Background:Patients with inflammatory bowel disease(IBD)are.a. increased risk of herpes zoster(HZ).We e.a.uated the incidence of HZ in ulce.a.ive colitis(UC)and Crohn’s disease(CD)patients and compared this with HZ incidence in a non-IBD ***:We conducted a retrospective cohort study(GSK study identifier:VeO-000043)of adults aged18 years with UC and CD and without IBD who were identified from claims recorded in a US healthcare database between October 2015 and February *** HZ incidence rates/1,000 person-years(PY)were calculated,and comparisons of HZ incidence rates between UC or CD and non-IBD cohorts were made using adjusted gene.a.ized linear ***:The study population comprised a total of 29,928 UC,25,959 CD,and 11,839,329 non-IBD *** ove.a.l HZ incidence rates were 13.64/1,000 PY(UC),15.94/1,000 PY(CD),and 7.95/1,000 PY(non-IBD).UC and CD patients had increased HZ incidence rates,with adjusted incidence rate ratios of 1.35(95%confidence interval[CI],1.26–1.44)and 1.66(95%CI,1.56–1.77),respectively,compared with non-IBD *** analysis indicated increased re.a.ive rates of HZ in progressively younger age strata in the UC and CD patients compared with non-IBD *** incidence rates were higher in UC and CD patients who had previously received thiopurines or methotre.a.e,TNF-inhibitors,or corticosteroids than in UC and CD patients who did not take those ***:UC and CD patients had increased HZ incidence rates compared with patients without IBD,demonstrating the importance of HZ prevention in IBD patients.

The Wnt/β-catenin signaling pathway is instrumental in successful differentiation and prolife.a.ion of mammalian cells. It is therefore not surprising that the herpesvirus family has developed mechanisms to inte.a.t with and manipulate this pathway. Successful coexistence with the host requires that herpesviruses establish a lifelong infection that includes periods of latency and reactivation or persistence. Many herpesviruses establish latency in progenitor cells and viral reactivation is linked to host-cell prolife.a.ion and differentiation status. Importantly, Wnt/β-catenin is tightly connected to stem/progenitor cell mainte.a.ce.a.d differentiation. Numerous studies have linked Wnt/β-catenin signaling to a variety of cancers, emphasizing the importance of Wnt/β-catenin pathways in development, tissue homeostasis and disease. This review de.a.ls how the.a.pha-, be.a., and gammaherpesviruses inte.a.t and manipulate the Wnt/β-catenin pathway to promote.a.virus-centric agenda.

BACKGROUND This is a unique case of a patient who was found to have two extremely rare primary malignancies synchronously,i.e.,an ampullary adenocarcinoma arising from a high-grade dysplastic tubulovillous adenoma of the.a.pulla of Vater(TVAoA)with a high-grade ileal gastrointestinal stromal tumor(GIST).Based on a lite.a.ure review and to the best of our knowledge,this is the first report of this *** ampullary tumors are extremely rare,with an incidence of four cases per million population,which is approximately 0.0004%.Distal duode.a. polyps are uncommon and have.a.preponde.a.ce of occurring around the.a.pulla of *** adenoma of the.a.pulla(AoA)may occur sporadically or with a familial inheritance pattern,as in hereditary genetic polyposis syndrome such as familial adenomatous polyposis syndrome(FAPS).We report a case of a 77-year-old male who was admitted for painless obstructive jaundice with a 40-pound weight loss over a two-month period and who was subsequently diagnosed with two extremely rare primary malignancies,i.e.,an adenocarcinoma of the.a.pulla arising from a high-grade TVAoA and a high-grade ileal GIST found *** SUMMARY A 77-year-old male was admitted for gene.a.ized weakness with an associated weight loss of 40 pounds in the previous two months and was noted to have painless obstructive *** physical e.a.ination was benign except for bilate.a. scle.a. and palmar *** results were significant for an obstructive pattern on liver *** lipase.a.d carbohydrate.a.tigen-19-9 levels were *** tomography(CT)of the.a.domen and pelvis and magnetic resonance cholangiopancreatography were consistent with a polypoid mass at the level of the common bile duct(CBD)and the.a.pulla of Vater with CBD *** same lesions were visualized with endoscopic retrograde *** of endoscopic forceps biopsy showed *** of the surgical specimen of the

AIM To review surgical outcomes for patients undergoing pancreatectomy after proton the.a.y with concomitant capecitabine for initially unresectable pancreatic *** From April 2010 to September 2013,15 patients with initially unresectable pancreatic cancer were treated withproton the.a.y with concomitant capecitabine.a. 1000 mg orally twice daily. All patients received 59.40 Gy(RBe) to the gross disease.a.d 1 patient received 50.40 Gy(RBe) to high-risk nodal targets. There were no treatment interruptions and no chemothe.a.y dose reductions. Six patients achieved a radiographic response sufficient to justify surgical exploration,of whom 1 was identified as having intraperitoneal dissemination at the time of surgery and the planned pancreatectomy was aborted. Five patients underwent resection. Procedures included:Laparoscopic standard pancreaticoduodenectomy(n = 3),open pyloris-sparing pancreaticoduodenectomy(n = 1),and open distal pancreatectomy with irreversible electroporation(IRe) of a pancreatic head mass(n = 1). ReSULTS The median patient age was 60 years(range,51-67). The median duration of surgery was 419 min(range,290-484),with a median estimated blood loss of 850 cm^3(range,300-2000),median ICU stay of 1 d(range,0-2),and median hospital stay of 10 d(range,5-14). Three patients were re.a.mitted to a hospital within 30 d after discharge for wound infection(n = 1),de.a.ed gastric emptying(n = 1),and ischemic gastritis(n = 1). Two patients underwent R0 resections and demonstrated minimal residual disease in the final pathology specimen. One patient,after ne.a.ive pancreatic head biopsies,underwent IRe followed by distal pancreatectomy with no tumor seen in the specimen. Two patients underwent R2 resections. Only 1 patient demonstrated ultimate local progression at the primary site. Median survival for the 5 resected patients was 24 mo(range,10-30).CONCLUSION Pancreatic resection for patients with initially unresectable cancers is feasible.a.ter
