Variability in Vitamin A Intake of Pregnant Women in Ngaoundere-Cameroon with Geographic Origin, Socio-Professional and Demographic Factors

Variability in vitamin A (VA) intake of pregnant women in Ngaoundere town according to geographic origin, socio-professional and demographic factors was studied. A total of 100 pregnant women attending ante natal visits at the Regional Hospital in Ngaoundere were involved in the survey. A questionnaire was used to obtain information on geographic origin, socio-professional status, birth history, demographic and anthropometric factors. Dietary intake was assessed using 24-hour dietary recall. Meals potentially rich in VA consumed by these women were collected, their carotenoids contents quantified and VA activity determined by conversion. Results indicated that daily VA intake of these women varied significantly (p level of education and age of pregnancy. VA intake of pregnant women of Northern origin (Adamawa, North and Far North Regions) was significantly (p lower (496 μg/day/woman) than that of women of Southern origin (588 μg/day/woman), although both were below the recommended intake of 800 μg/day/woman. The more educated a woman and the older her pregnancy, the higher her VA intake. VA consumption of pregnant women of Northern origin was significantly influenced by their age, whereas for pregnant women of Southern origin, BMI (Body Mass Index) equally had a significant influence on their VA intake. Socio-professional status, marital status and number of children did not significantly influence the daily VA intake of these women. Thus, a pregnant woman originating from the Northern part of the country and having no formal education, consumed less foods rich in carotenoids and therefore was more at risk for vitamin A deficiency.

Effect of a program to control perioperative blood glucose on the incidence of nosocomial infections in patients with diabetes: A pilot study

Aim: To evaluate the safety and effectiveness of a pilot program to control perioperative blood glucose in patients with diabetes. Methods: A pre-post intervention study was conducted in a 280-bed hospital in Spain. In the year 2008 we implemented perioperative insulin protocols aimed at blood glucose values from 80 to 180 mg/dl. Surgical patients with diabetes admitted on year 2009 (intervention group) were compared with a control group of patients with diabetes admitted for surgery on year 2007, matched 1:1 by traditional wound class. Results: We analyzed 96 patients. Implemented protocols were followed in 48% of patients intra-operatively and 75% of patients postoperatively. Patients in the intervention group had reductions in blood glucose at surgery 150 +/- 61 mg/dl vs. 172 +/- 53 mg/dl;p = 0.05), greater proportion of target glucose values throughout hospitalization (67% vs. 55%;p = 0.07), and reductions in the incidence of nosocomial infections after controlling for confounders (Odds Ratio: 0.20;95% Confidence intervals: 0.06 - 0.72;p = 0.014) when they were compared with the control group: The incidence of hypoglycemia was similar between two groups (0.12% vs. 0.10%, p = 0.867), respectively. Conclusion: Although our protocol needs improvements to increase implementation it was useful.to control blood glucose safely and for reducing nosocomial infections.

Mobile Application for Research Knowledge Sharing and Dissemination: The Case of Nm-Aist Univeristy Tanzania

The utilization of mobile and web applications has surpassed all other platforms in terms of disseminating researchers’ knowledge among diverse communities throughout the world. The current method of disseminating researchers’ knowledge to the communities in the Arusha region in Tanzania is through meetings, workshops, and focus group discussions held by researchers, agricultural extension officers and community members after every three months or during field study. Yet the strategy is inefficient and ineffective in practice. The purpose of this study was to determine the most efficient and successful method of disseminating knowledge in communities. The study began with a qualitative phase, utilizing an interpretive technique and a qualitative multiple case study research design. The Arusha region in Tanzania was selected as a case study where different social activities were undertaken, including farming, livestock keeping, tourism activities and fishing. Individual participants were interviewed by using a semi-structured questionnaire. In addition, focus group discussions were conducted to gather more information regarding the needs of the mobile application. Through the implementation of the application, the second phase of the study led to the development of a mobile application that includes community members, agricultural extension officers, and researchers that will enable anyone to install the application on their mobile phones to access knowledge regarding activities undertaken in Arusha. According to the findings of the first phase of the research, a substantial percentage of community members own mobile phones, and hence a mobile application would be sufficient. The research also found that most researcher-community interactions occur at the data collection and intervention assessment (field trials) stages. Hence, the mobile application will benefit community members, district agricultural, irrigation, and cooperative officers (DAICO), and researchers.

A pilot study of the modulation of sirtuins on arylamine N-acetyltransferase 1 and 2 enzymatic activity

Arylamine N-acetyltransferase(NAT; E.C. 2.***.1.5) enzymes are responsible for the biotransformation of several arylamine and hydrazine drugs by acetylation. In this process, the acetyl group transferred to the acceptor substrate produces NAT deacetylation and, in consequence, it is susceptible of degradation. Sirtuins are protein deacetylases, dependent on nicotine adenine dinucleotide,which perform post-translational modifications on cytosolic proteins. To explore possible sirtuin participation in the enzymatic activity of arylamine NATs, the expression levels of NAT1, NAT2,SIRT1 and SIRT6 in peripheral blood mononuclear cells(PBMC) from healthy subjects were examined by flow cytometry and Western blot. The in situ activity of the sirtuins on NAT enzymatic activity was analyzed by HPlC, in the presence or absence of an agonist(resveratrol) and inhibitor(nicotinamide) of sirtuins. We detected a higher percentage of positive cells for NAT2 in comparison with NAT1, and higher numbers of SIRT1t cells compared to SIRT6 in lymphocytes. In situ NAT2 activity in the presence of NAM inhibitors was higher than in the presence of its substrate, but not in the presence ofresveratrol. In contrast, the activity of NAT1 was not affected by sirtuins. These results showed that NAT2 activity might be modified by sirtuins.

