Glycemic management in critically ill patients

Hyperglycemia associated with critical illness,also called "stress hyperglycemia" or "stress diabetes",is a consequence of many pathophysiologic hormonal responses including increased catecholamines,cortisol,glucagon,and growth *** in multiple biochemical pathways result in increased hepatic and peripheral insulin resistance with an uncontrolled activation of gluconeogenesis and *** has a negative impact on the function of the immune system,on the host response to illness or injury,and on infectious and overall *** degree of glucose elevation is associated with increased disease *** randomized controlled trials including the Van den Berghe study,the NICE-SUGAR trial,VISEP and GLUCONTROL have shown that the control of glucose levels in critically ill patients has implications on outcome and that both hyperglycemia and hypoglycemia are detrimental and should be *** variability has also been shown to be *** glucose control strategies have changed due to the concerns of hypoglycemia and therefore intermediate target glucose control strategies are most often *** patient populations may vary with regards to optimal glucose targets,timing and approach for glucose control,and with regards to the prognostic significance of glucose excursions and ***,surgical,and trauma patients may benefit at different rates from glucose control and the approach may need to be adapted to various medical settings and to correspond to the workflow of health *** modifiers for the success of insulin therapy for hyperglycemia include the methods of nutritional supplementation and exogenous glucose *** research is required to improve insulin protocols for glucose control,to further define glucose targets,and to enhance the accuracy of glucose measuring technologies.

This paper proposes an innovative fault-diagnosis system for an Asynchronous machine. Stator and rotor faults are identified and analyzed such as inter turn faults resulting in the shorting of one or more circuits of stator phase winding, broken rotor bars. The model used is the one developed by [13].The proposed strategy consists in developing an observer based on H Control theory followed by a sliding mode estimator. The performance index, the parameter that permit to differentiate between faults and disturbances or noise, for the H observer followed by the sliding mode estimator is the amplitude and the frequency. Simulations with a nonlinear Machine model were carried out which demonstrated the effectiveness of the proposed algorithm. Moreover, the robustness properties of the used H observer and the sliding estimator exhibited significant rejection of disturbances and noise-effects attenuation which make the proposed strategy adequately close to the conditions of the real physical system.

Induction machines are critical components in several industrial processes and are frequently integrated in commercially available equipments. The reliability of an induction motor is of great Importance in industrial as well as commercial, aerospace and military applications. in This paper two kinds of induction motor faults, stator short circuit and broken rotor bar fault, are *** model used is the one developed by [14].The proposed strategy consists in developing an observer based on analysis Control theory, characterized by robustness against model mismatches and disturbances, followed by a sliding mode estimator. The performance index, the parameter that permit to differentiate between faults and disturbances or noise, for the observer followed by the sliding mode estimator is amplitude and frequency. Simulations with nonlinear Machine model were carried out which demonstrated the effectiveness of the proposed algorithm. Moreover, analysis observer and sliding estimator exhibited robustness against unmodeled dynamics simulated by multiplicative uncertainty and significant rejection of disturbances and noise-effects attenuation which make the proposed strategy adequately close to the conditions of the real physical system.

background and purpose Radial artery catheterisation is an alternate route of access that has recently started to gain more widespread use for neuroendovascular procedures,including acute stroke *** this small case series,we present our institution’s outcomes in patients undergoing acute stroke interventions via transradial *** and methods We present a retrospective study of 15 patients who underwent acute stroke intervention via radial artery *** analyse these patients’periprocedural and clinical outcomes after undergoing mechanical *** A total of 15 consecutive patients were included in the study(9 males and 6 females),and all patients were able to successfully undergo mechanical thrombectomy via radial artery *** mean time of arterial puncture to reperfusion was 50±28 min(range:15-104).A TICI 2b/3 revascularisation was achieved in 13/15 patients(87%);a TICI 1 and TICI 2a outcome was achieved on the other two *** patient incurred an iatrogenic vessel dissection during the *** of out 15 patients(53%)had favourable mRS(0-3)at the time of discharge from the *** Radial artery catheterisation is technically feasible for performing acute stroke interventions with favourable time to revascularisation and good overall clinical outcomes.

＜正＞This paper proposes an innovative fault-diagnosis system for a turbocharged diesel engine with variable-geometry turbocharger *** and diversified actuator faults are identified and analyzed such as air-leakage in the admission collector, compressor malfunctioning,intake-valves fault,intercooler fault,deterioration in the turbine-compressor coupling,defect in the variable geometry of the ***,a complete non-linear engine model with four state variables is *** proposed strategy consists in developing Fault Detection and Estimation algorithm（FDE） based on the theory ofμanalysis control,operated on a linearization LTI model of the diesel,permits a fault detection and estimation while attenuating the effect of uncertainties,disturbances and *** with LTI Diesel model in the presence of noise and uncertainties were carried out which demonstrated the effectiveness of the proposed ***,the robustness properties of the used H-infinity/μFDE filters exhibited significant rejection of disturbances and noise-effects attenuation as well as robustness against uncertainties which make the proposed strategy adequately close to the conditions of the real physical system.

Noninvasive alternatives for DNA collection from threatened rodents

Many rodent species are currently under conservation threat. However, population monitoring and status assessment are extremely challenging because of small body size, low abundance and elusive behavior of rodents. Furthermore, invasive methods of capture and tissue collection commonly used to address such studies can induce an unacceptable amount of stress to sensitive species. As a result, noninvasive techniques have become more widely used, but relatively few studies have applied noninvasive techniques to rodents. Here we present two noninvasive alternatives for the collection of DNA from Franklin’s ground squirrels (Poliocitellus franklinii). We compared the quantity, purity and degradation of DNA extracted from plucked hair and fecal pellets to tail snip tissues. We recovered more DNA from tail snips than either plucked hair or fecal pellets. Both hair and fecal pellets recovered DNA with purity ratios similar to tail snips. As expected, DNA recovered from fecal pellets exhibited a high degree of degradation compared to hair and tail tissues. Careful planning of field and laboratory protocols is therefore necessary to compensate for challenges associated with noninvasive tissue types. While there is no tissue that can universally be applied to all research projects, both hair and feces are viable alternatives to traditional invasive procedures and can be applied to threatened and endangered rodent species.

In this article, we propose a topology of a TLC-HAPF power filter as a harmonic compensator for an optimization of the pollution control of electrical networks. This filter consists of an active part and a passive part in order to reduce or limit switching losses during current injection into networks thanks to its TLC module. This topology also provides solutions dynamic performance issues, resonance and lack of compensation capacity for imbalance cases. It also offers a greater range of compensation than conventional active models which do not offer as well as an intermediate circuit voltage in the order of 105 V to 109 V relatively lower than others models (600 v). A modulated hysteresis control of this topology is therefore also developed in this article and allows to obtain a network analysis on the three phases at three levels: source side, load side, and finally at the connection of the filter to the network, allowing to specify for these different positions the value of the current spectrum and its THD at this well-defined moment.
