Ten years of a pediatric living donor liver transplantation program in Brazil

BACKGRoUND Pediatric living-donor liver transplantation is considered a safe alternative for the treatment of children with end-stage liver *** tertiary centers and specialized medical staff are necessary to ensure compatible long-term survival rates and quality-of-life for these *** To report the results and the 10-year learning curve of a pediatric living-donor liver transplantation *** We conducted a retrospective cohort study of pediatric recipients from 2013 to ***-transplant outcomes and patient survival rates were compared between two 5-year periods of the *** A total of 25 and 48 patients underwent transplantation in the first(2013-2017)and second period(2018-2023),*** vein and hepatic artery thrombosis occurred in 11(15.1%)and seven(9.6%)patients,*** complications were observed in 39 of 73 patients(53.4%).A lower warm ischemia time was observed in the second period compared to the first(32.6±8.6 minutes vs 38.4±9.8 minutes,P=0.018,respectively).Patient survival rates at 1 and 5 years were 84%in the first period and 91.7%in the second period,with no significant difference(P=0.32).CoNCLUSIoN The reported indications and outcomes align with the current *** findings provide crucial evidence regarding the feasibility of establishing a living donor program with consistent results over time.

Machine learning applied to road safety modeling:A systematic literature review

Road safety modeling is a valuable strategy for promoting safe mobility,enabling the development of crash prediction models(CPM)and the investigation of factors contributing to crash *** modeling has traditionally used statistical techniques despite acknowledging the limitations of this kind of approach(specific assumptions and prior definition of the link functions),which provides an opportunity to explore alternatives such as the use of machine learning(ML)*** study reviews papers that used ML techniques for the development of CPM.A systematic literature review protocol was conducted,that resulted in the analysis of papers and their *** types of models were identified:crash frequency,crash classification by severity,and crash frequency and *** first is a regression problem,the second,a classificatory one and the third can be approached either as a combination of the preceding two or as a regression model for the expected number of crashes by severity *** main groups of techniques used for these purposes are nearest neighbor classification,decision trees,evolutionary algorithms,support-vector machine,and artificial neural *** last one is used in many kinds of approaches given the ability to deal with both regression and classification problems,and also multivariate response *** paper also presents the main performance metrics used to evaluate the models and compares the results,showing the clear superiority of the ML-based models over the statistical *** addition,it identifies the main explanatory variables used in the models,which shows the predominance of road-environmental aspects as the most important factors contributing to crash *** review fulfilled its objective,identifying the various approaches and the main research characteristics,limitations,and opportunities,and also highlighting the potential of the usage of ML in crash analyses.

Assessment of hybrid geoids in Chile and Spain,combining GGM and GNSS/Leveling observations

Reliability analysis for mine blast performance based on delay type and firing time

Mining blasts may be defined as the use of explosive charges in a controlled manner by following a tightly controlled timing sequence according to an assigned firing order. Changes of timing between charges may result in an altered firing order and failure of the blasting sequence, which can cause high vibration levels, poor fragmentation, and/or an undesirable rock mass movement direction. Despite the importance of timing in determining mine blast results, there exists a lack of methodologies or tools with which to assess performance of a complete blast based on delay type and timing sequence. This document applies reliability engineering principles to evaluate the performance of a mine blast. The analyses are based on test results of the accuracy and precision of electronic and pyrotechnic detonators for typical firing times used in a surface coal mine, but may be applied to a variety of mines and timing scenarios.

String similarity join is an essential operation of many applications that need to find all similar string pairs from two given collections. A quantitative way to determine whether two strings are similar is to compute their similarity based on a certain similarity function. The string pairs with similarity above a certain threshold are regarded as results. The current approach to solving the similarity join problem is to use a unique threshold value. There are, however, several scenarios that require the support of multiple thresholds, for instance, when the dataset includes strings of various lengths. In this scenario, longer string pairs typically tolerate much more typos than shorter ones. Therefore, we proposed a so- lution for string similarity joins that supports different simi- larity thresholds in a single operator. In order to support dif- ferent thresholds, we devised two novel indexing techniques： partition based indexing and similarity aware indexing. To utilize the new indices and improve the join performance, we proposed new filtering methods and index probing tech- niques. To the best of our knowledge, this is the first work that addresses this problem. Experimental results on real-world datasets show that our solution performs efficiently while pro- viding a more flexible threshold specification.

