Autunite-Group Minerals and Their Paragenesis from the Sheared Granite of Gabal el Sela, South Eastern Desert, Egypt

G. el Sela is located in the Southern Eastern Desert of Egypt cropping as two parts, occupied by monzogranites that were categorized as biotite granite, muscovite granite and two mica granites. The northern part is more significant according its high concentrations of uranium that controlled by complicated structure regime;faulting, infrastructures and shearing are the most common structural criteria of this part. The Egyptian Nuclear Materials Authority (NMA) mined this part to produce the uranium ore. The previous mineralogical studies indicated that this granite was dominated by primary uranium minerals (pitchblende and coffinite) and secondary minerals belong to the autunite group (autunite, metautunite, phurcalite) in addition to uranophane. In the present work, petrographic and mineralogical studies are applied for the granites using the polarized and stereo microscopes and followed by electron microscope and XRD. The result of the microscopic examinations revealed the tectonic regime controlling the radioactivity and recognized the sodic autunite (meta-natroautunite) beside the pre-mentioned autunite group minerals completing the paragenetic sequrnce of these minerals. In this study, it is concluded that the sheared biotite granite is monzogranite originated during the episode of the continental plate collision (syncollision). The study finished to presence of two main types of the alteration corresponding to the two high levels of radioactivity (moderate and anomalous). The first is the thermal alteration (saussiritization, sericitization, kaolinization, silicification and hematization) and the second is the chemical transformation (oxidation, dehydration, ion substitutions and confusion) responsible for formation of the secondary uranium minerals. The temperature needed for the thermal alteration is sourced by the hydrothermal solutions, while the temperature needed for the uranium minerals transformation may be generated during the episode of the continent

Screening for hepatocellular carcinoma by Egyptian physicians

AIM: To assess the practice of Egyptian physicians in screening patients for hepatocellular carcinoma(HCC). METHODS: The study included 154 physicians from all over Egypt caring for patients at risk for HCC. The study was based on a questionnaire with 20 items. Each questionnaire consisted of two parts:(1) personal information regarding the physician(name, age, specialty and type of health care setting); and(2) professional experience in the care of patients at risk for HCC development(screening, knowledge about the cause and natural course of liver diseases and HCC risk). RESULTS: Sixty-eight percent of doctors with an MD degree, 48% of doctors with a master degree or a diploma and 40% of doctors with a Bachelor of Medicine, Bachelor of Surgery certificate considered the hepatitis C virus(HCV) genotype as risk factor for HCC development(P ell as liver cirrhosis(P < 0.05). Eighty-six percent of doctors in University Hospitals and 69% of Ministry of Health(MOH) doctors consider HCV infection as the leading cause of HCC in Egypt(P < 0.05). Seventy-two percent of doctors with an MD degree, 55% of doctors with a master degree or a diploma, 56% of doctors with an MBBCH certificate, 74% of doctors in University Hospitals and 46% of MOH hospital doctors consider abdominal ultrasonography as the most important investigation in HCC screening(P < 0.05). Population Pharmacokinetics of Methotrexate in Egyptian Children with Lymphoblastic Leukemia

