Double balloon enteroscopy in children:Diagnosis,treatment,and safety

AIM:To assess the feasibility and utility of double balloon enteroscopy(DBe)in the management of small bowel diseases in children. MeTHODS:Fourteen patients(10 males)with a median age of 12.9 years(range 8.1-16.7)underwent DBe; 5 for Peutz-Jeghers syndrome(PJ syndrome),2 for chronic abdominal pain,4 for obscure gastrointestinal (GI)bleeding,2 with angiomatous malformations(1 blue rubber bleb nevus syndrome)having persistent GI bleeding,and 1 with Cowden's syndrome with multiple polyps and previous *** procedures were performed under general anesthesia and 3 with deep sedation. ReSULTS:The entire small bowel was examined in 6 patients,and a length between 200 cm and 320 cm distal to pylorus in the remaining *** patients had both antegrade(trans-oral)and retrograde(transanal and via ileostomy)*** patient underwent DBe with planned laparoscopic *** remaining 6 had trans-oral examination *** median examination time was 118 min(range 95-195). No complications were *** were detected and successfully removed in all 5 patients with PJ syndrome,in a patient with tubulo-villous adenoma of the duodenum,in a patient with significant anemia and occult bleeding,and in a patient with Cowden's syndrome.A diagnosis was made in a patient with multiple angiomata not amenable to endotherapy,and in 1 with a discrete angioma which was treated with argon plasma *** source of bleeding was identified in a further patient with *** was normal or revealed minor mucosal friability in the remaining 3 *** a diagnostic yield of 11/14 with therapeutic success in 9/14 was achieved. CONCLUSION:Double balloon enteroscopy can be a useful diagnostic and therapeutic tool for small bowel disease in children,allowing endo-therapeutic intervention beyond the reach of the conventional endoscope.

Huntington’s Disease(HD)is a genetically dominant trinucleotide repeat disorder resulting from CAG repeats within the Huntingtin(HTT)gene exceeding a normal range(>36 CAGs).Symptoms of the disease manifest in middle age and include chorea,dystonia,and cognitive *** latency from diagnosis to death is 20 *** are currently no disease-modifying therapies available to HD *** is a potentially curative therapy for HD.A popular line of research employs siRNA or antisense oligonucleotides(ASO)to knock down mutant Huntingtin mRNA(mHTT).Unfortunately,this modality requires repeated dosing,commonly exhibit off target effects(OTes),and exert renal and hepatic *** contrast,a single AAV-mediated short-hairpin RNA(shRNA)dose can last years with low *** addition,we highlight research indicating that shRNA elicits fewer OTes than siRNA when tested *** this promise,shRNA therapy has been held back by difficulties controlling expression(oversaturating cells with toxic levels of RNA construct).In this review,we compare RNAi modalities for HD and propose novel methods of optimizing shRNA expression and on-target fidelity.

Reverse cholesterol transport was originally described as the high-density lipoprotein-mediated cholesterol flux from the periphery via the hepatobiliary tract to the intestinal lumen, leading to fecal excretion. Since the introduction of reverse cholesterol transport in the 1970s, this pathway has been intensively investigated. In this topic highlight, the classical reverse cholesterol transport concepts are discussed and the subject reverse cholesterol transport is revisited.

Predation rate with relation to species, sex and water temperature was tested among four different gammaridean species:Dikerogammarus villosus, Gammarus roeselii, Gammarus pulexandGammarus *** were performed in microcosms in climate-controlled rooms at five different *** magna,a common water flea, served as prey. On averageD. villosusshowed the highest consumption rate ofDaphnia magnaover the entire temperature range, followed in decreasing order byG. p u le x,G. roeseliiandG. fossarum. The predation rate of all species showed a distinct peak at 20°C. Correction of predation rates for body size gave somewhat different results.D. villosusis then still the most predatory of all gammaridean species tested followed byG. pulex,G. fossarumandG. roeselii. The outcome of theDaphniatests is consistent with results of other studies with different prey. This supports that theDaphniatest is a good and quick indicator of the predatory abilities in gammaridean species at varying temperatures, and allows the prediction of how changing temperature regimes influence invasion impacts.

