Hereditary Pancreatitis(HP)has emerged as a significant cause of acute,acute recurrent and chronic pancreatitis in the pediatric *** that it presents similarly to other causes of pancreatitis,a positive family history and/or isolation of a gene mutation are vital in its *** patterns remain complex,but mutations involving the PRSS1,SPINK1,CFTR and CTRC genes are commonly *** being first described in 1952,dozens of genetic alterations that modify the action of pancreatic enzymes have been *** children,these variants have been isolated in more than 50%of patients with chronic *** research has noted that such mutations in PRSS1,SPINK1 and CFTR genes are also associated with a faster progression from acute pancreatitis to chronic *** with HP are at increased risk of developing diabetes mellitus,exocrine pancreatic insufficiency,and pancreatic *** follows a multi-disciplinary approach with avoidance of triggers,surveillance of associated conditions,treatment of pancreatic insufficiency and use of endoscopic and surgical interventions for *** significant sequela,morbidity and a progressive nature,a thorough understanding of the etiology,pathophysiologic mechanisms,diagnostic evaluation,current management strategies and future research considerations for this evolving disease entity in pediatrics is warranted.

Vegetation Response To Soil And Morphological Properties In An Arid Region Alluvial Fan Landscape. Pre-Andes Of San Juan， Argentina

The Monte Desert is characterized by a great diversity of landforms created with fluvial， alluvial which the vegetation patterns are related to. The present work has the following objectives: 1: determine whether topographical attributes， surface characteristics， soil properties and vegetation patterns vary between alluvial landforms， and 2: define whether morphometric， soil and surface properties influence vegetation patterns along alluvial landscape. Morphometric data were obtained by processing a 5 m digital elevation model. The coverage of rock fragments， fine sediments and mulch was quantified. Observations and descriptions of the soil profiles were restricted to the uppermost 50 cm. Vegetation properties were calculated using a Point Quadrat Method. The relationship between variables was evaluated through multivariate statistical analysis. The main results show the presence of 45 plant species distributed in 19 families， where shrubs are dominant. The wind effect， topographic wetness and dissection of the landscape are limiting factors of diversity. The coverage of superficial rock fragments influence vegetation coverage through the distribution and availability of rainwater. Furthermore， the different soil textures reveal that the silt content favors an increase in vegetation coverage. The presence of V horizon could condition the installation and development of vegetation in the early stages of growth.

Patient Safety in the Use of Mechanical Restraints: Regulatory Compliance among Hospitals in the City of Buenos Aires and Proposal for Its Improvement

Introduction: Mechanical or physical restraint is an exceptional therapeutic resource to immobilize a subject and thus guarantee the safety of the patient and/or third parties in the face of high-risk behaviors, but it entails multiple crossings (bioethical, philosophical, medical, psychological, legal). Framed in the so-called “safety culture” developed by the WHO, based on the Protocol for its implementation of the CABA and attentive to its frequent use in CABA by different hospital services (medical clinic, geriatrics, intensive care and medical guards) we consider it necessary its study in terms of compliance with the risks it entails and its management. Objectives: Identify regulatory compliance with the GCABA Mechanical restraint (MR) Protocol from a patient safety perspective, as well as describe the clinical and medicolegal aspects, and propose the usefulness of a tool for its management and control. Methodology: Observational, descriptive, transversal and prospective work through the analysis of Clinical Records with indication of MR using a rubric-type form. 177 cases were analyzed between September-November 2023 from three hospitals of the Government of the City of Buenos Aires, statistical parameters were applied and graphs were made. Results: Only 12.99% complied with the Protocol. In the mental health specialized hospital compliance was almost 5 times greater than in the general one, and in the emergency services compliance was 12 times greater than in Inpatient services. We found that the start or end time of MR was not recorded and only 43% described the causes/justifications for the indication (mostly in Emergency and Specialized hospitals), with the MR average time being shorter in Emergency. Conclusions: Only 1.3 out of 10 patients reliably completed the Protocol and it was mostly in the mental health specialized hospital and the emergency services. 目的:ABI-007是一种不用聚氧乙基代蓖麻油(Cremophor E1),且蛋白质稳定的紫杉醇纳米制剂。不使用聚氧乙基代蓖麻油Cremophor EI可使ABI-007给药前不用进行常规预防超敏反应的预处理。此外,这种新制剂可使得紫杉醇在溶液中以更高的浓度存在,这样就可以减少输液的体积和时间,I期临床试验是要研究ABI-007的毒性,最大耐药剂量(MTD)和药物动力学。实验设计:ABI-007在门诊病人中使用,病人不用预先用药,30分钟滴注。ABI-007的剂量范围是从135(0级)到375mg/m2(3级)。16个病人参加了药物动力学的研究。结果:十九个病人在进行输液治疗期间没有观察到急性超敏反应。血药毒性是缓和的且无而非蓄积性。以3级(375mg/m2)治疗的6个病人中的3人发生剂量限制性毒性,口腔粘膜炎有2个病人,2个病人出现轻微的角膜炎。因此最大耐受剂量定为300 mg/m2(2级)。药物动力学分析显示当ABI-007的剂量在135-300mg/m2范围内时紫杉醇的Cmax和曲线下面积线形增加,Cmax和曲线下面积与病人个体毒性呈现出很好的相关性。结论:ABI-007提供了几个临床研究感兴趣的方面,包括快速输液率,不用预先用药处理和较高的紫杉醇最大耐药剂量,我们的研究结果为II期临床试验提供了该药抗肿瘤活性的依据。

