Evaluating the influence of a structured nursing protocol on targeted outcomes in rheumatoid arthritis patients

Objective:Rheumatoid arthritis(RA)requires comprehensive *** nursing protocols may enhance outcomes,but evidence is *** study evaluated the effect of a structured nursing protocol on RA *** and Methods:In this one-group pre-post study,30 Egyptian RA patients completed assessments before and after a 12-week nursing protocol comprising education,psychosocial support,and self-management *** included clinical evaluation of joint counts,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)and patient-reported Arthritis Self-Efficacy Scale(ASES),Health Assessment Questionnaire(HAQ),Visual Analog Scale(VAS)for pain,and Hospital Anxiety and Depression Scale(HADS).Results:The study demonstrated significant improvements in both clinical-and patient-reported *** count decreased from 18.4±4.2 to 14.2±3.8(P<0.001),ESR from 30.1±6.8 mm/h to 25.5±6.8 mm/h(P<0.01),and CRP levels from 15.2±3.6 mg/L to 11.8±2.9 mg/L(P<0.01)***-reported outcomes showed a marked increase in ASES score from 140±25 to 170±30(P<0.001)and reductions in HAQ from 1.6±0.4 to 1.3±0.3(P<0.01),VAS pain score from 7.8±1.7 to 6.2±1.2(P<0.001),and HADS anxiety and depression scores from 11±3 to 8±2(P<0.05)and 10±2 to 7±1(P<0.05),***:A structured nursing protocol significantly improved clinical disease activity,physical functioning,pain,self-efficacy,and emotional well-being in RA patients.A multifaceted nursing intervention appears beneficial for optimizing RA outcomes.

Rotavirus is the leading cause of acute gastroenteritis and responsible for 20% of death in children under 5 years of age approximately 111 million episodes of gastroenteritis due to Rotavirus in under 5 children, which result in 25 million visits to clinic, 2 million hospitalization and 352,000 - 592,000 deaths. Children in the poorest countries account 82% of rotaviruses death. Diarrhea disease are major cause of death and disease among children under five years, a child on average suffers 2 to 3 attacks of diarrhea every year. Stools samples were collected from 196 children suffering from diarrhea and were tested for rotaviruses by immune chromatography test antigen (ICT-Ag), ELISA and RT PCR The data were analyzed using statistical package for the Social Sciences (SPSS). The results showed that prevalence of rotavirus was 26/196 (13.3%) by ICT Ag and ELISA and confirmed by RT-PC R. 87.8% of the children were vaccinated by Rota vaccine, 70% of their mothers not using soap for cleaning the children and thought diarrhea not infectious disease and believed it was caused by teething. In conclusion there was a decrease in prevalence of diarrhea by Rotavirus due to insertion of Rota vaccine in routine vaccination, rotavirus infection mostly occurs in poor family that not able to provide safe water and due to poor sanitation and low education of mother beside that some children suffered from malnutrition, so it recommended to incorporate ELISA as a diagnostic tool in routine diagnosis of rotavirus among children suffered from gastroenteritis.

A noteworthy public health problem,antimicrobial resistance(AMR)has been impeded in many ways by the coronavirus disease 2019(COVID-19)*** narrative review discusses the two-sided impact of COVID-19 on the magnitude of *** pandemic has put tremendous strain on healthcare systems,diverting resources,personnel,and attention away from AMR diagnosis and management toward COVID-19 diagnosis and contact tracking and *** research has been severely hampered,and surveillance and antimicrobial stewardship(AMS)programs have been de-emphasized,delayed,or ***,particularly broad-spectrum,were prescribed more frequently without diagnostic confirmation of bacterial infection than before the ***,the COVID-19 pandemic has highlighted the vulnerability of healthcare systems in controlling infectious disease threats and raised awareness of the importance of infection prevention and ***,the pandemic has created opportunities to capitalize on positive effects on AMR *** review concludes that it is now more important than ever to focus on AMR and strengthen AMS programs to ensure appropriate antibiotic use and other AMR prevention measures in *** must ensure that one of the COVID-19 legacies is increased support for AMR research,diagnostic implementation,appropriate diagnostic stewardship,and the strengthening of our health *** COVID-19 pandemic has demonstrated that prevention is better than *** will need to step up their efforts to combat AMR as a multidisciplinary *** must prepare our public health systems to combat multiple threats at the same time.