Prenatal Playful-Educational Activities, Psycho-Emotional Well-Being of Pregnant Women and Perinatal Outcomes: A Pilot Study

Objective: To present the results of a pilot study that evaluated the psycho-emotional well-being, and the perinatal results of a group of 44 women who attended a prenatal program that incorporates playful and educational activities, in a Child Development Center in Durango. Mexico. Methods: A group of 44 women with informed consent participated for 28 weeks in the prenatal program. The condition of psycho-emotional well-being during the first and third trimesters and the perinatal results were analyzed with validated instruments. Physiological stress was also assessed with salivary cortisol in the same trimesters. Results: More than 90% of the women evaluated their psycho-emotional condition as satisfactory and very satisfactory. The evaluation of anxiety in its Trait/State modalities placed the participants in the category of mild and moderate. Salivary cortisol showed values within a normal range. Perinatal results showed that 98% of deliveries were at term, 93% of newborns weighed more than 2500 grams, and 98% of newborns had an APGAR score of 9 at 5 minutes. Conclusion: The results suggest that the incorporation of playful and educational actions to prenatal surveillance positively influenced the biopsychosocial condition of the pregnant woman and favorable perinatal outcomes.

Spanish Version of Test MIS (*) with Delayed Memory Recall Normative Values and Results in a Population with Mild Cognitive Impairment

Background: Cognitive impairment becomes more common with ageing and may benefit from intervention. In a Spanish speaking population, detection of cognitive impairment by a general practitioner in Primary Care can be a problem, as many of the standard tests target English speaking populations. The Memory Impairment Screen (MIS-A) is a validated test using English words to detect Alzheimer’s Disease (AD) and other dementias. We have modified this test to suit a Spanish speaking population and added a new component, delayed recall. We have called our new test the Memory Impairment Screen with Delayed Recall (MIS-D). Objectives: 1) To test a Spanish version of MIS-A and MIS-D. 2) To assess the discriminative validity of MIS-D as a screening tool for the amnestic variant of Mild Cognitive Impairment (aMCI) in a group of Spanish speaking people aged 65 years old and over. Methods: A case-control study of a cohort of 739 native Spanish speaking residents of Buenos Aires aged 65 years old and over, of whom 436 were healthy controls and 303 had a diagnosis of aMCI. Measurements: Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NVP) were estimated for MIS-D and MIS-A. Results: Normative values for MIS-A and MIS-D were obtained from the control population. Both age and education significantly affected these values (p l participants showed significant differences for both modalities, MIS-A and MIS-D. The cut-off for MIS-A should be 7.5 and for MIS-D, 5.5. Comparison between control population and aMCI population using ROC curve gave a result of 5.5 in MIS-D, with 97% specificity and 76% sensitivity. Conclusion: MIS-D was positively predictive of aMCI, with 97% specificity and 76% sensitivity in a sample of Spanish speaking patients aged 65 years old and over in Buenos Aires.

Dietary Intake of Vitamin A and Macronutrients among Pregnant Women in Ngaoundere Town, Adamawa Region, Cameroon

This paper examines the risk of Vitamin A deficiency among selected pregnant women in Ngaoundere, Adamawa Region, Cameroon. A total of one hundred (100) pregnant women attending ante natal visits at the Regional Hospital in Ngaoundere were involved in a survey conducted in 2012 which used a 24-hour dietary recall.to assess their dietary intake. Food models were used to assist memory and portion sizes of food taken. Carotenoids were quantified in the meals commonly consumed, as well as proteins, sugar, fibers and oil, nutrients intervening in carotenoids absorption and vitamin A metabolism. The results indicated that the dietary intake of vitamin A was inadequate with 37.5% of the selected pregnant women exposed to acute vitamin A deficiency, while 20% of them were exposed to the severe form. The daily consumption of vitamin A was about 539.09 ± 43.09 μg, corresponding to a contribution of 67.39% to the vitamin A requirement of the pregnant women. The dietary intake of macronutrients was also inadequate with a daily consumption of 57.28 ± 12 g for oils, 177.88 ± 29 g for sugars, 47.34 ± 11 g for proteins and 11.28 ± 3 g for fibers corresponding respectively to a contribution of 63.6%, 70.8%, 78.90% and 45.1% of these nutrients requirement in these women. Given that over 50% of the women surveyed were exposed to vitamin A deficiency, there is a need for urgent intervention programs in the area to alleviate this situation.
