Context: Individual contributions of obesity and physical fitness (physical ac tivity and functional capacity) to risk of coronary heart disease in women remai n unclear. Objective: To investigate the relationships of measures of obesity (b ody mass index [bMI], waist circumference, waist hip ratio, and waist height r atio) and physical fitness (self reported Duke Activity Status Index [DASI] a nd Postmenopausal Estrogen Progestin Intervention questionnaire [PEPI Q] score s) with coronary artery disease (CAD) risk factors, angiographic CAD, and adverse cardiovascular (CV) events in women evaluated for suspected myocardial ischemia. Design, Setting, and Participants: The national Heart, Lung, and blood Institut e sponsored Womens Ischemia Syndrome Evaluation (WISE) is a multicenter prosp ective cohort study. From 1996-2000,936 women were enrolled at 4 US academic me dical centers at the time of clinically indicated coronary angiography and then assessed (mean follow up, 3.9 [SD,1.8] years) for adverse outcomes. Main Outc om e measures: Prevalence of obstructive CAD (any angiographic stenosis ≥50%) and incidence of adverse CV events (all cause death or hospitalization for nonfata l myocardial infarction, stroke, congestive heart failure, unstable angina, or o ther vascular events) during follow up. Results: Of 906 women (mean age, 58 [S D , 12] years) with complete data, 19%were of nonwhite race, 76%were overweight (bMI ≥25), 70%had low functional capacity (DASI scores nt to ≤7 metabolic equivalents [meTs]), and 39%had obstructive CAD. During follow up, 3 37 (38%)-women had a first adverse event, 118(13%) had a major adverse event, and 68 (8%) died. Overweight women were more likely than normal weight women t o have CAD risk factors, but neither bMI nor abdominal obesity measures were sig nificantly associated with obstructive CAD or adverse CV events after adjusting for other risk factors (P=.05 to .88). Cytokine storms are crucial in the development of various inflammatory diseases,including sepsis and autoimmune *** immunosuppressive cytokine InTERLEUKIn(IL)-37 consists of five isoforms(IL-37a-e).We identified IL-37a as a nuclear cytokine for the first *** to IL-37b,IL-37a demonstrated greater efficacy in protecting against Toll-like receptor-induced cytokine hypersecretion and lethal endotoxic *** full-length(FL)form of IL-37a and the n-terminal fragment,which is processed by elastase,could translocate into cell nuclei through a distinctive nuclear localization sequence(nLS)/importin nuclear transport *** forms exerted their regulatory effects independent of the IL-1R8 receptor by transcriptionally upregulating the nuclear receptor peroxisome proliferator-activated receptor(PPARγ).This process involved the recruitment of the H3K4 methyltransferase complex WDR5/MLL4/C/EbPβand H3K4me1/2 to the enhancer/promoter of *** receptor-independent regulatory pathway of the nuclear IL-37a–PPARγaxis and receptor-dependent signaling by secreted IL-37a maintain homeostasis and are potential therapeutic targets for various inflammatory diseases,including sepsis.

Chemical investigation of beouverio felina,a marine-derived entomopathogenic fungus,obtained two new destruxin hexadepsipeptides(1 and 2)containing an/.-Ala^(5)residue instead of the usual A/-me-L-Ala^(5).Interestingly,the presence of the/.-Ala^(5)residue enabled two slowly-exchanging solution conformers with cis/trons(ILE^(3))-(A/-me-Val^(4))amide b.n. for 1 and 2 with 1.5:1 and 1:1 cis/trans ratios,respectively,while only the conformer with c/s amide b.n. was found in the *** solution and solid-state conformers were studied by nMR,X-ray diffraction and solution and solid-state ECD measurements,as well as by TDDFT-ECD ***(conformational)parameter-activity correlations suggested that the extended destruxin geometry with the preferred c/s(ILE^(3))-(A/-me-Val^(4))b.n. probably resulted in improved bioactivity and reduced toxicity for the destruxin *** was the first report on the interconversion phenomenon of cis/trans amide b.n.s of destruxin class,which could give insights in the development of bioactive destruxin agents with lower toxicity.

The reaction of triphenylstannyl-bis(3,5-dimethyl-4-ethylpyrazol-1-yl)methane with W(CO)5THF yields the title compound (C37.5H39.5n4O3SnW, Mr = 896.79) which is of triclinic, space group P with a = 9.755(3), b = 13.696(4), c = 14.359(5) , a = 96.202(7), b = 93.313(6), g = 94.845(6)o, V = 1896(1) 3, Z = 2, Dc = 1.571 g/cm3, l(MoKa) = 0.71073 , m = 3.728 mm-1, F(000) = 881, R = 0.0491 and wR = 0.0856 for 4242 observed reflections (I≥2s(I)). The crystal structural analysis indicates that bis(3,5-dimethyl-4-ethylpyrazol-1-yl)methide acts as a tridentate monoanionic k3-[n,C,n] chelating ligand, and four-membered metallacycles are found in this heterodinuclear complex.

Microscopic enteritis(me) is an inflammatory condition of the small bowel that leads to gastrointestinal symptoms,nutrient and micronutrient *** is characterised by microscopic or sub-microscopic abnormalities such as microvillus changes and enterocytic alterations in the absence of definite macroscopic changes using standard modern *** work recognises a need to characterize disorders with microscopic and submicroscopic features,currently regarded as functional or non-specific entities,to obtain further understanding of their clinical *** consensus working party reviewed statements about the aetiology,diagnosis and symptoms associated with me and proposes an algorithm for its investigation and *** the 5th International Course in Digestive Pathology in bucharest in november 2012,an international group of 21 interested pathologists and gastroenterologists formed a working party with a view to formulating a consensus statement on me.A five-step agreement scale(from strong agreement to strong disagreement) was used to score 21 statements,*** was strong agreement on all statements about me histology(95%-100%).Statements concerning diagnosis achieved 85% to 100% agreement.A statement on the management of me elicited agreement from the lowest rate(60%) up to 100%.The remaining two categories showed general agreement between experts on clinical presentation(75%-95%) and pathogenesis(80%-90%) of *** was strong agreement on the histological definition of *** agreement on management indicates a need for further investigations,better definitions and clinical trials to produce quality guidelines for *** me consensus is a step toward greater recognition of a significant entity affecting symptomatic patients previously labelled as non-specific or functional enteropathy.

目的比较拉米夫定联合胸腺素α1与单独应用拉米夫定治疗慢性乙型肝炎（CHb）患者的临床疗效及安全性。方法采用多中心、完全随机、对照的方法，按1：1比例将68例人选的CHb患者随机分为实验组和对照组，实验组服用拉米夫定12个月，同时注射胸腺素α16个月；对照组单独服用拉米夫定12个月。在治疗第3、6、12和18个月时（停药6个月随访）观察2组患者血清HbVDnA转阴率、HbeAg转阴率、HbeAg血清学转换率、ALT复常率以及胸腺素α1的安全性。结果治疗12个月时，实验组HbVDnA转阴率、HbeAg转阴率和ALT复常率与对照组比较差异有统计学意义（，值分别为31．17、7．17和5．92，P值均〈0．05）；停药6个月随访，实验组的HbVDnA转阴率和HbeAg转阴率与对照组比较差异有统计学意义（X^2值分别为4．53和7．17，P值均〈0．05）；在停药6个月的随访过程中，实验组有2例患者检测不出HbVDnA。治疗中及之后随访观察期间未出现严重不良反应。结论拉米夫定联合胸腺素α1治疗CHb患者的疗效优于单用拉米夫定，且安全性良好。
