Background Coronary heart disease （CHD） risk increases with age; yet lipid-lowering therapies are significantly under-utilized in patients 〉 65 years. The objective was to evaluate the safety and efficacy of lipid-lowering therapies in older patients treated with atorvastatin 10 nag ＋ ezetimibe 10 mg （eZ/Atorva） vs. increasing the atorvastatin dose to 40 mg, methods Patients 〉 65 years with atherosclerotic vascular disease （LDL-C ≥1.81 mmol/L） or at high risk for coronary heart disease （LDL-C 〉 2.59 mmol/L） were randomized to eZ/Atorva for 12 wk vs. uptitration to atorvastatin 20 nag for 6 wk followed by atorvastatin 40 nag for 6 wk. The percent change in LDL-C and other lipid parameters and percent patients achieving prespecified LDL-C levels were assessed after 12 wk. Results eZ/Atorva produced greater reductions in most lipid parameters vs. uptitration of atorvastatin in patients 〉 75 years （n = 228）, generally consistent with patients 65-74 years （n = 812）. more patients achieved LDL-C targets with combination therapy vs. monotherapy in both age groups at 6 wk and in patients 〉 75 years at 12 wk. At 12 wk, more patients 〉 75 years achieved LDL-C targets with monotherapy vs. combination therapy. eZ/Atorva produced more favorable improvements in most lipids vs. doubling or quadrupling the atorvastatin dose in patients 〉 75 years, generally consistent with the findings in patients 65-74 years. Conclusions Our results extended previous findings demonstrating that ezetimibe added to a statin provided a generally well-tolerated therapeutic option for improving the lipid profile in patients 65 to 74 years and 〉 75 years of age.

Switchgrass Biochar effects on Plant Biomass and microbial Dynamics in Two Soils from Different Regions

Biochar amendments to soils may alter soil function and fertility in various ways, including through induced changes in the microbial community. We assessed microbial activity and community composition of two distinct clayey soil types, an Aridisol from Colorado （CO） in the U.S. Central Great Plains, and an Alfisol from Virginia （va） in the southeastern USA following the application of switchgrass （Panicum virgatum） biochar. The switchgrass biochar was applied at four levels, 0%,0, 2.5%, 5%, and 10%, approximately equivalent to biochar additions of 0, 25, 50, and 100 t ha^-1, respectively, to the soils grown with wheat （Triticum aestivum） in an eight-week growth chamber experiment. We measured wheat shoot biomass and nitrogen （N） content and soil nutrient availability and N mineralization rates, and characterized the microbial fatty acid methyl ester （FAme） profiles of the soils. Net N mineralization rates decreased in both soils in proportion to an increase in biochar levels, but the effect was more marked in the va soil, where net N mineralization decreased from -2.1 to -38.4 mg kg^-1. The 10% biochar addition increased soil pH, electrical conductivity, mehlich- and bicarbonate-extractable phosphorus （P）, and extractable potassium （K） in both soil types. The wheat shoot biomass decreased from 17.7 to 9.1 g with incremental additions of biochar in the CO soil, but no difference was noted in plants grown in the va soil. The FAme recovery assay indicated that the switchgrass biochar addition could introduce artifacts in analysis, so the results needed to be interpreted with caution. Non-corrected total FAme concentrations indicated a decline by 457o and 34% with 10% biochar addition in the CO and va soils, respectively, though these differences became nonsignificant when the extraction efficiency correction factor was applied. Sol-gel-derived nanoporous ZnO film has been successfully deposited on glass substrate at 200 °C and subsequently annealed at different temperatures of 300, 400 and 600 °C. Atomic force micrographs demonstrated that the film was crack-free, and that granular nanoparticles were homogenously distributed on the film surface. The average grain size of the nanoparticles and RmS roughness of the scanned surface area was 10 nm and 13.6 nm, respectively, which is due to the high porosity of the film. Photoluminescence （PL） spectra of the nanoporous ZnO film at room temperature show a diffused band, which might be due to an increased amount of oxygen va- cancies on the lattice surface. The observed results of the nanoporous ZnO film indicates a promising application in the development of electrochemical biosensors due to the porosity of film enhancing the higher loading of biomacromolecules （enzyme and proteins）.