Background: Individualization of high dose regimen of methotrexate (MTX) in pediatric patients with acute lymphoblastic leukemia based on pharmacokinetic (PK) parameters can help in optimization of the dose and better control of the disease. Building up of a pharmacokinetic model can help dose optimization. Objectives: A NONMEM based population (POP) PK model has been subsequently developed to evaluate the effect of demographics as covariates to address variability in pharmacokinetics of MTX. Method: Forty one patients (24 males & 17 females) with ranges of age, body weight and height of 3 - 15 years, 13 - 54 kg and 100 - 177 cm respectively and diagnosed as acute lymphoblastic leukemia (ALL) were involved in the study. MTX was administered as i.v. infusion at a dose of 2 gm/m2 over a period of two hour and its plasma concentrations were determined primarily at 24 hr post-dose to be utilized in the building-up of PK *** initial/prior estimates of volumes of central (V1) and peripheral compartments (V2) and clearance (CL) and inter-compartmental clearance (Q2) for MTX were extracted from literature. The inter-subject variability was estimated for V1 & CL. The influence of different covariates on model performance and parameter estimates was assessed by evaluating the difference in objective function value. The final POP PK model was validated by bootstrap re-sampling procedures (1000 runs) and the 95% confidence intervals for the estimates were calculated. Results: The POP estimates for CL, V1 and V2 were 2.18 L/h, 5.74 L and 11.2 L respectively. The inter-individual variability for the CL and V1 was 23% and 30% respectively. The covariates analysis showed effect of body surface area and sex on the CL and weight on V1. Conclusions: The POP-PK model developed adequately defines the population PK of MTX in pediatric patients with lymphoblastic leukemia. Background: Nutritional status of mothers is considered one of the major problems facing mothers in the world. The nutrition plays a major role in mothers and child health. Objective: This study aims to assess mothers’ nutritional status and its determinants in Alemtidad area, Khartoum. Methods: A descriptive community based survey was conducted in the Alemtidad area of Khartoum between November 2012 and December 2012. One hundred and eighty mothers were selected by simple randomization. Data were collected by using structured designed questionnaire which was then analyzed by (SPSS) program version 20. Results: The study conducted among mothers in Alemtidad area;their age ranged between 19 and 86 years with the mean age 38 ± 12 years. From those mothers about half were unemployed, and having mainly primary and secondary educational level. 81.1% of mothers were not able to select the proper food according to age and nutrients and about 71.1% of mothers took two meals per day and 23.3% one meals per day, and only 5.6 take three meals per day. The Body Mass Index (BMI) was calculated to measure the nutrition status of mothers;this study shows that mothers in Alemtidad are mainly overweight and obese, 45.6% and 18% respectively and only 35% within normal range of Body Mass Index (BMI). Conclusion: The study concludes that mothers are either overweight or obese (45.6%, 17.7% respectively). Compared to national and international statistics the findings of this study reflect high Body Mass Index (BMI) of mothers in Alemtidad.

Evaluation of the systematic set-up errors using electronic portal image device in the radiotherapy procedures

Objective： The aim of this work was to quantify the extent of set-up errors to conduct a quality assurance （QA） aspect of treatment delivery, verification of the treatment field＇s position on different days using electronic portal. Methods： This study was carried out on 12 patients, treated for pelvis tumor; and total of 240 images obtained by electronic portal image device （EPID） were analyzed. The EPIs acquire using EPID attached to the Siemens linear accelerator. The anatomy match- ing software （Theraview） was used and displacement in two dimensions were noted for each treatment field to study patient setup errors. Results： The percentages of mean deviations less than 5 mm in X direction were 65% ＆ 92%, from 5-10 mm were 31% ＆ 19% and more than 10 mm were 11% ＆ 9% forNP and lateral direction respectively. The percentages of mean deviations less than 5 mm in Y direction were 65% ＆ 63%, from 5-10 mm were 33% ＆ 28% and more than 10 mm were 22% ＆ 29%. The mean deviations in 2D-vector errors were 〈 5 mm in 47% and 46%, 5-10 mm in 36% and 37% and 〉 10 mm in 37% and 37% of images in the NP and lateral direction respectively. Conclusion： The results revealed that the ranges of set up errors are immobilization method to improve reproducibility. The observed variations were not within the limits..