Feeding Calcium Salts of Linseed Oil on Metabolic Status and Reproductive Traits in Grazing Dairy Cows

This experiment aimed to evaluate the effect of calcium salts of linseed oil (rich in alpha-linolenic acid (ALA, C18:3n-3) on metabolic and reproductive traits in high-producing dairy cows under grazing. Thirty-six Holstein dairy cows were randomly assigned, in a complete block design, to receive ALA supplementation (0.85 kg∙day−1 of calcium salts of linseed oil) or to remain as untreated control (CON). The concentrate was formulated to offer the same amount of energy across treatments (CON cows received an extra kg of corn to compensate for the higher energy density of ALA treatment). A PMR + Alfalfa pasture was offered to all cows at the same time. A fixed time artiﬁcial insemination (FTAI) at 80 DIM, preceded by a Presynch plus Ovsynch protocol was implemented for the first service and later, on return to estrus, heat detection and artiﬁcial insemination (AI) were performed. Pregnancy diagnosis was checked at 30, 42, 60, and 90 d after AI. Blood and milk samples were taken biweekly. Treatment affected plasma cholesterol concentration (160.36 vs. 186.70 mg∙dl−1, p = 0.03, for ALA and CON, respectively) and on size of corpus luteum (CL, 17.6 vs. 13.7 mm, p = 0.02, for ALA and CON, respectively). Supplementation tended (p = 0.136) to increase conception rate by 200 DIM (81.69% vs. 55.43% in ALA and CON cows, respectively). However, treatment had no effect (p > 0.05) on body weight (BW), body condition score (BCS), and circulating levels of beta-hydroxybutyrate (BHBA), glucose, insulin, growth hormone (GH) and insulin-like growth factor (IGF-I). Our results suggest that supplementation with calcium salts of linseed oil could enhance ovarian function without affecting energy metabolism in early lactation dairy cows.

1 Introduction Sacubitril/valsartan(SV)is a first in class dual action molecule of the neprilysin(NEP)inhibitor prodrug sacubitril(AHU377)and the angiotensin II receptor(Ang-II)type 1 antagonist valsartanJ11 It is the first angiotensin receptor-neprilysin inhibitor(ARNI)whose pharmacodynamic effects are consistent with a simultaneous stimulation of the natriuretic peptides system(via NEP inhibition)and the blockade of the renin-angiotensin-aldosterone system(valsartan effect)that finally results in systemic vasodilation,increased diuresis and natriuresis,reduction of plasmatic volume and diminution of peripheral vascular resistance.

Changing the treatment of heart failure with reduced ejection fraction： clinical use of sacubitril-valsartan combination

Despite significant therapeutic advances, patients with chronic heart failure （HF） remain at high risk of morbidity and mortality. Sacubitill valsartan （previously known as LCZ696） is a new oral agent approved for the treatment of symptomatic chronic heart failure in adults with reduced ejection fraction. It is described as the fast in class angiotensin receptor neprilysin inhibitor （ARNI） since it incorporates the neprilysin inhibitor, sacubitril and the angiotensin Ⅱ receptor antagonist, valsartan. Neprilysin is an endopeptidase that breaks down several vasoactive peptides including natriuretic peptides （NPs）, bradykinin, endothelin and angiotensin II （Ang-II）. Therefore, a natural consequence of its inhibition is an increase of plasmatic levels of both, NPs and Ang-Ⅱ （with opposite biological actions）. So, a combined inhibition of these both systems （Sacubitril / valsartan） may enhance the benefits of NPs effects in HF （natriuresis, diuresis, etc） while Ang-Ⅱ receptor is inhibited （reducing vasoconstriction and aldosterone release）. In a large clinical trial （PARADIGM-HF with 8442 patients）, this new agent was found to significantly reduce cardiovascular and all cause mortality as well as hospitalizations due to HF （compared to enalapril）. This manuscript reviews clinical evidence for sacubitril valsartan, dosing and cautions, future directions and its considered place in the therapy of HF with reduced ejection fraction.

1 Introduction Heart failure （HF） is a global health problem with an estimated prevalence of over 5.8 million in the USA and over 23 million *** represents the most common cause of hospitalization in elderly patients （〉 65 years） and its incidence has a growing trend mainly due to the aging of the population. Neurohumoral activation plays a major role in the pathophysiology. So in consequence, the cornerstone of its medical treatment is based on the inhibition of the renin-angiotensin-aldosterone system （RAAS） and the sympathetic nervous system, According to this, all patients with HF and reduced ejection fraction should be treated with an angiotensin converting enzyme inhibitor （ACEI） or an angiotensin receptor blocker （ARB） plus a beta blocker （BB） and if needed, a mineralocorticoid recep- tor antagonist （MRA）.