目的分析中国澳门地区2001至2005年结核病的耐药情况。方法选取近5年澳门地区新发结核病患者及复治结核病患者结核分枝杆菌分离株进行4种抗结核药物(异烟肼、利福平、链霉素、乙胺丁醇)的耐药性测定。结果 1460株结核分枝杆菌的总耐药率为16.2%(236株),其中初始和获得性耐药率分别为15.0(203/1357)和32.0(33/103);耐多药率为3.2%(47/1460),其中初始和获得性耐多药率分别为2.3%(31/1357)和15.5%(16/103);4种抗结核药物的耐药率由高到低依次为异烟肼(11.5%)、链霉素(9.5%)、利福平(3.6%)、乙胺丁醇(2.5%);耐1种、2种、3种和4种药物的初始耐药率分别为9.9%、2.9%、1.0%和1.2%,获得性耐药率分别为12.6%、3.9%、8.7%和6.8%。近5年新发和复治患者的耐药率没有上升趋势(X^2值分别为0.27和0.03,P 均>0.05);不同性别和年龄组间初始耐药率的差异无统计学意义(X^2=5.7,P>0.05)。结论本次流行病学调查结果与1996至1999年澳门地区结核病流行病学调查资料比较,获得性耐药率和获得性耐多药率显著下降,差异有统计学意义(X^2值分别为6.04和7.47,P 均<0.05),但初始耐药率和获得性耐药率高于2004年第3次全球多国耐药监察结果,仍处于高耐药水平,尤其是耐多药率较高的问题,应引起重视。

Over the past decade, there has been a substantial increase in the number of men who are treated with testosterone. Despite this increase in the use of testosterone, the risks of adverse cardiovascular events are unclear as meta-analyses have reported conflicting findings and no clinical studies have been large enough or long enough to adequately assess for cardiovascular risks. The goal of this paper is to review large prescription database studies of testosterone treatment and adverse cardiovascular events and mortality with the aim of providing some guidance for clinicians and researchers in this controversial area.

This is a commentary on the often-observed phenomenon of observing task-based brain signaling differences between clinical populations and healthy control participants in the absence of any behavioral decrements in the clinical group. We offer several explanations for why brain-based differences amid normative performance may be of interest to researchers and clinicians. First, neural processing in the clinical group may not be as efficient as that in the control group. Second, differences in activation could reveal important differences in the cognition behind the (normative) behavior. Third, differences in activation may be prognostic biomarkers of injury or decline. In addition, we contend that similar behavior between groups is important in properly interpreting brain data. Finally, we offer caveats and future directions to further clarify brain mechanisms underlying behavior in clinical populations.

AIm:To assess tumor necrosis factor-a(TNF-a),infliximab(IFX)concentrations,and antibodies against IFX molecules in patients with inflammatory bowel disease(IBD)who develop loss of response,side effects,or allergic reaction during anti TNF-a ***:Blood samples of 36 patients with response loss,side effects,or hypersensitivity to IFX therapy(Group?Ⅰ)and 31 patients in complete clinical remission(GroupⅡ)selected as a control group were collected to measure trough serum TNF-a level,IFX,and anti-IFX antibody(ATI)*** examined the correlation between loss of response,the development of side effects or hypersensitivity,and serum TNF-a,IFX trough levels,and ATI ***:The serum TNF-a level was shown to be correlated with the presence of ATI;ATI positivity was significantly correlated with low trough levels of *** were detected in 25%of IBD patients with loss of response,side effects,or hypersensitivity,however no association was revealed between these patients and antibody positivity or lower serum IFX *** use of IFX correlated with the development of ATI,although concomitant immunosuppression did not have any impact on ***:On the basis of the present study,we suggest that the simultaneous measurement of serum TNF-a level,serum anti TNF-a concentration,and antibodies against anti TNF-a may further help to optimize the therapy in critical situations.
