Fully bioresorbable vascular scaffolds(BVSs)aim to overcome the limitations of metallic drug-eluting stents(DESs).However,polymer-based BVSs,such as Abbott’s Absorb,the only US FDA-approved BVS,have had limited use due to increased strut thickness(157μm for Absorb),exacerbated tissue inflammation,and increased risk of major cardiac events leading to inferior clinical performance when compared to metallic *** we report the development of a drug-eluting BVS(DE-BVS)through the innovative use of a photopolymerizable,citrate-based biomaterial and a high-precision additive manufacturing *** with a clinically relevant strut thickness of 62μm can be produced in a high-throughput manner,*** BVS per minute,and controlled release of the anti-restenosis drug everolimus can be achieved by engineering the structure of polymer coatings to fabricate drug-eluting *** achieved the successful deployment of BVSs and DE-BVSs in swine coronary arteries using a custom-built balloon catheter and BVS delivery system and confirmed BVS safety and efficacy regarding maintenance of vessel patency for 28 days,observing an inflammation profile for BVS and DE-BVS that was comparable to the commercial XIENCE^(TM)DES(Abbott Vascular).

The purpose of this paper is to determine the optimal size and number of tubes for a generic attenuator that is similar to Pelamis P2, the wave energy converter. Simulations using ANSYS Workbench, Design Modeler, and AQWA are performed to study the energy absorption at the nodes between the tubes. The analysis is limited to linearized hydrodynamic fluid waves loading on floating bodies by employing three-dimensional radiation/diffraction theory in regular waves in the frequency domain. Three sets of tests are conducted by varying total tube number, each tube length and the order of tubes with different lengths. After a systematic study in the frequency domain, the optimal size and number of the genetic attenuator is recommended.

A Property Based Approach for Simultaneous Process and Molecular Design

In this work, property clustering techniques and group contribution methods are combined to enable simultaneous consideration of process performance requirements and molecular property constraints. Using this methodology, the process design problem is solved to identify the property targets corresponding to the desired process performance. A significant advantage of the developed methodology is that for problems that can be satisfactorily described by only three properties, the process and molecular design problems can be simultaneously solved visually on a ternary diagram, irrespective of how many molecular fragments are included in the search space. On the ternary cluster diagram, the target properties are represented as individual points if given as discrete values or as a region if given as intervals. The structure and identity of candidate components is then identified by combining or ＂mixing＂ molecular fragments until the resulting properties match the targets.

AIM: To evaluate whether a higher prevalence of Giardia lamblia infection is associated with an increase in irritable bowel syndrome (IBS) prescriptions at the county level in Michigan. METHODS: The Michigan Disease Surveillance System (MDSS) was used to ascertain both the numbers of Giardia lamblia infections as well as the total number of foodborne illnesses per population by county in Michigan during 2005. This was compared with Blue Cross Blue Shield (BCBS) of Michigan numbers of drug prescriptions for IBS per one thousand members per county in 2005. These data were also analyzed for associations with per capita income by county and the number of refugees entering each county in 2005. RESULTS: There were a total of 786 confirmed cases of Giardia lamblia reported to MDSS in 2005. During the same time period, the number of prescriptions for IBS varied from 0.5 per 1000 members up to 6.0 per 1000 members per month. There was no trend towards higher numbers of IBS prescriptions in the counties with more Giardia lamblia infections. Per capita income was not associated with either IBS prescriptions or Giardiasis. There was a significant linear association between the number of refugees entering each county, and the number of Giardia lamblia cases per 100 000 population. CONCLUSION: In this ecological study, there was no association found between BCBS prescriptions for IBS and Giardia lamblia infections in Michigan counties. Our findings may have been influenced by the disparate number of refugees admitted per county.

