a dramatic reduction in cardiovascular mortality over three decades, normal myocardial perfusion is not restored in approximately one-third of these patients. Several mechanisms may contribute to myocardial reperfusion failure, in particular distal embolization of the thrombus and plaque fragments. In fact, this is a possible complication during PPCI, resulting in microvascular obstruction and no-reflow phenomenon. The presence of a visible thrombus at the time of PPCI in patients with STEMI is associated with poor procedural and clinical outcomes. Aspiration thrombectomy during PPCI has been proposed to prevent embolization in order to improve these outcomes. In fact, the most recent guidelines suggest the routine use of manual aspiration thrombectomy during PPCI(class Ⅱa) to reduce the risk of distal embolization. Even though numerous international studies have been reported, there are conflicting results on the clinical impact of aspiration thrombectomy during PPCI. In particular, data on long-term clinical outcomes are still inconsistent. In this review, we have carefully analyzed literature data on thrombectomy during PPCI, taking into account the most recent studies and meta-analyses.

汽车行驶稳定动力学优化控制,旨在通过确定汽车行驶状态下纵向车速、横摆角速度、质心侧偏角等重要的状态变量,提前预知汽车未来时刻的可能的行驶状态,并将其输送到汽车底盘主动控制系统,实现动力学优化控制,提高汽车的主动安全性,减少道路交通事故。寻找一种低成本、高精度且能够实时获得车辆重要状态参数的方法,是汽车稳定行驶动力学优化控制的关键技术之一。利用Matlab/Simulink仿真工具,分别建立了汽车动力学仿真模型和车辆行驶状态Kalman滤波估计仿真模型,可以同时实现对车辆行驶状态的仿真和对车辆行驶过程中横摆角速度、侧向加速度和质心侧偏角的估计,并且模型具有可扩展性。最后进行了实车场地试验,完成了阶跃曲线、双移线等操作,通过模型仿真、试验数据和状态估计结果的比较得出,三者一致性较好,同时验证了车辆动力学仿真模型和状态估计算法仿真模型的有效性和通用性。
