Assessment of mechanism, type and severity of injury in multiple trauma patients： A cross sectional study of a trauma center in Iran

Purpose： To accurately assess the mechanism, type and severity of injury in Iranian multiple trauma patients of a trauma center. Methods： Patients with multiple traumas referring to the emergency department of Hasheminejad University Hospital in Mashhad, lran, entered this cross sectional study from March 2013 to December 2013. All the patients with injury severity score （ISS）〉 9 were included in this study. Data analysis was performed by SPSS software （Version 11.5） and P values less than 0.05 were considered as significant differences. Results： Among the 6306 hospitalized trauma patients during this period, 148 had ISS〉9. The male fe- male ratio was 80%. The mean age of the patients was （33.5 m 19.3） years. And 71% of the patients were younger than 44 years old. There were 19 （13%） deaths from which 68.5% were older than 44 years old. The mean transfer time from the injury scene to hospital was （55 e 26） minutes. The most frequent mechanisms of injury were motorcycle crashes and falling from height, which together included 66.2% of all the injuries. A total of 84% of hospital deaths occurred after the first 24 h of hospitalization. Head and neck were the most common body injured areas with a prevalence of 111 cases （75%）. Conclusion： Motorcycle crashes have high frequency in lran. Since most victims are young males, injury prevention strategies should be considered to reduce the burden of injuries.

Sperm fertility and motility requires the presence of a functional flagellum to act as a motor to enable sperms to move in *** adenosine monophosphate(cAMP),as a regulator of many biological pathways,contributes in mammalian sperms to the production of soluble adenyl cyclase,a process which is regulated through Ca2+and HCO3-dependent mechanisms and triggers widespread physiological alterations essential to sperm *** aim of this study was to investigate the effect of sodium hydrogen carbonate(NaHCO3)on human sperm adenylyl cyclase *** show that flagellum is essential for sperms to keep their ***,multiple cellular and molecular players and pathways(i.e.,capacitance)are critical for normal maturation of sperms in final stages[1].Sperm capacity is an incorrect maturation process observed in the female reproductive system or can be tested in specific environments,the end point of which has been shown to allow the sperm to withstand the acrosome reaction and fertilize the *** between membrane components(e.g.,ion channels)and intracellular skeletal proteins are strictly regulated and are important for promoting these signaling pathways.

AIM: To assess the prevalence of hepatitis B virus(HBV) and hepatitis C virus(HCV) infection and association with drug induced liver injury(DILI) in patients undergoing anti-tuberculosis(TB) ***: Four hundred and twenty nine patients with newly diagnosed TB- either active disease or latent infection- who were due to commence antiTB therapy between September 2008 and May 2011 were included. These patients were prospectively tested for serological markers of HBV, HCV and human immunodeficiency virus(HIV) infections- hepatitis B core antigen(HBc Ag), hepatitis B surface antigen(HBs Ag), hepatitis B e antigen, Ig G and Ig M antibody to HBc Ag(anti-HBc), HCV Ig G antibody and HIV antibody using a combination of enzyme-linked immunosorbent assay, Western blot assay and polymerase chain reaction techniques. Patients were reviewed at least monthly during the TB treatment initiation phase. Liver function tests were measured prior to commencement of antiTB therapy and 2-4 wk later. Liver function tests were also performed at any time the patient had significant nausea, vomiting, rash, or felt non-specifically unwell. Fisher's exact test was used to measure significance in comparisons of proportions between groups. A P value of less than 0.05 was considered statistically ***: Of the 429 patients, 270(62.9%) had active TB disease and 159(37.1%) had latent TB infection. 61(14.2%) patients had isolated anti-HBc positivity, 11(2.6%) were also HBs Ag positive and 7(1.6%) were HCV-antibody positive. 16/270 patients with active TB disease compared to 2/159 patients with latent TB infection had markers of chronic viral hepatitis(HBs Ag or HCV antibody positive; P = 0.023). Similarly the proportion of HBs Ag positive patients were significantly greater in the active vs latent TB infection group(10/43 vs 1/29, P = 0.04). In this work,we demonstrate that ultraviolet(UV)laser photolysis of hydrocarbon species alters the flame chemistry such that it promotes the diamond growth rate and film *** emission spectroscopy and laser-induced fluorescence demonstrate that direct UV laser irradiation of a diamond-forming combustion flame produces a large amount of reactive species that play critical roles in diamond growth,thereby leading to enhanced diamond *** diamond growth rate is more than doubled,and diamond quality is improved by 4.2%.Investigation of the diamond nucleation process suggests that the diamond nucleation time is significantly shortened and nondiamond carbon accumulation is greatly suppressed with UV laser irradiation of the combustion flame in a laser-parallel-to-substrate geometry.A narrow amorphous carbon transition zone,averaging 4 nm in thickness,is identified at the film–substrate interface area using transmission electron microscopy,confirming the suppression effect of UV laser irradiation on nondiamond carbon *** discovery of the advantages of UV photochemistry in diamond growth is of great significance for vastly improving the synthesis of a broad range of technically important materials.

