One in four patients with gastrointestinal bleeding develops shock or hemodynamic instability:A systematic review and meta-analysis

BACKGROUND Hemodynamic instability and shock are associated with untoward outcomes in gastrointestinal ***,there are no studies in the existing literature on the proportion of patients who developed these outcomes after gastrointestinal *** To determine the pooled event rates in the available literature and specify them based on the bleeding *** The protocol was registered on PROSPERO in advance(CRD42021283258).A systematic search was performed in three databases(PubMed,EMBASE,and CENTRAL)on 14^(th) October *** proportions with 95%CI were calculated with a random-effects model.A subgroup analysis was carried out based on the time of assessment(on admission or during hospital stay).Heterogeneity was assessed by Higgins and Thompson’s I^(2) *** Joanna Briggs Institute Prevalence Critical Appraisal Tool was used for the risk of bias *** Reference Citation Analysis(https://***/)tool was applied to obtain the latest highlight *** We identified 11589 records,of which 220 studies were eligible for data *** overall proportion of shock and hemodynamic instability in general gastrointestinal bleeding patients was 0.25(95%CI:0.17-0.36,I^(2)=100%).In non-variceal bleeding,the proportion was 0.22(95%CI:0.14-0.31,I^(2)=100%),whereas it was 0.25(95%CI:0.19-0.32,I^(2)=100%)in variceal *** proportion of patients with colonic diverticular bleeding who developed shock or hemodynamic instability was 0.12(95%CI:0.06-0.22,I^(2)=90%).The risk of bias was low,and heterogeneity was high in all *** One in five,one in four,and one in eight patients develops shock or hemodynamic instability on admission or during hospitalization in the case of non-variceal,variceal,and colonic diverticular bleeding,respectively.

Background:It remains unclear if the vascular and connective tissue structures of primary and metastatic tumors are intrinsically determined or whether these characteristics are defined by the host *** we examined the microanatomical steps of vasculature and connective tissue development of C38 colon carcinoma in different ***:Tumors produced in mice at five different locations(the cecal wall,skin,liver,lung,and brain)were ana-lyzed using fluorescent immunohistochemistry,electron microscopy and quantitative real-time polymerase chain ***:We found that in the cecal wall,skin,liver,and lung,resident fibroblasts differentiate into collagenous matrix-producing myofibroblasts at the tumor *** activated fibroblasts together with the produced matrix were incorporated by the *** connective tissue development culminated in the appearance of intratumoral tissue columns(centrally located single microvessels embedded in connective tissue and smooth muscle actin-expressing myofibroblasts surrounded by basement membrane).Conversely,in the brain(which lacks fibroblasts),C38 metasta-ses only induced the development of vascularized desmoplastic tissue columns when the growing tumor reached the fibroblast-containing ***:Our data suggest that the desmoplastic host tissue response is induced by tumor-derived fibrogenic molecules acting on host tissue *** concluded that not only the host tissue characteristics but also the tumor-derived fibrogenic signals determine the vascular and connective tissue structure of tumors.

Alzheimer’s disease is a progressive brain disorder and complex mechanisms are involved in the physiopathology of Alzheimer’s ***,there is data suggesting that inflammation plays a role in its development and ***,some non-steroidal antiinflammatory drugs,such as meloxicam,which act by inhibiting cyclooxygenase-2 have been used as neuroprotective agents in different neurodegenerative disease *** purpose of this study was to investigate the effects of co-nanoencapsulated curcumin and meloxicam in lipid core nanocapsules(LCN)on cognitive impairment induced by amyloid-beta peptide injection in *** were prepared by the nanoprecipitation *** Swiss mice received a single intracerebroventricular injection of amyloid-beta peptide aggregates(fragment 25–35,3 nmol/3μL)or vehicle and were subsequently treated with curcumin-loaded LCN(10 mg/kg)or meloxicam-loaded LCN(5 mg/kg)or meloxicam+curcumin-co-loaded LCN(5 and 10 mg/kg,respectively).Treatments were given on alternate days for 12 days(i.e.,six doses,once every 48 hours,by intragastric gavage).Our data showed that amyloid-beta peptide infusion caused long-term memory deficits in the inhibitory avoidance and object recognition tests in *** the inhibitory avoidance test,both meloxicam and curcumin formulations(oil or co-loaded LCN)improved amyloid-beta-induced memory impairment in ***,only meloxicam and curcumin-co-loaded LCN attenuated non-aversive memory impairment in the object recognition ***,the beneficial effects of meloxicam and curcuminco-loaded LCN could be explained by the anti-inflammatory properties of these drugs through cortical cyclooxygenase-2 *** study suggests that the neuroprotective potential of meloxicam and curcumin co-nanoencapsulation is associated with cortical cyclooxygenase-2 *** study was approved by the Committee on Care and Use of Experimental Animal Resources,the Federal University of Pampa,Brazil(app