The Expression of miRNAs in Sudanese Patients with Systemic Lupus Erythematosus in Khartoum State

Background: MicroRNAs (miRs) are noncoding gene regulators that may have a role as diagnostic or prognostic biomarkers in systemic lupus erythematosus (SLE). Aim: To measure the blood levels of miR-146a, miR-126 and miR-30a in Sudanese SLE patients and to investigate their potential role in disease pathogenesis and utility as biomarkers for SLE. Material and Methods: A total of 48 SLE patients and 20 matched healthy individuals were enrolled in this study. SLE disease activity index (SLEDAI) was assessed. The blood levels of miR-146a, miR-126 and miR-30a were determined by Real-time polymerase chain reaction (PCR) in all participants. Γ-INF and IL-2 were analyzed by elISA, and CD markers were used in flow cytometry. Results: The mean age of the patients was 31.5 ± 8.5 years with disease duration > 5 years. In SLE patients, the mean blood level fold changes of miR-146a (0.33 ± 0.277;P 0.305) compared to controls. Down regulation of miR-146a increase expression of γ-INF (P ely, miR-30a was insignificantly associated with SLE disease (P value > 0.05) as no differences between the SLE patients and healthy control. Conclusion: Circulating miR-146a and miR-126 could be a potential noninvasive biomarker in SLE. This study provides an overview of the current state of research on the role of miRNAs in the immune pathogenesis and regulation of SLE. Further studies are needed in miRNAs profiling expressions of SLE diseases.

Strain Rate Effect on the Response of Blast Loaded Reinforced Concrete Slabs

Dynamic Increase Factor (DIF) due to strain rate effect was examined with documented experimental work done by Razaqpur, et al. In the experiment work, two 1000 × 1000 × 70 mm reinforced concrete slabs were constructed. The slabs were subjected to blast loads generated by the detonation of either 22.4 kg or 33.4 kg of ANFO located at a 3.0 m standoff. Blast wave characteristics, including incident and reflected pressures and reflected impulses were measured. The slabs were modeled by explicit analysis with or without strain rate effect to study their behavior under blast load to compare their predicted and observed behavior. The predicted post-blast damage and mode of failure for each model is compared with the observed damage of experimental work. It was concluded that when the dynamic increase factor added to concrete and reinforcement materials due to strain rate effect, the behavior of model under blast load become closer to experimental work.

Implicit and Explicit Analysis of the Response of Blast Loaded Reinforced Concrete Slabs

Implicit and explicit analyses were examined with experimental work done by Razaqpur et al. In the experiment work, two 1000 × 1000 × 70 mm reinforced concrete slabs were constructed. The slabs were subjected to blast loads generated by the detonation of either 22.4 kg or 33.4 kg of ANFO located at a 3.0 m standoff. Blast wave characteristics, including incident and reflected pressures and reflected impulses were measured. The slabs were modeled by implicit and explicit analysis to study their behavior under blast load to compare their predicted and observed behavior. The post-blast damage and mode of failure of each slab were observed. It was concluded that explicit analysis provides better modeling than implicit analysis.

How does epidemiological and clinicopathological features affect survival after gastrectomy for gastric cancer patientssingle Egyptian center experience

AIM:To investigate the clinicopathological features and the significance of different prognostic factors which predict surgical overall survival in patients with gastric ***:This retrospective study includes 80 patients diagnosed and treated at gastroenterology surgical center,Mansoura University,Egypt between February 2009 to February *** factors were assessed by cox proportional hazard ***:There were 57 male and 23 *** median age was 57 years(24-83).One,3 and 5 years survival rates were 71%,69% and 46% *** median survival was 69.96 *** the follow-up period,13 patients died(16%).Hospital morbidity was reported in 10 patients(12.5%).The median number of lymph nodes removed was 22(4-41).Lymph node(LN) involvement was found in 91% of *** R0 resection,depth of wall invasion,LN involvement and the number(> 15) of retrieved LN,LN ratio and tumor differentiation predict *** multivariable analysis,tumor differentiation,curability of resection and a number of resected LN superior to 15 were found to beindependent prognostic ***:Surgery remains the cornerstone of *** differentiation,curability of resection and a number of resected LN superior to 15 were found to be independent prognostic *** LN dissection does not increase the morbidity or mortality rate but markedly improves long term survival.
