Assessment of Left Ventricular Torsion in Hypertensive Patients

Background: Hypertension has important effects on left ventricle and its early detection helps to avoid the cardiac complications. LV Twist in systole helps in storing potential energy during diastole;LV untwist releases this energy. Speckle tracking echocardiography (STE) is angle-independent so it allows a more detailed evaluation of myocardial deformation. Objective: This work aimed to assess the left ventricular torsion by speckle tracking in hypertensive patients. Patients and Methods: The study was applied on 40 hypertensive patients and 20 age and sex matched control groups;all had speckle tracking echocardiography of the LV, and the basal and apical rotation were measured to assess the LV torsion. The 18 segments of LV strain were assessed and the net LV global longitudinal strain was calculated and demonstrated as bull's eye figure. Results: Hypertensive patients had higher mean values of both apical rotation and left ventricular twist than control group by a highly significant value (9.76 ± 4.98 vs 4.03 ± 2.15, P value 0.001) and (15.25 ± 4.10 vs 9.90 ± 1.47, P value 0.001) respectively. Hypertensive patients had a higher basal rotation than controls but did not reach a significant value (-6.01 ± 3.25 vs -5.52 ± 2.63, P value 0.558). On the contrary, the global LV longitudinal strain was lower in hypertensive patients than control but did not reach a significant value (-18.48 ± 3.94 VS -19.48 ± 3.57, P value 0.341). Conclusion:Hypertension affects myocardial structure and its systolic and diastolic functions;the left ventricular twist is an essential component of the systolic function that increases in hypertension as an early compensation for the systolic impairment. The detection of these changes achieved accurately by 2-D speckle tracking, can help in monitoring the treatment modalities of the patients for a better direction of treatment and thus, prevent further deterioration.

Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy in epithelial ovarian cancer: State of the art

Advanced stage epithelial ovarian cancer(EOC) is diffi cult to treat with low overall cure rates. A new strategy combining maximal cytoreductive surgery(CRS) with intraoperative hyperthermic intraperitoneal chemotherapy(HIPEC) has been proposed to treat advanced stage EOC in the primary setting. Numerous small, heterogeneous studies have been conducted exploring outcomes in patients with predominantly advanced, recurrent or refractory disease treated with CRS + HIPEC. Although morbidity rates approaching 35% have been reported, oncologic outcomes are promising. Incorporation of HIPEC for the treatment of primary EOC has continued to gain interest. Several prospective phase 2 clinical trials were recently completed evaluating the impact of CRS + HIPEC in the primary setting. This article will briefl y discuss the benefi ts of optimal surgical cytoreduction and the theoretical basis of intraperitoneal chemotherapy in patients with advanced stage EOC, and will then review existing literature describing oncologic outcomes in EOC patients treated with HIPEC in the primary setting.

High-Frequency Electrostatic SW at the Boundary between Quantum Plasma and Metal

It is shown that high-frequency electrostatic surface waves (SW) could be propagated at right angles to an external magnetic field on the boundary between metal and gaseous plasma due to a finite pressure electron gas in quantum plasma by using the quantum hydrodynamic QHD equations. The dispersion relation for those surface waves in uniform electron plasma is derived under strong external magnetic field. We have shown that the electrostatic surface waves exist also in the frequency for the ranges where electromagnetic SW is impossible. The surface plasma modes are numerically evaluated for the specific case of gold metallic plasma at room temperature. It has been found that dispersion relation of surface modes depends significantly on these quantum effects (Bohm potential and statistical) and should be into account in the case of magnetized or unmagnetized plasma.

Study of Phytochemical Screening and Antimicrobial Activity of Citrus aurantifolia Seed Extracts

Citrus aurantifolia, the family Rutaceae, which consists of 150 genera and 900 species, is gaining grounds as important source for treatment in complementary medicine, and commonly called limone. Chloroform, methanol and ethanol extract exhibited significant antimicrobial activity and highlighted the biological monitoring of activity from the methanol extract of seeds. Phytochemical screening of the diethyl ether, methanol and aqueous extracts of seeds confirmed the presence of alkaloids, saponins, sterols and triterpenes, carotenoids, coumarins, tannins and carbohydrates. Chloroform extract was fractionated by sing liquid solid column chromatographic technique and column fractions eluted with chloroform and methanol. The antibacterial secondary metabolites were achieved by fractionation of the active. The seed extracts were monitored analytically by TLC preparative and some chemical analyses of seeds have been done such as determination of ash, nitrogen, protein content.