Nurses’ Knowledge and Attitudes towards Pain Management in Children Admitted in the Paediatric Department of Queen Elizabeth Central Hospital, Blantyre, Malawi

Background: Inadequate pain management is a problem in hospitalized children. Objectives: To explore knowledge and attitudes of nurses in management of pain in children. Methods: A descriptive design using qualitative methods was used to conduct the study. 17 nurses with prior pain management training were interviewed using a semi-structured interview guide. Ethical approval was obtained from College of Medicine Research and Ethics Committee. Data were analysed using thematic content analysis. Results: Nurses had some knowledge of pain assessment methods, treatment and use of morphine. Gaps, however, existed on how to use pain assessment scales, analgesics to use at each step of the WHO analgesic ladder, and how to effectively use morphine for pain relief. Although most nurses had positive attitudes towards children’s pain and use of morphine, negative attitudes were evident in some nurses regarding administration of morphine to neonates or acutely ill children. Conclusion and Implications: The study has revealed knowledge and attitude gaps which may reflect deficiencies in the pain education nurses received, non-availability of pain scales and lack of support for nurses to link theory with practice. Nurse leaders are challenged to provide repeated comprehensive education for nurses on pain assessment and management. Furthermore, user friendly pain scales should be developed, and nurses educated and demonstrated on their use. In addition, opportunities for supportive supervision with nurses in the clinical setting should be created for linkage of theory and practice.

The distribution of parotid gland neoplasms in a veteran population

Objective:Salivary gland tumors account for 6%-8%of head and neck neoplasms with the parotid gland as the most common primary *** adenomas(PA)are considered the most common benign parotid gland neoplasms,followed by Warthin tumors(WT).The goal of this study was to investigate the distribution of parotid gland neoplasms among a United States veteran ***:Retrospective chart ***:Washington DC Veterans Affairs Medical ***:Veterans who underwent fine needle aspiration(FNA)for a parotid gland mass from 2000 to 2018 were *** records were reviewed for gender,age,tobacco use,surgery date,and pathology *** outcome measures:Changes in the distribution of parotid neoplasms and tobacco use over an 18-year ***:Of 141 patients with parotid gland masses,86.5%(n=122)were benign,9.9%(n=14)were malignant,and 3.5%(n=5)were *** benign tumors,WT accounted for the majority at 51.6%,followed by PA at 40.2%.When stratified by decade(2000-2009 and 2010-2018),the proportion of WT compared to all other benign and malignant neoplasms increased from 31.6%to 53.6%,whereas the proportion of PA decreased from 36.8%to 33.3%.The rate of tobacco use was unchanged at approximately 32.0%among our cohort from 2000 to ***:Among our cohort of veteran patients,WT was the most common benign parotid tumor and has increased in incidence over the last two decades despite an unchanged smoking rate.

背景与目的关于原发和转移性肿瘤的血管和结缔组织结构是由本身决定还是由宿主组织决定,目前尚不清楚。因此,我们研究了不同组织中C38结直肠癌血管和结缔组织发展的显微解剖过程。方法采用免疫荧光染色、电镜和实时定量聚合酶链反应等方法对小鼠盲肠壁、皮肤、肝脏、肺和大脑5个不同部位生成的肿瘤进行分析。结果我们发现在盲肠壁、皮肤、肝脏和肺的成纤维细胞分化成胶原蛋白基质,在肿瘤周围产生肌成纤维细胞。这些活化的成纤维细胞和生成的基质被肿瘤吸收利用。结缔组织的发展最终引起瘤内组织柱的出现(位于中心位置的单个微血管包埋在结缔组织和表达平滑肌肌动蛋白的肌成纤维细胞中,最外围由基底膜包围)。相反,在缺乏成纤维细胞的大脑中,仅当增生的肿瘤接触到含有成纤维细胞的脑膜时,C38转移才能诱导血管生成的促结缔组织增生性纤维柱的形成。结论我们的数据表明,作用于宿主组织成纤维细胞的肿瘤源性纤维生成分子诱导了促结缔组织增生的宿主组织反应。我们认为宿主组织特征和肿瘤来源的纤维生成信号共同决定了肿瘤的血管和结缔组织的结构。