Advanced Glycation End Products (AGEs) have been associated as a possible cause in inflammation-mediated chronic diseases such as diabetes, Alzheimer’s and cardiovascular disorders. Thus, inhibition of AGE formation represents a prospective therapeutic target for the prevention and treatment of these complications. This study investigated the individual and combined effects of dietary ingredients, spices, on lowering AGEs formation in meat patties. In the study, Carboxymethyllysine (CML), a well-investigated AGE is used as a marker for AGEs and malondialdehyde (MDA) as an indicator for lipid peroxidation. Nine spices were selected based on their ability to inhibit the formation of AGEs at different stages of Maillard reactions. Individually, all the 9 selected spices significantly inhibited the formation of AGEs. Among the 33 combinations of spices, 26 combinations significantly inhibited the formation of AGEs. The highest reduction (84%) was found by the combination of Black Pepper-Rose-Cumin. The individual spices failed to significantly lower the MDA concentration;however, all 33 combinations were able to significantly reduce MDA concentration. Farmers perception about availability and utilization of Spirulina (Arthrospira fusiformis) were assessed and its nutritive value compared against reference proteins (soybean and nuge cake). Samples of Spirulina were collected from Arenguade and Chitu lakes of rift valley of Ethiopia. A survey was conducted on purposively selected 100 households (HH) living around the Lakes which are located in Gerbicha and Labu subuqa districts of Debre Zeit town and west Arsi zone, respectively. Chemical composition and in vitro dry matter digestibility (IVDMD) and total gas volume (TGV) of feed samples were determined. Respondents of both districts are not aware of Spirulina as animal feed. Spirulina was available during dry season in Arenguade and whole year in Chitu. The highest (p 0.05) in crude fiber (CF), neutral detergent fiber (NDF), acid detergent fiber (ADF), acid detergent lignin (ADL) and metabolizable energy (ME) contents between Spirulina from both lakes. Spirulina from Chitu produced the highest TGV (31.2 ml) and fermented faster than Spirulina from Arenguade. Lowest (p < 0.05) ash, EE, CP and IVDMD and higher (p < 0.05) CF, NDF, ADF, ADL and TGV were recorded for reference proteins than Spirulina. ME content of Spirulina (3200 kcal/kg) was higher (p < 0.05) than that of nuge cake (3011 kcal/kg) but lower (p < 0.05) than that of soybean (3474 kcal/kg). Therefore, the chemical composition and IVDMD showed the potential of Spirulina to be used as a substitute of the reference feeds.

背景足够的产科家庭医生对于确保患者获取必需的产科服务至关重要。然而,既往研究表明执业范围包含产科的家庭医生数量正在逐年减少,如今从事新生儿接生工作的家庭医生不到10%。目的旨在探究希望将产科纳入其执业范围的新毕业家庭医生如何发现和选择工作,并进一步了解当前就业形势对家庭医生的执业范围,特别是产科、新生儿接生方面的影响。方法于2017年进行问卷调查和定性访谈的混合方法研究。对美国家庭医生进行电子问卷调查并在目的性子抽样后对这些医生进行深度半结构化电话访谈。问卷调查总结了家庭医生未从事产科工作的原因,为进一步明确选择从事产科工作的家庭医生的具体情况,采用基于群体的沉浸式方法来转译定性访谈的结果。本研究向2098例毕业于2014—2016年并希望从事新生儿接生工作的美国家庭医学专业实习医生发送调查问卷,回复1016份,回复率48.43%,其中56例接受了电话访谈。结果问卷调查结果显示,未能找到工作范围包含产科的工作是希望从事产科工作的家庭医学毕业生未从事该工作的主要原因。定性访谈结果显示,家庭医生通常通过人脉关系或人才招聘的途径找到产科相关工作,并根据地理位置偏好、家庭义务及生活方式要求等个人考虑因素做出选择。同时,求职过程和择业决策也受到工作结构、执业特点及缺乏产科相工作经验等因素的限制。结论虽然个人意向决定大部分医学生的工作选择,但其选择仍受到多种不可控因素限制,特别是家庭医学工作提供产科相关工作的能力。美国毕业医学生从医院实习生到执业医师的转变同时影响着毕业生的求职选择和医疗机构患者寻求医疗服务的质量。因此,了解就业情况对家庭医生就业范围的影响方式,有助于进一步明确如何协助家庭医生在其希望从事的范围内工作并对其进行相应协助,从而确保每一个家庭可以获得更好的医疗服务。

Splenic Marginal Zone Lymphoma in a Patient with Positive Hepatitis B Virus Serology

We present a case in which an elderly woman diagnosed with a splenic marginal zone lymphoma (MZL) was found to have positive Hepatitis B serology. Link with Hepatitis C virus is well documented but reports of association of Hepatitis B virus (HBV) with splenic marginal zone lymphoma are still emerging. A 69-year-old lady presented with weight loss, pancytopenia and marked splenomegaly. Prior to commencing treatment, Hepatitis B serology confirmed Hepatitis B infection. She was treated with Chlorambucil along with anti-hepatitis B prophylaxis and HBV PCR monitoring. She had an excellent response to treatment with resolution of symptoms and splenomegaly. This case highlights the importance of testing for hepatitis B serology in patients diagnosed with splenic MZLs as causative agent. Although the association between HCV is well documented in the literature, a relationship between HBV may also be important. Also, chemotherapy +/- Rituximab for splenic MZL is associated with the reactivation of latent infections;hence providing prophylactic cover for pre-existing latent HBV infection may be required to prevent reactivation as in this case.