Utilization of Carob (<i>Ceratonia siliqua</i>L.) Extract as Functional Ingredient in Some Confectionery Products

The aim of this study was to investigate the physicochemical and antioxidant activity of carob pod powder and its use in preparing of functional drinks, and some confectionery products. The raw and heated (at 110°C and 130°C/20 min) carob powder was used in preparing of drinks. The concentrate carob drink was used in preparing of some confectionery products. The drying process significantly decreased the moisture and total sugars values in carob powder. The antioxidant activity (was measured by two different assays, which are DPPH radical scavenging activity and FRAP) of heated carob powder drinks was lower than the raw carob powder drink, while total phenolic not affected. The best sensory acceptability was observed with raw carob powder drink at ratio 1:2 (carob powder: water). The concentration process caused significant increase in total phenolic and antioxidant activity of carob drink. According to the sensory evaluation results, the carob concentrate was used in preparation of Toffee, Jelly candy and Turkish delight by 5%

Effects of Lidocaine Infusion on Quality of Recovery and Agitation after Functional Endoscopic Sinus Surgery: Randomized Controlled Study

Background: After functional endoscopic nasal surgery, emergence agitation is not uncommon. The aim of this trial was to investigate the effect of perioperative lidocaine infusion on postoperative early recovery quality and incidence of emergence agitation in patient undergoing functional endoscopic sinus surgery. Study Design: Prospective, randomized, double-blinded, placebo-controlled trial. Methods: 100 patients of ASA I and II, aged 18 - 50 years, of both sexes scheduled for FEES, were assigned into two groups. In Group L;patients received an intravenous bolus infusion of 1.5 mg/kg lidocaine just before induction of anesthesia followed by a continuous infusion of 2 mg/kg/h during the operation and until the end of the surgery. In Group C;patients received normal saline infusion with the same volume as group L according to the same protocol. The primary endpoints were incidence of emergence agitation and postoperative recovery quality (QoR-40) score on first postoperative day (POD1). Results: Incidence of emergence agitation was significantly lower in group L (P 0.05) compared with group C. Global QoR-40 scores on POD1 were significantly lower in both groups compared with preoperative assessment, it was significantly higher in group L on POD1 (P 0.05) than in group C. Among the five dimensions of QoR-40, the scores for physical comfort and pain were superior in group L compared to group C (P 0.05) at POD1. Conclusion:

The influence of rolling bearing clearances on diagnostic signatures based on a numerical simulation and experimental evaluation

This paper presents investigations into the influences of bearing clearances on the diagnostic features of monitoring rolling-bearings. A nonlinear dynamic model of a deep groove ball bearing with five degrees of freedom is developed for numerical analysis under increased radial clearances which are due to not only the scenarios of bearing grades but also gradual wear with bearing service lifetime. The model incorporates local defects and clearance increments in order to gain the insight into the bearing dynamics under different fault cases along with clearance changes. Numerical results show that the vibrations at fault characteristic frequencies exhibit clear inconsistency with common understandings for different cases of increased clearances. This study highlights that it has to take into account the clearance effect, especially for the inner race fault, in order to avoid the under-estimate of fault sizes which may be indicated by the feature amplitude reduction.

In this work, the Lagrangean Relaxation method has been discussed to solve different sizes of capacitated facility location problem (CFLP). A good lower bound has been achieved on the solution of the CFLP considered in this paper. This lower bound has been improved by using the Volume algorithm. The methods of setting two important parameters in heuristic have been given. The approaches used to gain the lower bound have been explained. The results of this work have been compared with the known results given by Beasley.