The evolution of the Internet of Things(IoT)has empowered modern industries with the capability to implement large-scale IoT ecosystems,such as the Industrial Internet of Things(IIoT).The IIoT is vulnerable to a diverse range of cyberattacks that can be exploited by intruders and cause substantial reputational andfinancial harm to *** preserve the confidentiality,integrity,and availability of IIoT networks,an anomaly-based intrusion detection system(IDS)can be used to provide secure,reliable,and efficient IIoT *** this paper,we propose an anomaly-based IDS for IIoT networks as an effective security solution to efficiently and effectively overcome several IIoT *** proposed anomaly-based IDS is divided into three phases:pre-processing,feature selection,and *** the pre-processing phase,data cleaning and nor-malization are *** the feature selection phase,the candidates’feature vectors are computed using two feature reduction techniques,minimum redun-dancy maximum relevance and neighborhood components *** thefinal step,the modeling phase,the following classifiers are used to perform the classi-fication:support vector machine,decision tree,k-nearest neighbors,and linear discriminant *** proposed work uses a new data-driven IIoT data set called *** experimental evaluation demonstrates our proposed model achieved a high accuracy rate of 99.58%,a sensitivity rate of 99.59%,a specificity rate of 99.58%,and a low false positive rate of 0.4%.

Non-alcoholic fatty liver disease(NAFLD)denotes a spectrum of fatty liver disease in individuals without significant alcohol *** is set to be the most common etiology of serious liver diseases in numerous nations when accompanied by obesity and type 2 *** is further histologically categorized into the non-alcoholic fatty liver(NAFL;steatosis without hepatocellular injury)and non-alcoholic steatohepatitis(NASH)which is characterized by the coexistence of hepatic steatosis and inflammation and is accompanied by hepatocyte injury(ballooning),either with or without *** is considered the benign and reversible stage arising from the excessive accumulation of triglycerides in ***,NASH is a more progressive stage of NAFLD,due to the increased risks of evolving more serious diseases such as cirrhosis,hepatocellular *** concept,however,has been lately challenged by a hypothesis of multiple parallel hits of NAFLD,in which steatosis and NASH are separate entities rather than two points of the NAFLD spectrum,not only from a set of histological patterns but also from a pathophysiological *** current review highlights the epidemiology and pathophysiology of NAFLD,and its progression towards steatohepatitis,with special focus on the novel imminent therapeutic approaches targeting the molecular aspects and the pathogenic pathways involved in the development,and progression of NAFLD.

Paleontological and sedimentological analyses were carried out in sediment core from southeastern Mediterranean Sea,offshore Egypt to reconstruct the past environmental changes in trophic state and temperature during *** achieve our goals,grain size,total organic carbon,planktic and benthic foraminifera,dinoflagellate cysts have been *** micropalaentological studies have been done in the studied area and none of them used the combined proxies of benthic foraminifera and dinoflagellate *** combined proxies reflects more comprehensive paleoenvironmental *** and abiotic data have been analysed with multivariate technique including Redundancy Analysis(RDA).Diversity indices such as:Fisher alpha index(α)and Shannon index(H)have been *** foraminiferal study yields 9 planktic species and 10 benthic *** assemblages have low species diversity indices especially at sapropel layer *** sapropels are layers with elevated organic carbon concentrations that contrast with surrounding organic poor *** occur periodically in sedimentary sequences of the last millions years,which have been the subject of extensive previous *** Analysis(RDA)yields two groups of foraminiferal assemblages depending on the changes of total organic carbon and clay *** dinocysts study yields 15 species,the majority of cyst types belonging to the order *** association of dinoflagellate cyst shows two depositional phases in the *** sapropel layer S1,with anoxic condition and warmer temperature,is recorded at depth 28-46 cm where heterotrophic taxa dominate and the post-sapropel layer is recorded at depth 0-28 cm;where autotrophic taxa dominate.

Objective： Differentiated thyroid carcinomas （DTCs） are classified into papillary thyroid carcinoma （PTC） and follicular thyroid carcinoma （FTC）. DTCs are analyzed as a single group in clinical studies that investigated the prognostic factors and prognosis of these malignancies. However, the biological behaviors of these carcinomas significantly differ. In the present study, we aimed to detect differences in the outcomes between PTC and FTC in Mansoura University Hospital in Egypt. Methods： A total of SS8 patients with histologically proven thyroid carcinomas from January 2003 to December 2012 were retrospectively enrolled. The clinical and pathological data of patients were reviewed. Results： Large primary tumor size, lymph node involvement, extrathyroid extension, and distant metastasis were significant poor prognostic factors for overall survival （OS） in old PTC patients. Cox hazard analysis showed that the patient＇s age, extra thyroid extension, and distant metastasis were the only independent prognostic factors. In FTC patients, only the distant metastasis and degree of tumor invasion were significant poor prognostic factors in OS univariate analysis. However, these factors were nonsignificant in multivariate analysis. The 10-year OS rates were 97% and 89% for PTC and FTC, respectively （P=0.003）. The 10-year disease-free survival （DFS） rates were 77.2% in PTC vs. 65% in FTC （P=0.179）. Conclusion： The significant prognostic factors vary between the two types of DTCs. Therefore, PTC and FTC patients need to be analyzed and reported independently. PTC survival is widely and significantly affected by age, extrathyroid extension, and distant metastasis. By contrast, these factors were nonsignificant in FTC, which showed poorer survival than PTC.