Management of 634 Consecutive Patients with Chronic Pilonidal Sinus: A Nine-Year experience of a Single Institute

Objectives: Different surgical techniques, with variable morbidity and recurrence rates, have been advocated for the management of chronic pilonidal sinus (PNS). This study was conducted to report the outcome of surgical treatment of 634 cases of chronic PNS at a single institute between January 2001 and January 2010. Methods: Karydakis flap was performed in 244 patients (38.5%, Group 1). excision and midline closure was performed in 371 patients (58.5%, Group 2), while the open method was used in 19 (3.0%). Data regarding patient and sinus characteristics, operative de-tails, postoperative course, complications and recurrence were recorded. Mean follow-up was 73.5 months. Results: 571 patients were male (90.1%) and 63 were female (9.9%). Their ages ranged between 16 - 44 years (mean 25.7 years). The mean body mass index was 31.2 (range 23.6 - 41.9), and 71.5% (453/634) were hirsute. Chronic PNS was the first presentation (primary) in 504 patients (79.5%) and recurrent in 130 (20.5%).Overall complication rate was 16.1% (102/ 634) and overall recurrence rate was 8.4% (53/634). Comparing both groups showed that they were similar regarding demographic characteristics, clinical presentation, hospital stay, healing time and time off work. Operative time was insignificantly longer with Karydakis technique (mean 43.2 versus 39.1 minutes, respectively). Complications were sig-nificantly more in patients with midline closure (21%, 78/371) as compared with Karydakis procedure (9%, 22/244) (P = 0.0001). Likewise, there was a significantly (P = 0.0001) higher rate of recurrence with midline closure (12.1%, 45/371) as opposed to Karydakis technique (2.5%, 6/244). Conclusions: 1) PNS affects mainly young male adults who are usually, obese and hirsute, 2) Karydakis technique for the management of chronic PNS, whether primary or recurrent, is a non-lengthy, efficient procedure that has less overall complications and a lower recurrence rate than conventional excision and midline clo

试验旨在探索高温条件下,饲粮中添加瘤胃维生素丸,缓释多种维生素对泌乳期荷斯坦奶牛血液代谢物和生产繁殖性能的影响。试验期间,最高温湿指数(THI)达到77.7。50头高产奶牛随机分到两个试验组中:1组为试验组,饲粮中补充维生素丸;2组为空白对照组,饲粮中无添加。每个维生素丸含有矿物质(铜16.2 g、硒0.251 g、钴0.236 g、碘0.497 g、锰8.28 g、锌13.32 g)和维生素(维生素A 545.6×103U、维生素D3 109.1×106U、维生素e 1 092 U)。试验结果表明,与对照组相比,饲粮中添加维生素丸可使奶牛产奶量有增加趋势(P=0.07),显著提高乳脂肪含量(P=0.03),极显著增加乳蛋白含量和非脂乳固体(SNF)含量(PeFA)含量降低,但与对照组相比差异不显著(P=0.09)。饲喂维生素丸可使空怀期(产犊后时期)显著缩短(P=0.02),在第5次人工授精时引发的妊娠反应显著增强(P=0.01)。试验结果表明,补充缓释矿物质和维生素超过NRC推荐标准时,显著提高了高温环境下的高产奶牛生育和生产性能。

Invasive candidiasis(IC) bears a high risk of morbidity and mortality in the intensive care units(ICU). With the current advances in critical care and the use of widespectrum antibiotics, invasive fungal infections(IFIs) and IC in particular, have turned into a growing concern in the ICU. Further to blood cultures, some auxil-iary laboratory tests and biomarkers are developed to enable an earlier detection of infection, however these test are neither consistently available nor validated in our setting. On the other hand, patients' clinical status and local epidemiology data may justify the empiric antifungal approach using the proper antifungal option. The clinical approach to the management of IC in febrile, non-neutropenic critically ill patients has been defined in available international guidelines; nevertheless such recommendations need to be customized when applied to our local practice. Over the past three years, Iranian experts from intensive care and infectious diseases disciplines have tried to draw a consensus on the management of IFI with a particular focus on IC in the ICU. The established IFI-clinical forum(IFI-CF), comprising the scientific leaders in the field, has recently come up with and updated recommendation on the same(June 2014). The purpose of this review is to put together literature insights and Iranian experts' opinion at the IFI-CF, to propose an updated practical overview on recommended approaches for the management of IC in the ICU.