Complications of hip fractures: A review

Nowadays, fracture surgery represents a big part of the orthopedic surgeon workload, and usually has associated major clinical and social cost implications. These fractures have several complications. Some of these are medical, and other related to the surgical treatment itself. Medical complications may affect around 20% of patients with hip fracture. Cognitive and neurological alterations, cardiopulmonary affections(alone or combined), venous thromboembolism, gastrointestinal tract bleeding, urinary tract complications, perioperative anemia, electrolytic and metabolic disorders, and pressure scars are the most important medical complications after hip surgery in terms of frequency, increase of length of stay and perioperative mortality. Complications arising from hip fracture surgery are fairly common, and vary depending on whether the fracture is intracapsular or extracapsular. The main problems in intracapsular fractures are biological: vascularization of the femoral head, and lack of periosteum-a major contributor to fracture healing- in the femoral neck. In extracapsularfractures, by contrast, the problem is mechanical, and relates to load-bearing. Early surgical fixation, the role of anti-thromboembolic and anti-infective prophylaxis, good pain control at the perioperative, detection and management of delirium, correct urinary tract management, avoidance of malnutrition, vitamin D supplementation, osteoporosis treatment and advancement of early mobilization to improve functional recovery and falls prevention are basic recommendations for an optimal maintenance of hip fractured patients.

Compressing supercapacitor(SCs)electrode is essential for improving the energy storage characteristics and minimizing ions’distance travel,faradaic reactions,and overall ohmic *** comprising the ion dynamics in SC electrodes under compression are still ***,the ionic dynamics of five aqueous electrolytes in electrodes under compression were studied in this work for tracking electrochemical and structural changes under mechanical stress.A superionic state is formed when the electrode is compressed until the micropores match the dimensions with the electrolyte’s hydrated ion sizes,which increases the *** excessive compression is applied,the accessible pore regions decrease,and the capacitance ***,as the studied hydrated ions have different dimensions,the match between ion/pore sizes *** the LioH and NaClo4electrolytes,increasing the pressure from 60 to 120 and 100 PSI raised the capacitance from 13.5 to 35.2 F g^(-1)and 30.9 to 39.0 F g^(-1),***,the KoH electrolyte with the lowest and LiCl with the biggest combination of hydrated ion size have their point of maximum capacitance(39.5 and 36.7F g^(-1))achieved at 140 and 80 PSI,*** LiCl and KCl electrolytes,overcompression causes a drop in capacitance higher than 23%.

Effects of laying on of hands with and without a spiritual context on pain and functionality of older women with knee osteoarthritis:study protocol for a randomized controlled trial

Background： Laying on of hands （LooH） is a culturally-accepted therapy in several traditions. In Brazil, ＂Spiritism＂（third-largest religious tradition in number of followers） uses LooH with the name of ＂Spiritist Passe＂（SP）. However, there are few studies assessing SP＇s role in medical outcomes. The present study aims to investigate the effects of LooH, with and without a spiritual context, on pain, joint stiffness, and functional capacity of older women （≥60 years old） with knee osteoarthritis （KoA）. Methods/design： In this triple-blind, prospective randomized controlled trial, older women with KoA are assigned to three groups： LooH with a spiritual context group, LooH group without a spiritual context, and a control group receiving no intervention. Patients are assessed by a blinded researcher at baseline, 8 weeks, and 16 weeks. Pain, joint stiffness, and functional capacity are assessed using Western ontario and McMaster Universities osteoarthritis Index. other measures such as anxiety, depression, gait speed, and quality of life will also be assessed. To detect differences between groups, a post-intervention com- parison between the three groups and a mean change （post-pre） comparison among the three groups will be done using analysis of variance. All statistical analyses will be performed using an intention-to-treat approach and a per-protocol analysis. Discussion： A pragmatic design using SP, LooH without spiritual context, and no LooH may provide further scientific evidence on SP＇s feasibility and efficacy for KoA patients.
