回顾性研究3种针刀技术的胆管套管插入术

Goals: to assess the outcome differences following different precut biliary sphincterotomy needle-knife techniques. Background: Precut biliary needle-knife sphincterotomy (NKS) allows biliary access when standard cannulation techniques fail. Little comparative data exist on the outcome of precut NKS. Study: Retrospective comparison of outcome differences of three NKS techniques performed by three pancreaticobiliary endoscopists at a tertiary referral center. Results: A total of 139 consecutive biliary NKS were performed. In 44 cases (technique A), NKS was performed using a precut fistulotomy technique avoiding the papillary orifice, with pure cutting current, and occasional pancreatic duct (PD) stenting (6 of 44). In 47 cases (technique B), NKS was performed starting from the papillary orifice cutting upward with blended current, and no PD stenting. In 48 cases (technique C), NKS was performed as B but using pure cutting current and frequent PD stenting (15 of 48).NKS was successful in 95.5% ,95.7% , and 89.6% at initial endoscopic retrograde cholangiopancreatography and 100% , 97.8% , and 95.6% after a second endoscopic retrograde cholangiopancreatography. total complications were not significantly different between the three groups; however, a lower incidence of pancreatitis occurred using technique A compared with techniques B and C (not significant). Conclusions: NKS techniques result in a high success rate of biliary cannulation with a similar overall complication rate. Avoiding cutting at the papillary orifice may reduce the risk of pancreatitis. When cutting at the papillary orifice, pancreatic duct stenting, pure cutting current, or both may reduce the incidence of pancreatitis.

Quality assessment of graphene： Continuity, uniformity, and accuracy of mobility measurements

With the increasing availability of large-area graphene, the ability to rapidly and accurately assess the quality of the electrical properties has become critically important. For practical applications, spatial variability in carrier density and carrier mobility must be controlled and minimized. We present a simple framework for assessing the quality and homogeneity of large-area graphene devices. the field effect in both exfoliated graphene devices encapsulated in hexagonal boron nitride and chemical vapor-deposited （CVD） devices was measured in dual current-voltage configurations and used to derive a single, gate-dependent effective shape factor, t, for each device, β is a sensitive indicator of spatial homogeneity that can be obtained from samples of arbitrary shape. All 50 devices investigated in this study show a variation （up to tenfold） in β as a function of the gate bias. Finite element simulations suggest that spatial doping inhomogeneity, rather than mobility inhomogeneity, is the primary cause of the gate dependence of β, and that measurable variations of β can be caused by doping variations as small as 10^10 cm^-2. Our results suggest that local variations in the position of the Dirac point alter the current flow and thus the effective sample shape as a function of the gate bias. We also found that such variations lead to systematic errors in carrier mobility calculations, which can be revealed by inspecting the corresponding β factor.

目的评价经颈静脉肝内门腔分流术（tIPS）专用覆膜支架（Viatorr支架）在tIPS中的应用价值。方法回顾性分析37例在美国俄勒冈州健康生命科技大学Dotter介入放射学研究所采用Viatorr支架行’nPs治疗的患者资料，随访时间为（15．2±9．3）个月（3～42个月）。tIPS指征包括门静脉高压相关性的急慢性消化道出血，经药物及内镜治疗无效者17例；顽固性肝源性胸、腹水18例，***综合征2例。采用配对t检验比较手术前后门腔静脉压力差（PSG）的变化，以Kaplan-Meier曲线分析支架开通率。结果37例共置入41枚Viatorr支架，其中3枚直径为8mm，38枚为10mm，支架带膜长度为4～8cm，无相关手术并发症。PSG由术前的（22．4±8．4）mmHg（1mmHg=0．133kPa）降为（8．1±3．2）mmHg，差异有统计学意义（t=12．754，P〈0．01）。17例出血患者术后出血均停止，1例于术后17个月复发。18例严重顽固性腹水及肝性胸水患者中，4例术后腹水不消退，其余14例随访期间有2例腹水复发。2例（5．4％）发生分流道阻塞，Kaplan-Meier曲线分析结果显示1年的开通率为97．0％。术后1个月内无病死患者，2例分别于术后3个月及15个月死于多器官功能衰竭，晚期病死率为5．4％，死亡前1周内复查支架均通畅。肝移植患者5例（13．5％）。结论Viatorr支架能明显提高tIPS术后开通率，选择合适的支架，采用正确的释放技术能进一步提高疗效，但长期效果评价尚需验证。

Aim:to evaluate the protective efficacy of a DNA vaccine encoding *** ROP5 and GRA15 ***:We constructed eukaryotic plasmids expressing pVAX-ROP5 and pVAX-GRA15,and measured the immune responses to these DNA ***:Kunming mice immunized with pVAX-ROP5 or pVAX-GRA15 showed significantly increased serum IgG2 a titers;th1 responses association with the production of IFN-γ,IL-2,IL12 p40 and IL-12 p70;cell-mediated cytotoxic activity with increased frequencies of IFN-γ secreting CD8+t cells(CD8+ IFN-γ+ t cells),as well as prolonged survival time(19.4±4.9 days for ROP5;17.8±3.8 days for GRA15) and brain cyst reduction(57.4%for ROP5;65.9%for GRA15) compared to control ***-administration with pVAX-ROP5 and pVAX-GRA15 boosted the cellular and humoral immune responses,and significantly increased cyst reduction(79%) and prolonged the survival of immunized mice(22.7±7.2 days).Conclusion:Co-immunization of pVAX-ROP5 and pVAX-GRA15 increase the protective efficacy.