The negative effect of soil erosion and soil compaction is well documented for the purpose of optimum rangeland functioning, while the impact of rangeland degradation on effective soil depth is seldom quantified. The aim of this study was to quantify the response of vegetation cover and soil properties, particularly effective soil depth and soil texture to rangeland degradation. Forty-one farms were sampled in the arid and semi-arid climate of South Africa. Within these farms, data was collected over a vegetation degradation gradient. Results showed a significant decline in relative basal cover (94% ± 15% to 39% ± 17%) and soil depth (90% ± 14% to 73% ± 24%) as rangeland degraded. Soil texture changes over the degradation gradients vary for different homogeneous vegetation types. Indications regarding the loss of a functioning rangeland ecosystem were also demonstrated, using objective long-term relations between rangeland conditions and grazing capacity. The study highlights the importance of sustainable rangeland management practices to reduce the loss in effective soil depth and to ensure the sustainable utilization of the rangeland ecosystem. These results can probably extrapolate to other arid and semi-arid rangelands worldwide.

The highly effective direct-acting antivirals(DAAs)for the treatment of chronic hepatitis C virus(HCV)infection were well-received by both patients and physicians as sustained virological response(SVR)was associated with an improved overall survival in patients with stable liver disease,even in case of cirrhosis(1).Yet,at that time these results were derived from retrospective cohort studies with long-term follow-up after interferon-based *** patients with compensated liver disease there is confidence that SVR is a clinically relevant endpoint based on consistent positive results with smart use of statistics in numerous cohort ***,however,there remains a need for long-term follow-up data showing a clinical benefit of DAA-induced SVR,preferably with high-quality prospective *** results from the French ANRS CO22 Hepather cohort,as published by *** and colleagues in the April issue of the Lancet,thus bring an important contribution to the field(2).It’s the first prospective cohort study with reasonable follow-up time to assess all-cause mortality after DAA therapy.

Over 2 billion people worldwide are estimated to have nonalcoholic fatty liver disease(NAFLD),defined by excess hepatic ***,progression into nonalcoholic steatohepatitis(NASH)is characterized by the onset of liver inflammation following exacerbated steatosis.1 This suggests that reducing liver inflammation(*** lifestyle interventions involving exercise training)may come secondary to a reduction of ***,both the metabolic and inflammatory processes involved in NAFLD are under circadian control and could respond differently to exercise at different times of day.2 To investigate the time-of-day–dependent effect of exercise training on NAFLD amelioration in the early disease stages we trained high-fat high-cholesterol(HFHC)-fed APOe*3-Leiden cholesteryl ester transfer protein(CeTP)mice during their early or late active *** mouse model was chosen because of its humanized lipid metabolism and its ability to develop all hallmarks of human NAFLD upon HFHC feeding.

This paper describes the effects that temperature changes in the Rhine river distributaries have on native and exotic fish diversity. Site-specific potentially affected fractions (PAFs) of the regional fish species pool were derived using species sensitivity distributions (SSDs) for water temperature. The number of fish species in the river distributaries has changed remarkably over the last century. The number of native rheophilous species declined up until 1980 due to anthropogenic disturbances such as commercial fishing, river regulation, migration barriers, habitat deterioration and water pollution. In spite of progress in river rehabilitation, the native rheophilous fish fauna has only partially recovered thus far. The total number of species has strongly increased due to the appearance of more exotic species. After the opening of the Rhine-Main-Danube waterway in 1992, many fish species originating from the Ponto-Caspian area colonized the Rhine basin. The yearly minimum and maximum river temperatures at Lobith have increased by circa 40C over the period 1908-2010. exotic species show lower PAFs than native species at both ends of the temperature range. The interspecific variation in the temperature tolerance of exotic fish species was found to be large. Using temporal trends in river temperature allowed past predictions of PAFs to demonstrate that the increase in maximum river temperature negatively affected a higher percentage of native fish species than exotic species. Our results support the hypothesis that alterations of the river Rhine’s temperature regime caused by thermal pollution and global warming limit the full recovery of native fish fauna and facilitate the establishment of exotic species which thereby increases competition between native and exotic species. Thermal refuges are important for the survival of native fish species under extreme summer or winter temperature conditions.
